Introduction: Open tendoachilles injuries are rare and associated with significant soft tissues complications. The objective of the present study was to assess the clinical outcome and safety of a simple and minimally invasive technique, with a goal to assess if it may help minimise flap and wound related complications in open tendoachilles injuries. Materials and methods: This prospective study of four years duration included 20 patients with open tendoachilles injuries managed with a simple minimally invasive tunnel technique. The primary outcome variable was occurrence of a major soft tissue complication. The secondary outcome variables included functional outcome measured using AOFAS Ankle hind foot score, re-rupture of tendoachilles and need for revision surgery. Results: None of the patients in the present series developed a serious soft tissue complication. Based upon the AOFAS hind foot scoring system, good to excellent outcome was achieved in 19 (95%) patients. All the patients were able to perform tip toe walking at six months post-surgery. None of the patients had a re-rupture of the tendoachilles and no patient needed a revision surgery. The complications encountered include thickening of the tendon at the repair site (15%), superficial wound infection (5%), stitch granuloma (5%) and hypertrophic scar (5%). Conclusion: This technique seems to be promising in reducing the soft tissue complications associated with the surgical management of open tendoachilles injuries. Most patients had a good final clinical outcome. The technique is safe, simple and reproducible. However, further randomised control studies with a larger sample size assessing the technique are recommended.

The overall prevalence of stunting in children was 40.5%, among children 36.1% boys and 46.3% of girls were stunted. The prevalence of stunting in under-five children was 50.7%. Female children (OR=1.35, 95% CI:0.94-2.0), children aged 13-24 months (OR=6.5, 95% CI: 3.0-13.9), mothers aged 15-24 years (OR=4.4, 95% CI: 2.6-7.2), joint family (OR=2.1, 95% CI: 1.4-3.0) did not have access to improved drinking water (OR=3.3, 95% CI: 1.9-5.9), and the toilet facility (OR=2.8, 95% CI, 1.9-4.3), while the children from district Nowshera (OR=1.7, 95% CI: 0.9-3.2) were significantly (P<0.05) associated in univariate analysis. The regression model revealed that child age, maternal age, family type, quality of water, and toilet facility, were the significant (P<0.05) factors contributing to child stunting in the flood-hit areas. Identification of key factors might be helpful for policymakers in designing comprehensive community-based programs for the reduction of stunting in flood-affected areas. In disasters such as flood, the detrimental consequences of the stunting problem could be even more on children. Este estudo avaliou a prevalência de nanismo e os fatores de risco associados de nanismo entre crianças em idade pré-escolar e em idade escolar em áreas afetadas por inundações do Paquistão. Foi realizado um estudo transversal visitando 656 domicílios por meio de amostragem em múltiplos estágios. As medidas antropométricas do entrevistado, informações sociodemográficas e instalações de saneamento foram exploradas. Um modelo de regressão logística foi usado para determinar os determinantes do nanismo, controlando todos os possíveis fatores de confusão. A prevalência geral de baixa estatura em crianças foi de 40,5%, entre as crianças 36,1% dos meninos e 46,3% das meninas com baixa estatura. Alveolar soft part sarcoma (ASPS) accounts for less than 1% of all sarcomas. More frequently encountered within the lower limbs, the authors present a 24-year-old male with ASPS presenting as an asymptomatic swelling of the lateral tongue. At 12 months post wide local excision of the lesion, the patient remains well with no evidence of local or regional recurrence. Histological and immunohistochemical features are diagnostic of ASPS. Whilst rare, head and neck surgeons should be aware of ASPS as a potential cause of slow growing lesions, as early surgical resection is vital in view of the propensity for metastatic spread.

Annexin 1 (ANXA1) is a member of the annexin family of phospholipid- and calcium-binding proteins with a well demonstrated role in early delayed (30 min to 3 h) inhibitory feedback of glucocorticoids in the pituitary. We have examined corticotrophs in wild-type and ANXA1 knockout mice to determine the effects of lack of ANXA1 in male and female animals. Anterior pituitary tissue from ANXA1 wild-type, heterozygote and null mice was fixed and examined (i) by confocal immunocytochemistry to determine the number of corticotrophs and (ii) by electron microscopy to examine the size, secretory granule population and secretory machinery of corticotrophs. No differences in these parameters were detected in female mice. In male ANXA1 null mice, there were approximately four-fold more corticotrophs than in wild-type animals. However, the corticotrophs in ANXA1 null mice were smaller and had reduced numbers of secretory granules (the reduction in granules paralleled the reduction in cell size). No differences in the numerical density of folliculo-stellate, gonadotroph, lactotroph or somatotroph cells were detected in male ANXA1 null mice. Plasma corticosterone, adrenocorticotrophic hormone (ACTH) and pituitary pro-opiomelanocortin mRNA were unchanged but pituitary ACTH content was increased in male ANXA1 null mice. Interleukin (IL)-6 pituitary content was significantly elevated in male and reduced in female ANXA1 null mice compared to wild-type. In conclusion, these data indicate that ANXA1 deficiency is associated with gender-specific changes in corticotroph number and structure, via direct actions of ANXA1 and/or indirect changes in factors such as IL-6.

PIP: Color-coded measuring tapes have been developed as surrogates for measuring birth weight with a scale. The tapes have been correlated with birth weight to indicate low birth weight. In this study, arm circumference and thigh circumference were measured with tapes indicating low birth weight: 7.0 to 8.5 cms for mid arm circumference and 11.5 and 14.5 cms for thigh circumferences. The area in between the numbers was color coded in yellow to indicate a birth weight of 2.0 to 2.5 kg. The area under the lowest number was coded red, and the are above the highest number was coded green. The strips were tested on 281 infants to determine the accuracy of this form of measurement. Use of strips and birth weights were recorded within 48 hours of the birth. The results indicated that 54% of the low birth weight infants were correctly identified with the mid arm circumference tapes. There were 13.88% who were recorded with low birth weight out of the 281 sampled. The tape reliably measured 90% of the normal birth weight infants. The thigh circumference tape was a better measure of low birth weight. 80% of low birth weight infants were identified with the thigh circumference tape, but only 85% of normal weights were identified. When the tapes were used concurrently, accuracy of low birth weight classification was increased to 87%, but normal weight remained much the same at 85% accuracy. It is recommended that these tapes be used in combination by traditional birth attendants and other grassroots workers to identify low birth weight infants and then refer these infants for special care.^ieng

PIP: There remains a need to determine low birth weight in settings such as the home, where scales are not easily available. This study examined the calf circumference at a prominent point on a semi-flexed leg with a standard measuring tape to the nearest .1 cm and birth weight to the nearest 5 g. The sample included 257 full term singleton infants. The results of the correlation coefficient analysis was r = .74, which indicates a high degree of correlation between the two measures. The mean of birth weight was 2.96 g plus or minus .41 and the mean calf circumference was 9.87 cm plus or minus 1.07. Birth weight can be calculated from calf circumference by adding .172 to the multiplication of .283 times calf circumference. The calf circumference appears to exhibit the strongest correlation with birth weight regardless of the sample. A color-coded tape had been prepared which demarcated in yellow 6.5 cm and 8.25 cm as the range equivalent to 2.0 kg and 2.5 kg low birth weight. The area below 6.5 cm was colored red and the area below 8.25 was colored green. Currently, data is being collected on the efficacy and positive predictive value of using the tapes.^ieng
