RESUMEN

As systems evolve over time, their natural tendency is to become increasingly more complex. Studies in the field of complex systems have generated new perspectives on the application of management strategies in health systems. Much of this research appears as a natural extension of the cross-disciplinary field of systems theory. Since writing my 1st article for Managing Organizational Complexity in 2004, much has happened to further our understanding of complexity in healthcare systems. The growth of new computational methods in the fields of data science and data analytics has allowed scientists to identify signals or patterns in large complex data sets (big data) that in the past were seemingly hidden. Rather than relying on historical statistical methods to infer outcomes, these advanced methods combined with increased computer processing power allow machines to learn the structure of data and create artificial intelligence (AI). In our ongoing efforts to find solutions for complex healthcare problems, AI is becoming more and more an accepted method. The purpose of this edition of Managing Organizational Complexity is to define AI and machine learning, discuss the recent resurgence of AI, and then provide examples of how AI can provide value to healthcare with an emphasis on nursing.

BACKGROUND: Despite the role caregivers play in the delivery of care, the interactions and training methods used with caregivers during an inpatient stay are not clear. PURPOSE: The purpose was to examine interactions and training methods used with caregivers during hospital care. METHODS: A mixed-methods case study was conducted. Observations were summarized and interviews were analyzed using thematic analysis. RESULTS: The frequency of caregiver engagement varied at different points in the care process but was highest among observations during the stay care point. Providers were most commonly using written and verbal instructions to train caregivers. Three themes emerged from the interviews and were described to be both facilitators and barriers to caregiver involvement: experience, time, and relationship. CONCLUSIONS: High-quality person and family-centered care depends upon coordinated efforts among health care systems, providers, patients, and caregivers. Future caregiver initiatives should aim to decrease disengagement, increase assessment, and broaden the use of training methods.

Introdução: A pandemia do Corona vírus (COVID-19) provocou em caráter emergencial a necessidade dos gestores das faculdades e universidades de todo mundo de reinventar novas formas de prover o ensino preservando a qualidade dele. Com as novas portarias do Ministério da Educação e Saúde, houve uma abertura para que todos os cursos utilizassem de metodologias remotas para continuidade do ano letivo, surgindo novos desafios e paradigmas com essa proposta metodológica: prover para o usuário, a sensação de imersão, de estar dentro do ambiente, a partir da navegação e interação nesse meio virtual, ao mesmo tempo que o educador, respeitando os princípios educacionais e a abordagem pedagógica que acredita, não transformar esse momento em uma simples educação à distância. Objetivo: Descrever o estado da arte sobre o ensino de enfermagem e os desafios do uso de tecnologias remotas em época de pandemia do Corona vírus. Método: Trata-se de estudo reflexivo consubstanciado por fontes secundárias da literatura pertinente à temática, considerando artigos de periódicos nacionais e internacionais e produções recentes sobre educação, formação em saúde, tecnologias remotas, o COVId-19 e saúde pública. Resultados:Evidencia-se que vivenciar os efeitos da pandemia de corona vírus (COVID-19) no setor educacional na formação em saúde, em especial no campo da enfermagem, vai além de uma reorganização estrutural dos cursos, pois implica em mudança atitudinal dos gestores, docentes e discentes para que reformulem as práticas de ensino (por vezes com ferramentas tradicionais), em práticas inovadoras preservando um ensino que propicie ao estudante a criticidade, reflexão, diálogo, vínculo e interação; elementos que fazem parte de uma formação que visa a transformação, o empoderamento e não apenas a transmissão do conhecimento. Nesse contexto, a pandemia do COVID-19 provocou mudanças de paradigmas talvez ainda não superados pelas instituições na área da saúde, pois ao se perceberem dentro de uma realidade em que gerou mudanças nos aspectos políticos, econômicos, culturais e sociais em nível mundial, estas tiveram que se reinventar e inserir no seu processo de trabalho as novas formas de ensinar; tiveram que discutir sobre as diferentes abordagens educacionais e diante das necessidades do readequar os métodos de ensino em saúde, inseriram as tecnologias remotas como ferramentas essenciais para atender a real necessidade da continuidade das aulas no formato não presencial. Para muitos um desafio, pois permeia atualmente uma reflexão sobre o cuidado do ensino à distância no campo da enfermagem e demais cursos da área da saúde. Entretanto, à medida que se abriu para discussões sobre novas formas de ensinar mediadas pela inovação, pode-se dizer que esse será o maior impacto da pandemia para o ensino: a contribuição das novas tecnologias de informação e comunicação no processo ensino aprendizagem para formação em saúde, assim como a reflexão sobre a educação à distância e seus conceitos, diferenciando-a dos conceitos de metodologia remota e o uso das tecnologias. Conclusão: No ensino da enfermagem, a discussão relacionada ao uso de tecnologias remotas em sala de aula sempre foi um ponto de debate. Entretanto, com a necessidade da inclusão dessas ferramentas para a continuidade de aulas no formato não presencial decorrente da estratégia do isolamento social motivada pela pandemia do COVID-19, pode oportunizar a se ter um novo olhar sobre o assunto e que talvez com esse momento vivido se haja oportunidade de ampliar o debate sobre uso dessas metodologias remotas no ensino em saúde, buscando uma reflexão sobre a interação destas com os demais métodos de ensino já implementados.(AU)


