Objetivo: Validar el contenido de un instrumento para la documentación de las etapas del proceso enfermero, utilizando los sistemas de lenguaje de NANDA-I, NOC y NIC, considerando los pacientes hospitalizados en una unidad de cuidados intensivos. Método: Investigación metodológica realizada en 3 etapas: diseño del instrumento existente a partir de los sistemas NANDA-I, NOC y NIC; validación de contenido por 13 jueces a partir de una escala tipo Likert de 4 puntos, evaluando los ítems en cuanto a claridad y pertinencia; aplicabilidad: opinión de 40 enfermeros asistenciales sobre el contenido respecto a la claridad, facilidad de lectura y presentación. Se calculó el índice de validez de contenido y el coeficiente de Kappa para medir la proporción de relevancia y claridad, así como para verificar el nivel de concordancia entre especialistas en cada ítem. Resultados: Se consideró el instrumento claro y pertinente, con índice de validez de contenido por encima de 0,8 en la mayoría de los ítems, e índice de concordancia global de 0,90, evidenciando un nivel satisfactorio de concordancia entre los jueces. En cuanto a la aplicabilidad se consideró el instrumento claro, de fácil lectura y con presentación adecuada por la mayoría de los enfermeros asistenciales, siendo validado a través de 11 diagnósticos con sus respectivos resultados e intervenciones de enfermería. Conclusión: El instrumento ha demostrado ser válido y aplicable para el grupo estudiado. Se espera que este estudio contribuya a la mejora del proceso enfermero en cuidados intensivos


Objective: to validate the content of an instrument for documenting the steps of the Nursing Process, using the standardized languages NANDA-I, NOC, and NIC (NNN), aiming at hospitalized patients in an Intensive Care Unit (ICU). Method: A methodological research performed in three steps: design of the existing instrument from the systems NANDA-I, NOC and NIC; content validation by 13 judges, from a four-point Likert-type scale – items were evaluated as to their clarity and pertinence; applicability: judgement of the content regarding clarity, reading ease, and presentation for 40 critical-care nurses. The Content Validity Index (CVI) and the Kappa coefficient (k) was calculated to measure the proportion of relevance and clarity, was well as to verify the level of agreement between the experts in each item. Results: The instrument was considered clear and pertinent, with CVI above 0.8 in most items and overall Concordance Index (CI) of 0.90, showing a satisfactory level of agreement between judges. Regarding applicability, the instrument was deliberated clear, of easy reading, and with proper presentation by most critical-care nurses, being validated through 11 diagnoses with their respective results and nursing interventions. Conclusion; The instrument showed to be valid and applicable for the group studied. It is expected that this study is able to contribute to the improvement of the Nursing Process in intensive care

OBJECTIVE: to relate nonspecific low back pain within the nursing work context with their workloads, attrition processes and the risks of illness. METHOD: a cross-sectional study with 301 workers from a general hospital in the south of the country. The Nordic Musculoskeletal Questionnaire and the Work Context Assessment Scale composed of three dimensions were used: working conditions, work organization and socio-professional relations. The association of variables with low back pain was tested using bivariate and multivariate analyzes. The measure of association used was the Odds Ratio and its respective intervals with 95% confidence. The data collected were discussed under the theoretical framework of the work process within the marxist conception and the theory of social determination of the health-disease process. RESULTS: there was a statistically significant association between the dimensions of work organization and working conditions with low back pain and they obtained a critical classification meaning moderate risks to the professional illness. CONCLUSION: the study allowed a better understanding of the nursing work process and its relation with nonspecific low back pain and signaled that changes in the organization and working conditions should occur in order to reduce the risks of nursing workers' illness.

