RESUMEN

Los sistemas sanitarios tienen la responsabilidad de dar respuesta a las nuevas necesidades de salud de la población, que se caracterizan por factores como el envejecimiento, la cronicidad y/o las situaciones de dependencia y que requieren de cuidados de calidad, especializados y adaptados a diferentes ámbitos donde la atención se presta, cuidados ofertados por profesionales formados y cada vez más cualificados para mejorar los resultados en salud de las personas que cuidan. En el año 2016, en Andalucía se publica el marco normativo por el que se crea la categoría profesional estatutaria de enfermero/a especialista, entre las que se incluye la especialidad de Enfermería de Salud Mental en el Servicio Andaluz de Salud. En el ámbito de la Salud Mental, el desarrollo de este marco normativo y la definición y ocupación de puestos permitirá al sistema sanitario conjugar el papel de las enfermeras especialistas con el de las enfermeras que prestan cuidados generales, enfermeras de Grado, en pro de avanzar en la mejor respuesta a las necesidades de salud de la ciudadanía en este ámbito de atención. El desarrollo de la especialidad podrá suponer un valor añadido tanto para la mejora de los resultados en salud de las personas con problemas de salud mental como para mejorar la calidad asistencial, la eficiencia y la sostenibilidad de los sistemas sanitarios


Health systems have a responsibility to respond to the new health needs of the population, which are characterized by factors such as aging, chronicity and/or dependency situations and which requires quality and specialized care adapted to different areas where care is provided, care offered by trained and increasingly qualified professionals to improve the health outcomes of the caregivers. In 2016, in Andalusia the regulatory framework by which is created the statutory professional category of nurse/specialists is published, including the specialty of Mental Health Nursing in the Andalusian Health Service. In the field of Mental Health, the development of this normative framework and the definition and occupation of positions, will allow the health system to combine the role of nurse specialist nurses with that of nurses who provide general care, registered nurses, in order to advance in the best response to the health needs of citizens in this area of care. The development of the specialty will be an added value both to improve the health outcomes of people with mental health problems, and to improve the quality of care, efficiency and sustainability of health systems

The impact of culture on health has gained considerable importance in care delivery. This review discusses the complex interaction of culture and social determinants, and the combined impact of these on emotional well-being. Examples of this interaction are presented and recommendations for change within nursing to improve care are discussed.

A key government priority, artificial intelligence (IA) in healthcare is a real opportunity for nursing professionals. Faced with the daily difficulties encountered, AI could bring a new perspective to nursing care in psychiatry and free up time for professionals which they can then spend with the patient. More training and a multi-disciplinary approach are required.

RESUMEN

Objetivo: Descrever aspectos referentes à sexualidade das pessoas com sofrimento psíquico atendidas em uma instituição de saúde mental no município do Rio de Janeiro. Métodos: Pesquisa qualitativa e descritiva. Participaram 12 usuários. Utilizaram-se entrevistas semiestruturadas para coleta de dados e análise de conteúdo. Resultados: Os participantes possuem vivências sexuais que refletem no seu comportamento social, como: abuso sexual, infecções sexualmente transmissíveis (IST), masturbação, entre outras. Conclusão: O tema sexualidade é tratado com superficialidade pelos profissionais de saúde; as pessoas com sofrimento psíquico precisam de educação em saúde sexual e reprodutiva para exercerem sua sexualidade de forma saudável e segura, melhorarem sua qualidade de vida e diminuir o estigma social que sofrem.(AU)


Objective: describe aspects related to the sexuality of people with psychic suffering attended at a mental health institution in the city of Rio de Janeiro. Methods: Qualitative and descriptive research. Twelve users participated. Semi-structured interviews were used for data collection and content analysis. Results: Participants have sexual experiences that reflect in their social behavior, such as sexual abuse, sexually transmitted infections (STIs), masturbation, among others. Conclusion: The topic sexuality is treated with superficiality by health professionals; People with psychic suffering need sexual and reproductive health education to exercise their sexuality in a healthy and safe way, improve their quality of life and reduce the social stigma they suffer.(AU)


