In the spring of 2020, a novel virus known as COVID-19 (coronavirus disease 2019) was introduced to the human population, and the world faced a global pandemic with far-reaching consequences. One of the most difficult challenges that nurses faced in the midst of the crisis was the lack of proper personal protective equipment (PPE). The lack of PPE left health care professionals with a complicated ethical dilemma: Is there an ethical duty to care for patients in the absence of proper PPE? This article seeks to help the individual nurse (a) understand the ethical dilemma and the tensions that it brings, (b) look to the literature for guidance, and (c) understand how individuals can apply these ethical principles. After careful analysis, the recommendation is for the individual nurse to make a thorough assessment of their personal situation. This will include consideration for their family, community, financial responsibilities, legal protections, organizational policies, and personal health risk. Finally, this article serves as a call to organizations and professional leaders to increase their support of frontline health care workers and provide the individual nurse with the information they need in order to make sound decisions.

The conduct of nurse managers, and health service managers more widely, has been subject to scrutiny and critique because of high-profile organisational failures in healthcare. This raises concerns about the practice of nursing management and the use of codes of professional and managerial conduct. Some responses to such failures seem to assume that codes of conduct will ensure or at least increase the likelihood that ethical management will be practised. Codes of conduct are general principles and rules of normative standards, including ethical standards, and guides for action of agents in particular roles. Nurse managers seem to stride two roles. Contra some accounts of the roles of a professional (nurse) and that of a manager, it is claimed that there is no intrinsic incompatibility of the roles though there is always the possibility that it could become so and likewise for codes of conduct. Codes of conduct can be used to support nurse managers in making practical decisions via an 'outside in' approach with an emphasis on the use of principles and an 'inside out' approach with an emphasis on the agent's character. It is claimed that both approaches are necessary, especially as guides to ethical action. However, neither is sufficient for action because judgement and choice will always be required (principles always underdetermine action) as will a conducive environment that positively influences good judgement by being supportive of the basic principles and values of healthcare institutions. The response to the Covid-19 pandemic has created a unique set of circumstances in which the practical judgement, including ethical judgement, of nurse managers at all levels is being tested. However, the pandemic could be a turning point because staff and institutions (temporarily) freed from managerialism have demonstrated excellent practice supportive of ethical and other practical decision making. Organisations need to learn from this post pandemic.

New nurse leaders may lack the foresight to understand ethical dilemmas encountered in their leadership practice. Two sets of ethical dilemmas faced by nurse leaders were identified as part of a leadership class: patient care and work environment, exacerbated by the COVID-19 pandemic. Professional development opportunities for new and evolving leaders should include robust opportunities to identify and resolve ethical dilemmas. [J Contin Educ Nurs. 2021;52(1):8-10.].

Objetivo apreender conceitos e fundamentos sobre direitos humanos na compreensão de profissionais de enfermagem em unidade de emergência hospitalar. Método pesquisa fenomenológica realizada com 11 profissionais de enfermagem. As entrevistas foram gravadas e a interpretação dos dados deu-se pela configuração Triádica Humanística Existencial Personalista, embasada na Declaração Universal dos Direitos Humanos. Resultados emergiram duas categorias "Profissionais de Enfermagem conceituam os direitos tomando como referência a Declaração Universal dos Direitos Humanos" e "Profissionais de Enfermagem fundamentam a compreensão sobre Direitos Humanos em princípios bioéticos e valores". Saúde, habitação, segurança, cuidados médicos e sociais foram expressos como direitos humanos. O respeito ao outro e aos Direitos Humanos foi compreendido como independente da condição de hospitalização. Conclusão as profissionais de enfermagem compreendem os Direitos Humanos com base na Declaração Universal dos Direitos Humanos. Os aspectos bioéticos e valores que emergiram como fundamentos para essa compreensão foram autonomia, dignidade, beneficência, alteridade e privacidade.

