RESUMEN

OBJECTIVE: To study a role of central sensitization (CS) in patients with Failed Back Surgery Syndrome (FBSS) after decompression of the lumbar and/or sacral roots. MATERIAL AND METHODS: The study included three groups of patients: 1) early FBSS (e-FBSS, n=23), pain after surgery decreased, but continued to significantly bother the patient, or recurred in the first 6 months after surgery; 2) middle FBSS (m-FBSS, n=42), pain after surgery completely stopped, but recurred within 6-12 months; 3) late FBSS (l-FBSS, n=31), if pain relapse occurred more than 12 months later. Neurological status assessment, study of muscle trigger zones (MTZ), postoperative scar trigger zones (TZS), pain assessment according to the Visual Analogue Scale (VAS), CS assessment according to the Russian version of the Central Sensitization Inventory (CSI) were performed. Treatment included the following stages: stage 1 (non-steroidal anti-inflammatory drug (NSAID) meloxicam 15 mg 7 days); stage 2 (aminophenylbutyric acid hydrochloride (APAH) 250 mg 3 times a day 14 days); stage 3 (fluvoxamine 50 mg daily 8 weeks); stage 4 (MTZ and TZS treatment, fitness program). In addition to the previously described 96 patients, 15 patients (4 men and 11 women, aged 36-47 years) with FBSS and widespread pain, corresponding to the criteria for fibromyalgia ACTTION-APS Pain Taxonomy, who took only milnacipran 25-50 mg daily for two months (WSP-FBSS group), were studied. RESULTS: At baseline, CSI and VAS are as follows: CSI=72.2±6.5; VAS=58.5±8.8 in the e-FBSS group; CSI=49.2±9.0; VAS=39.5±5.3 in the m-FBSS group; CSI=18.1±5.9; VAS=18.1±5.4 in the l-FBSS group. All patients have active MTZ and TZS. The differences between MTZ and TZS subgroups were in the reproduction of pain - the appearance of pain characteristic of the patient during stimulation (pressure) of the MTZ or TZS. In the e-FBSS group, there is the low efficacy of NSAIDs, the moderate efficacy of APAH, the high efficacy of fluvoxamine. In the m-FBSS group, the moderate efficacy of NSAIDs and APAH and the high efficacy of fluvoxamine are observed. In the l-FBSS group, there is the high efficacy of NSAIDs. In the WSP-FBSS group, VAS and CSI are 58.8±9.2 mm and 75.1±8.04, respectively, before treatment, 15.51±5.1 mm (p=0.00032) and 25.6±8.2 (p=0.0002), respectively, after 2 months of treatment. CONCLUSIONS: In patients with FBSS, MTZ and TZS should be treated taking into account CS. The study shows the efficacy of APAH, fluvoxamine and milnacipran in the presence of CS.

OBJECTIVE: A comparative analysis of the parameters of lipid peroxidation - antioxidant defense system in menopausal women with- and without insomnia, depending on the GSTT1 polymorphism. MATERIAL AND METHODS: The study included 181 menopausal Caucasian women (ethnic group - Russians), who according to the results of the clinical-anamnestic examinations were divided into insomnia group (n=120, age 53.68±4.61 years, body mass index 26.47±3.73 kg/m2) and control group (n=61, age 52.64±4.91 years, body mass index 27.69±3.11 kg/m2). The insomnia group was formed according to the clinical guidelines for the diagnosis and treatment of chronic insomnia in adults, and scores of the Insomnia Severity Index. The study of the GSTT1 gene polymorphism was carried out using polymerase chain reaction. The lipid peroxidation - antioxidant system parameters were determined by spectrofluorophotometric methods. RESULTS: In the insomnia group, carriers of the normal GSTT1 genotype had higher diene conjugates with lower glutathione peroxidase activity as compared to controls. When comparing groups of women with the deletion in GSTT1, an increase in the content of substrates and lipid peroxidation products was observed at all stages of lipid peroxidation, oxidized glutathione, glutathione S-transferase activity with a decrease in the content of reduced glutathione and retinol in the insomnia group as compared to controls. In the insomnia group, carriers of the deletion genotype had a higher content of ketodienes/conjugated trienes and active products of thiobarbituric acid, a lower content of retinol and α-tocopherol as compared to the patients with normal GSTT1 genotype. Conclusions. Insomnia in the menopausal women is accompanied with the intensification of lipid peroxidation only in the carriers of the GSTT1 deletion genotype.