ntrooduction: The Corona virus (COVID-19) pandemic caused, among others, the need for colleges and universities managers around the world to reinvent new ways of providing education preserving its good quality at the same time. With the new ordinances of the Ministry of Education and Health, all courses can use remote methodologies for the continuity on the current school year. With new challenges and paradigms emerging from this methodological proposal: provide for the user the feeling of immersion, of being in the class room, from the navigation and interaction in this virtual environment, at the same time that the educator, respecting the educational principles and the pedagogical approach that he believes, does not transform this moment into a simple distance education. Objective: To describe the state of the art on nursing education and the challenges of using remote technologies in the time of Corona virus pandemic. Methods:This is a reflective study based on secondary sources of literature relevant to the theme, considering articles from national and international journals and recent productions on education, health training, remote technologies, COVId-19 and public health. Results:It is evident that experiencing the effects of the corona virus pandemic (COVID-19) in the health educational sector, especially in the field of nursing, goes beyond a structural reorganization of courses. It implies change attitude of managers, teachers and students to reformulate educational practices (sometimes with traditional tools), with innovative practices preserving a methodology that provides to the student criticality reflection, dialogue, bonding and interaction; elements that are part of a training aimed at transformation, empowerment and not just the transmission of knowledge. In this context, the COVID-19 pandemic caused paradigm shifts perhaps not yet overcome by health science institutions , because when they perceived themselves within a reality that generated changes in the political, economic, cultural and social aspects at a global level, they had to reinvent and insert new ways of teaching in their work process; they had to discuss different educational approaches and, given the needs to readjust health teaching methods, they inserted remote technologies as essential tools to meet the real need for continuity of classes in non-face-to-face model. For many, it is a challenge, as it currently permeates a reflection on the attention of distance learning in the field of nursing and other courses in the health area. However, as it opened up to discussions about new ways of teaching mediated by innovation, it can be said that this will be one of the greatest impact of the pandemic in benefit the education: the contribution of new information and communication technologies in the teaching-learning process for training in health, as well as the reflection on distance education and its concepts, differentiating it from the concepts of remote methodology and the use of technologies. Conclusion: In nursing education, the discussion related to the use of remote technologies in the classroom has always been a point of debate. However, with the need to include these tools for the continuity of classes in the non-face-to-face model resulting from the social isolation strategy motivated by the pandemic of COVID-19, it can provide an opportunity to have a new look on the subject and perhaps there is an opportunity to expand the debate on the use of remote methodologies in health education, seeking a reflection on their interaction with the other teaching methods already implemented.(AU)

RESUMEN

At this crucial stage in nursing's development, key organizations around the world are voicing the need for nurses to become more involved in leadership, advocacy and policymaking. The importance of leadership and health policy training cannot be overemphasized. However, the widespread education and training of nurses about policy is yet to be realized. Moving nurses from being the recipients and implementers of health policy decisions to being leaders with a strong voice in the development or reform of policy will take concerted, strategic effort into the future. We argue that for nurses around the world to take their place at decision-making tables and to be rightfully engaged in policy, health reform and advocacy, nurse leaders need to provide them with access to well-thought-out policy training programmes. This access needs to be wide-ranging, from exposure to policy knowledge in undergraduate education to more specialized graduate programs focused in every specialization on some aspects of policy, through to a variety of continuing educational opportunities.