Objetivo: describir aspectos relacionados con la sexualidad de las personas con sufrimiento psíquico atendidas en una institución de salud mental en la ciudad de Río de Janeiro. Métodos: Investigación cualitativa y descriptiva. Doce usuarios participaron. Se utilizaron entrevistas semiestructuradas para la recopilación de datos y el análisis de contenido. Resultados: Los participantes tienen experiencias sexuales que se reflejan en su comportamiento social, como el abuso sexual, las infecciones de transmisión sexual (ITS), la masturbación, entre otros. Conclusión: El tema de la sexualidad es tratado con superficialidad por profesionales de la salud; Las personas con sufrimiento psíquico necesitan educación sobre salud sexual y reproductiva para ejercer su sexualidad de manera saludable y segura, mejorar su calidad de vida y reducir el estigma social que sufren.(AU)

Background: A new short-term risk assessment instrument, the Mechanical Restraint - Confounders, Risk, Alliance Score (MR - CRAS) checklist, including three subscales with altogether 18 items, has been developed in close collaboration with forensic mental health nurses, psychiatrists' etc., and shows evidence of being comprehensible, relevant, comprehensive and easy to use for assessing the patient's readiness to be released from mechanical restraint. Aim: The aim of this study was to investigate whether the subscales: confounders, risk and parameters of alliance constituted separate subscales and needed further revisions. Materials and methods: MR - CRAS was field-study tested among nurses, nurse assistants and social and health care assistants in 13 Danish closed forensic mental health inpatient units, and a Mokken analysis of scalability and a Spearman correlation analysis were performed. Results: MR - CRAS was completed by clinicians in 143 episodes of mechanical restraint, representing 88 patients, with a mean duration of 63.25 hours. Most patients were younger men, diagnosed within the schizophrenia spectrum. One-third of the patients had repeated mechanical restraint episodes ranging between 2 and 8 episodes. MR - CRAS and especially the parameters of alliance were perceived as usable for assessment of the patient's readiness to be released from mechanical restraint. The psychometric analyses showed that the three subscales were unidimensional. Conclusions: The study shows evidence of the construct validity of MR - CRAS among clinicians at closed forensic mental health inpatient units. MR - CRAS contributes with a common language and structured, systematic and transparent observations and assessments on an hour by hour basis during mechanical restraint.

Research concerning this issue demonstrates that nursing students initially feared interacting with and caring for patients with suicidal tendencies. However, there is a lack of research, which examines the care that is provided to patients that are suicidal, by nursing students. The aim of this study was to develop a theory to guide nursing students when caring for patients with suicidal tendencies on their psychiatric clinical practicum. A qualitative approach using Grounded Theory was used. A total of 22 nursing students who had provided care for suicidal patients were interviewed. The core category that emerged from the data was the 'changing of mindsets towards caring for suicidal patients and promotion of suicidal care competencies'. Other key categories linked to and enfolded within this core category were: suicidal risk assessment; protecting patients' safety; and, developing therapeutic communication competencies to advance suicidal care. This study could help fill a theory-practice gap for both psychiatric nursing teachers and students. Nurse teachers could use this theory as a map to help guide students caring for patients with suicidal tendencies and develop nursing students' suicidal care competencies.

RESUMEN

Critical thinking in nursing is largely theorized as a clinically-based idea. In the context of mental health education, this presents a problem, given documented evidence of a shift to demedicalize mental illness. Using institutional ethnography, this article examines the critical thinking of nursing faculty in a baccalaureate nursing program in a Canadian university by way of focus group interviews, observation periods, and the analysis of a number of institutional and legislative texts. The findings suggest that the critical thinking of nursing faculty is caught within a constrained institutional-textual order. Drawing on critical theory and Foucauldian philosophy, recommendations for nursing education are made in order to diversify and extend critical thinking in mental health nursing.

Feelings of empowerment in nursing staff can contribute to increased job satisfaction, improved quality of care and a higher retention rate within healthcare organizations. Nursing is critical at healthcare organizations, with direct and significant impact on the quality of care provided; yet, nurses often feel undervalued for the work they do. The strength-based approach of appreciative inquiry is one way to engage individuals in finding collective value through the identification of successes, enabling the creation of a shared vision and strategy for the future.