RESUMEN

Most mental disorders occur before the age of 25 years, and if left without intervention, they can transform into more severe mental disorders. Despite the great need to protect the mental health of children and adolescents, in Russia and most countries of the world, there is a shortage of qualified personnel in this area. The article presents data on the availability of personnel in different countries of the world, including Russia. The issues of organizing the training of child and adolescent psychiatrists in Russia and various countries are examined, and the available data on training programs are analyzed. Educational opportunities for child and adolescent psychiatry in Russia are examined in detail.

OBJECTIVE: To assess the mental health of health workers during a pandemic. MATERIAL AND METHODS: Eight hundred and twelve medical workers took part in a cross-sectional Internet survey. The questionnaire included a socio-demographic block, a block of questionnaires for assessing the level of symptoms of anxiety and depression (PHQ-9 and GAD-7). The data were processed using statistical methods. RESULTS: The prevalence of anxiety and depression among healthcare providers during the pandemic was 48.77% and 57.63% respectively. Subjectively poor quality of sleep was noted by 37.4% of respondents. Cluster analysis distinguished 4 groups of respondents: group 1 was characterized by high scores on PHQ-9 and GAD-7 and a low subjective assessment of sleep quality; group 2 had low scores on PHQ-9 and GAD-7 and a high subjective assessment of sleep quality; respondents from clusters 3 and 4 had average total scores on PHQ-9 and GAD-7, however, group 4 was characterized by significantly lower values of subjective sleep quality assessment. CONCLUSION: The division into clusters makes it possible to understand which groups of medical workers require psychological (psychotherapeutic) support in the first place. Cluster 1 respondents with high levels of anxiety and depression, as well as poor sleep quality, need priority assistance.

RESUMEN

This paper explores the potential of recent research on metacognition to offer new avenues to assess and address the phenomenon of fragmentation in schizophrenia, which was described by E.Bleuler as «splitting¼. The concepts of metacognition characterize and quantify alterations or decrements in the processes by which fragments or pieces of information are integrated into a coherent sense of self and others. A method for assessing metacognition is presented along with research examining the presence and importance of metacognitive deficits in schizophrenia. Greater levels of metacognitive deficits have been detected in different phases of schizophrenia and linked to poorer psychosocial outcomes. These data were obtained both in foreign and preliminary Russian studies. The authors suggest that treatments, which successfully target metacognitive capacity, may uniquely promote wellness and recovery in schizophrenia.

RESUMEN

In December 2019, a new SARS-CoV-2 virus, called COVID-19, was identified in the Wuhan province of China, the spread of which determined the development of a pandemic. The lack of publications with evidence-based clinical studies dictate the necessity to cover various, including rare, pathological effects of the virus on the human body and the subsequent effect on it. Nowadays, there is already evidence that the SARS-CoV-2 virus causes specific damage to the vessels (endothelium), myocardium, and kidneys. The pathophysiologic mechanisms leading to acute kidney injury during COVID-19 infection are unclear, but may be due to direct exposure of the kidney tubules and endothelial cells to the virus. We present a clinical case of a kidney infarction in a 17-year-old girl with a severe course of a new coronavirus infection. At the time of the description of this clinical case, there are no publications on COVID-19 associated with kidney infarction in children in the Russian and foreign literature which is available to us.

Late diagnosis of renal tuberculosis leads to complications that cannot be eliminated by treatment. A clinical observation of renal tuberculosis, complicated by total ureteral obliteration, in a comorbid patient is presented. He underwent planned bowel substitution of the right ureter. In the postoperative period, pyelonephritis developed, which was resolved by drug therapy. In this case, there is the correct tactics of outpatient urologists. When hydronephrosis was diagnosed, a nephrostomy tube was put, which allowed to preserve the kidney, Then the patient was immediately referred to a phthisiatrician to exclude urogenital tuberculosis. In the local TB dispensary, the patient did not have the opportunity to receive necessary treatment, and he was transferred to the TB Research Institute of Ministry of Health of Russia, where a reconstructive laparoscopic procedure was performed.