OBJECTIVE: To identify work process-related causes associated with nursing errors reported in newspapers. METHOD: This was a documentary and qualitative study based on the work process theory and hermeneutic analysis that examined 112 news articles published between 2012 and 2016 in 21 high-circulation Brazilian newspapers, organized and codified using Atlas.ti software. RESULTS: The causes associated with the reported errors were associated with workforce (lack of professionals and training, turnover, work overload, lack of information, recklessness, negligence, and distraction); work instruments (similar labels or packages, storage, lack of product identification and information, and medical prescriptions); and the object of nursing work (overcrowding and specific characteristics of patient). CONCLUSION: Analysis of the possible causes of reported errors identified the negative outcomes of nursing work, while also identifying elements of the work process that influenced these results. The findings emphasize the importance of understanding these errors so they can be avoided and of reviewing nursing work conditions to guarantee quality and safety of care.

Objetivo: Identificar en la literatura científica, los matices identitarios de la enfermería en diversos contextos, especialmente en los campos de la gestión y de la enseñanza, para contribuir y desplegar nuevos apuntes sobre la temática propuesta.Método: Revisión integrativa, con el fin de aclarar la siguiente cuestión: ¿Cuáles son las configuraciones identitarias de la Enfermería como profesión en el contexto gerencial y de enseñanza? Los datos fueron recolectados en junio de 2017, sin delimitación temporal o de idioma, que estuvieran íntegramente, en la base de datos de Cinahl, en el portal de la BVS y del Dedalus, y en el directorio de revistas de Scielo. Al final del análisis de los datos, siete estudios fueron seleccionados.Resultados: Se percibió que estudios inherentes a la identidad profesional de la enfermera gestora insertada en un Programa de Integración Docente Asistencial, son todavía poco explorados. Sin embargo, se pudo discutir acerca de la referida temática sumergiéndose en publicaciones que reflexionan acerca de la identidad profesional de la enfermera docente y gerente.Consideraciones finales: La literatura enumerada en este estudio permitió vislumbrar las variantes que impactan en la construcción de un arquetipo identitario, permitiendo con ello, que nuevos desdoblamientos surgen como consecuencia de la necesidad de profundizarse en ese asunto eminentemente específico y peculiar, habida cuenta de que es un tema instigador, rico de posibilidades y de transformaciones

Objective: To identify, in the scientific literature, the identity nuances of Nursing in several contexts, especially in the fields of management and teaching, to contribute and unfold new notes on the proposed subject.Method: Integrative review, aiming to solve the following question: What are the identity settings of Nursing as a profession in both managing and teaching context? Data was collected in June 2017, without time or language delimitation, in full, from Cinahl database, from BVS and Dedalus portal, and from Scielo magazine directory. At the end of the data analysis, seven studies had been selected.Results: It was noticed that studies that are inherent to the professional identity of the nurse manager at IDA Program are still little explored. However, it was possible to discuss the subject matter by immersing itself in publications that reflect on the professional identity of the teaching and management nurse.Final considerations: The literature mentioned in this study allowed us to glimpse the variants that affect the construction of an identity archetype, thus allowing new developments to arise because of the need to explore this eminently specific and peculiar subject, since it is an exciting subject, abundant in possibilities and transformations

RESUMEN

Advanced practice registered nurses (APRNs) are leaders in clinical practice, systems-level care delivery, nursing practice, and policy. The challenge is that in the current environment, their leadership is not highlighted. With the national shortage of palliative care clinicians, APRN leaders will be necessary to care for all the patients and their families with serious illness. While many programs describe their APRNs as leader in clinical care, there is not much written about their leadership in other areas. This paper provides a description of APRN palliative care leadership and opportunities across all domains of clinical care, education, advocacy/policy, research, and administration/management.

RESUMEN

Coordination of information and resources is central to efficient ward management. Tools to aid coordination include standardized procedures and paper lists and, more recently, electronic tools such as whiteboards and mobile solutions. This study reports on the experiences and effects of designing and implementing collaboration technology, and how the use of light-weight technology has enabled redesign of traditional work processes. The process followed the principles for user-centered development and is evaluated through observations and informal interviews of users and stakeholders. In addition to the expected changes in work processes, we found that short redesign cycles with heavy user involvement afforded by this technology enabled implementation of new ways of structuring and combining information that were not foreseen. This turned the collaboration technology into a tool that also guided prioritization between tasks and contributed to the effects seen in use of resources, user satisfaction, and quality of treatment.

