RESUMEN

Studying the use of femtosecond laser in patients with complicated cataracts remains a topical issue in phaco surgery. PURPOSE: To comparatively analyze the results of using hybrid (femtosecond laser-assisted) phacoemulsification in hypermature cataract in patients with lens subluxation. MATERIAL AND METHODS: The study included 78 patients (78 eyes) with hypermature cataract in combination with lens subluxation of I-II degree. In the 1st group (45 eyes) capsulorhexis was performed using a femtosecond laser. In the 2nd group (33 eyes) manual capsulorhexis was performed. RESULTS: On the first day after surgery, best corrected visual acuity of 0.8-1.0 was obtained in 57.8% of cases in the 1st group and 33.3% in the 2nd group. Transient corneal edema during the first 1-3 days was noted in 4.4% of cases in the 1st group and in 15.2% - in the 2nd group of patients. Pachymetry in the center of the cornea showed an increase in its thickness on the 1st day after surgery from 0.564±0.027 mm to 0.591±0.033 mm in the 1st group and from 0.561±0.023 mm to 0.637±0.043 mm (p<0.05) in the 2nd group of patients. The loss of corneal endothelial cells amounted to 10.9±1.8% in the 1st group and 17.4±2.9% in the 2nd group (p<0.05). Macular edema with decreased visual acuity confirmed by OCT was detected in the 1st group of patients in 2.2% of cases, in the 2nd group - in 9.1% of cases. With a follow-up period of up to 3 years, IOL optics decentration by more than 1 mm was detected in 2.2% of cases in the 1st group of patients and in 12.1% of cases in the 2nd group. CONCLUSION: Femtosecond laser-assisted capsulorhexis significantly improves the results of surgery in patients with hypermature cataract combined with lens subluxation.

PURPOSE: To evaluate the first outcomes of non-contact navigated laser retinopexy for peripheral tears and rhegmatogenous degenerations of the retina. MATERIAL AND METHODS: This prospective single-center interventional pilot study included 58 patients (68 eyes), among which 19 males and 39 females aged 47.5±16.9 years with peripheral retinal degenerations and symptomatic retinal tears who received non-contact laser retinopexy with the Navilas 577s navigated laser system and non-contact PRP widefield objective. All patients (68 eyes) underwent laser treatment for the following disorders: horseshoe tear in 13 eyes, retinal hole in 11 eyes, operculated retinal hole in 3 eyes, lattice degeneration in 19 eyes, snail track degeneration in 1 eye, vitreoretinal tufts in 16 eyes, and progressing bullous retinoschisis in 5 eyes. Pain intensity was assessed using a four-point verbal rating scale. RESULTS: All patients were followed up for 5 to 21 months (9.0±3.3 months on average). Stable condition of the retina was observed in 65 cases (95.5%). In 3 eyes (4.4%), newly formed retinal tears prompted additional laser treatment: horseshoe tear in 2 eyes and retinal hole in 1 eye. The mean pain score during non-contact navigated laser retinopexy was 0.9±0.5. No complications associated with laser treatment were observed. CONCLUSION: Non-contact navigated laser retinopexy performed with the Navilas 577s navigated laser system is a safe and well-tolerated procedure with effectiveness comparable to conventional laser retinopexy.

The morphofunctional state of erythrocytes significantly changes by the onset and progression of various diseases, as well as in critical conditions, injuries, accompanied by severe tissue damage. This leads to a violation of microcirculation and has an effect on hemorheology in general. In connection with this, there is a need for the development of adequate methods for assessing the functional activity of erythrocytes. The aim of the work was to study the possibilities of laser interference microscopy in the analysis of the functional state of erythrocytes. The results allowed us to quantify the state of the phase height and phase diameter of erythrocytes. Analysis of the surface nanostructure gave a qualitative assessment of the functional activity of cells. The following patterns were revealed: the action of adrenaline causes the appearance of echinocytes with an increase in phase height and decrease phase diameter and the appearance of spicules on the surface of cells. The action of cortisol leads to a decrease in the phase height and the phase diameter with the appearance of numerous loosening of the structure of the cell surface. Moreover, the traditional method of analyzing the distribution of erythrocytes in a population by morphology (discocytes, stomatocytes, and degeneratively altered cell forms) did not reveal statistically significant differences. Thus, the results indicate that the interference microscopy method allows a qualitative assessment of the morphofunctional activity of native cells. This increases the information content of the analysis and leads to the objectification of data on the functional capabilities of erythrocytes.