Longitudinal monitoring studies (between 2006 and 2019) of the chemical composition of the water from the Angara River source (the runoff of Lake Baikal) revealed the interannual, year-round and monthly cyclicity in the distribution of some trace elements vis-à-vis their concentrations. The change in the concentrations of elements was contingent on the season, the temperature of the air and water, the activity of phyto- and zooplankton and regional changes in the environment (floods, earthquakes, fires, tourism, ships, technogenesis etc.). We compared the concentrations of trace elements present in the water samples from the Angara source and the water samples from Lake Baikal with the maximum permissible concentration values for drinking water. The calculated (median) concentrations for water samples from the Angara source, which spanned the entire study period, were close to the data obtained for the Baikal water.

OBJECTIVE: The aim of the study evaluate the quality of life of employees of the North Eastern Federal University. PATIENTS AND METHODS: Materials and methods: The respondents to the WHO QoL 100 questionnaire were 37 teachers and 32 support staff with an average age of 44 years. RESULTS: Results and conclusions: There is a higher correlation between assessments of overall satisfaction with the quality of life and individual areas in the group of surveyed teachers compared to the group of support staff. Comparison of the parameters of the QOL areas of teachers of NEFU with the data of similar surveys of universities in Central Russia revealed a higher assessment of such areas as the "environment", the "physical" sphere among the Yakutia's. These results indicate the adaptability of NEFU employees to environmental conditions. Also, among the respondents of the Yakutia's, there was revealed a great significance for the QOL of such a subsphere as support for relatives, friends and colleagues.

The aim of the study was to determine the influence of various drinking liquids on an the dentoalveolar system in rats. The study was carried out on 25 white male rats from the nursery of Krolinfo branch of the Moscow region which were kept in standard vivarium conditions. Age of rats-3-6 months, weight - 150-200 grams. The study was carried out in accordance with the ethical principles of humane treatment of animals, in accordance with the current legislation of the Russian Federation. To assess the effect of consumed fluids, groups of 5 animals of the same sex (males, females) were used. The animals were randomly assigned to groups. Observation of animals was carried out for 6 months, during this period the animals were in conditions of free access to food and as drink received the studied liquids ad libitum. Experimental groups within 3 months received the following types of drink: I (control group) - tap water; The II group - Coca-Cola; The III group - honey water; The IV group - the distilled water; The V group - light water. The results obtained by the authors and the data provided in literature confirm that among the factors that affect the teeth condition, in particular the characteristics of the hardness of enamel and dentin, the use of various sugar-containing drinks, as well as the duration, quantity, and frequency of their use, can play a significant role. These factors remain largely unexplored at present. It is obvious that in order to stop caries, it is necessary to develop and carry out a set of preventive measures, including limiting the consumption of sweets and beverages containing sugar, along with regular dental care and a balanced diet.

Increasing research shows that migrants are disproportionately exposed to coronavirus disease 2019 (COVID-19). However, little is known about their lived experience and related meaning-making. This qualitative study maps COVID-19-related experiences among respondents from three migrant groups living in Finland: Somali-, Arabic- and Russian-speakers (N = 209). The data were collected by telephone interviews over four weeks in March and April 2020. Using inductive thematic analysis, we identified seven themes that illustrate respondents' multifaceted lived experiences during the first phase of pandemic. The themes depict respondents' difficulties and fears, but also their resilience and resources to cope, both individually and collectively. Experiences varied greatly between individuals and migrant groups. The main conclusion is that although the COVID-19 pandemic may be an especially stressful experience for migrant populations, it may also provide opportunities to deepen cooperation and trust within migrant communities, and between migrants and their country of settlement. Our analysis suggests that cooperation between local authorities and migrants, trust-building and effective information-sharing can foster positive and functional adaptations to disease-related threats and changing social environments.

