Promoting handwashing is essential to minimize transmission of SARS-COV-2, the virus that causes COVID-19, and to save lives. One of the most commons ways of transmission of SARS-COV-2 is from contaminated hands touching the mouth, nose and eyes. The virus can also be transferred from one surface to another through contaminated hands. Free access and mandatory use of handwashing stations would help to minimize SARS-COV-2 transmission and save lives.

The strategic roadmap "Accélérer le virage numérique" presented by the Minister of Health aims to give greater coherence to the development of e-health in France, by developing a clear and shared framework that is likely to strengthen the development of digital uses in health. The Agence nationale d'appui à la performance des établissements de santé et médico-sociaux is fully in line with this approach, in particular by coordinating a vast professional network on this topic.

RESUMEN

The shift in our healthcare system towards organisational models based on patient care management is one of the structural changes that have taken place in recent years. Digital technology represents a major lever to support this transformation, which has high stakes for improving the quality and efficiency of patient care. Positive regulation of the associated ethical issues can be achieved through the principle of a human guarantee of digital technology and artificial intelligence in health care, which is currently being recognised in the framework of the revision of the bioethics law.

The e-track, a real challenge for caregivers. We have known for more than twenty years that in order to "defragment" healthcare plan, break down the barriers between the city and the hospital and improve coordination between healthcare professionals, a better flow and sharing of information is essential. We are now talking about a digital shift in the light of accelerating technical progress with the rapid adoption of smartphones and mobile applications, and now the themes of big data and artificial intelligence, which are very present in the media.

RESUMEN

Digital health technologies offer significant opportunities to reshape current health care systems. From the adoption of electronic medical records to mobile health apps and other disruptive technologies, digital health solutions have promised a better quality of care at a more sustainable cost. However, the widescale adoption of these solutions is lagging behind. The most adverse scenarios often provide an opportunity to develop and test the capacity of digital health technologies to increase the efficiency of health care systems. Catalonia (Northeast Spain) is one of the most advanced regions in terms of digital health adoption across Europe. The region has a long tradition of health information exchange in the public health care sector and is currently implementing an ambitious digital health strategy. In this viewpoint, we discuss the crucial role digital health solutions play during the coronavirus disease (COVID-19) pandemic to support public health policies. We also report on the strategies currently deployed at scale during the outbreak in Catalonia.

RESUMEN

Digital health is uniquely positioned to enhance the way we detect and manage infectious diseases. This commentary explores the potential of implementing digital technologies that can be used at different stages of the COVID-19 outbreak, including data-driven disease surveillance, screening, triage, diagnosis, and monitoring. Methods that could potentially reduce the exposure of healthcare providers to the virus are also discussed.

[RESUMEN]. La Comisión Lancet de Hipertensión determinó que una medida clave para responder a la carga mundial que representa la hipertensión arterial era mejorar la calidad de las mediciones de la presión arterial, mediante la utilización de dispositivos cuya exactitud haya sido validada. En la actualidad existen 3000 dispositivos comercializados, pero muchos no tienen datos publicados sobre pruebas de exactitud conformes a las normas científicas establecidas. La falta de regulación o su ineficiencia, que permiten la autorización de dispositivos para uso comercial sin una validación oficial, posibilitan este problema. Además, han surgido tecnologías nuevas de medición de la presión arterial (por ejemplo, los sensores sin brazalete) sobre las cuales no existe unanimidad en la comunidad científica con respecto a las normas de exactitud de la medición. En conjunto, estos aspectos contribuyen a la disponibilidad generalizada de tensiómetros de consultorio o domiciliarios que ofrecen una exactitud limitada o incierta, que llevan a diagnósticos, manejo y farmacoterapia inapropiados de la hipertensión a escala mundial. Los problemas más importantes relacionados con la exactitud de los dispositivos de medición de la presión arterial se pueden resolver mediante el requisito regulatorio de una validación independiente obligatoria de los dispositivos, en consonancia con la norma ISO universalmente aceptada. Esta es una recomendación básica y constituye una necesidad internacional acuciante. Otras recomendaciones clave son la elaboración de normas de validación específicas para las tecnologías nuevas de medición de la presión arterial y la publicación en línea de listas de los dispositivos nuevos exactos que están a la disposición de los usuarios y los profesionales de salud. Las recomendaciones están en consonancia con las políticas de la Organización Mundial de la Salud sobre los dispositivos médicos y la atención universal de la salud. El cumplimiento de las recomendaciones aumentará la disponibilidad mundial de dispositivos de medición de la presión arterial que sean exactos y tendrá como efecto un mejor diagnóstico y tratamiento, reduciendo así la carga mundial de la hipertensión.