Objetivo: Describir los hallazgos registrados en la literatura científica sobre el uso del lenguaje estandarizado enfermero como estrategia para fomentar el cuidado a la gestante en los servicios materno-perinatales. Método: Revisión integrativa de la literatura de artículos científicos publicados en bases de datos entre 1994 y 2017 en español, inglés y portugués. Se seleccionaron 63 artículos. Resultados: Se establecieron tres categorías que aluden al Conocimiento Contemporáneo de Enfermería (CCE), al Proceso de Atención de Enfermería (PAE) y a las perspectivas para la aplicación del lenguaje enfermero estandarizado (LEE) en el cuidado materno-perinatal. Conclusión: Se plantea una propuesta denominada Modelo Educación, Práctica e Investigación (EPI), que pretende fortalecer el saber enfermero acorde con sus competencias.


Objective: To describe the findings recorded in the scientific literature about the use of the standardized nursing language as a strategy to promote the care for pregnant women and in the maternal-perinatal services. Method: This is a comprehensive review of the literature covering scientific articles published in different databases between 1994 and 2017 in Spanish, English and Portuguese; 63 articles were selected. Results: Articles were grouped in three categories regarding the Contemporary Nursing Knowledge (CNK), the Nursing Care Process (NCP), and perspectives for the application of the Standardized Nursing Language (SNL) in the maternal-perinatal care practice. Conclusion: This work provides a proposal named Model of Education, Practice and Research (EPR) intended to strengthen the nursing knowledge based on the competences.


Objetivo: Descrever os achados registrados na literatura científica sobre o uso da linguagem enfermeira estandardizada como estratégia para fomentar o cuidado à gestante nos serviços materno-perinatais. Método: Revisão integrativa da literatura de artigos científicos publicados em bases de dados entre 1994 e 2017 em espanhol, inglês e português. 63 artigos foram selecionados. Resultados: Três categorias que aludem ao Conhecimento Contemporâneo de Enfermagem (CCE), o Processo de Atendimento de Enfermagem (PAE) e às perspectivas para a aplicação da linguagem enfermeira estandardizada (LEE) no cuidado materno-perinatal foram estabelecidas. Objective: to make a comprehensive assessment of a pacient who has a thoracoscopy or thoracic window that needs wound care. Through the functional patterns of Marjory Gordon and the NANDA taxonomy (North American Nursing Diagnosis Association), NIC (Nursing Interventions Classification), and NOC (Nursing Outcomes Classification). As well as helping the healing and closing time of the wound to be shorter, reducing the risk of infection and improving the quality of life of the patient. Case presentation: the clinical case of the patient with thoracic window is described. The real and potential diagnoses were established, some objectives or NOC were set and the necessary activities or NIC were carried out to reach them. Results: on a Likert scale (1 to 5), a wound healing was obtained, from 1 to 3, a level of anxiety, from 3 to 4, a problem coping, from 3 to 4, a level of self-care, from 2 to 4, the promotion of health, from 2 to 4, and the therapeutic regimen, from 2 to 4, after one year of follow-up. Conclusions: the comprehensive approach that should be performed to the patient, not focused only on the reason for the consultation, but assessing the different functional patterns to know which are altered and take into account the evolution of the process and better care and quality improvement of the patient's life