OBJECTIVE: To estimate the frequency of early postoperative neurological complications in patients undergoing planned surgery on the ascending aortic and arch of the aorta, and their long-term outcomes. MATERIAL AND METHODS: The prospective study included 100 patients who were operated on at the FSBSI Petrovsky Russian Research Center of Surgery. In group I (n=50), operations were performed on the aortic arch with hypothermic circulatory arrest (26 °C) and antegrade cerebral perfusion. Patients in group II underwent prosthetics of the ascending aorta with extracorporeal circulation and moderate hypothermia (32 °C). All patients underwent monitoring of cerebral and tissue oxygenation, transcranial Doppler and testing of cognitive functions before and after surgery, and after 5 and 10 years of follow-up. RESULTS: Postoperative stroke in group I was observed in 1 (2%) patient, no cases were observed in group II. Delirium was detected in 14% of patients in group I and 6% of patients in group II, its subsyndromal form was found in 6 and 4%, respectively. Moderate cognitive impairment in the immediate postoperative period was found in 42 and 26%; severe in 8% of group I. After 5 years of follow-up, the number of patients with moderate and severe cognitive impairment was 23.1 and 12.8%, respectively. After 10 years, severe disorders were identified in 37.5 and 21.9% of patients. CONCLUSION: In cardiac surgery patients, intraoperative multimodal monitoring allows dynamic regulation of antegrade cerebral perfusion. Dynamic testing of cognitive functions and early detection of delirium in the immediate postoperative period improve long-term neurological treatment outcomes.

Aim        To analyze first results of using the Aterostop calculator for a comprehensive evaluation of the risk for cardiovascular diseases (CVD).Material and methods        A cross-sectional study analyzed major and additional risk factors in 460 subjects without apparent disease and in patients with documented CVD of atherosclerotic origin using the application (calculator) Aterostop developed in the National Medical Research Center of Cardiology in Moscow, Russia.Results   45.4% of evaluated persons belonged to the categories of very high and extreme risk. Age and frequencies of smoking, arterial hypertension, and diabetes mellitus (DM) increased with the increase in risk; the growth of DM was exponential. 129 (28%) individuals used lipid-lowering medications at the time of study. Their plasma levels of low-density lipoprotein cholesterol (LDL-C) were significantly lower than in those who did not received this treatment. However, achieving the target level was inversely proportional to the risk: the greatest proportion of individuals who reached the LDL-C target was in the category of low risk and the smallest proportion was in the category of extreme risk (75 % vs. 3.7 %, respectively).Conclusion            The results obtained with the calculator Aterostop were consistent with earlier reports of insufficient effectiveness of primary and secondary prevention of atherosclerotic CVDs, which requires more tight and fruitful cooperation of the physician and the patient.

Aim    To present clinical characteristics of patients after hospitalization for acute decompensated heart failure (ADHF) and to analyze hemodynamic indexes and compliance with the treatment at two years depending on the conditions of outpatient follow-up.Material and methods    The study included 942 patients with chronic heart failure (CHF) older than 18 years who had been hospitalized for ADHF. Based on patients' decisions, two groups were isolated: patients who continued the outpatient follow-up at the Center of CHF (CCHF) (group 1, n=510) and patients who continued the follow-up in outpatient multidisciplinary clinics (OMC) at their place of residence (group 2, n=432). The clinical portrait of patients was evaluated after ADHF, and hemodynamic parameters were evaluated on discharge from the hospital. Also, the patient compliance with the treatment was analyzed during two years of follow-up. Statistical analysis was performed with Statistica 7.0 for Windows.Results    The leading causes for CHF included arterial hypertension, ischemic heart disease, atrial fibrillation, and type 2 diabetes mellitus. With the mean duration of hospitalization of 11 inpatient days, 88.1 % and 88.4 % of patients of groups 1 and 2 were discharged with complaints of shortness of breath; 62 % and 70.4 % complained of palpitations; and 73.6 % and 71.8 % complained of general weakness. On discharge from the hospital, the following obvious signs of congestion remained: peripheral edema in 54.3 % and 57.9 %; pulmonary rales in 28.8 % and 32.4 %; orthopnea in 21.4 % and 26.2 %; and cough in 16,5 % and 15.5 % of patients of groups 1 and 2, respectively. For the time of hospitalization, CHF patients did not achieve their targets of systolic BP (SBP), diastolic BP (DBP) and heart rate (HR). Patients of group 1 achieved the recommended values of SBP, DBP and HR already at one year of the follow-up at CCHF. Patients of group 2 had no significant changes in hemodynamic indexes. At one and two years of the follow-up, group 2 showed a considerable impairment of the compliance with the basis therapy for CHF compared to group 1.Conclusions    During the short period of hospitalization (11 inpatient days), the patients retained pronounced symptoms of HF and clinical signs of congestion and did not achieve their hemodynamic targets. The patients who were followed up for a long time at CCHF were more compliant with the basis therapy, which resulted in improvement of hemodynamic indexes, compared to the patients who were managed in OMS at the place of residence.