[ABSTRACT]. The Lancet Commission on Hypertension identified that a key action to address the worldwide burden of high blood pressure (BP) was to improve the quality of BP measurements by using BP devices that have been validated for accuracy. Currently, there are over 3 000 commercially available BP devices, but many do not have published data on accuracy testing according to established scientific standards. This problem is enabled through weak or absent regulations that allow clearance of devices for commercial use without formal validation. In addition, new BP technologies have emerged (e.g. cuffless sensors) for which there is no scientific consensus regarding BP measurement accuracy standards. Altogether, these issues contribute to the widespread availability of clinic and home BP devices with limited or uncertain accuracy, leading to inappropriate hypertension diagnosis, management and drug treatment on a global scale. The most significant problems relating to the accuracy of BP devices can be resolved by the regulatory requirement for mandatory independent validation of BP devices according to the universally-accepted International Organization for Standardization Standard. This is a primary recommendation for which there is an urgent international need. Other key recommendations are development of validation standards specifically for new BP technologies and online lists of accurate devices that are accessible to consumers and health professionals. Recommendations are aligned with WHO policies on medical devices and universal healthcare. Adherence to recommendations would increase the global availability of accurate BP devices and result in better diagnosis and treatment of hypertension, thus decreasing the worldwide burden from high BP.

A Comissão Lancet sobre Hipertensão Arterial identificou que uma iniciativa central para enfrentar a carga mundial da hipertensão arterial seria a melhoria na qualidade da mensuração da pressão arterial pelo uso aparelhos de pressão arterial validados quanto à acurácia. Atualmente, existem mais de 3 000 aparelhos de pressão arterial disponíveis comercialmente; entretanto, muitos não têm dados publicados sobre testes de acurácia realizados de acordo com padrões científicos estabelecidos. Este problema resulta de regulamentação fraca ou inexistente, o que permite a aprovação para uso comercial de dispositivos sem validação formal. Além disso, surgiram novas tecnologias de mensuração da pressão arterial (por exemplo, sensores sem algemas) sem consenso científico quanto aos padrões de acurácia. No conjunto, essas questões contribuem para a oferta generalizada de dispositivos de pressão arterial clínica e domiciliar com acurácia limitada ou incerta, levando a diagnóstico, gerenciamento e tratamento inadequados da hipertensão em escala global. Os problemas mais significativos relacionados com a acurácia dos dispositivos de pressão arterial podem ser resolvidos por regulamentação que imponha a obrigatoriedade de validação independente dos aparelhos de pressão arterial, de acordo com a norma universalmente aceita pela Organização Internacional de Normalização. Esta é uma recomendação fundamental para a qual existe uma necessidade internacional urgente. Outras recomendações essenciais incluem o desenvolvimento de padrões de validação especificamente para novas tecnologias de mensuração da pressão arterial e listas on-line de aparelhos com acurácia adequada que sejam acessíveis aos consumidores e profissionais de saúde. As recomendações estão alinhadas com as políticas da Organização Mundial da Saúde (OMS) sobre dispositivos médicos e atenção universal à saúde. A adesão às recomendações aumentaria a oferta global de dispositivos de pressão arterial com acurácia adequada e resultaria em melhor diagnóstico e tratamento da hipertensão arterial, diminuindo assim a carga mundial dessa doença.

Soft silicone's flexibility, adhesive capacity and non-toxic, non-odourous and hypoallergenic nature have made it an established material for adhesive and protective therapeutic devices. In wound care, silicone is a component of contact layer dressings for superficial wounds and silicone gel sheeting for reducing the risk of scarring, as well as of barriers for incontinence-associated dermatitis. Regarding stoma accessories, silicone is established in barrier films to prevent contact dermatitis, adhesive removers to prevent skin stripping and filler gels to prevent appliance leaks. Until recently, silicone has not been used in stoma appliances flanges, as its hydrophobic nature has not allowed for moisture management to permit trans-epidermal water loss and prevent maceration. Traditional hydrocolloid appliances manage moisture by absorbing water, but this can lead to saturation and moisture-associated skin damage (MASD), as well as increased adhesion and resultant skin tears on removal, known as medical adhesive-related skin injury (MARSI). However, novel silicone compounds have been developed with a distinct evaporation-based mechanism of moisture management. This uses colloidal separation to allow the passage of water vapour at a rate equivalent to normal trans-epidermal water loss. It has been shown to minimise MASD, increase wear time and permit atraumatic removal without the use of adhesive solvents. Trio Healthcare has introduced this technology with a range of silicone-based flange extenders and is working with the University of Bradford Centre for Skin Sciences on prototype silicone-based stoma appliance flanges designed to significantly reduce the incidence of peristomal skin complications, such as MARSI and MASD. It is hoped that this will also increase appliance wear time, reduce costs and improve patient quality of life.

