RESUMEN

Objetivo: identificar evidências acerca do uso seguro da hipotermia terapêutica em recém-nascidos. Método: revisão integrativa realizada entre junho e julho de 2018, em fontes eletrônicas da Biblioteca Virtual de Saúde e PubMed, por meio da pergunta:"Que evidências podem subsidiar o cuidado de enfermagem voltado para a redução de sequelas em recém-nascidos submetidos à hipotermia terapêutica?".Foram eleitos nove artigos para análise, sendo oito internacionais e um nacional. Resultados:o resfriamento deve acontecer por 72 horas, com hipotermia leve. As indicações para inclusão no protocolo foram: primeiras seis horas de vida, idade gestacional maior que 35 semanas e acidose na primeira hora de vida.São cuidados essenciais: monitoração hemodinâmica, observação da pele, controle térmico retal, vigilância do Eletroencefalograma de Amplitude Integrada. Conclusão: a terapêutica apresenta benefícios, porém sua aplicação depende de protocolo institucional e treinamento das equipes com foco nas potenciais complicações.


Objective: to identify the evidence on safe use of therapeutic hypothermia in newborns. Method: integrative review of the literature, conducted between June and July of 2018, in electronic sources from the Virtual Health Library and PubMed, through the question: "What evidence can support nursing care aimed at reducing sequelae in newborns undergoing therapeutic hypothermia?". Analysis was conducted for nine selected article, being eight from international literature and one from Brazilian national literature. Results: cooling should occur for 72 hours with mild hypothermia. Indications for inclusion in the protocol were: first six hours of life, gestational age greater than 35 weeks and acidosis in the first hour of life. Essential care includes hemodynamic monitoring, skin observation, rectal thermal control, Integrated Amplitude Electroencephalogram surveillance. Conclusion: the therapy has benefits, but its application depends on institutional protocol and team training focusing on potential complications.


Objetivo: identificar la evidencia sobre el uso seguro de la hipotermia terapéutica en recién nacidos. Método: revisión integradora de la literatura, realizada entre junio y julio de 2018, en fuentes electrónicas de la Biblioteca Virtual de Salud y PubMed, a través de la pregunta: "¿Qué evidencia puede apoyar la atención de enfermería dirigida a reducir las secuelas en los recién nacidos que sufren hipotermia terapéutica?". Se realizaron análisis para nueve artículos seleccionados, ocho de literatura internacional y uno de literatura nacional brasileña. Resultados: el enfriamiento debe ocurrir durante 72 horas con hipotermia leve. Las indicaciones para la inclusión en el protocolo fueron: primeras seis horas de vida, edad gestacional mayor de 35 semanas y acidosis en la primera hora de vida. El cuidado esencial incluye monitoreo hemodinámico, observación de la piel, control térmico rectal, vigilancia integrada de electroencefalograma de amplitud. Conclusión: la terapia tiene beneficios, pero su aplicación depende del protocolo institucional y del entrenamiento del equipo, enfocándose en posibles complicaciones.

RESUMEN

Objetivo: conhecer as concepções do enfermeiro frente à utilização de protocolos de urgência psiquiátrica no atendimento préhospitalar móvel. Métodos: estudo qualitativo, descritivo exploratório, com enfermeiros do Serviço de Atendimento Móvel de Urgência. Os dados foram coletados por meio de entrevista semiestruturada e submetidos à análise de conteúdo. Resultados: foram elencadas duas categorias: Protocolos de atendimento de urgência psiquiátrica e seu emprego no serviço de atendimento pré-hospitalar; e Fatores que influenciam o atendimento de urgência e emergência psiquiátrica. Os protocolos se direcionam contra os preceitos da reforma psiquiátrica e colocam sua aplicabilidade como fator de interferência no manejo da pessoa em crise, dando continuidade à ação ideológica social da contenção física. Considerações finais: o estudo mostrou o contexto da prática dos enfermeiros nas urgências psiquiátricas e evidenciou que os enfermeiros do Serviço de Atendimento Móvel de Urgência não estão devidamente instrumentalizados para atuarem nas urgências/emergências psiquiátricas.


Objective: to ascertain nurses' conceptions regarding the use of psychiatric emergency protocols in mobile pre-hospital care. Methods: in this qualitative, descriptive exploratory study of nurses in a Mobile Emergency Care Service, data were collected through semi-structured interviews and submitted to content analysis. Results: two categories were listed: Psychiatric emergency care protocols and their use in the pre-hospital care service; and Factors that influence emergency care and psychiatric emergency. The protocols are at odds with the tenets of psychiatric reform and frame its applicability as a factor that interferes with management of persons in crisis, thus serving to perpetuate the ideological social action of physical restraint. Final considerations: the study showed the context of nurses' practice in psychiatric emergencies and showed that nurses of the Mobile Emergency Care Service are not properly equipped to work in psychiatric emergencies.