Objetivos: hacer una valoración integral de un paciente que presenta una toracostomía o ventana torácica, que precisa de cuidados de la herida a través de los patrones funcionales de Marjory Gordon y la taxonomía NANDA (North American Nursing Diagnosis Association), NIC (Nursing Interventions Classification) y NOC (Nursing Outcomes Classification), así como ayudar a que el tiempo de cicatrización y cierre de la herida sea más corto, reduciendo el riesgo de infección y mejorando la calidad de vida del paciente. Presentación del caso: se describe el caso clínico del paciente con ventana torácica. Se establecieron los diagnósticos reales y potenciales, se fijaron unos objetivos (NOC) y se realizaron las actividades necesarias (NIC) para alcanzarlos. Resultados: en una escala de tipo Likert, con rangos de medición del 1 al 5, se obtuvo una curación de la herida del 1 al 3, un nivel de ansiedad del 3 al 4, un afrontamiento de problemas del 3 al 4, un nivel de autocuidado del 2 al 4, el fomento de la salud del 2 al 4, y el régimen terapéutico del 2 al 4, tras un año de seguimiento. Conclusiones: el abordaje integral que se debe realizar al paciente no debe centrarse solo en el motivo de la consulta, sino que es necesario también valorar los distintos patrones funcionales, para saber cuáles están alterados y tener en cuenta la evolución del proceso, la mejor atención y la mejora de la calidad de vida del paciente

OBJECTIVE: To conduct a situational diagnosis of the Nursing Care Systematization (NCS) in a Basic Health Unit from the perception of the nursing team. METHOD: This was a quantitative, descriptive-exploratory study, conducted in a Basic Health Unit in the interior of São Paulo state, Brazil. A structured questionnaire containing Likert scale was used for data collection, previously validated by specialists. RESULTS: The questionnaire was applied to 21 nursing professionals. It was verified that implementation of NCS and the nursing process is still incipient in the Basic Health Units. Regarding its use in all nursing actions: 19% of respondents believe that it is never used, 38% thought that it is rarely implemented, and 29% responded sometimes. The main difficulties identified were related to the lack of institutional structure, highlighting a lack of capacity by the institution (81%). The team has a reasonable understanding about the subject, but pointed out that professional education does not prepare them for performing NCS in primary care. CONCLUSION: Institutional interest in NCS is necessary to enable implementation. The study can contribute to the effective implementation in this unit and point out guidelines for its implementation in other similar contexts.

RESUMEN

The current Primary Health Care reform is focused on effectiveness patterns, tar-geted at ensuring the best possible health gains for its users. In regard to this ap-proach the aim is to assess health gains as a result of the implementation of the Dynamic Model for Assessment and Family Intervention. For this quantitative study, the focus of attention areas described in the opera-tive dimensions of this Nursing Model. Data collection was performed based on the information produced by Primary Health Care Nurses, through the Information System in use. The family health gains indexes computed into the Microsoft Office Excel 2007 were used for data processing and analysis. In the structural dimension, the major health gains were found in the residential house (50%). In the development dimension, the interventions targeted at family planning were found effective for 85.19% of the families, with the adaptation to pregnancy showing the lowest rates in health gains (50%). As to the functional di-mension, the highest rates in health gains were found for an adequate caregiver role in 33% of the families, whilst in the family process, health gains rates were at 5.56%. The implementation of the MDAIF had a positive impact on health gains outco-mes for families, fostering the development of assessment and family intervention competencies as well as the identification of training needs in specific intervention areas

Introduction: In the current health care environment, the needs of the population provided the perfect opportunity for nurses to (re)define their practice and professional career. Within this framework, this study aimed thus at identifying the bases that are underlying the practice, as well as the factors that stimulate or jeopardise the quality of care and a practice that is consistent with the desired grounding. Method: Qualitative study with a phenomenological nature carried out in 19 public hospitals in mainland Portugal with the participation of 56 nurses. The tool used for collecting data was a semi-structured interview. Results: Regarding the factors that stimulate or jeopardise the quality of care a prac-tice that is consistent with the desired grounding, we could highlight features that were perfectly integrated in the triad structure, process and result. In relation to "structure", we could point out the following: organizational resources, human and material resources, service organization, nursing sustainment practices and organization of nursing care. Concerning "process", we could analyse: decision-making process, guiding principles for the professional practice, scientific methodology of health care service, health care documentation process, communication process, collaborative practice and management practice. In what concerns "result", the features were less evident in the participants' speech, and more stressed among clients and nurses. Conclusion: The explanatory approach to the grounding of the professional nursing practice, outlined in a three-dimensional perspective, makes clear the theoretical framework that grounds the practice, as well as the factors in hospital environment, that stimulate or jeopardise the quality of the nursing care