RESUMO: Modelo do estudo: Revisão integrativa da literatura. Objetivo: Analisar evidências científicas disponíveis na literatura sobre a utilização do método INTERMED pela equipe multidisciplinar em pacientes com trans-tornos mentais. Metodologia: A seleção de artigos foi realizada nas bases de dados National Library of Medicine (PubMed), American Psychological Association (PsycINFO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Eletronic Library Online (SciELO). Resultados: Ao final, cinco estudos concentrados entre 2000 e 2008, foram selecionados para análise. Os artigos analisados mostraram que o método INTERMED teve resultados positivos na identificação de pacientes que precisam de cuidados complexos, o que direcionou a realização de intervenções psiquiátricas rápidas que diminuíram o tempo de permanência no ambiente hospitalar, o número de internações melhorou a qualidade de vida dos pacientes e, consequentemente, diminuiu custos com a saúde. Conclusão: Os estudos analisados mostraram que há poucas evidências sobre o tema investigado. O método INTERMED tem boa aplicabilidade junto a outros instrumentos e foi eficaz na identificação de pacientes que precisam de cuidados complexos. (AU)


ABSTRACT Study design: Integrative literature review. Purpose: Analyzing the scientific evidence available in the literature about the use of the INTERMED method by the multidisciplinary team in patients with mental disorders. Method: The selection of articles was performed using the National Library of Medicine (PubMed), American Psychological Association (PsycINFO), Latin American and Caribbean Literature in Health Sciences (LILACS) and Scientific Electronic Library Online (SciELO). Results: Five studies, published between 2000 and 2008, were selected for analysis. The data analyzed highlights that the INTERMED method had positive results in the identification of patients who needed complex care, which led to the implementation of rapid psychiatric interventions that reduced the length of staying in the hospital environment, the number of hospitalizations, improved the quality of life of the patients and consequently decreased health costs. Conclusion: We may affirm that there is little evidence on the subject investigated. The INTERMED method has good applicability with other instru-ments and has been effective in identifying patients who need complex care. (AU)

O objetivo foi caracterizar os atendimentos as urgências psiquiátricas realizadas pelo Serviço de Atendimento Móvel de Urgência de Sobral, em 2017. Tem- se uma abordagem quantitativa, do tipo documental retrospectiva. A amostra foi composta por 301 fichas de ocorrências utilizadas nos atendimentos dos pacientes. Os dados foram coletados através de um formulário e analisados por meio do software IBM SPSS Statistics. Obteve parecer favorável pelo CEP da Santa Casa de Misericórdia de Sobral. Foram analisados e discutidos dados referentes a sexo e faixa etária, principais motivos das ocorrências, sinais, sintomas e queixas apresentados pelos paciente e desfecho da ocorrência. O elevado número de ocorrências de emergências psiquiátricas, evidenciou a importância da discussão dos resultados desse trabalho como uma forma de melhorar a articulação e integração da Rede de Atendimento Psicossocial e a Rede de Urgência e emergência, no município, criando novas estratégias para minimizar as internações hospitalares dos pacientes. The objective was to characterize the psychiatric emergency services performed by the Mobile Emergency Service of Sobral in 2017. A quantitative, retrospective, documentary approach is available. The sample consisted of 301 records of occurrences used in patient care. The data were collected through a form and analyzed using the IBM SPSS Statistics software. He obtained a favorable opinion by the CEP of Santa Casa de Misericórdia de Sobral. Data regarding gender and age group, main reasons for occurrences, signs, symptoms and complaints presented by the patients and outcome of the event were analyzed and discussed. The high number of occurrences of psychiatric emergencies highlighted the importance of discussing the results of this work as a way to improve the articulation and integration of the Psychosocial Care Network and the Emergency and Emergency Network in the city, creating new strategies to minimize hospitalizations patients.(AU)


El objetivo fue caracterizar las atenciones a las urgencias psiquiátricas realizadas por el Servicio de Atención Móvil de Urgencia de Sobral, en 2017. Se tiene un abordaje cuantitativo, del tipo documental retrospectivo. La muestra fue compuesta por 301 fichas de ocurrencias utilizadas en las atenciones de los pacientes. Los datos fueron recolectados a través de un formulario y analizados a través del software IBM SPSS Statistics. Obtuvo un dictamen favorable por el CEP de la Santa Casa de Misericordia de Sobral. Se analizaron y discutieron datos referentes a sexo y grupo de edad, principales motivos de las ocurrencias, signos, síntomas y quejas presentados por los pacientes y desenlace de la ocurrencia. El elevado número de ocurrencias de emergencias psiquiátricas, evidenció la importancia de la discusión de los resultados de ese trabajo como una forma de mejorar la articulación e integración de la Red de Atención Psicosocial y la Red de Urgencia y emergencia, en el municipio, creando nuevas estrategias para minimizar las internaciones hospitalarios de los pacientes.(AU)