The medical community, researchers and healthcare organizers are constantly challenged by comparing key indexes reflecting the effectiveness of cardiovascular care, primarily for the dynamic assessment and implementation of the world's best practices to reduce cardiovascular mortality. The analysis of health care for patients with cardiovascular diseases (CVD) is a complex, multicomponent process, the structure and key tools of which differ from country to country. Using different data sources, methodological and analytical approaches creates certain limitations and barriers to the assessment. In order to update the ideas about the modern coordinate system and tools for assessing cardiovascular care, the authors presented practices for analyzing major indexes in Russia, European countries, and the United States. The review presents sources of statistical data, principles for assessing risk factors, cardiovascular morbidity and mortality, and specific features of monitoring the availability and quality of cardiovascular care.

RESUMEN

The new testate amoeba species Meisterfeldia bitsevi is described from the linden-tree trunk hollow in the urban park in Moscow (Russia). New species is characterized by elongated ovoid bilaterally symmetrical shell, which is laterally not compressed. The shell is composed of proteinaceous material without mineral particles. Almost circular subterminal aperture is placed on ventral side. The key to the species of the genus Meisterfeldia is developed.

RESUMEN

This paper provides a review of the plant bug subfamily Bryocorinae (Hemiptera: Miridae) of European Russia and the Caucasus. Although several bryocorine species are widely used as biological control agents, species delimitation remains challenging even in the relatively well-studied European fauna. We provide diagnoses, distribution data, host information, and illustrated keys to six genera and 21 species found in the region. The following new records are given: Dicyphus (B.) albonasutus Wagner, 1951 (Crimea and Caucasus, North Ossetia), Dicyphus (B.) digitalidis Josifov, 1958 (Caucasus, Abkhazia), Dicyphus (B.) geniculatus (Fieber, 1858) (Armenia and Ingushetia), Dicyphus (D.) epilobii Reuter 1883 (Abkhazia and Ingushetia), Dicyphus (D.) hyalinipennis Burmeister, 1835 (Armenia). Previous records of Dicyphus (B.) annulatus (Wolff, 1804) from the Northern Caucasus (Konstantinov Zinovyeva 2017), and Crimea were based on misidentifications and should be referred to D. albonasutus. Records of Dicyphus (D.) pallidus (Herrich-Schaeffer, 1836) from Krasnodar Terr and Azerbaijan should be referred to D. errans (Wolff, 1804).

RESUMEN

We assembled a checklist by documenting and curating previously published data as well as previously unpublished records of bat flies from the Russian Federation. A total of 20 bat fly species are listed, belonging to 4 genera. Basilia mongolensis nudior Hurka, 1972 and Basilia nattereri (Kolenati, 1857) are recorded from Russia for the first time. The following new host associations are reported: Basilia mongolensis nudior ex Myotis nattereri (Kuhl), Basilia nattereri ex Eptesicus nilssonii (Keyserling Blasius), Basilia rybini Hurka, 1969 ex Myotis dasycneme (Boie) and Eptesicus nilssonii, and Nycteribia quasiocellata Theodor, 1966 ex Vespertilio murinus Linnaeus. We provide data on nine major Russian regions for which nycteribiid records were previously lacking.

RESUMEN

Four new species of zerconid mites of the genus Halozercon are described and illustrated based on material collected from litter and soil in Altai and Tuva mountains, South Siberia, Russia-H. alataus sp. n., H. gryphus sp. n., H. kumir sp. n. and H. aesopi sp. n. Morphological characters for the new Halozercon species are given. A key for all known species of Halozercon is attached.