El objetivo de este estudio fue evaluar el grado de precisión de los diagnósticos de enfermería de pacientes ingresados en un hospital universitario. Es una investigación documental, transversal, retrospectiva con análisis cuantitativo. Los datos fueron recogidos a partir de los registros médicos de pacientes hospitalizados en el servicio de neurología de un hospital universitario en la región oeste del estado de Paraná, Brasil. Se evaluaron 292 historias clínicas, tras el análisis de los criterios de inclusión y exclusión, 12 (4,1%) fueron parte del estudio. Se identificaron 19 diagnósticos de enfermería, documentado 94 veces. La evaluación de la exactitud del diagnóstico de enfermería se realizó a través de EADE-2. La mayoría de los diagnósticos de enfermería (n=88; 93,6%) fue evaluada como muy precisa. El grado de precisión más frecuente fue de 13,5 (n=87; 92,6%) y el grado de precisión promedio fue de 12,9, con una variación de 2 a 13,5. Se concluye que los diagnósticos de enfermería son muy precisos, pero es notoria la carencia en la aplicación del proceso de enfermería en la institución investigada


O estudo objetivou avaliar o grau de acurácia dos diagnósticos de enfermagem de pacientes internados em um hospital universitário. Trata-se de uma pesquisa documental, transversal, retrospectiva com análise quantitativa. Os dados foram coletados de prontuários de pacientes internados no setor de neurologia de um hospital de ensino da região oeste do Paraná, Brasil. Foram avaliados 292 prontuários, após a análise dos critérios de inclusão e exclusão, 12 (4,1%) fizeram parte do estudo. Foram identificados 19 diagnósticos de enfermagem, documentados 94 vezes. A avaliação da acurácia dos diagnósticos de enfermagem foi feita por meio EADE-2. A maioria dos diagnósticos de enfermagem (n=88; 93,6%) foi avaliada como sendo altamente acurada. O grau de acurácia mais frequente foi 13,5 (n=87; 92,6%) e a média do grau de acurácia foi 12,9, com variação de 2 a 13,5. Conclui-se que os diagnósticos de enfermagem analisados foram altamente acurados, porém é notória a carência na realização do processo de enfermagem na instituição pesquisada


The study aimed to evaluate the degree of accuracy of the nursing diagnoses of patients hospitalized in a university hospital. This is a documentary, cross-sectional and retrospective research with quantitative analysis. Data were collected from records of patients hospitalized in the neurology sector of a teaching hospital in the western region of Paraná, Brazil. 292 patients' records were evaluated and after the analyses of inclusion and exclusion criteria, 12 (4.1%) were part of the study. Nineteen nursing diagnoses were identified, documented 94 times. The evaluation of the accuracy of the nursing diagnoses was done using EADE-2. Most of nursing diagnoses (n = 88, 93.6%) were evaluated as highly accurate. The most frequent degree of accuracy was 13.5 (n = 87; 92.6%) and the mean accuracy was 12.9, with a variation from 2 to 13.5. It is concludedthat the nursing diagnoses analyzed were highly accurate, but it's notorious the lack of nursing process at the institution studied