BACKGROUND: Trans-sacral epiduroscopic laser decompression (SELD) using slender epiduroscope and a holmium YAG laser is one of the minimally invasive surgical options for lumbar disc herniation. However, the learning curve of SELD and the effect of surgical proficiency on clinical outcome have not yet been established. We investigated patients with lumbar disc herniation undergoing SELD to report the clinical outcome and learning curve. METHODS: Retrospective analysis of clinical outcome and learning curve were performed at a single center from clinical data collected from November 2015 to November 2018. A total of 82 patients who underwent single-level SELD for lumbar disc herniation with a minimum follow-up of 6.0 months were enrolled. Based on the findings that the cut-off of familiarity was 20 cases according to the cumulative study of operation time, patients were allocated to two groups: early group (n = 20) and late group (n = 62). The surgical, clinical, and radiological outcomes were retrospectively evaluated between the two groups to analyze the learning curve of SELD. RESULTS: According to linear and log regression analyses, the operation time was obtained by the formula: operation time = 58.825-(0.181 × [case number]) (p < 0.001). The mean operation time was significantly different between the two groups (mean 56.95 min; 95% confidence interval [CI], 49.12-64.78 in the early group versus mean 45.34 min; 95% CI, 42.45-48.22 in the late group; p = 0.008, non-parametric Mann-Whitney U test). Baseline characteristics, including demographic data, clinical factors, and findings of preoperative magnetic resonance imaging, did not differ between the two groups. Also, there was no significant difference in terms of surgical outcomes, including complication and failure rates, as well as clinical and radiological outcomes between the two groups. CONCLUSION: The learning curve of SELD was not as steep as that of other minimally invasive spinal surgery techniques, and the experience of surgery was not an influencing factor for outcome variation.

The treatment of the moderate to advanced resorbed edentulous maxilla with a fixed prosthesis is complicated due to the presence of pneumatized maxillary sinuses posteriorly and the nasal aperture in the premaxilla. To treat patients with this condition using endosseous implants to support a functional prosthesis, grafting procedures have been advocated. The multiple surgical steps involved in the reconstruction of the resorbed maxilla, however, can be a significant barrier to patient acceptance of grafting treatment protocols. The success of graftless concepts using tilted and zygomatic implants has led to a higher case acceptance as the treatment can be accomplished in one appointment. Over the past two decades, the experience clinicians have gained using zygomatic implants has resulted in a redesign of the implant aimed at addressing the challenges associated with the treatment of patients with moderate to advanced resorbed maxillae to better serve these patients.

Over the last year, urologic progress remains driven by the quick technologic evolution, with a focus on Laser and robotics. The latter appears to potentially contribute to the drift towards ambulatory surgery, in particular for distinct sub-populations harbouring kidney or prostate cancer. The Da Vinci robot appears also to increase the drive towards minimally invasiveness with respect to female incontinence and the laparoscopic placement of an artificial urethral sphincter. Last, a new Laser is in the midst to offer its polyvalence in terms of endoscopic stone surgery, so as that of benign prostatic hyperplasia. Facing this technologic pressure, it remains of utmost importance to scrupulously validate on a short so as longer-term basis the true hoped advantages.

Implant dentistry has emerged as a first line of treatment to replace missing teeth for both the edentulous and partially dentate patients. Implant dentistry is accompanied by the onset of peri-implantitis (PIM). PIM is characterized by the inflammatory destruction of the implant-supporting tissues, because of biofilm formation on the implant surface. A history of periodontitis, poor oral hygiene, and smoking are considered as risk factors for PIM. Occasionally PIM is associated with iatrogenic factors, that, only recently, have been acknowledged as direct cause of PIM, i.e.: non-parallel adjacent implants or the presence of a gap, between fixture and prosthetic components. The use both of traditional protocols of nonsurgical periodontal therapy and the laser seems to be an effective alternative treatment modality for PMI. By the application of laser-assisted non-surgical peri-implant therapy the periodontal pocket depth was reduced. The present article illustrates the nonsurgical management of one case, where failure to remove residual cement, from an implant-supported dental prosthesis, seemed to cause PMI.