Objetivo aprehender conceptos y fundamentos sobre los derechos humanos en la comprensión de los profesionales de enfermería en una unidad de emergencias hospitalarias. Método investigación fenomenológica realizada con 11 profesionales de enfermería. Las entrevistas fueron grabadas y la interpretación de los datos fue dada por la configuración Tríadica Humanista Existencial Personalista, basada en la Declaración Universal de Derechos Humanos. Resultados surgieron dos categorías "Los profesionales de enfermería conceptualizan los derechos tomando como referencia la Declaración Universal de Derechos Humanos" y "Los profesionales de enfermería basan su comprensión de los Derechos Humanos en principios y valores bioéticos". La salud, la vivienda, la seguridad, la atención médica y social se expresaron como derechos humanos. El respeto por el otro y los derechos humanos se entendía como independiente de la condición de hospitalización. Conclusión los profesionales de enfermería entienden los derechos humanos sobre la base de la Declaración Universal de Derechos Humanos. Los aspectos bioéticos y los valores que surgieron como fundamentos para este entendimiento fueron la autonomía, la dignidad, la beneficencia, la alteridad y la privacidad.

Objective to grasp concepts and principles about human rights in the understanding of nursing professionals in a hospital emergency unit. Method phenomenological research conducted with 11 nursing professionals. The interviews were recorded and the interpretation of the data was given by the Humanistic Existential Personalist Triadic configuration, based on the Universal Declaration of Human Rights. Results two categories emerged "Nursing professionals conceptualize rights taking as reference the Universal Declaration of Human Rights" and "Nursing professionals base their understanding of Human Rights on bioethical principles and values". Health, housing, safety, medical and social care were expressed as human rights. Respect for the other and human rights was understood as independent of the condition of hospitalization. Conclusion nursing professionals understand human rights based on the Universal Declaration of Human Rights. The bioethical aspects and values that emerged as principles for this understanding were autonomy, dignity, beneficence, otherness and privacy.

ABSTRACT Objective: to identify the elements of moral sensitivity held by nurses working in a medical clinic unit. Method: this exploratory-descriptive study with a qualitative approach addressed 18 nurses from a medical clinic of a university hospital located in southern Brazil using semi-structured interviews, which were analyzed using discursive textual analysis. Results: data were structured into six categories: relational orientation; experiencing moral dilemmas; following rules; benevolent motivation; structuring moral meaning; and autonomy. These categories enabled the identification of important elements of moral sensitivity, such as acknowledging the ethical dimension of one's attitudes, acknowledging the uniqueness of each patient, dealing with conflict between workers and patients and/or their companions, adapting to the workplace, empathy, dialogue, clinical decision-making, meeting the needs of patients, understanding patients' health condition, respect, welcoming patients' desires and providing guidance that concerns patients' requests and refusals. Conclusion: the elements of moral sensitivity identified in this study contribute to support nurses when making clinical decisions, especially when facing ethical issues arising in a medical clinic setting.

RESUMEN Objetivo: identificar los elementos de sensibilidad moral presentes en las enfermeras que trabajan en una Unidad de Clínica Médica. Método: investigación cualitativa, descriptiva exploratoria, desarrollada a través de entrevistas semiestructuradas analizadas mediante análisis discursivo textual, con 18 enfermeras trabajando en la unidad de clínica médica de un hospital universitario en el sur de Brasil. Resultados: se estructuran en seis categorías: orientación relacional; experimentando conflicto moral; seguir las reglas; motivación benevolente; Estructuración del significado moral y la autonomía que permitieron identificar elementos importantes de la sensibilidad moral, como el reconocimiento de la dimensión ética de las actitudes, el reconocimiento de la singularidad de los pacientes, la forma de hacer frente a los conflictos entre el profesional y el paciente y / o compañero, la adaptación en el entorno de trabajo. , empatía, diálogo, toma de decisiones clínicas, satisfacción de las necesidades de los pacientes, comprensión de su estado de salud, respeto, bienvenida a sus deseos y orientación con respecto a sus solicitudes y rechazos. Conclusión: los elementos de sensibilidad moral identificados en este estudio contribuyen a que las enfermeras puedan tomar decisiones clínicas, especialmente frente a problemas éticos experimentados en el entorno de la clínica médica.

