INTRODUCTION: We aim to provide better insights into the demographic, epidemiological, and seasonal characteristics of scorpion envenomation reported in the Rio Doce Valley, Brazil. METHODS: We conducted this monocentric retrospective descriptive study with data on Investigation Forms of Accidents by Venomous Animals, only envenoming cases of scorpion stings, between January 1, 2017 and December 31, 2020, belonging to the compulsory notification system of the Municipal Hospital of Governador Valadares. RESULTS: There were 3032 accidents, and the male-to-female ratio was 1.17:1 (male, 54%; females, 46%). Accidents occurred every month, but there was an increase in June, July, and October to January. In most cases, the time between the scorpion sting and medical care was between 1 and 3 h (n=1304; 43%). The most frequent clinical and systemic manifestations were pain at the bite site (94%) and vagal symptoms (7%). Cases were mild (n=2750, 91%), moderate (n=221, 7%), and severe (n=56, 2%). Children younger than 10 y constituted the most cases, 522 (17%). There was a significant difference between clinical severity and age (P<0.01). Ten patients developed acute pulmonary edema. Two 4-y-old children died. All severely envenomated patients as well as 74% and 2% of mild and moderately envenomated patients, respectively, received specific scorpion antivenoms. CONCLUSIONS: Scorpion envenomation is of greater severity in children aged <10 y. The clinical presentation includes mainly local pain associated with vagal symptoms. Accidents occur every month, but there was an increase in June, July, and October to January.

Scorpionism is responsible for most accidents involving venomous animals in Brazil, which leads to severe symptoms that can evolve to death. Scorpion venoms consist of complexes cocktails, including peptides, proteins, and non-protein compounds, making separation and purification procedures extremely difficult and time-consuming. Scorpion toxins target different biological systems and can be used in basic science, for clinical, and biotechnological applications. This study is the first to explore the venom content of the unexplored scorpion species Rhopalurus crassicauda, which inhabits exclusively the northernmost state of Brazil, named Roraima, and southern region of Guyana. Here, we pioneer the fractionation of the R. crassicauda venom and isolated and characterized a novel scorpion beta-neurotoxin, designated Rc1, and a monomeric hyaluronidase. R. crassicauda venom and Rc1 (6,882 Da) demonstrated pro-inflammatory activities in vitro and a nociceptive response in vivo. Moreover, Rc1 toxin showed specificity for activating Nav1.4, Nav1.6, and BgNav1 voltage-gated ion channels. This study also represents a new perspective for the treatment of envenomings in Roraima, since the Brazilian scorpion and arachnid antivenoms were not able to recognize R. crassicauda venom and its fractions (with exception of hyaluronidase). Our work provides useful insights for the first understanding of the painful sting and pro-inflammatory effects associated with R. crassicauda envenomings.

OBJECTIVE: To describe elements of vulnerabilities and clinical manifestations of the victims of scorpionism in Santarem-Pará. METHOD: Quantitative and qualitative study with a theoretical framework of Twenty-eight patients stung by scorpions were interviewed by applying a semi-structured instrument. Data were analyzed according to the technique of discourse analysis. RESULTS: Seventeen patients (60.7%) were male and eight (28.6%) were illiterate. Twenty-two (78.5%) patients were small farmers. This study revealed that these patients had an important individual, social and programmatic vulnerability. Interviews revealed limited knowledge about prevention and first aid after the envenomation. Programmatic vulnerability is highlighted because there is difficulty in accessing health services due to the great geographical extension of the Amazon region and also a lack of experience of the health care workers in caring for scorpion sting victims. CONCLUSION: This research accurately reveals previously described neuromuscular and sensory nervous system disorders in these victims of scorpionism. As far as we know, this is the first study that addresses the vulnerability of scorpionism using qualitative methodology.

Scorpionism has a high incidence rate in Brazil. It is considered a serious public health problem mainly in tropical and subtropical regions around the world. The number of scorpion accidents have increased over the years and the highest frequencies have been reported mainly in the Brazilian Northeast region. Therefore, in this study we report a retrospective clinical and epidemiological analysis of scorpion stings from 2007 to 2017 in Alagoas State, Northeast Brazil, at a referral hospital for assistance and treatment of accidents by venomous animals. During the analyzed period, the referral hospital treated 27,988 cases, and an increase in the number of cases has taken place over the years. The highest frequency of scorpion stings was observed in females, and the age range most affected was from 20 to 29 years old. The most stung body site was the foot, followed by finger, toe or hand. Regarding the severity, most severe cases were reported in children up to 4 years old (69.4%) and 50% of the total cases treated with serotherapy corresponded to patients in this age range. Interestingly, it was also found that the occurrence of systemic manifestations and the severity of the cases were significantly associated with pediatric patients. In this way, this study highlights the scorpionism as an environmental public health problem in Alagoas State, Northeast Brazil, as well as the need to intensify the epidemiological surveillance and educational campaigns to prevent and control scorpion accidents throughout the year.

