RESUMEN

BACKGROUND/PURPOSE: To evaluate the presentation, diagnosis and management of congenital urethral polyps (CUP) in children and to report the results of the endoscopic resection of polyp with long-term follow-up. METHODS: Between April 1995 to March 2010, 18 children (14 boys, 4 girls) with CUP were treated. The most common presentation was urinary outflow obstruction/retention, hematuria or protruding polyp from the urethra meatus in girls. Six patients presented with vesicoureteral reflux (VUR). All children (except one) underwent a transurethral resection of the CUP. RESULTS: Following the endoscopic resection of the polyps, there was no polyp recurrence, and all patients became symptom-free. The children exhibited no reflux, urinary retention, hematuria or urinary tract infection (UTI) following endoscopic management. Abnormal uroflowmetry patterns returned to normal following the resection of the polyp for one year after the operation. CONCLUSIONS: Urethral polyps must be considered in every child with history of triad of recurrent intermittent urinary retention, hematuria and lower urinary tract symptoms. The cure can be achieved in all cases by an endoscopic approach. This type of tumor is always benign and very rarely recurs, unless the pedicle stalk is not resected. The endoscopic management of reflux is unnecessary in this group of patients due to their natural history of secondary reflux.

Urethral polyp is a rare cause of bladder outlet obstruction, voiding dysfunction, and hematuria in the pediatric age group. Urethral polyps are rarely associated with other congenital urinary tract anomalies. In this study, we report a case of solitary posterior urethral polyp with type I posterior urethral valve in a 7-day-old neonate presented with urinary retention and deranged renal function. The polyp was diagnosed on cystoscopy. Transurethral resection of the polyp with posterior urethral valve fulguration was performed. Pathologic assessment revealed a fibroepithelial lesion, which was consistent with congenital posterior urethral polyp.

RESUMEN

Los pólipos fibroepiteliales de uretra comprenden una patología infrecuente, suelen ser diagnosticados en edad pediátrica por hematuria o síndrome obstructivo bajo y requieren tratamiento quirúrgico para su resolución. Presentamos tres casos clínicos, dos varones de seis y nueve años que fueron diagnosticados y tratados de esta patología tras presentar un cuadro de hematuria franca y dificultad miccional, y una niña de dos años que debutó con clínica de prolapso de masa polipoide por uretra (AU)


Urethral polyps are a rare pathology that is usually diagnosed in childhood hematuria and obstructive urinary symptoms are the most common findings and transurethral resection is the best choice of the treatment .We report three cases, a 6-years-old boy and 9-years-old boy diagnosed and treated of posterior urethral polyp after showing hematuria and obstructive urinary symptoms, and a 2-years-old girl that showed to prolapse of urethral polyp (AU)

RESUMEN

A case of giant congenital urethral polyp on a female newborn is reported. The lesion appeared as a large fleshy, exuberant tumor at the introitus between the labia minora. The mass was constituted by orderly arranged layers of transitional and squamous epithelium outside and loose connective tissue and smooth muscle in its core. The patient was treated by simple excision of the lesion.

Congenital posterior urethral polyps are rare benign lesions that can cause a variety of symptoms in young boys, the diagnosis is usually made by cystourethrogram and ultrasonography where the polyp appears as a soft tissue mass arising at the base of the urinary bladder. We present a case of verumontanum polyp in a 7-year-old boy who presented to us in October 2001 with terminal hematuria, dysuria, interrupted stream and suprapubic pain. The polyp was diagnosed by ultrasonography and cystourethrogram. Transurethral resection of the polyp was performed and pathological assessment revealed a fibroepithelial lesion which is consistent with congenital posterior urethral polyp. After 18 months follow up, the patient was free of symptoms. We reviewed the literature to identify the presentation, diagnosis, treatment options and prognosis of these polyps. In the past 20 years the posterior urethral polyp has become more common than before, and it should be considered in boys with lower urinary tract dysfunction and hematuria.

RESUMEN

OBJECTIVE: To describe an unusual case of congenital fibroepithelial polyp of the prostatic urethra in an adult, presented with acute urinary retention. METHODS: Cystouretrographic, endoscopical and pathological investigations are discussed. RESULTS: The definitive diagnosis of the tenth case of this benign lesion was made only after endoscopic resection and pathological examination. The complete resolution of the symptoms has been quick. CONCLUSIONS: We stress this unusual pathology in order to focus its peculiarity in aetiopathogenesis, diagnosis and treatment.

RESUMEN

Congenital posterior urethral polyps are an uncommon but well-described disease process known to cause hematuria or bladder outlet obstruction in boys. Conventional treatment has included transurethral resection or open cystotomy for complete removal of the polyp. We report the first use of the neodymium:yttrium-aluminum-garnet laser for endoscopic excision of a posterior urethral polyp in a 3-year-old boy.

RESUMEN

A case of congenital urethral polyp in a 10-month-old boy is presented. These rare congenital tumors can lead to urinary retention, hematuria, urinary tract infections, hydronephrosis and hydroureter. Traditionally the diagnosis was based either on voiding cystourethrography or cystoscopy. The sonographic features of these polyps have only been reported in a few cases in the past. This modality is not infrequently the imaging procedure of choice in the work-up of children with signs and symptoms related to the urinary system. In the case presented herein the polyp was demonstrated initially by sonography as a soft tissue mass arising at the base of the urinary bladder. The differential diagnosis of these lesions is presented.

Congenital urethral polyps are an extremely rare cause of obstructive uropathy in childhood. Obstructing polyps arise on the seminal colliculus or the floor of the prostatic urethra. They are pedunculated, and the stalk can be several centimetres in length. Primarily they are composed of connective tissue covered by epithelium. Additionally, smooth muscles and islands of glandular cells and even nerve tissue have been found. We present three cases of children with congenital posterior urethral polyps causing severe infravesical obstruction. In two cases the polyps were excised at open cystotomy and in one case, transurethrally. The follow-up investigations confirmed a good prognosis of these patients: micturition behaviour had normalized and the symptoms of urinary obstruction had either markedly improved or completely disappeared. Dilatation of the upper urinary tract and loss of function resulting from recurrent urinary tract infection remained stable after relief of the obstruction by polypectomy.

RESUMEN

Congenital polyps of the prostatic urethra are an uncommon cause of obstructive uropathy, infection and/or hematuria in male children. A filling defect localized in the posterior urethra on the voiding cystourethrogram represents the peculiar diagnostic finding. Transurethral resection is the treatment of choice, according to the size of the polyp. Two cases of congenital posterior urethral polyps are reported and the main clinical and radiological features are discussed. This lesion has to be considered in the differential diagnosis of the voiding dysfunction in young boys.