This descriptive research study examined adaptation to advances in technology by healthcare technology management (HTM) departments in U.S.-based healthcare delivery organizations (HDOs). HDOs that have made or are making significant changes to address this technology evolution were identified through discussions with subject matter experts. Interviews were conducted with HDO leaders responsible for patient care technology and systems. The research described here examined drivers of change, response to change, evolution of strategies, and directions forward. This article shares common and unique interventions for the future of the HTM field, patterns and approaches for adapting to the evolution in health technology, and lessons learned from the research.

RESUMEN

Attention deficit disorder and hyperactivity (ADHD) is the main reason for consultation in most Pediatric Neurology units in Spain. The new technologies also associate benefits for both patients and professionals and the health system itself, which makes its rational implementation essential. Genetics, neuroimaging or virtual reality, for example, are clear exponents of the results that can be achieved by optimizing traditional processes. The different technologies that we collect in this article are fully operational and have thousands of experience in patients. The incorporation of them to the usual clinical practice is in our hands.

Introdução: a adolescência é uma fase da vida caracterizada por situações de vulnerabilidade que predispõem os adolescentes às situações de risco. Inovações tecnológicas podem promover à saúde dos adolescentes por meio de oficinas educativas para empoderá-los acerca do cuidado com sua saúde. Objetivo: conhecer a percepção de facilitadores sobre as tecnologias em saúde utilizadas em oficinas educativas com adolescentes. Método: estudo qualitativo realizado com 14 facilitadores do projeto de extensão Adolescer com Saúde. Os dados foram coletados por meio de entrevista semiestruturada, submetidos à análise de conteúdo em modalidade temática. Resultados: emergiram três categorias temáticas. Os facilitadores percebem as oficinas educativas como tecnologias em saúde adequadas e que potencializam a promoção da saúde. Apreendeuse que a utilização das oficinas educativas pode contribuir para o êxito das ações, especialmente quando direcionadas aos adolescentes. Conclusão: os facilitadores percebem que as oficinas educativas, como tecnologias em saúde, são importantes. Evidenciou-se o uso recorrente de tecnologias leves e representam um recurso que fortalece ações de educação em saúde.(AU)


Introduction: adolescence is a phase of life characterized by situations of vulnerability that predispose adolescents to risky situations. Technological innovations can promote the health of adolescents through educational workshops to empower them about caring for their health. Objective: to know the facilitators' perception of health technologies used in educational workshops with adolescents. Method: qualitative study carried out with 14 facilitators of the extension project Adolescer com Saúde (Adolescent with Health). Data were collected through semi-structured interviews, submitted to content analysis in thematic modality. Results: three thematic categories emerged. Facilitators perceive educational workshops as appropriate health technologies that enhance health promotion. It was apprehended that the use of educational workshops can contribute to the success of actions, especially when directed at adolescents. Conclusion: the facilitators realize that educational workshops, as health technologies, are important. The recurrent use of soft technologies was evidenced and represents a resource that strengthens health education actions.(AU)

Las innovaciones tecnológicas pueden promover la salud de estos jóvenes a través de talleres educativos con miras a capacitarlos en el cuidado de su salud. Objetivo: conocer la percepción de los facilitadores de las tecnologías de salud utilizadas en los talleres educativos con adolescentes. Método: estudio cualitativo realizado con 14 facilitadores del proyecto de extensión Adolescer com Saúde. Los datos, recogidos a través de entrevistas semiestructuradas, fueron sometidos al análisis de contenido en la modalidad temática. Resultados: surgieron tres categorías temáticas. Los facilitadores perciben los talleres educativos como tecnologías sanitarias apropiadas que potencian la promoción de la salud. Se comprobó que el uso de talleres educativos puede contribuir al éxito de las acciones, especialmente cuando se dirigen a adolescentes. Conclusión: los facilitadores reconocen que los talleres educativos son importantes como tecnologías sanitarias. También es evidente el uso recurrente de las tecnologías ligeras, un recurso que fortalece las acciones de educación en salud.(AU)

There are tens of thousands of mental health-related apps available today - representing extreme duplication in this digital age. Instead of a plethora of apps, there is a need for a few that meet the needs of many. Focusing on transparency and free sharing of software, we argue that a collaborative approach towards apps can advance care through creating customisable and future proofed digital tools that allow all stakeholders to engage in their design and use.