OBJECTIVE: to describe the epidemiological profile of scorpionism and to investigate factors associated with case severity in the Far South of Bahia, Brazil. METHODS: this was a descriptive study using data on scorpion accidents reported on the Notifiable Health Conditions Information System (SINAN) from 2010 to 2017; odds ratios (OR) were calculated to investigate factors associated with severe cases. RESULTS: 3,055 cases were reported, of which 411 (13.5%) were severe and included 10 deaths; highest incidence occurred in 2017 (1.3 per 1,000 inhabitants); cases predominated in rural areas (62.5%), among males (70.1%), those aged 20-49 years (51%), and Black people (84%); 22.7% of those stung received serotherapy; severe cases were more frequent among children under 15 years old (OR=3.26; 95%CI 2.55;4.74), and people stung in rural areas (OR=1.40; 95%CI 1.10;1.78). CONCLUSION: scorpionism incidence increased; case severity was associated with younger people and occurrence in rural areas.

Se ha observado la presencia de especies de Tityus en diferentes regiones del país, en las cuales su presencia no había sido comunicada previamente: 1- Tityus bahiensis en las provincias de Entre Ríos y Buenos Aires, en esta última en la localidad de Lanús y en San Clemente del Tuyú, y 2- Tityus confluens en la Ciudad Autónoma de Buenos Aires (CABA) y en la provincia de Buenos Aires en las localidades de Pilar, La Plata, Mar del Plata y Bahía Blanca. Estos hallazgos modifican el mapa de la distribución de escorpiones de importancia sanitaria en Argentina por lo que ante la picadura de escorpiones deben considerarse estos nuevos hallazgos. Esto es especialmente importante en el ámbito de la CABA y la provincia de Buenos Aires, en donde la enorme mayoría de los accidentes por escorpiones han sido causados por T. trivittatus y en donde ahora, al menos en algunas de sus regiones se pueden encontrar T. confluens y T. bahiensis. Se discuten posibles razones de esta nueva distribución así como la ocurrencia de accidentes graves en zonas donde no ocurrían históricamente y de sus posibles causas. En base a los casos graves producidos en los últimos tiempos y a este nuevo mapa de distribución, se hace énfasis en la necesidad de capacitación al personal de salud en general y de los médicos de guardia y terapistas en particular, para tratar adecuadamente los accidentes por escorpiones.
Several species of Tityus have been described in regions of Argentina where their presence had not been previously described. These are: 1- Tityus bahiensis in the provinces of Entre Ríos and Buenos Aires (in the localities of Lanús and San Clemente del Tuyú), and 2- Tityus confluens in the city of Buenos Aires and in the province of Buenos Aires in the localities of Pilar, La Plata, Mar del Plata and Bahía Blanca. These findings modify the distribution map of scorpions of sanitary importance in Argentina, reason for which this new distribution must be considered when facing a scorpion sting. This is especially important in the city of Buenos Aires and the province of Buenos Aires, where most of the accidents by scorpions are caused by Tityus trivittatus, and where at least in some of their regions, T. confluens or T. bahiensis can be found at present. The possible reasons of this new distribution, as well as the possible causes for the occurrence of severe envenomations in regions where these were not observed historically, are discussed. Based on the severe envenomations observed and on this new distribution map, emphasis is placed on the need to capacitate health personnel in general and intensivists or critical care physicians in particular to adequately treat scorpion accidents.

Although underreported across the Amazon region, scorpion stings are very prevalent in some areas and can be potentially life-threatening, especially in children. The most vulnerable populations are those living in locations far from the capitals, hence having limited access to the health system where the appropriate structure for the treatment of severe cases is found. An abundant and diverse fauna of scorpions is found in the region, but few studies have been conducted to decipher the clinical characteristics and therapeutic response of the available antivenoms in envenomings caused by the various species. Antivenom underdosage as well as delayed medical assistance are common among indigenous populations, resulting in poor outcome rates. An in depth understanding of the epidemiological, clinical and therapeutic aspects of scorpion sting envenomings in the Amazon is necessary to improve the outcome of these cases.

The present study analyzes cases of scorpionism in 11 dogs and a cat that were registered at a veterinary clinic in the city of Manizales, between 2009 and 2018. All eight cases where expert identification of the arthropod was possible, involved Centruroides gracilis (Latreille, 1804). None of the stings were lethal, though two cases were classified as severe envenomation and five moderate. The primary sign was local pain, in addition to lameness in all 10 cases that involved one of the limbs (83,3%, 10 of 12 cases). The other two cases had injuries involving the face. The established treatment was symptomatic with clinical and patient monitoring. Antivenom could not be used due to its high cost and scarcity. However, even the severest cases improved satisfactorily, and all patients were discharged without complication.