RESUMO Objetivo: identificar os elementos da sensibilidade moral presentes em enfermeiros atuantes em uma Unidade de Clínica Médica. Método: pesquisa qualitativa, do tipo exploratório-descritiva, desenvolvida por meio de entrevistas semiestruturadas analisadas mediante análise textual discursiva, com 18 enfermeiros atuantes na unidade de clínica médica de um hospital universitário do sul do Brasil. Resultados: se estruturam em seis categorias: orientação relacional; experimentando o conflito moral; seguir regras; motivação benevolente; estruturação do significado moral e autonomia a qual foi possível identificar importantes elementos da sensibilidade moral como reconhecimento a dimensão ética das atitudes, reconhecer a singularidade dos pacientes, a forma de enfrentamento dos conflitos entre profissional e paciente e/ou acompanhante, adaptação no ambiente de trabalho, empatia, diálogo, tomada de decisão clínica, atendimento as necessidades dos pacientes, compreensão da sua condição de saúde, respeito, acolhimento aos seus desejos e orientação quanto as suas solicitações e recusas. Conclusão: os elementos da sensibilidade moral identificados nesse estudo contribuem para habilitar os enfermeiros para a tomada de decisão clínica, principalmente diante de problemas éticos vivenciados no ambiente de clínica médica.

ABSTRACT Objective: to compare the moral values that nursing teachers and students consider important for vocational training with those they believe are promoted throughout undergraduate study. Method: a qualitative research; an ethnographic study conducted at a public nursing school in the state of Rio de Janeiro, in 2018, involving 40 interviews with teachers and students and a participant observation phase. Results: the moral values that teachers and students consider important for vocational training, as well as those that are promoted, converge on prudence, respect, responsibility, and empathy. It is noteworthy that the knowledge was much cited by the respondents and this article interpreted it as prudence. However, in relation to the values promoted during undergraduation, students warn that these are stimulated when referring to the binomial student-user of health, because they do not feel to the same intensity the presence of these moral values in the teacher-student relationship. Conclusion: a powerful strategy for teaching about moral values is to promote the care of their own students, teachers and staff. In this way, mismatches are avoided between what is said and what is done by experiencing values such as prudence, respect, responsibility and empathy within interpersonal relationships and in the daily life of the school.

RESUMEN Objetivo: comparar los valores morales que los profesores y estudiantes de enfermería consideran importantes para la formación profesional con los que creen que se promueven a lo largo de la carrera de grado. Método: investigación cualitativa; un estudio etnográfico realizado en el año 2018 en una facultad pública de enfermería del estado de Río de Janeiro, por medio de 40 entrevistas con profesores y estudiantes y una fase de observación de los participantes. Resultados: los valores morales que los profesores y estudiantes evalúan como importantes para la formación profesional, al igual como los que se promueven, convergen en la prudencia, el respeto, la responsabilidad y la empatía. Cabe destacar que el conocimiento fue muy citado por los entrevistados y, en este artículo, se lo interpretó como prudencia. Pese a ello, en relación con los valores promovidos a lo largo de la carrera de grado, los estudiantes manifiestan que se los fomenta cuando se refieren al binomio estudiante-usuario de servicios de salud, puesto que no sienten con la misma intensidad la presencia de estos valores en la relación profesor-estudiante. Conclusión: estamos frente a una poderosa estrategia para enseñar sobre los valores morales y promover el cuidado de sus propios estudiantes, profesores y personal administrativo. De esta manera, se evitan desencuentros entre lo que se dice y lo que se hace al respetar valores como la prudencia, el respeto, la responsabilidad y la empatía dentro de las relaciones interpersonales y en la vida cotidiana de la facultad.

RESUMO Objetivo: comparar os valores morais que professores e estudantes de enfermagem consideram importantes para a formação profissional com aqueles que eles acreditam que são promovidos ao longo da graduação. Método: pesquisa qualitativa; um estudo etnográfico realizado em uma faculdade pública de enfermagem do estado do Rio de Janeiro, em 2018, envolvendo 40 entrevistas com professores e estudantes e uma fase de observação participante. Resultados: os valores morais que os professores e estudantes avaliam ser importantes para a formação profissional, bem como aqueles que são promovidos, convergem para a prudência, o respeito, a responsabilidade e a empatia. Vale destacar que o conhecimento foi muito citado pelos entrevistados e, este artigo, interpretou-o como prudência. Todavia, em relação aos valores promovidos ao longo da graduação, os estudantes alertam que estes são estimulados quando referem-se ao binômio estudante-usuário de saúde, pois não sentem na mesma intensidade a presença desses valores morais na relação professor-estudante. Conclusão: uma potente estratégia para ensinar sobre os valores morais é promover o cuidado dos seus próprios estudantes, professores e funcionários. Dessa maneira, evitam-se desencontros entre o que se diz e o que se faz ao vivenciar os valores como a prudência, o respeito, a responsabilidade e a empatia dentro das relações interpessoais e no cotidiano da faculdade.