OBJECTIVE: to validate the terms of the specialized nursing language used in care for hospitalized patients with Pressure Injury, identified in nurses' records, mapping them with the terms of the International Classification for Nursing Practice (ICNP® 2017). METHODS: methodological study, carried out at a School Hospital in 2018. It was performed: extraction of terms of medical records; normalization; cross-mapping between extracted terms and those in ICNP®; distribution in the seven axes; theoretical definition and validation of terms. RESULTS: 27,756 terms were extracted. The normalization resulted in 370 relevant terms, being: 225 listed and 145 not listed in the ICNP®, being 60 similar, 13 more comprehensive, 38 more restricted and 34 without agreement, all of which are validated. CONCLUSION: this study identified and validated terms used by nurses to assist people with Pressure Injury. These may contribute to the unification of professional nursing language in care for these clients.

OBJECTIVES: To identify useful terms for nursing practice in the care of people with leprosy and to cross-mapp the identified terms with the ICNP® terms. METHOD: A descriptive, documentary study that included publications of the Ministry of Health in the area of leprosy launched between 2002 and 2017. The terms identified in these publications were extracted and underwent a normalization proces and then, were cross-mapped with terms of the ICNP® version 2015 for the identification of constant and non-constant terms in this terminology. RESULTS: In total, were extracted 1,177 terms, of which 26.76% were equal, 4.59% similar, 4.26% were broader, 19.62% were more restricted, and 44.77% were totally different from the ICNP® terms. CONCLUSIONS: The large number of terms identified and not included in the ICNP® show the need for its expansion in order to contemplate the phenomena of clinical practice more effectively and allow better representation of nursing care for people with leprosy.

Objetivo: evaluar la calidad de la asistencia en una Unidad de Cuidados Intensivos maternos, ha tenido por base indicadores de estructura proceso y resultados. Material y método: estudio descriptivo, de análisis documental con abordaje cuantitativo. La colecta de datos ocurrió entre enero y febrero de 2017 en la Unidad de Cuidados Intensivos maternos de una maternidad de referencia en Piauí. Fue utilizado un guion observacional con muestra censal de 72 pacientes. Resultados: el eje higiene, confort y seguridad mostró una asistencia global deseable, excepto para los indicadores de nutrición e hidratación que presentaron asistencia tolerable o limíte. El eje que presentó mejores resultados fue las anotaciones de enfermería con asistencia deseable en gran parte de los artículos analizados. Conclusión: el estudio evidenció resultados satisfactorios de forma general, sin embargo, es importante que ocurran mejoras en la asistencia de enfermería a las pacientes ingresadas en la unidad de cuidados intensivos maternosl, sobre todo para los indicadores de nutrición e hidratación


Objetivo: avaliar a qualidade da assistência em uma Unidade de Terapia Intensiva Materna, tendo por base indicadores de estrutura, processo e resultados. Material e método: estudo descritivo, de análise documental com abordagem quantitativa. A coleta de dados ocorreu entre os meses de janeiro e fevereiro de 2017 na Unidade de Terapia Intensiva materna de uma maternidade de referência do Piauí. Utilizou-se roteiro observacional com amostra censitária de 72 pacientes. Resultados: o eixo higiene, conforto e segurança demonstraram uma assistência global desejável, exceto para os indicadores nutrição e hidratação que apresentaram assistência sofrível ou limítrofe. O eixo que apresentou melhores resultados foi anotações de enfermagem com assistência desejável em grande parte dos itens analisados. Conclusão: o estudo evidenciou resultados satisfatórios de forma geral, entretanto, chama-se atenção para melhoras na assistência de enfermagem às pacientes internadas na Unidade de Terapia Intensiva Materna, sobretudo para os indicadores nutrição e hidratação


Objective: to evaluate the quality of care provided in a maternal intensive care unit, based on indicators of structure, process and results. Material and methods: descriptive study of documentary analysis with a quantitative approach. Data collection happened between January and February 2017 at the Maternal Intensive Care Unit of a reference maternity hospital in Piauí. An observational script with a census sample of 72 patients was used. Results: the hygiene, comfort and safety axis showed adequate care. However, the nutrition and hydration indicators presented with risky and undesirable care. The axis that presented the best results was nursing records with adequate care in most of the analyzed items. Conclusion: the study showed satisfactory results in general; however, attention should be given to improvements in nursing care for patients hospitalized in the Maternal Intensive Care Unit, especially for the nutrition and hydration indicators