Objetivo: conocer las concepciones de las enfermeras sobre el uso de protocolos de emergencia psiquiátrica en la atención prehospitalaria móvil. Métodos: en este estudio cualitativo, descriptivo, exploratorio de enfermeras en un Servicio Móvil de Atención de Emergencias, los datos fueron recolectados a través de entrevistas semiestructuradas y sometidos a análisis de contenido. Resultados: se enumeraron dos categorías: protocolos de atención de urgencias psiquiátricas y su uso en el servicio de atención prehospitalaria; y Factores que influyen en la atención de emergencia y la emergencia psiquiátrica. Los protocolos están en desacuerdo con los principios de la reforma psiquiátrica y enmarcan su aplicabilidad como un factor que interfiere con el manejo de las personas en crisis, sirviendo así para perpetuar la acción social ideológica de la restricción física. Consideraciones finales: el estudio mostró el contexto de la práctica de las enfermeras en emergencias psiquiátricas y mostró que las enfermeras del Servicio Móvil de Atención de Emergencias no están debidamente equipadas para trabajar en emergencias psiquiátricas.

Objetivo: apresentar atualizações para a ressuscitação cardiopulmonar em pacientes suspeitos e confirmados com COVID-19. Método: revisão compreensiva da literatura, com síntese narrativa das evidências de diretrizes e recomendações da Organização Mundial de Saúde, Associação de Medicina Intensiva Brasileira, American Heart Association, Resuscitation Council UK, American College of Surgions Committee on Trauma e National Association of Emergency Medical Technicians. Resultados: as principais atualizações trazem informações sobre especificidades das manobras de ressuscitação cardiopulmonar; preparação do ambiente, recursos humanos e materiais, reconhecimento da parada cardiorrespiratória e ações iniciais; estratégias de ventilação e acesso invasivo da via aérea; ajustes do ventilador mecânico e manobras de ressuscitação cardiopulmonar em pacientes pronados. Considerações finais: profissionais de saúde envolvidos no atendimento à parada cardiorrespiratória de pacientes suspeitos e/ou confirmados com COVID-19 podem encontrar inúmeros desafios, portanto devem seguir com rigor o protocolo estabelecido para maximizar a efetividade das manobras de ressuscitação e minimizar o risco de contágio pelo vírus e sua disseminação.


Objective: to present updates for cardiopulmonary resuscitation in suspected and confirmed patients with COVID-19. Method: comprehensive literature review with narrative synthesis of the evidence of guidelines and recommendations from World Health Organization, Associação de Medicina Intensiva Brasileira, American Heart Association, Resuscitation Council UK, American College of Surgions Committee on Trauma and National Association of Emergency Medical Technicians. Results: the main updates bring information about the specifics of cardiopulmonary resuscitation maneuvers; preparation of the environment and human and material resources, recognition of cardiorespiratory arrest and initial actions; ventilation and invasive airway access strategies; mechanical ventilator adjustments and cardiopulmonary resuscitation maneuvers in patients in the prone position. Final considerations: health professionals involved in the care of cardiorespiratory arrest of suspected and/or confirmed patients with COVID-19 can face numerous challenges, so they must strictly follow the protocol established to maximize the effectiveness of resuscitation maneuvers and minimize the risk of contagion by the virus and its spread.


Objetivo: apresentar actualizaciones para la reanimación cardiopulmonar en pacientes sospechos os y confirmados con COVID-19. Método: revisión exhaustiva de la literatura con síntesis narrativa de la evidencia de guías y recomendaciones de la Organización Mundial de la Salud, Associação de Medicina Intensiva Brasileira, American Heart Association, Resuscitation Council UK, American College of Surgions Committee on Trauma and National Association of Emergency Medical Technicians. Resultados: las principales actualizaciones aportan información sobre los detalles de las maniobras de reanimación cardiopulmonar; preparación del medio ambiente y recursos humanos y materiales, reconocimiento de paro cardiorrespiratorio y acciones iniciales; estrategias de ventilación y acceso invasivo a las vías aéreas; ajustes del ventilador mecánico y maniobras de reanimación cardiopulmonar en pacientes en decúbito prono. Consideraciones finales: los profesionales de la salud involucrados en la atención del paro cardiorrespiratorio de pacientes sospechosos y/o confirmados con COVID-19 pueden enfrentar numerosos desafíos, por lo que deben seguir estrictamente el protocolo establecido para maximizar la efectividad de las maniobras de reanimación y minimizar el riesgo de contagio por el virus y supropagación.

