RESUMEN

The school nurse role parallels the growing complexity of health care, education, and the social needs of youth in the United States. The complex and chaotic environment of the school setting requires leaders to be flexible and to have the ability to recognize and tackle the ever-changing needs of this environment. School nurses are in a prime position to enact change and drive the culture of the community, giving it purpose, while allowing its members to have a focus on their work. Nurses, as complexity leaders, are positioned to understand that change is a continual process that stems from collaboration, complex systems thinking, and innovation mindsets.

RESUMEN

School nurses are often the primary health professional promoting children's health-and address not just individuals, but communities. This article, featuring the key principle of community/public health, is the first in a series focusing on The Framework for 21st Century School Nursing Practice™ and presents a case study on asthma and air quality issues comparing the more traditional individual approaches to health (labeled downstream and midstream) with population-level approaches (labeled upstream). Through collaboration with key community stakeholders, school nurses can successfully implement policies and programs to address social determinants of health and, in turn, improve the health of the entire community. As we look to advance school nursing and support best practices, concentrating on population-level, upstream approaches align with recommendations outlined in the Framework for 21st Century School Nursing Practice™.

This invited testimony was given during a Robert Wood Johnson Foundation and Campaign for Action meeting held in New Orleans, LA. The meeting was held in conjunction with the Future of Nursing Town Hall in Chicago that focused on social determinants of health. The focus of the meeting was school health and social determinants of health. The author, serving as Director of Research for NASN, was asked to specifically focus on NASN's efforts related to data and research, as well as social determinants of health.

The Future of Nursing (FON) 2020-2030 will be an important roadmap for advancing the profession of nursing. The final FON document is meant to address nursing as a whole-not specific specialties-as well as address changes needed in the entire healthcare system that would facilitate patient safety and care. To ensure inclusion of the needs of school-age children and nurses employed outside the traditional hospital setting in the proceedings, the NASN offered comments at the first public meeting of the Committee on the Future of Nursing in March 2019 (Figure 1) and followed the other town hall meetings carefully. NASN submitted the following observations and suggestions in the form of a memo to the FON 2020-2030 Committee for consideration in the final report.

School nursing is part of America's hidden healthcare system. Recently, the Institute of Medicine commissioned the Robert Wood Johnson Foundation and the AARP (formerly the American Association of Retired Persons) to review progress on its Future of Nursing 2010 study. Additionally, the AARP and the Future of Nursing Campaign for Action held town halls that focused on school nursing. This article is a summary of the testimony offered to the Future of Nursing and Campaign for Action leaders about the value that school nursing brings to the health of our nation and the recommendations for action.

The Robert Wood Johnson Foundation in partnership with the National Academy of Medicine held Town Meeting discussions the summer of 2019 at three sites across the country on the Future of Nursing 2030. Included in the testimony were both invited presentation and public testimony. This article is the transcript of the invited testimony of Robin Cogan, a school nurse who has established a voice for advocacy and the creator of the blog titled The Relentless School Nurse and is one of four presentations featured in this issue of the NASN School Nurse. In her January 2020 letter, NASN's President Laurie Combe discussed the Future of Nursing 2030 and the importance of school nurse advocacy.

When the Robert Wood Johnson Foundation in partnership with the National Academy of Medicine announced the Town Meeting discussions of the Future of Nursing 2030, the president of the National Association of State School Nurse Consultants felt that the voice of School Nursing was crucial to informing the national discussion on health equity and health promotion. Sharonlee Trefry, State School Nurse Consultant, wanted to be sure that the leadership team of the Future of Nursing 2030 heard about the needs of the nation's 56.6 million elementary and secondary public and independent school students. Serving in an even greater role today with regard to promoting equity in healthcare and educational access, school nurses recognize and address social determinants of health every day all day working to grow the next generation of healthy citizens.

La enfermería escolar cuenta con amplias competencias respecto a la mejora de la salud de la comunidad educativa. Sin embargo, en España presenta una baja implementación y visibilidad. El desarrollo de la investigación en enfermería permite aportar la evidencia científica que ponga de manifiesto la importancia que tiene en la mejora de la calidad de vida del alumnado y sus familias. Por ello, el objetivo ha sido conocer el estado de la investigación a nivel nacional, a partir del estudio de asociaciones y grupos de trabajo. Los resultados muestran que el panorama nacional es muy plural y diverso. Existen asociaciones en cuatro comunidades y grupos de trabajo en nueve provincias. Por ello es necesaria la continuidad en la investigación en enfermería escolar y en la creación de grupos de trabajo, ya que su objetivo es implantar esta figura en sus correspondientes comunidades autónomas


The school nurse has broad competencies regarding the improvement of the health of the educational community. However, in Spain it has low implementation and visibility. The development of nursing research allows us to provide scientific evidence that shows the importance it has in improving the quality of life of students and their families. Therefore, the objective has been to know the state of research in Spain, from the study of associations and working groups. The results show that the national panorama is very plural and diverse. There are associations in four communities and working groups in nine provinces. Therefore, continuity in research in school nursing and in the creation of work groups is necessary, since its objective is to implement this figure in their corresponding autonomous communities


A enfermeira da escola possui amplas competências em relação à melhoria da saúde da comunidade educacional. No entanto, na Espanha, tem baixa implementação e visibilidade. O desenvolvimento da pesquisa em enfermagem nos permite fornecer evidências científicas que mostram a importância que ela tem na melhoria da qualidade de vida dos estudantes e de suas famílias. Portanto, o objetivo tem sido conhecer o estado da pesquisa a nível nacional, a partir do estudo de associações e grupos de trabalho. Os resultados mostram que o panorama nacional é muito plural e diversificado. Existem associações em quatro comunidades e grupos de trabalho em nove províncias. Portanto, é necessária a continuidade da pesquisa em enfermagem escolar e na criação de grupos de trabalho, uma vez que seu objetivo é implementar esse número nas comunidades autônomas correspondentes