INTRODUÇÃO: Trata-se de um estudo sobre a sistematização da assistência de enfermagem (SAE) com a operacionalização do processo de enfermagem (PE) no desenvolvimento da primeira fase deste processo, avaliação inicial de enfermagem. OBJETIVO GERAL: Implementar Tecnologia Educacional visando aumentar a adesão dos Enfermeiros à SAE/PE, quanto ao preenchimento do Instrumento de Avaliação Inicial, no módulo informatizado. OBJETIVOS ESPECÍFICOS: Descrever o nível de conhecimento técnico-científico e de valor atribuído pelos Enfermeiros sobre SAE/PE/Avaliação Inicial do paciente oncológico; desenvolver oficinas de sensibilização e de capacitação para o registro da avaliação inicial, no módulo informatizado e monitorar a adesão dos enfermeiros ao preenchimento desse instrumento, após a capacitação. MÉTODO: Estudo comparativo, antes e depois, tipo observacional, descritivo e transversal, com abordagem quantitativa e para o desenvolvimento dos produtos, o processo de análise administrativo de Cury, desenvolvido em três fases. Realizado no INCA/HC I, em uma amostra de 54 enfermeiros. A coleta de dados se deu por meio de questionário estruturado e por dados coletados no Sistema Absolute e na Intranet do INCA. Os dados foram organizados e armazenados num banco de dados construído numa planilha Microsoft Excel® 2007 e transportados para o software R® versão 3.4.4, para análise. A análise foi processada por estatística descritiva, utilizando-se de tabelas e figuras. RESULTADOS: Quanto ao questionário, os participantes da pesquisa eram predominantemente do sexo feminino, tinham em média 39 anos de idade, mais de 10 anos de formados; 98% dos respondentes possuíam curso de Pós-Graduação e tinham mais de 6 anos de INCA. A aplicação do questionário, evidenciou áreas de desconhecimentos sobre SAE/PE/ Avaliação Inicial; a maioria (83.34%) assinalou que existem dificuldades para a implantação da SAE/PE na unidade em que trabalham, 93% acreditam que a SAE/PE trazem benefícios para o paciente, para o enfermeiro e para Instituição. A maioria assinalou que conhecimento (96.30%), capacitação (96.30%), quantitativo adequado de RH (94,11%), educação permanente (81.48%), entre outros, facilita a aplicação da SAE/PE. Observou-se que a SAE/PE não está implementada no local da pesquisa. Quanto ao estudo comparativo, houve aumento dos registros em todos os locais do estudo, após a capacitação. CONSIDERAÇÕES FINAIS: Os dados coletados do questionário e analisados possibilitaram uma visão mais ampla do processo de trabalho realizado pelos enfermeiros participantes da pesquisa. Os dados obtidos do estudo comparativo, antes e após a Oficina de Sensibilização e Capacitação, evidenciaram aumento da adesão dos enfermeiros a esse processo de trabalho. A meta foi atingida, nos Serviços de Pronto-atendimento Adulto (70.80%) e Pediátrico (70%), e no Serviço de Oncologia Pediátrica (83%). Urge a necessidade de continuidade dessa Oficina, de modo a contemplar todo o efetivo de enfermeiros do HC I, fomentando e fortalecendo a discussão sobre essa temática, ampliando o nível de conhecimento, e de competência nesse quesito. Neste sentido, o convênio CAPES/COFEN/UNIRIO reforça o desenvolvimento de pesquisa na Sistematização da Assistência de Enfermagem a partir de melhorias nos processos de trabalho e consequentemente ocasiona impacto direto na prática profissional, na qualidade dos cuidados e segurança do paciente