RESUMEN

Objetivo: Validar el contenido de las actividades de atención al anciano desarrolladas en los centros de día. Métodos: Estudio metodológico con validez de contenido realizada por nueve especialistas, según los atributos relevancia, claridad, pertinencia y simplicidad. El índice de consenso establecido correspondió al 80%. La recopilación de datos se llevó a cabo entre septiembre y noviembre de 2014. Resultados: Ocho actividades fueron validadas y dos incluidas. En la dimensión salud fueron: capacidad funcional y fragilidad, alimentación y nutrición, higiene y confort, medicación continuada, manejo y estímulo cognitivo; en participación: actividades culturales, acogida y supervisión, apoyo espiritual/religioso; en seguridad, accesibilidad y control de factores de riesgo de caída y en educación, actividades educativas. De los 82 ítems evaluados, el 100% alcanzó el consenso respecto a la relevancia; el 91,4% respecto a la claridad; el 97,5% respecto a la pertinencia, el 92,6% respecto a la simplicidad. Conclusión: Creemos que la validez y la sistematización de las actividades contribuirán en el proceso de evaluación de la calidad en esta modalidad de atención

Métodos: Estudo metodológico, com validação de conteúdo realizada por nove especialistas, segundo os atributos relevância, clareza, pertinência e simplicidade. O índice de consenso estabelecido correspondeu a 80%. A coleta de dados ocorreu entre setembro e novembro de 2014 Resultados: Oito atividades foram validadas e duas incluídas. Na dimensão saúde, foram: capacidade funcional e fragilidade, alimentação e nutrição, higiene e conforto, medicamentos de uso contínuo, manejo e estímulo cognitivo; na participação as atividades socioculturais, acolhimento e monitoramento, apoio espiritual/religioso; na segurança, acessibilidade e controle dos fatores de risco para queda e na educação, as atividades educativas. Dentre 82 itens avaliados, 100% atingiram o consenso quanto à relevância; 91,4% à clareza; 97,5% à pertinência; 92,6% à simplicidade. Conclusão: Acreditamos que a validação e a sistematização das atividades contribuirão para subsidiar a avaliação da qualidade nessa modalidade de atenção


Objective: Validate the content of care activities for elderly developed in day-care centers. Conclusion: We believe that validation and systematization of activities contribute to support the assessment of quality in this type of attention

Introducción: Existe un problema de sobrecarga asistencial en el ámbito de la Atención Primaria (AP), al tiempo que las enfermeras han aumentado progresivamente sus competencias para el abordaje de problemas habituales en AP, de modo que es ya una realidad la enfermera que gestiona la demanda. Por ello, el objetivo de esta revisión es identificar la mejor evidencia disponible sobre la gestión enfermera de la demanda de pacientes no programados en AP y su eficacia, y aportar recomendaciones para la mejora de los servicios de AP. Métodos: Scoping review. Forma de síntesis de la evidencia disponible en un área determinada. Su objetivo es realizar un mapa del conocimiento respondiendo a la pregunta: ¿Qué alcance tiene el conocimiento en este campo?. Resultados: Las enfermeras en el ámbito de la AP pueden abordar y resolver la mayoría de los problemas de carácter leve, de manera autónoma y eficiente, utilizando guías o protocolos de intervenciones junto a una adecuada formación. Conclusiones: Los resultados representan una oportunidad para el sistema sanitario por la necesidad de abordar la cuestión de la presión asistencial, y sitúan a la enfermera como una puerta de entrada eficaz y eficiente ante los problemas agudos de pacientes en AP


Introduction: There is a problem of health care overload in Primary Care (PA), due to the increase in demand by the population and a reduced workforce of health workers. Simultaneously, nursing professionals have increased their competencies to deal with common PA problems, and the nurse who manages the demand is already a reality. Therefore, the objective of this review is to identify the best available evidence on the nurse management of the demand for non-programmed patients and its effectiveness, and, together, to elaborate recommendations for the improvement of PA services. Method: Scoping review. Form of synthesis of the available evidence in a certain area. Its objective is to map knowledge by answering the question: What is the scope of knowledge in this field?. Results: The main result of this study is that nurses in the field of PA can address and solve most problems of a mild nature, autonomously and efficiently using interventions guidelines or protocols together with training. Conclusions: The results represent an opportunity for the health system because of the need to address the question of the pressure on PA. The recommendation to investigate this issue is of great importance since nursing is shown as an effective and efficient gateway to the acute problems of PA patients

