RESUMEN

OBJECTIVE: To relate the levels of air pollution and hospital admissions for cardiovascular and respiratory diseases in the city of Manaus in Brazil from 2008 to 2012. METHOD: This is an ecological time-series study among children (under 5 years of age) and elderly (above 60 years of age). Data on the daily number of hospitalizations for cardiovascular and respiratory diseases, pollutants (PM2.5), temperature, and humidity were used. Poisson generalized additive models were used to estimate the association between variables. Increases in hospitalizations for cardiovascular and respiratory diseases were estimated for the interquartile range (IQR) daily mean level of each variable studied, with a confidence interval of 95%. RESULTS: Respiratory diseases and children: -0.40% (95% CI: -1.11, 0.30), 0.59% (95% CI: -0.35, 1.52), and 0.47% (95% CI: -3.28, 4.21) for PM2.5, temperature, and humidity, respectively. Respiratory diseases and elderly: 0.19% (95% CI: -0.93, 1.31), -0.10% (95% CI: -1.85, 1.65), and -6.17% (95% CI: -13.08, 0.74) for PM2.5, temperature, and humidity, respectively. Cardiovascular diseases and elderly: -0.18% (95% CI: -0.86, 0.50), -0.04% (95% CI: -1.10, 1.03), and -3.37% (95% CI: -7.59, 0.85) for PM2.5, temperature, and humidity, respectively. CONCLUSIONS: The time-series study found no significant association between PM2.5, temperature, humidity, and hospitalization, unlike the evidences provided by the present academic literature. Since there is no air quality monitoring network in Manaus and the option available in the present study was to reproduce some information obtained from remote sensing, there is a need for implementation of ground monitoring stations for health and environmental studies in the region.

RESUMEN

Human health is the main role of medical research. Scientists were always intrigued by disease prevention, their diagnostics and proper treatment. In fact, research in medicine is always directed towards the improvement of the health care and improvement of the quality of life of the target population. Nowadays, physiological research, which is the base stone for clinical research, progresses fast forward, providing new information about body functions in health and diseases. This obvious progress is associated with modern methods, such as neuronal tracing, patch-clamp methods, electrophysiology, molecular biology and many more, which supported by comprehensive information technology guarantees high quality and complex data. Our younger colleagues, young scientists, post-docs or PhD students are well-trained and qualified in utilizing these new methods.

OBIETTIVI: valutare l'andamento temporale della mortalità per patologie respiratorie nelle province pugliesi utilizzando dati omogenei per fonte e metodologia di calcolo. DISEGNO: analisi ecologica storica degli andamenti temporali di mortalità per tumori e patologie dell'apparato respiratorio nelle province pugliesi, in Puglia e nelle ripartizioni geografiche italiane dal 1933 al 2010. SETTING E PARTECIPANTI: i dati di mortalità e le popolazioni residenti sono di fonte Istat. Sono state esaminate tutte le cause di decesso, il tumore della laringe, il tumore del polmone, l'insieme dei tumori respiratori, la bronchite, la polmonite e la broncopolmonite considerate congiuntamente, e l'insieme delle patologie respiratorie. Le analisi sono disaggregate per sesso dal 1969. PRINCIPALI MISURE DI OUTCOME: rapporti standardizzati di mortalità (SMR%) in riferimento all'Italia, con intervalli di confidenza al 95%, e tassi di mortalità standardizzati col metodo diretto (TSD ) in riferimento alla popolazione standard europea. RISULTATI: dal 1933 al 2010, i TSD per tumori respiratori e per bronchiti diminuiscono in tutte le aree analizzate. Tuttavia, nelle province di Taranto, Brindisi e Lecce, l'SMR% per tumori respiratori, inferiore al riferimento nazionale fino agli anni Sessanta, si allinea (a Brindisi) e supera (a Lecce e Taranto) il riferimento negli anni successivi. Nelle province di Foggia e Bari il numero dei decessi per tumore del polmone è costantemente inferiore all'atteso. CONCLUSIONI: la ricostruzione storica e l'analisi dei trend temporali di mortalità dal 1933 al 2010 mostrano alcune criticità sanitarie in periodi specifici. L'elaborazione dei dati di mortalità per un arco temporale di circa 80 anni ha messo in evidenza la maggiore rilevanza di queste criticità con l'avvio dello sviluppo industriale.

RESUMEN

John Darwall's (1796-1833) contribution to medicine has not attracted the attention it merits from medical historians. Although the Birmingham physician died at a relatively young age, his work in several fields of early nineteenth century medicine, including paediatrics, deserves to be both recovered and addressed, if only due to the light it sheds on medical practice in England during the first decades of the nineteenth century. Given its pioneering nature, his work on occupational health has attracted some attention, albeit minor.(1) However his contribution to the development of paediatrics, as represented by his most substantial publication, Plain Instructions for the Management of Infants (1830), has been completely ignored by historians of child health. This paper seeks to redress this particular oversight.

RESUMEN

Since its initial description by Max Wilms over a century ago, nephroblastoma has benefited from considerable improvements both in terms of basic knowledge about it and management of it. Today, the majority of these very young patients can expect a long-term survival in excess of 90% at the price of a light therapy that combines surgical resection, chemotherapy based on ill-toxic agents, and in selected cases, radiotherapy of remarkably low toxicity. The contribution of large international studies will be emphasized here.

RESUMEN

The 1960s were a period of great flowering in the recognition of neurologic disorders in children. The so-called ataxic cerebral palsies were an especially fertile field waiting for clarification. Congenital ataxia coupled with hyperpnea-apnea, abnormal eye movements, and retardation was identified as an autosomal-recessive syndrome eponimically associated with the senior author, Marie Joubert. The disorder, though rare, is increasingly recognized and a lay society dedicated to family support and research has been formed. In preparation for a recent symposium the original proband was re-examined 30 years later and the manifestations in adults clarified. Severe dysarthria was the most striking feature in this man, the hyperpnea-apnea had diminished, and the abnormal eye movements were less striking. Ataxia was still present but not severe. Poor judgment and borderline intelligence rounded out the clinical picture. Modern imaging has clarified, in part, the anatomic basis of this syndrome.