INTRODUCTION: This is a study about the systematization of nursing care (SAE) with the operationalization of the nursing process (PE) in the development of the first phase of this process, initial nursing assessment. GENERAL OBJECTIVE: To implement Educational Technology to increase the adhesion of the Nurses to the SAE / PE, regarding the completion of the Initial Evaluation Instrument, in the computerized module. SPECIFIC OBJECTIVES: To describe the level of technical-scientific knowledge and value attributed by the Nurses on SAE / PE / Initial Evaluation of the cancer patient; develop awareness and training workshops to register the initial evaluation in the computerized module and monitor the adhesion of nurses to the completion of this instrument after the training. METHODS: A comparative, before and after, observational, descriptive and cross - sectional study, with a quantitative approach and for product development, the Cury administrative analysis process, developed in three phases. Performed in INCA / HC I, in a sample of 54 nurses. Data collection was done through a structured questionnaire and data collected in the Absolute System and INCA Intranet. The data were organized and stored in a database built on a Microsoft Excel® 2007 worksheet and transported to R® software version 3.4.4 for analysis. The analysis was processed by descriptive statistics, using tables and figures. RESULTS: As to the questionnaire, the participants were predominantly female, averaging 39 years of age, over 10 years of age; 98% of the respondents had a postgraduate course and had more than 6 years of INCA. The application of the questionnaire revealed areas of lack of knowledge about SAE / PE / Initial Evaluation; the majority (83.34%) pointed out that there are difficulties for the implantation of SAE / PE in the unit in which they work, 93% believe that the SAE / PE brings benefits to the patient, the nurse and to the Institution. The majority reported that knowledge (96.30%), training (96.30%), adequate quantitative HR (94.11%), permanent education (81.48%), among others, facilitates the application of SAE / PE. It was observed that the SAE / PE is not implemented at the research site. As for the comparative study, there was an increase in the records in all the study sites, after the training. FINAL CONSIDERATIONS: The data collected from the questionnaire and analyzed, allowed a broader view of the work process performed by the nurses participating in the research. The data obtained from the comparative study, before and after the Workshop on Sensitization and Training, showed an increase in nurses' adherence to this work process. The goal was achieved in Adult (70.80%) and Pediatric (70%) and in the Pediatric Oncology Service (83%). It is urgent that this Workshop be continued, so as to include all the HC I nurses, encouraging and strengthening the discussion on this topic, increasing the level of knowledge and competence in this area. In this sense, the CAPES / COFEN / UNIRIO agreement reinforces the development of research in Nursing Care Systematization based on improvements in work processes and consequently has a direct impact on professional practice, quality of patient care and safety

OBJECTIVE: To develop a journal club on management topics in Nursing. METHOD: Action research, adopting content analysis as methodological reference and the journal club strategy. 12 nursing managers of a public hospital in the state of São Paulo participated from August to November 2016. RESULTS: data showed the participation of nurses in the journal club in five meetings planned collectively with the following topics: "aspects regarding organization", with the subtopics "planning", "work process", "structure" and "people management"; and the topic "aspects regarding the team", with the subtopics "autonomy", "behavior/attitude", "awareness" and "social problems". CONCLUSION: The study showed the comprehensiveness of topics the journal club can address regarding management, and how scientific basis is important in the daily life of nursing managers to improve quality of care and the ability to manage.

RESUMEN

OBJECTIVE: To describe and analyze the national and international scientific production on Nursing care for the transgender or gender-variance population. METHOD: Integrative review of the literature, conducted throughout  the Virtual Health Library, Cumulative Index to Nursing and Allied Health Literature, Public Medline and Web of Science databases, without pre-established periods of time and using the descriptors "Transgender AND 'Nursing Assistance'" and "Transgender AND 'Nursing care'". RESULTS: We included 11 articles, published between 2005 and 2016, broadly North American with only one Brazilian, so categorized: I- Fragility in the care of transgender people; II - Health of the transgender population: general and specific demands; III- Public health policies for transgender people. Transgender people have not found yet answers to their health demands; they are victims of prejudices and violence in services and seek care in extreme cases of sickness. FINAL CONSIDERATIONS: Understanding their needs is primordial to build knowledge and practices that support nursing care.

RESUMEN

OBJECTIVE: to reflect on the everyday life in nursing work based on Michel de Certeau's theorization. RESULTS: everyday life in nursing work is permeated by tactics of subjects who practice the place. Their movements escape standards, protocols and rules, re-signifying the cultural system defined beforehand. There is a practice proper to professionals who (re)invent the care based on their intentions and pressures. Patients/users also move, create their own itinerary and, similarly to professionals, use strategies to achieve specific results in the therapeutic process. CONCLUSION: to think on the everyday life in nursing work as an object of research requires to consider the care (re)invention in every act in health care. We need to dive into the invisible dimension of the uncontrollable tactics of subjects that re-signify the social system.