RESUMEN

Importance: Complex medication regimens pose self-management challenges, particularly among populations with low levels of health literacy. Objective: To test medication management tools delivered through a commercial electronic health record (EHR) with and without a nurse-led education intervention. Design, Setting, and Participants: This 3-group cluster randomized clinical trial was performed in community health centers in Chicago, Illinois. Participants included 794 patients with hypertension who self-reported using 3 or more medications concurrently (for any purpose). Data were collected from April 30, 2012, through February 29, 2016, and analyzed by intention to treat. Interventions: Clinics were randomly assigned to to groups: electronic health record-based medication management tools (medication review sheets at visit check-in, lay medication information sheets printed after visits; EHR-alone group), EHR-based tools plus nurse-led medication management support (EHR plus education group), or usual care. Main Outcomes and Measures: Outcomes at 12 months included systolic blood pressure (primary outcome), medication reconciliation, knowledge of drug indications, understanding of medication instructions and dosing, and self-reported medication adherence. Medication outcomes were assessed for all hypertension prescriptions, all prescriptions to treat chronic disease, and all medications. Results: Among the 794 participants (68.6% women; mean [SD] age, 52.7 [9.6] years), systolic blood pressure at 12 months was greater in the EHR-alone group compared with the usual care group by 3.6 mm Hg (95% CI, 0.3 to 6.9 mm Hg). Systolic blood pressure in the EHR plus education group was not significantly lower compared with the usual care group (difference, -2.0 mm Hg; 95% CI, -5.2 to 1.3 mm Hg) but was lower compared with the EHR-alone group (-5.6 mm Hg; 95% CI, -8.8 to -2.4 mm Hg). At 12 months, hypertension medication reconciliation was improved in the EHR-alone group (adjusted odds ratio [OR], 1.8; 95% CI, 1.1 to 2.9) and the EHR plus education group (adjusted odds ratio [OR], 2.0; 95% CI, 1.3 to 3.3) compared with usual care. Understanding of medication instructions and dosing was greater in the EHR plus education group than the usual care group for hypertension medications (OR, 2.3; 95% CI, 1.1 to 4.8) and all medications combined (OR, 1.7; 95% CI, 1.0 to 2.8). Compared with usual care, the EHR tools alone and EHR plus education interventions did not improve hypertension medication adherence (OR, 0.9; 95% CI, 0.6-1.4 for both) or knowledge of chronic drug indications (OR for EHR tools alone, 1.0 [95% CI, 0.6 to 1.5] and OR for EHR plus education, 1.1 [95% CI, 0.7-1.7]). Conclusions and Relevance: The study found that EHR tools in isolation improved medication reconciliation but worsened blood pressure. Combining these tools with nurse-led support suggested improved understanding of medication instructions and dosing but did not lower blood pressure compared with usual care. RESUMEN

OBJECTIVE: To analyze the single therapeutic project and the nursing process regarding its specificities and intersection points from the interdisciplinary care perspective. METHODIntegrative review of the literature from articles available in the Lilacs, SciELO, MEDLINE and PubMed databases, published in Portuguese, English and Spanish from 2005 to 2015. RESULTS: It was identified 23 articles. From these, 17 on the nursing process, six on the single therapeutic project and one about multiprofessional residency. From the analysis, their specificities and intersection points that describe the alignment and similarities between them were identified in the primary and mental health cares. CONCLUSIONS: The single therapeutic project and the nursing process are aligned in health practices in primary and mental health cares. The multiprofessional residency allows this alignment among them, and the nurse contributes to the interdisciplinary care with the nursing process.

The project's main areas of focus are then rolled out through actions prioritising the bold, innovative dimensions of caregiving. This project highlights the skills of many nurses who demonstrate high levels of creativity in their nursing practices and management of patients.