Introducción: La Enfermería escolar existe en diversos países del mundo desde hace décadas. En España la presencia de Enfermería en los centros es aislada y no está regulada de manera uniforme. La promoción de la salud y Educación para la Salud juegan un papel clave en la salud actual de los escolares. Objetivo: Determinar el grado de consenso de expertos en salud pública e infantojuvenil sobre aspectos relacionados con la Enfermería escolar para esTablecer los contenidos que se deben desarrollar en la función asistencial y en Educación para la Salud. Materiales y métodos: Estudio prospectivo con enfoque cualitativo (técnica Delphi) en dos rondas con una muestra de 17 expertos. Se elaboró un cuestionario basal online pidiendo opiniones y propuestas sobre la Enfermería escolar. En la segunda ronda se incluyeron los porcentajes de respuesta y propuestas que se sometieron a priorización mediante valoración y consenso de los expertos. Resultados: La mayoría refrendan algún tipo de presencia física de enfermeras en los centros escolares, que contribuiría a mejorar la inclusión educativa y la atención a la diversidad. Se resalta su papel en materia de promoción de la salud y prevención, identificación precoz de enfermedades y atención a la salud mental. Conclusiones: Es preciso tener en cuenta el ámbito escolar como un nuevo y diferenciado entorno de actuación para Enfermería. El personal de Enfermería se convierte en un necesario agente de innovación en salud escolar que cuida de toda la comunidad educativa (escolares, profesorado y familia)


Introduction: The School Nursing service has existed in many countries around the world for decades. In Spain, the presence of Nursing in schools is isolated and not uniformly regulated. The promotion of health and Health Education plays a key role in students' current health. Objective: To determine experts' degree of consensus on public and child and youth health in terms of aspects related to School Nursing, with the aim of establishing the priority contents that should be developed in the care giving function and in Health Education. Materials and methods: A two-round prospective study with a qualitative approach (the Delphi method), based on a sample of 17 experts. An online baseline questionnaire was prepared, asking for opinions and proposals on School Nursing. The second round included the response percentages and proposals subject to prioritization by means of expert assessment and consensus. Results: Most endorse some type of physical presence of nurses in schools, which would contribute to improving educational inclusion and attention to diversity. Its role in the promotion of health and prevention, early identification of diseases, and attention to mental health is pointed out. Conclusions: The school environment should be taken into account as a new and differentiated operating environment for Nursing. Nursing staff becomes a necessary innovation agent in school health, looking after the entire educational community (students, teachers and family)

OBJECTIVE: To understand adolescents' perceptions on school health. METHOD: Qualitative and descriptive research grounded on Maurice Merleau-Ponty's phenomenology, which was developed with 90 adolescent students from a federal school of the state of Rio de Janeiro. Data were produced by gathering answers to the following question: what is your perception on school health? Those who chose to write their answer to the guiding question deposited the manuscripts in polls. RESULTS: School health is linked to hygienist practices and to the hegemonic assistentialist model. Nevertheless, we assigned senses and meanings to the practice of physical activity and health education by integrating and expanding behavioral strategies and healthy habits. FINAL CONSIDERATIONS: a healthy school environment implies the protagonism of adolescents in school health promotion actions.

Método: Investigación cualitativa, descriptiva, basada en la fenomenología de Maurice Merleau-Ponty, desarrollada con 90 alumnos adolescentes de una escuela federal del estado de Río de Janeiro. Se recogieron los datos por medio del discurso hablado en respuesta al cuestionamiento: ¿cuál es su percepción sobre la salud en la escuela? Aquellos que optaron por escribir la respuesta a esta cuestión principal la depositaron en urnas. Resultados: La salud en la escuela se encuentra vinculada a las prácticas higienistas y al modelo hegemónico asistencialista. Sin embargo, sentidos y significados fueron asignados a la práctica de actividad física y educación en salud -estrategias integradoras y multiplicadoras de comportamientos y hábitos saludables. Consideraciones finales: El hacer salud en la escuela pasa por el protagonismo del ser adolescente sobre su salud en la reorganización de las acciones de promoción de la salud en la escuela.

Os dados foram produzidos pelo discurso falado em resposta ao questionamento: conte para mim qual a sua percepção sobre a saúde na escola. Aqueles que optaram pela resposta escrita à questão norteadora depositaram os manuscritos nas urnas. Resultados: A saúde escolar encontra-se presa às práticas higienistas e ao modelo hegemônico assistencialista. No entanto, sentidos e significados foram atribuídos à prática da atividade física e de educação em saúde - estratégias integradoras e multiplicadoras de comportamentos e hábitos saudáveis. Considerações finais: O fazer saúde na escola passa pelo protagonismo do ser adolescente sobre sua saúde na reorganização das ações de promoção em saúde escolar.

This article builds on the historical precedents of NASN's history to develop a vision for its next 50 years. Examining the changing landscape of healthcare provides the outline of NASN's potential to advance child health.

While Parts 1 through 4 of this series focuses on the past and present, Part 5 is the first of two articles presenting the force of the future. In developing the content for Part 5, the authors posed a question to past and current NASN Officers, State Directors, and other school nursing colleagues. Areas of identified importance are grouped into the following major headings: research, education, collaboration, advocacy, image, technology, and leadership.