Objetivou-se realizar uma busca de publicações cientificas na base de dados sobre a depressão nos estudantes de enfermagem, com o propósito de responder à questão norteadora: Como a depressão pode prejudicar o desenvolvimento acadêmico, a saúde e futuro profissional dos graduandos de Enfermagem? Trata-se de uma revisão integrativa realizada em abril de 2019 com os descritores: "estudantes de enfermagem" e "depressão", com uso da estratégia de busca da aplicação do conector booleano "AND", na Biblioteca Virtual em Saúde. Foram selecionados 14 estudos e elaboradas as seguintes categorias para discussão: O Inventário Beck de Depressão como instrumento de avaliação, Principais fatores associados ao desenvolvimento da Depressão nos graduandos de enfermagem e Consumo de antidepressivos entre os acadêmicos de enfermagem. Conclui-se que há a importância de discutir a qualidade de vida desse estudante, a metodologia de ensino, e a implementação de núcleo de apoio psicológico. Considerando que para o aprendizado ocorrer de forma eficaz, o aluno precisa estar bem em todos aspectos, pois a formação desse profissional implicará no cuidado de outra pessoa, o paciente.(AU)


The aim of this study was to search the scientific literature in the database on depression in nursing students, in order to answer the guiding question: How can depression affect academic development, the health and professional future of Nursing undergraduate students? This is an integrative review conducted in April 2019 with the descriptors: "nursing students" and "depression", using the search strategy of the Boolean connector "AND", in the Virtual Health Library. studies and elaborated the following categories for discussion: The Beck Inventory of Depression as an instrument of evaluation, Main factors associated with the development of Depression in Nursing Graduates and Consumption of antidepressants among nursing academics. It is concluded that it is important to discuss the quality of life of this student, the teaching methodology, and the implementation of a nucleus of psychological support. Considering that for learning to occur effectively, the student needs to be well in all aspects, since the training of this professional will involve the care of another person, the patient.(AU)

TOWARDS ADVANCED PRACTICE FOR NURSES IN PSYCHIATRY: Advanced nursing practice in psychiatry is a major challenge for the discipline. It must meet the needs of the population as well as conform to national policies and tackle the problems within the mental health and psychiatric sector. For a long time, psychiatric nurses have developed their practices on the basis of precise indications and health policies. Advanced practice will not be able to replace these existing situations.

BACKGROUND: It is important to support nursing students during their education in developing positive attitudes toward people with mental illness. OBJECTIVES: To examine the impact of mental health nursing module, clinical practice, and anti-stigma program on the attitudes of nursing students toward mental illness. DESIGN: The study was a single group pretest and posttest quasi-experimental design. SETTINGS: This study was conducted in a nursing school in the west of Turkey. PARTICIPANTS: The sample of the study consisted of 64 undergraduate nursing students. METHODS: The data were collected through the Beliefs Toward Mental Illness Scale and Social Distance Scale. RESULTS: The mean scores of the dangerousness subscale significantly decreased after mental health nursing module, clinical practice and anti-stigma program. The results revealed significant positive differences in the mean scores of the social distance scale after the theory, clinical practice, and anti-stigma program were completed. CONCLUSIONS: The mental health nursing curriculum should be focused on replacing stereotypes with accurate information. Anti-stigma programs should be included in standard mental health nursing courses.