For over two decades photoacoustic imaging has been tested clinically, but successful human trials have been limited. To enable quantitative clinical spectroscopy, the fundamental issues of wavelength-dependent fluence variations and inter-wavelength motion must be overcome. Here we propose a real-time, spectroscopic photoacoustic/ultrasound (PAUS) imaging approach using a compact, 1-kHz rate wavelength-tunable laser. Instead of illuminating tissue over a large area, the fiber-optic delivery system surrounding an US array sequentially scans a narrow laser beam, with partial PA image reconstruction for each laser pulse. The final image is then formed by coherently summing partial images. This scheme enables (i) automatic compensation for wavelength-dependent fluence variations in spectroscopic PA imaging and (ii) motion correction of spectroscopic PA frames using US speckle tracking in real-time systems. The 50-Hz video rate PAUS system is demonstrated in vivo using a murine model of labelled drug delivery.

This study characterizes the temporal and spectral property of the mechanical impulsive waves produced by a clinical ballistic shock wave therapy device. The impulsive waves were generated by a projectile impacting a metallic shock wave transmitter, and were measured using a laser vibrometer that detects vibrations on the front surface of the shock wave transmitter. The shock wave transmitter considered in the measurement has a circular flat surface tip with a diameter of 15 mm. The results showed that the measured impulsive wave was resonated at 93.4 kHz and modulated with a relatively low frequency of 3.7 kHz to form a heavily damped pulse wave lasting for longer than 2 ms. Multiple impulsive waves produced at high output settings, were observed for the first time, and their clinical implication needs to be elucidated in the future study. Despite measurements made on the cavitation free air born impulsive waves, the study was justified by the experimental clarification that their temporal and spectral properties were characteristically similar to those of the impulsive waves produced in water, more representative to clinical conditions. The present results are expected to deliver critical information for assessing the shock wave dose delivered from a clinical radial extracorporeal shock wave device to patients.

As a nutritious and popular seafood among consumers, Sargassum fusiforme is susceptible to the toxic heavy metals because of its strong adsorption properties. In this study, laser-induced breakdown spectroscopy (LIBS) coupled with a simple framework (only remove some noise and low-intensity variables, and then combine with PLS algorithm) was used to establish the detection models to simultaneously and quantitatively analyze the content of heavy metals arsenic (As), chromium (Cd), cadmium (Cr), copper (Cu), mercury (Hg), lead (Pb) and zinc (Zn) in Sargassum fusiforme. As comparisons, three classic variable methods of successive projections algorithm (SPA), uninformative variable elimination (UVE) and variable importance in projection (VIP) were adopted. The final results showed that six of seven heavy metal models from the TV-PLSR model were optimal. These results demonstrate that the TV-PLSR framework combined with LIBS technique is an effective framework for quantitatively analyzing the heavy metals in Sargassum fusiforme.

Laser-induced breakdown spectroscopy (LIBS) was used to rapidly detect heavy metals in mulberry leaves. For the purpose of increasing detection stability and accuracy, a novel analysis framework consisting of a Kohonen self-organizing map (SOM), a variable selection method using the successive projection algorithm (SPA) and uninformative variable elimination (UVE), and a consensus modeling strategy was proposed for processing LIBS data to determine copper (Cu) and chromium (Cr) content. Results showed that the best regression model for Cu and Cr content achieved the residual predictive deviation (RPD) values of 10.0494 and 8.3874, respectively, and root mean square error of prediction (RMSEP) values of 110.4550 and 41.4561, respectively. The proposed strategy provides a high-accuracy and rapid alternative to the traditional method for monitoring heavy metals in mulberry leaves, which could guarantee the quality of mulberry leaves and potentially be used in food-related industries.

Discharge of heavy metals from various sources of industrial wastewater poses significant environmental and health concerns. Thus, efficient recovery of precious metals from wastewater employing sustainable, rapid, and cost-effective treatment methods is highly desirable. In this work, palladium nanoparticles (Pd NPs) were successfully recovered from industrial wastewater using a pulsed laser process in the absence of additives or reducing agents. Notably, the developed approach is faster and more environmentally friendly than other conventional recovery methods. The recovered Pd NPs were characterized by X-ray powder diffraction (XRD), field emission scanning electron microscopy (FE-SEM), transmission electron microscopy (TEM), and inductively coupled plasma optical emission spectroscopy (ICP-OES). Various pulsed laser parameters (i.e., laser wavelength, power, and irradiation time) were optimized to obtain ideal conditions for the pulsed laser ablation process. Effective recovery of the Pd metal from industrial wastewater was achieved at a laser wavelength of 355 nm, power of 40 mJ/pulse, and irradiation time of 30 min. The Pd NPs exhibited excellent catalytic activity toward the reduction of 4-nitrophenol. Thus, the recovered materials showed remarkable potential for application in degradation of toxic aromatic nitro compounds in the environment.