Com a recente atualização do Código de Ética dos Profissionais de Enfermagem, essa tornou-se conhecida apenas por parte dos profissionais. Objetivo: avaliar o conhecimento dos enfermeiros sobre o Código de Ética dos Profissionais de Enfermagem. Método: Trata-se de um estudo observacional, descritivo e transversal, com dados coletados no período de setembro a outubro de 2018, por meio de um questionário online, via Plataforma Google Forms®. Participaram da pesquisa 217 enfermeiros, sendo 190 (87,46%) do sexo feminino e 27 (12,44%) do sexo masculino. Resultado: Em relação aos direitos, os participantes obtiveram respostas semelhantes, considerando a alternativa mais coerente. Quanto aos deveres e proibições, apresentaram grande diversidade e discordância entre as alternativas escolhidas, demonstrando dúvida ou equívoco por parte dos participantes. Conclusão: Foi possível observar que o conhecimento dos enfermeiros acerca do Código de Ética Profissional de Enfermagem é limitado em relação aos deveres e proibições da profissão.(AU)

With the recent update of the Nursing Professionals Code of Ethics, this became known only by professionals. Objective: to evaluate nurses' knowledge of the Nursing Professionals Code of Ethics. Method: This is an observational, descriptive and cross-sectional study, with data collected from September to October 2018, through an online questionnaire, via the Google Forms® Platform. 217 nurses participated in the research, 190 (87.46%) female and 27 (12.44%) male. Result: Regarding rights, the participants obtained similar responses, considering the most coherent alternative. As for duties and prohibitions, they showed great diversity and disagreement between the alternatives chosen, showing doubt or misunderstanding on the part of the participants. Conclusion: It was possible to observe that the nurses' knowledge about the Nursing Professional Code of Ethics is limited in relation to the duties and prohibitions of the profession.(AU)

Con la reciente actualización del Código de Ética de los Profesionales de Enfermería, esto solo se dio a conocer por los profesionales. Objetivo: evaluar el conocimiento de los enfermeros sobre el Código de Ética para Profesionales de Enfermería. Método: Se trata de un estudio observacional, descriptivo y transversal, con datos recolectados de septiembre a octubre de 2018, a través de un cuestionario en línea, vía Google Forms® Platform. En la investigación participaron 217 enfermeras, 190 (87,46%) mujeres y 27 (12,44%) hombres. Resultado: En cuanto a derechos, los participantes obtuvieron respuestas similares, considerando la alternativa más coherente. En cuanto a deberes y prohibiciones, mostraron gran diversidad y desacuerdo entre las alternativas elegidas, mostrando dudas o incomprensiones por parte de los participantes. Conclusión: se pudo observar que el conocimiento de las enfermeras sobre el Código de Ética Profesional de Enfermería es limitado en relación a los deberes y prohibiciones de la profesión.(AU)

Objective. To determine the relationship between ethical climate and burnout in nurses working in Intensive Care Units (ICUs). Methods. This cross-sectional and multi-center study was conducted among 212 nurses working in adult ICUs of six hospitals affiliated to Shiraz University of Medical Sciences, Iran in 2019. The participants were selected using systematic random sampling technique. Data was collected using valid instruments of Olson's Hospital Ethical Climate Survey (HECS) and Maslach Burnout Inventory (MBI). Results. Ethical climate was favorable (3.5±0.6). The intensity (32.2±12.4) and frequency (25.5±12.4) of burnout were high. Ethical climate had significant and inverse relationships with frequency of burnout (r =-0.23, p=0.001) and with intensity of burnout (r=-0.186, p=0.007). Ethical climate explained 5.9% of burnout. Statistically significant relationships were also found between these factors: age with ethical climate (p=0.001), work shifts with burnout (p=0.02), and gender and with intensity frequency of burnout in ICU nurses (p=0.038). The results of Spearman correlation coefficient showed significant and inverse relationships between ethical climate and job burnout (r=-0.243, p<0.001). Conclusion. Nurses in ICUs perceived that ethical climate was favorable however, burnout was high. Therefore, burnout can be affected by many factors and it is necessary to support ICU nurses since they undertake difficult and complicated task. It is recommended to assess factors that increase burnout and adopt specific measures and approaches to relieve nursing burnout.