Tardive dyskinesia (TD), the choreoathetoid movements of fingers, arms, legs, and trunk and irregular stereotypical movements of the mouth, face, and tongue, has been the scourge of antipsychotic medications since the approval of chlorpromazine. TD tends to occur late in treatment and sometimes remains after discontinuation of the antipsychotic medication. With the recent approval of two medications, valbenazine (Ingrezza®) and deutetrabenazine (Austedo®), there are now treatments for this disfiguring consequence of dopamine-blocking medications. The current article distinguishes the movement disorder adverse effects of dopamine antagonists, explains the putative mechanism of action, and describes how best to treat TD with the new vesicular monamine transporter 2 (VMAT2) medications now approved by the U.S. Food and Drug Administration. BACKGROUND: Intramuscular injections (IMIs) remain a frequent practice in mental health. Few studies have examined the issue of nurses' practices concerning IMI in this domain, and none considered specifically hygiene. Finally, no study appears to have looked at emergency situations and their possible influence on practices. The principal objective of our study was to assess the practices associated with IMI in mental health, especially the hygiene-related practices. The secondary objectives were as follows: 1) to assess the practices for the preparation and administration of IMI in mental health for criteria other than hygiene and 2) to determine whether professional experience and emergency situations influence these practices. DESIGN: Single-centre cross-sectional study in a psychiatric Hospital and adherence to the STROBE guidelines. METHODS: The participating nurses all worked with inpatients, and all volunteered for the study. A self-administered questionnaire was sent to all participants. The questionnaire included questions about knowledge and practices associated with IMI, some considered in two different situations: emergencies and planned injections. The distribution of the responses was tested by the chi-squared test or Fisher's exact test, as appropriate, or by McNemar's chi-squared test or Friedman's nonparametric chi-squared test for matched data. RESULTS: Response rate was 48.6%. Overall, 81% of nurses reported correct handwashing before preparation, 87.5% responded that the dorsogluteal site is currently recommended for IMI, and 74.6% that they did not know the "Z track" technique. In planned injections, 58.7% reported that the choice of needle was determined, at least in part, by the patient's body mass index. In emergency situations, adherence to guidelines was less frequent for all types of practices. CONCLUSIONS: This study shows the need to improve practices for the frequently used procedure of IMI among mental health nurses. RELEVANCE TO CLINICAL PRACTICE: Better professional education appears necessary to develop optimal practices, especially in emergency situations.

Objetivo: rastrear a depressão pós-parto entre mulheres jovens que estão na segunda semana e no sexto mês após o parto. Método: trata-se de estudo quantitativo, descritivo, exploratório e transversal cujas participantes foram mulheres com idades entre 18 a 26 anos, entre a segunda semana e o sexto mês após o parto. Coletaram-se os dados entre os meses de agosto/2017 a janeiro/2018, por meio da Escala de Depressão Pós-natal de Edimburgo e de um inquérito sociodemográfico. Utilizou-se, para a tabulação dos dados, o programa Microsoft Excel, digitando-os por dupla entrada e, posteriormente, transportando-os para o programa BioEstat, versão 5.0. Aplicou-se o teste qui-quadrado para a análise estatística descritiva e apresentaram-se os resultados em forma de tabela. Resultados: identificou-se uma provável depressão pósparto em 19,70% das puérperas e essa condição teve associação com os seguintes fatores: idade do bebê, multiparidade e baixo nível de escolaridade. Conclusão: evidencia-se que a depressão pós-parto precisa ser investigada na atenção primária em saúde, que deve valorizar os aspectos sociodemográficos e individuais para estabelecer um plano de cuidados integral desde o pré-natal, com vistas à prevenção desse frequente transtorno do puerpério.(AU)


Objective: to track postpartum depression among young women who are in the second week and in the sixth month postpartum. Method: it is a quantitative, descriptive, exploratory and cross-sectional study whose participants were women aged between 18 and 26 years, between the second week and the sixth month after delivery. The data was collected between August 2017 and January 2018, using the Edinburgh Postnatal Depression Scale and a sociodemographic survey. For the tabulation of data, the Microsoft Excel program was used, typing them by double entry and then transporting them to the BioEstat program, version 5.0. The chisquare test was applied for the descriptive statistical analysis and the results were presented in table form. Results: a probable postpartum depression was identified in 19.70% of the puerperae and this condition was associated with the following factors: baby age, multiparity and low level of schooling. Conclusion: it is evidenced that postpartum depression needs to be investigated in primary health care, which should value the sociodemographic and individual aspects to establish a comprehensive care plan from the prenatal period, with a view to preventing this frequent postpartum disorder.(AU)