El veneno contiene varias neurotoxinas capaces de generar una "tormenta autonómica" responsable de las manifestaciones clínicas. Los sistemas cardiovascular y respiratorio son los más afectados y su grado de compromiso determinará la morbimortalidad. Un 10 % de los casos en niños evolucionan a su forma grave. En esta serie retrospectiva, se describe la epidemiología y evolución de 17 pacientes ingresados a la Unidad de Terapia Intensiva Pediátrica entre enero de 2010 y enero de 2014. Si bien no hubo mortalidad, sí una alta morbilidad: 12 pacientes presentaron edema agudo de pulmón que requirió ventilación mecánica; 11 pacientes desarrollaron shock cardiogénico hipotensivo; 7, bajo gasto cardíaco grave y, en 6, se utilizó levosimendán por el carácter refractario de su bajo gasto.

Scorpions are distributed throughout Iran and the genus Hemiscorpius is particularly important in this region. Hemiscorpius lepturus is the most significant species within the genus in the country. Since scorpionism provoked by Hemiscorpius comprises a medical emergency, the present study is focused on this important issue. In order to perform the present work, a review of the medical and health-related literature was carried out in several databases. The current findings indicate that six species of Hemiscorpius are found in 15 states of Iran, mainly in the south and southwest. Deaths caused by stings were reported only for two species. The morphological characteristics and geographical distribution of H. lepturus in Iran, its venom and the toxic compounds, epidemiologic data and clinical manifestations of envenomation as well as treatment for affected people are herein reviewed and described. H. lepturus venom toxicity differs from other Iranian scorpions regarding duration and severity. Scorpionism is an important public health problem in Iran, especially in southwest and south regions and in urban areas. It is more prevalent in children and young people. H. lepturus venom is primarily a cytotoxic agent and has hemolytic, nephrotoxic and to some extent hepatotoxic activity. The use of polyvalent antivenom to prevent scorpion sting symptoms is recommended. A well-planned health education program might be useful in preventing scorpionism.(AU)

Tityus serrulatus (Ts) is the main scorpion species of medical importance in Brazil. Ts venom is composed of several compounds such as mucus, inorganic salts, lipids, amines, nucleotides, enzymes, kallikrein inhibitor, natriuretic peptide, proteins with high molecular mass, peptides, free amino acids and neurotoxins. Neurotoxins are considered the most responsible for the envenoming syndrome due to their pharmacological action on ion channels such as voltage-gated sodium (Nav) and potassium (Kv) channels. The major goal of this review is to present important advances in Ts envenoming research, correlating both the crude Ts venom and isolated toxins with alterations observed in all human systems. The most remarkable event lies in the Ts induced massive releasing of neurotransmitters influencing, directly or indirectly, the entire body. Ts venom proved to extremely affect nervous and muscular systems, to modulate the immune system, to induce cardiac disorders, to cause pulmonary edema, to decrease urinary flow and to alter endocrine, exocrine, reproductive, integumentary, skeletal and digestive functions. Therefore, Ts venom possesses toxins affecting all anatomic systems, making it a lethal cocktail. However, its low lethality may be due to the low venom mass injected, to the different venom compositions, the body characteristics and health conditions of the victim and the local of Ts sting. Furthermore, we also described the different treatments employed during envenoming cases. In particular, throughout the review, an effort will be made to provide information from an extensive documented studies concerning Ts venom in vitro, in animals and in humans (a total of 151 references).

Deaths by venomous animals are medical emergencies that can lead to death and thus constitute sanitary problems in some regions of the world. In the South of America, the accidents by these animals are a common sanitary problem especially in warm, tropical or subtropical regions, related with rural work in several countries. Argentina is located in the extreme South of South America and a minor part of the continental surface is in tropical or subtropical regions, where most of the accidents by venomous animals happen. However, in the big cities in the center and South of the country, with no relation to rural work, scorpionism, mostly due to the synanthropic and facultative parthenogenetic scorpion Tityus trivittatus, has become a sanitary problem in the last few decades. This scorpion is present in the biggest cities of Argentina and in the last decades has killed over 20 children in provinces of the center and north of the country, mostly in big cities. In addition, it seems that this species is growing and spreading in new regions of the cities. In this revision, some characteristics of this scorpion regarding its habitat, spreading in Buenos Aires city, combat measures and available treatments are discussed.