Objetivo. Determinar la percepción de las enfermeras sobre el clima ético y el agotamiento laboral en las Unidades de Cuidados Intensivos (UCI). Métodos. Este estudio de corte transversal y multicéntrico se realizó con 212 enfermeros que trabajaban en UCI para adultos de seis hospitales afiliados a la Universidad de Ciencias Médicas de Shiraz, Irán, en 2019. Los participantes se seleccionaron mediante una técnica de muestreo aleatorio sistemático. Los datos se recolectaron a partir de la utilización de instrumentos Encuesta de Clima Ético en Hospitales (HECS) de Olson y el Inventario de Burnout de Maslach (MBI). Resultados. El clima ético fue favorable (3.5±0.6). La intensidad (32.2±12,4) y la frecuencia (25.5±12.4) del agotamiento fueron altas. El clima ético tuvo relaciones significativas e inversas con la frecuencia de Burnout (r=-0.23, p=0.001) y con la intensidad del burnout (r=-0.186, p=0.007). El clima ético explica el 5.9% del agotamiento. También se encontraron relaciones estadísticamente significativas entre edad y clima ético (p=0.001), turnos de trabajo y burnout (p=0.02), además de género y frecuencia de intensidad de burnout (p=0.038). El coeficiente de correlación de Spearman mostró relaciones significativas e inversas entre el clima ético y el agotamiento laboral (r=-0.243, p<0.001). Conclusión. Las enfermeras en las UCI percibieron que el clima ético era favorable, sin embargo, el desgaste fue alto. Por tanto, el burnout puede verse influido por muchos factores y es necesario apoyar a estas enfermeras que cumplen una tarea difícil y complicada. Se recomienda evaluar los factores que aumentan el agotamiento y adoptar medidas y enfoques específicos para aliviar el burnout de enfermería.

Este estudo transversal e multicêntrico foi realizado com 212 enfermeiras que trabalham na UTI adulto de seis hospitais afiliados à Universidade de Ciências Médicas de Shiraz, Irão, em 2019. Os participantes foram selecionados por meio de uma técnica de amostragem aleatória sistemática. Os dados foram coletados usando os instrumentos Enquete de Clima Ético em Hospitais (HECS) de Olson e o Inventário Burnout de Maslach (MBI). Resultados. O clima ético era favorável (3.5±0.6). A intensidade (32.2±12.4) e a frequência (25.5±12.4) de exaustão foram altas. O clima ético teve relações significativas e inversas com a frequência de Burnout (r=-0.23, p=0.001) e com a intensidade de Burnout (r=-0.186, p=03007). O clima ético explica 5,9% do esgotamento. Também foram encontradas relações estatisticamente significativas entre: idade e clima ético (p=0.001), turnos de trabalho e Burnout (p=0,02), além de gênero e frequência da intensidade do Burnout (p=0.038). O coeficiente de correlação de Spearman mostrou relações significativas e inversas entre clima ético e Burnout (r=-0243, p<0.001). Conclusão. As enfermeiras das UTIs perceberam que o clima ético era favorável, porém, o desgaste foi alto. Portanto, o Burnout pode ser influenciado por diversos fatores e é necessário apoiar essas enfermeiras que cumprem uma tarefa difícil e complicada. Recomenda-se avaliar os fatores que aumentam o Burnout e adotar medidas e abordagens específicas para aliviar o Burnout da enfermagem.