60 patients of the Narcological Clinic "Neogene" were enrolled into the study. The following participation criteria were used: a diagnosis of mental and behavioral disorders associated with ephedrone use; withdrawal state. Toxic encephalopathy; age - 20-65 years; gender - male. Patients were selected and diagnosed for the study according to ICD-10 criteria. The study was conducted in compliance with bioethical principles, based on informed consent. The main study group (Group I) consisted of 45 patients and the control group (Group II) - of 15 patients. The study strictly observed anonymity and confidentiality of the participants. Thiogamma drug (alpha-lipoic acid, meglumine salt, manufacturer WÖRWAG PHARMA) with a daily dose of 600 mg was added for a duration of one month, to the standard treatment regimen of the patients of the Group I in both inpatient and outpatient settings. The patients (both inpatient and outpatient) in the Group II were treated with placebo drug along with standard treatment for the same period. Clinico-neurological disorders of the patients were evaluated before and after a one-month therapy course. Neurological examination and Unified Parkinson Disease Rating Scale - UPDSR - were used. Based on the study materials it can be reported that the users of homemade ephedron group psychostimulants ("Jeff") had neurological disorders, mainly manifested with the symptoms of Parkinson's disease; with well-expressed dystonia, postural instability, pseudobulbar and vegetative syndromes. As a result of the treatment, a decrease in the scores on the UPDSR was observed (improved condition) in patients of both groups, however, the improvement was greater expressed in the patients of Group I where Thiogamma (alpha-lipoic acid meglumine salt) drug was added to the standard treatment protocol. The above results clearly speak to the effectiveness of "Thiogamma" in the treatment of neurological disorders caused by the use of homemade psychostimulants ("Jeff").

In-hospital resuscitation practices have changed by necessity in the Covid-19 era, principally due to precautions intended to protect caregivers from infection. This has resulted in serious delays in resuscitation response.ILCOR has recently modified its guidelines to separate defibrillation from other interventions, recognizing that shock success is extremely time-dependent and that defibrillation poses relatively little risk of Covid-19 transmission. The new recommendation calls for sending one caregiver into the isolation room in order to initiate bedside monitoring and defibrillate if indicated, while the code team is donning their personal protective equipment. Implementing this change requires focused training in that specific role. This can be accomplished by intensively training a subset of clinical staff to assume the responsibility and act without hesitation when a code occurs.Focused defibrillation training promises to avoid compromising the care of patients experiencing tachyarrhythmic arrests in the setting of Covid-19. Such a training program might even result in better survival than before the pandemic for this subset of patients.

SARS-CoV-2 shares nearly 80% of its' genomic sequence with SARS-CoV and MERS-CoV, both viruses known to cause respiratory symptoms and liver impairment. The emergence of pediatric cases of multisystem inflammatory syndrome related to the SARS-CoV-2 infection (PIM-TS) has raised concerns over the issue of hepatic damage and liver enzyme elevation in the critically ill pediatric population with COVID-19. Some retrospective cohorts and case series have shown various degrees of ALT/AST elevation in SARS-CoV-2 infections. A limited number of liver histopathological studies are available that show focal hepatic periportal necrosis. This liver damage was associated with higher levels of inflammatory markers, C-reactive protein (CRP), and pro-calcitonin. Proposed pathophysiological mechanisms include an uncontrolled exacerbated inflammatory response, drug-induced liver injury, direct viral infection and damage to cholangiocytes, hypoxic-ischemic lesions, and micro-thrombosis in the liver. Based on the physiopathological characteristics described, our group proposes a clinical protocol for the surveillance, evaluation, management, and follow-up of critically ill pediatric COVID-19 patients with liver damage.

Generalized parodontal diseases (GPD) consistently occupy one of the leading places in the structure of dental diseases. Early diagnosis of the initial degree of generalized parodontitis (GP) is an effective way of secondary prevention. This is due to the complexity of understanding the etio-pathogenetic mechanisms of the development generalized parodontal diseases (GPD) and the high association of them with a number of diseases of the internal organs and systems with common points of contact between interdependence and mutual influence, in particular with anorexia nervosa (AN). The aim of this research was to develop a protocol for the treatment of GP in patients with AN. The object - 60 patients (mean age 26±3.8 years), with a diagnosis of GP, I-II degree, chronic, and AN, restrictive, which by simple randomization were divided into three groups (randomized by sex, age of patients, underlying and comorbidy diagnosis) to study the clinical effectiveness of our proposed method. Clinical, radiological, hygienic, immunological, biochemical, psychological and statistical methods were used. Conclusions. Thus, as a result of the proposed treatment protocol, the largest number of satisfactory treatment results was observed in group III patients (85.0±8.0%) with the inclusion of drugs that affect the pathogenetic mechanisms of the disease, including normalization of local immunity, markers of decline oxidative-antioxidant stress, radiological data of normalization of bone tissue of the alveolar process, a tendency to reduce microbial and tissue sensitization.