Objective: To analyze the perioperative maternal complications of twin-twin transfusion syndrome (TTTS) after fetolascopic laser photocoagulation (FLP). Methods: A retrospective study was conducted among 182 cases with TTTS received FLP in Shanghai First Maternity and Infant Hospital from January 2010 to December 2018. The types, incidence and related factors of perioperative maternal complications as well as the changes of maternal laboratory parameters before and after FLP were analyzed. Results: The age of 182 TTTS pregnant women was (29.8±3.9) years old, body mass index (BMI) before pregnancy was (21.3±2.9) kg/m2. The median gestational week of FLP treatment was 22.0 weeks, the preoperative cervical length was (34.1±9.0) mm, and the median preoperative maximum vertical pocket was 12.0 cm. During the perioperative period of FLP treatment, 22 cases (12.1%, 22/182) presented maternal complications, among which 4 cases (2.2%, 4/182) presented severe postoperative maternal complications, including 3 cases of pulmonary edema and 1 case of pulmonary embolism accompanied with right cardiac insufficiency. There were 18 cases (9.9%, 18/182) of common maternal complications during the perioperative period, including 6 cases (3.3%, 6/182) of intraoperative hemorrhage, 5 cases (2.7%, 5/182) of intraoperative amniotic fluid leakage into the pelvic cavity, 5 cases (2.7%, 5/182) of premature rupture of membrane 72 hours after the operation, 1 case (0.5%, 1/182) of inevitable abortion, and 1 case (0.5%, 1/182) of infection. The analysis of related risk factors found that maternal complications were only related to BMI before pregnancy, and the BMI of TTTS pregnant women with complications was lower than that of those without complications, the difference was statistically significant (P<0.01). The hemoglobin level, hematocrit and albumin level of TTTS pregnant women were significantly decreased at 4-6 hours and 24 hours after FLP respectively, compared with those before surgery (P<0.01), and there were no significant correlations with the amount of amniodrainage during surgery (P>0.05 for all). Conclusions: The overall incidence of perioperative maternal complications in the treatment of TTTS by FLP is not high, among which the serious complications mainly include pulmonary edema and pulmonary embolism. Timely correction of maternal hemodilution that may occur in TTTS pregnant women could achieve a good prognosis after FLP.

PURPOSE: To compare the effectiveness and safety of panretinal laser photocoagulation targeting far-periphery (sPRP) versus conventional panretinal laser photocoagulation (cPRP) in patients with ischemic central retinal vein occlusion (iCRVO) in terms of the degree of macular edema. MATERIAL AND METHODS: The study included eight (5 males and 3 females, mean age 59.4±17.2 years) and seven (6 males and 1 female, mean age 75.7±9.1 years) patients in sPRP and cPRP groups, respectively. iCRVO was defined as a CRVO case with baseline best corrected visual acuity (BCVA) of <0.2, central retinal thickness (CRT) of >500 µm and severe intraretinal hemorrhages. sPRP included dense photocoagulation of far-periphery over 360˚ and standard photocoagulation of the mid-periphery. Primary outcome measures were CRT and macular volume at the end of the follow-up. BCVA served as the secondary outcome measure. RESULTS: The median of the follow-up period was 9.5 months (ranged from 1.5 to 19 months). In the sPRP group, at the end of the follow-up both CRT and macular volume significantly decreased from 892.0±149.4 µm to 391.8±131.2 µm (p<0.001) and from 16.6±4.1 mm3 to 9.7±1.4 mm3, respectively (p=0.008). In the cPRP group, at the end of the follow-up CRT and macular volume changes were statistically insignificant: from 761.0±162.1 µm to 705.0±181.8 µm (p=0.46) and from 13.6± 3.2 mm3 to 11.8±1.7 mm3, respectively (p=0.38). At the end of the follow-up, cPRP group did not have changes in BCVA, while in the sPRP group BCVA increased statistically significantly from 1.45±0.6 LogMAR (≈0.06) to 1.1±0.6 LogMAR (≈0.14) (p=0.03). There were no adverse events or complications associated with laser treatment in any of the study patients. CONCLUSION: This study demonstrates the superiority of sPRP compared with cPRP for treatment of patients with iCRVO, showing that selective laser treatment of the far-periphery results in significant reduction of macular edema associated with severe retinal ischemia.