INTRODUCCIÓN: Durante el curso 2017/2018 se realizó una intervención educativa sobre buenas prácticas éticas en el abordaje del sida y de la discapacidad. Los objetivos fueron: analizar, divulgar y concienciar a los estudiantes sobre los problemas éticos de estos colectivos y sobre las buenas prácticas. MÉTODOS: Participaron 185 estudiantes matriculados en la asignatura de Ética y Gestión de Enfermería. Los objetivos se abordaron mediante el trabajo autónomo de búsquedas de información y en 2seminarios presenciales, donde elaboraron pósteres, folletos y vídeos. Al finalizar la experiencia rellenaron un cuestionario de opinión. RESULTADOS: Se agruparon en 39 equipos de 4-5 estudiantes y entregaron 78 trabajos: 52 pósteres, 20 folletos y 6 vídeos. Los trabajos fueron expuestos, durante una semana, coincidiendo con el Día Mundial de la Lucha contra el Sida y con el Día Internacional de las Personas con Discapacidad. CONCLUSIONES: Esta experiencia ha servido para que los estudiantes tomen conciencia de los problemas de estos colectivos vulnerables, además de conseguir la divulgación de buenas prácticas éticas

INTRODUCTION: During the academic year 2017/2018, an educational intervention on good ethical practices was carried out about AIDS and Disability. The objectives were, to analyse, divulge and sensitise students about the ethical problems of these patient groups and about good practices. METHODS: The study included a total of 185 students enrolled in the subject of Ethics and Management of Nursing. The objectives were addressed using independent information searches and attending 2seminars, where they produced posters, brochures and videos. At the end of the experience they produced an opinion questionnaire. RESULTS: They were grouped into 39 teams of 4-5 students, and delivered 78 works, 52 of which were posters, 20 brochures, and 6 videos. The works were presented for a week, coinciding with the World AIDS Day and with the International Day of Persons with Disabilities. CONCLUSIONS: This experience has served to make students aware of the problems of these vulnerable groups, and also for the divulgation of good ethical practice

OBJECTIVE: to identify the main ethical problems and how these relate to the moral sensitivity of nurses working in a general medicine ward. METHOD: this qualitative, exploratory, and descriptive study was conducted in a university hospital located in the south of Brazil. A total of 18 nurses working in a general medicine ward were interviewed. A semi-structured interview script was used, and data were analyzed using discursive textual analysis. RESULTS: nurses considered the main ethical problems to include conflicts at the institutional level, situations involving conflicts with patients and/or family members, and conflicts within the staff. The perception of nurses and how they deal with these problems relate to moral sensitivity. Two categories emerged: experiencing ethical problems, and relationship with moral sensitivity. CONCLUSION: because of the multidimensional nature of moral sensitivity, it trains and enables nurses to recognize and deal with ethical problems faced in clinical practice so that nurses become able to make fair and prudent decisions, improving the quality of nursing care.

OBJECTIVE: to identify the association between moral distress and the supporting elements of moral deliberation in Brazilian nurses. METHOD: a cross-sectional study conducted with Brazilian nurses working in health services at different complexity levels. The research protocol consisted of the Brazilian Scale of Moral Distress in Nurses, a sociodemographic and labor questionnaire, and a list of bases and ethical elements used for moral deliberation. For analysis, descriptive statistics, chi-square test, and Poisson regression were used. RESULTS: 1,226 nurses took part in the study. The 12 elements associated with the moral deliberation process were classified as important for nurses' actions, especially the professional experience acquired, code of ethics/law of professional practice, and ethical and bioethical principles. The relationship of moral distress showed higher prevalence in the Beliefs, culture and values of the patient, Beliefs and personal values, and Intuition and Subjectivity elements. CONCLUSION: the results showed a balance between the subjective criteria of professional experience and the objective ones of deontology for moral deliberation.

Globally nurses and midwives are working hard to detect cases of COVID-19, to save lives or give comfort in the face of death, to educate themselves and the public about protective measures to stop the viral spread, while still caring for those not infected with the virus. In many countries nurses are working under virtual siege from this pandemic, with not enough resources or personal protective equipment, overwhelming numbers of patients, staff shortages, underprepared health systems and supply chain failures. Nurses and other health and emergency workers are suffering physical and emotional stress, and moral distress from conflicting professional values. They are faced with unpalatable and complex ethical issues in practice, with moral conflicts, high levels of acuity and patient deaths, and long working hours. A rising number of nurses are infected with SARS-CoV-2 or dying in the line of duty. Nurses need strong moral courage, stamina and resilience to work on the front lines of the pandemic, often while separated from their loved ones.