Os escorpiões são artrópodes quelicerados da ordem Scorpiones, sendo conhecidos aproximadamente 1.500 espécies agrupadas em 20 famílias e 165 gêneros, sendo a família Buthidaea de maior importância toxicológica.Estão distribuídos por todos os continentes,exceto a Antártida, mas a maior concentração está nas regiões tropicais e subtropicais. Habitam diversificados ambientes, incluindo desertos,cavernas profundas e altitudes superiores a 4.200m, como nos Andes e no Himalaia.

OBJECTIVES: Scorpion envenomation is a common public health problem in Venezuela. We report an envenoming case by Tityus breweri, endemic to the Guayana Shield, southeast Venezuela, and the outcome of its treatment with antivenom anti-Tityus discrepans. Toxin composition and antigenic reactivity of T breweri venom were also explored. T breweri distribution range was re-evaluated. METHODS: Clinical signs and symptoms in an adult male were recorded after envenoming and treatment with antivenom. Toxin composition and antigenicity of T breweri venom were investigated by polyacrylamide gel electrophoresis, immunoblotting, and mass spectrometry. T breweri distribution range was reassessed by mapping new records of the species. RESULTS: The moderately severe case (a 21-year-old man) presented autonomic manifestations, including cardiopulmonary and gastrointestinal effects. Full recovery was achieved after anti-T discrepans antivenom administration. T breweri venom contains toxins in the 6-8 kd range that affect voltage-sensitive sodium channels. Based on new records, T breweri distribution area reaches 12 155 km.(2) Inclusion of southeast Venezuela as an endemic area of scorpionism prompted the examination of clinical, immunological, and phylogenetic evidence for suggesting a partitioning of the Venezuelan Tityus fauna into toxinological provinces. CONCLUSIONS: The severity of the case reinforces categorization of the Guayana Shield region as a macroendemic area of scorpionism in Venezuela and allows classification of T breweri as a species of medical importance, with toxins immunologically related to central-eastern Venezuelan Tityus. Partitioning of the territory incorporating multiple criteria may help health authorities establish and implement preventive and therapeutic measures for scorpion envenoming in this region.

In Argentina, scorpion poisoning is an entity with high morbidity and mortality mainly produced by Tityus trivitatus sting. This species is distributed all around the center and north of the country. Clinical manifestations range from local pain and paresthesia to multisystemic failure followed by death. Diagnosis is done by recognizing clinical features, and is reinforced by the history of a sting and the identification of the scorpion. Specifc treatment is antitoxic serum. We strongly recommend preventive measures to keep scorpions far from building areas. At present, a new species, Tityus confluens, arouses in association with lethal events.

OBJECTIVE: Identify factors associated with scorpion stings among farm workers who pick corn in the Mexican state of Guerrero. MATERIAL AND METHODS: Cross-sectional survey in 14 randomly selected communities in the state of Guerrero.Simple frequencies were obtained and bivariate analysis was used to identify factors associated with scorpion stings while picking corn. Odds ratio was estimated to evaluate the magnitude of the effect. RESULTS: The incidence of scorpion stings was 15% (500/3 294) in 2003. Use of gloves was associated with a dramatic reduction in risk of scorpion stings to the hands (OR = 0.11; IC95% 0.06-0.18). Scorpion stings are a frequent occupational health issue for farm workers. If the results of this survey were reproduced in an unbiased trial, the implication would be that gloves could prevent 133 stings per 1,000 farm workers who currently do not use gloves. The cost of medical attention, transportation and time away from work due to a scorpion sting totaled 505.90 pesos (46 US dollars). CONCLUSIONS: The use of gloves by farmers who live in regions where scorpions are endemic should be promoted. The supply network for anti-scorpion serum should also be extended to all rural areas where very toxic species are predominant and farmers should be educated about the importance of seeking timely medical care at health clinics.

BACKGROUND: The scorpion puncture is a medical urgency. The greater frequency happens in Jalisco, Morelos and Guerrero. Controversy in the medical treatment persists, some use serum anti-scorpion, others only in critical cases. People by tradition are used the home remedies to cure the scorpion puncture. Our objective was to know the medical treatment and home remedies used in scorpion punctures in the State of Guerrero. METHODS: Retrospective, cross-sectional-descriptive, observational study through a non-probabilistic sample by quotas during a period of eight years with 1,174 patients with age above 18 years with a interview applied by medical students. RESULTS: All the affected patients used home remedies, 94% went to the physician, the more frequent remedies used were garlic, tobacco, cacahuananche, salt, eggs. The most frequent prescribed drugs were: antiscorpion serum, calcium, chloroquine, an antihistamine drugs. Of the total of affected patients, 45% were children and 53 % adults. CONCLUSIONS: In Guerrero, it is traditional to use different home remedies. Doctors used antiscorpion serum in the 56.73% of the cases and symptomatic treatment with different medications.