OBJECTIVE: To observe and compare the clinical efficacy of CO2 fractional laser in the early control of scar post-secondary repair in patients with a cleft lip. METHODS: In the treatment group, 43 patients with secondary repair of cleft lip were treated via CO2 fractional laser. The control group covered 70 patients post-cheiloplasty. The effect in the two groups after six months was compared. The duration from the beginning of the laser treatment to surgery and patient gender were analyzed to determine if they affected the efficacy of the laser treatment for scars. RESULTS: 1) The curative effect in the treatment group was better than that in the control group (P<0.000 1), the total effectiveness rate was 90.7% in the treatment group. 2) No significant statistical difference in efficacy was observed between men and women (P=0.487). Moreover, no significant statistical difference in efficacy (P=0.055) was observed among patients one year after surgery, within the duration of <3 months, and within the duration of ≥3 months. CONCLUSIONS: CO2 fractional laser had a definite effect on the treatment of scar post-secondary repair in patients with a cleft lip. No significant correlation was observed between treatment effect and patient gender, and no difference was noted among patients one year after surgery. In the beginning of the laser treatment, no difference was observed between the durations of <3 months and ≥3 months after the reconstruction, Therefore, early intervention of scars a year after secondary repair of a cleft lip can achieve good results.

OBJECTIVE: The aim the study was to investigate the impact of the shot peening process on the condition of the surface layer and biological properties of titanium alloy produced by means of the Direct Metal Laser Sintering (DMLS) method. MATERIAL AND METHODS: Specimens were prepared by the EOSINT M280 metal sintering laser system. The surfaces were subjected to the shot peening process using three different media, i.e. steel shot, nutshell granules and ceramic beads, after which they were subjected to profilometric analysis, scanning electron microscopic (SEM) observations and energy dispersive X-ray spectroscopy (EDS) tests, as well as to assessment of biological compatibility in terms of cytotoxicity (SH-SY5Y cell lines). RESULTS: The general results obtained from the tests indicate satisfactory biocompatibility of the examined surfaces and that the impact of the shot peening process on the titanium alloy cytotoxicity is acceptable. CONCLUSIONS: The lowest cytotoxicity was demonstrated by the surfaces modified by ceramic beads than the nutshells and the biggest steel shot correspondingly. Moreover, the shot peening process carried out by means of CrNi steel and ceramic shot caused the reduction of surface roughness when, for the surface processing by means of nutshell granules, the increase of surface roughness was observed compared with the unmodified surface of titanium alloy samples.

Objective: The current study aimed to determine the effectiveness of post-isometric relaxation and LASER treatment in minimizing pain and increasing neck range of motion and function in people with upper Trapezius trigger point pain. Methods: A total of 45 participants with upper Trapezius trigger point pain were studied. All the participants were randomly assigned into three equal groups (post-isometric relaxation, LASER, and Control) to receive the respective treatments for 1 week. The pressure threshold meter was used to assess the pain-pressure sensitivity of myofascial trigger point pain. Pain intensity and cervical range of motion were measured using the visual analogue scale and cervical goniometry, respectively. The neck disability index was used to assess neck function. Results: The results of within group analysis indicate significant changes in all the outcomes in three groups (All groups, P = 0.001) except an insignificant improvement of pain pressure threshold in the control group (P = 0.069). Additionally, there were significant differences noted among three groups on pain-pressure threshold and visual analogue scale scores at posttest day 1, day 5, and day 12 (all P = 0.001). However, there was non-significant improvements of neck disability index and side flexion on both sides noted at all timelines except (all P > 0.05), a significant improvement at posttest day 5 (P = 0.015) among three groups. Conclusions: Both PIR and LASER treatment have been effective in reducing pain-pressure threshold and pain intensity in the management of muscle trigger point pain. Long-term studies are warranted to further validate the effectiveness of these modalities in the management of upper Trapezius trigger point pain.
