RESUMEN

Women in medicine have made progress since Elizabeth Blackwell: the first women to receive her medical degree in the United States in 1849. Yet although women currently represent just over one-half of medical school applicants and matriculates, they continue to face many challenges that hinder them from entering residency, achieving leadership positions that exhibit final decision-making and budgetary power, and, in academic medicine, being promoted. Challenges include gender bias in promotion, salary inequity, professional isolation, bullying, sexual harassment, and lack of recognition, all of which lead to higher rates of attrition and burnout in women physicians. These challenges are even greater for women from groups that have historically been marginalized and excluded, in all aspects of their career and especially in achieving leadership positions. It is important to note that, in several studies, it was indicated that women physicians are more likely to adhere to clinical guidelines, provide preventive care and psychosocial counseling, and spend more time with their patients than their male peers. Additionally, some studies reveal improved clinical outcomes with women physicians. Therefore, it is critical for health care systems to promote workforce diversity in medicine and support women physicians in their career development and success and their wellness from early to late career.

Scientific progress and discovery of preventions and cures for life-threatening diseases depend on the vitality of the biomedical research workforce. We analyzed the workforce of cancer researchers applying for and receiving R01 awards from the National Cancer Institute (NCI) from fiscal years 1990 to 2016, the last year prior to implementation of the Next Generation Researchers Initiative. Here we report that the NCI R01 Principal Investigator (PI) workforce expanded 1.4-fold and aged over this time frame. We tracked 9 age groups and found that the number of PIs in the 3 oldest groups increased dramatically, in contrast with the younger groups. Sustained increases in the number of funded older PIs stemmed from increases in the number of older PIs submitting applications, rather than higher funding rates for older PIs. The decline in the number of funded younger PIs was driven in part by (a) a marked increase in time from PhD degree to first R01 application and award, as well as (b) a decrease in retention of PIs in the funded R01 workforce beyond their first R01 award. The NCI is using these and other analyses to inform strategies and policies for attracting, supporting, and retaining meritorious early-career researchers.

The United States currently has over one million restaurants, making food service one of the largest workforces and industry sectors in the nation's economy. Historically, concern for the health of early restaurant workers was tied largely to the hygiene of the food and thus the wellbeing of the customer rather than the individuals preparing the food. The landscape of occupational illness and injury that resulted is fraught with some of the starkest health disparities in wages, discrimination, benefits, injuries, and illness seen among US laborers. These disparities have consistently been associated with social class and economic position. Conditions identified during the early years of restaurant work, before the introduction of occupational safety and health protections, persist today largely due to tipped wages, dependence on customer discretion, and the management structure. Research and intervention efforts to control occupational health hazards should be directed toward the socioeconomic and structural roots of health problems among food service workers in the United States. Such efforts have important implications for enhancing worker protections, improving wages, and restructuring working conditions for restaurant and food service workers. They also suggest opportunities for occupational health practitioners and researchers to contribute to system-level change analysis to address centuries-old occupational health challenges still facing one of the largest sectors of workers in the country.

RESUMEN

Through a brief historical recovery, the article seeks to describe the main trends in the world of work in Brazil. The emphasis of the article focuses on the current early transition to the service society, whose significant change can be observed in the operation of the labor market itself. Also, it considers the predominance of massive open unemployment, the expansion of the underutilization of the labor force and the generalization of precariousness in professions due to the absence of economic growth and the return of the neoliberal reforms. The result has been the increasing polarization within the world of work.


Através de breve recuperação histórica busca-se descrever as principais tendências do mundo do trabalho no Brasil. A ênfase do ensaio focaliza a atual transição antecipada para a sociedade de serviços, cuja alteração significativa pode ser observada no próprio funcionamento do mercado de trabalho. Também se considera a predominância do massivo desemprego aberto, da ampliação da subutilização da força de trabalho e da generalização da precarização nas ocupações em função da ausência de crescimento econômico e do retorno das reformas neoliberais. O resultado tem sido a polarização crescente no interior do mundo do trabalho.

BACKGROUND: When one thinks of jobs with physical employment standards, the first thoughts typically center around firefighting, law enforcement, and military jobs. However, there are 100s of arduous jobs that exist in the public and private sectors that range from moderately demanding to strenuous. The Bureau of Labor Statistics reported that 28% of the workforce in the United States performs physically demanding jobs that involve construction, machinery installation and repair, public safety, and other professions. OBJECTIVE: This paper provides a historical perspective of physical employment standards for hiring workers into these arduous jobs, how we arrived at our current knowledge base, and the challenges faced today when determining and implementing physical employment standards. METHOD: This narrative review draws on evidence from 62 published sources. RESULTS: This paper focuses on the need for a multidisciplinary approach to identifying job requirements, the professions (e.g., medical, psychology, physiology) that underpin the methodologies, and the knowledge used by current researchers. Descriptions of test and cut score development, legal issues, and challenges for the future also are highlighted.

Importance: The current state of the US pathologist workforce is uncertain, with deficits forecast over the next 2 decades. Objective: To examine the trends in the US pathology workforce from 2007 to 2017. Design, Setting, and Participants: A cross-sectional study was conducted comparing the number of US and Canadian physicians from 2007 to 2017 with a focus on pathologists, radiologists, and anesthesiologists. For the United States, the number of physicians was examined at the state population level with a focus on pathologists. New cancer diagnoses per pathologist were compared between the United States and Canada. These data from the American Association of Medical Colleges Center for Workforce Studies' Physician Specialty Data Books and the Canadian Medical Association Masterfile were analyzed from January 4, 2019, through March 26, 2019. Main Outcomes and Measures: Numbers of pathologists were compared with overall physician numbers as well as numbers of radiologists and anesthesiologists in the United States and Canada. Results: Between 2007 and 2017, the number of active pathologists in the United States decreased from 15 568 to 12 839 (-17.53%). In contrast, Canadian data showed an increase from 1467 to 1767 pathologists during the same period (+20.45%). When adjusted for each country's population, the number of pathologists per 100 000 population showed a decline from 5.16 to 3.94 in the United States and an increase from 4.46 to 4.81 in Canada. As a percentage of total US physicians, pathologists have decreased from 2.03% in 2007 to 1.43% in 2017. The distribution of US pathologists varied widely by state; per 100 000 population, Idaho had the fewest (1.37) and the District of Columbia had the most (15.71). When adjusted by new cancer cases per year, the diagnostic workload per US pathologist has risen by 41.73%; during the same period, the Canadian diagnostic workload increased by 7.06%. Conclusions and Relevance: The US pathologist workforce decreased in both absolute and population-adjusted numbers from 2007 to 2017. The current trends suggest a shortage of US pathologists.

RESUMEN

This article reflects on the history of the NHS in Wales and how this has led to its current structure. How this structure supports integrated working across primary, community and secondary care and how further integration with social care is moving forward and its direct effects on district nursing are explored. This article describes how district nursing is meeting these challenges. Support for district nurses as part of integrated multiprofessional teams is being developed to promote appropriately staffed teams centred on meeting the requirements of people within a designated area and ensuring that home is the best and first place of care.
