ABSTRACT

BACKGROUND: To investigate the outcome and prognosis after Unicondylar knee arthroplasty (UKA) in patients with medial compartment arthritis of the knee combined with anterior cruciate ligament(ACL) dysfunction. METHODS: A total of 122 patients diagnosed with knee osteoarthritis and treated with medial mobile platform unicondylar replacement at our center from January 2019 to December 2021 were retrospectively included in the study, and were divided into two groups according to ACL function, namely the normal ACL function group (ACLF) and the poor ACL function (N-ACLF) group. The postoperative results and prognosis of the two groups were evaluated and compared. RESULTS: This study included 122 patients who underwent UKA surgery. There were no statistical differences in preoperative and postoperative posterior tibial tilt angle, knee mobility, KOOS, and prognosis between the two groups (P > 0.05). CONCLUSION: For medial compartment arthritis of the knee combined with ACL malfunction, surgery resulted in pain relief, improved quality of life and a good prognosis for such patients. It is hoped that clinicians will perform UKA in patients with ACL dysfunction after a comprehensive evaluation to improve their quality of life.

ABSTRACT

Sensor-based human activity recognition is now well developed, but there are still many challenges, such as insufficient accuracy in the identification of similar activities. To overcome this issue, we collect data during similar human activities using three-axis acceleration and gyroscope sensors. We developed a model capable of classifying similar activities of human behavior, and the effectiveness and generalization capabilities of this model are evaluated. Based on the standardization and normalization of data, we consider the inherent similarities of human activity behaviors by introducing the multi-layer classifier model. The first layer of the proposed model is a random forest model based on the XGBoost feature selection algorithm. In the second layer of this model, similar human activities are extracted by applying the kernel Fisher discriminant analysis (KFDA) with feature mapping. Then, the support vector machine (SVM) model is applied to classify similar human activities. Our model is experimentally evaluated, and it is also applied to four benchmark datasets: UCI DSA, UCI HAR, WISDM, and IM-WSHA. The experimental results demonstrate that the proposed approach achieves recognition accuracies of 97.69%, 97.92%, 98.12%, and 90.6%, indicating excellent recognition performance. Additionally, we performed K-fold cross-validation on the random forest model and utilized ROC curves for the SVM classifier to assess the model's generalization ability. The results indicate that our multi-layer classifier model exhibits robust generalization capabilities.

ABSTRACT

Purpose: Although the tumor-node-metastasis staging system is widely used for survival analysis of nasopharyngeal carcinoma (NPC), tumor heterogeneity limits its utility. In this study, we aimed to develop and validate a radiomics model, based on multiple-sequence magnetic resonance imaging (MRI), to estimate the probability of overall survival in patients diagnosed with NPC. Methods: Multiple-sequence MRIs, including T1-weighted, T1 contrast, and T2-weighted imaging, were collected from patients diagnosed with NPC. Radiomics features were extracted from the contoured gross tumor volume of three sequences from each patient using the least absolute shrinkage and selection operator with the Cox regression model. The optimal Rad score was determined using 12 of the 851 radiomics features derived from the multiple-sequence MRI and its discrimination power was compared in the training and validation cohorts. For better prediction performance, an optimal nomogram (radiomics nomogram-MS) that incorporated the optimal Rad score and clinical risk factors was developed, and a calibration curve and a decision curve were used to further evaluate the optimized discrimination power. Results: A total of 504 patients diagnosed with NPC were included in this study. The optimal Rad score was significantly correlated with overall survival in both the training [C-index: 0.731, 95% confidence interval (CI): 0.709-0.753] and validation cohorts (C-index: 0.807, 95% CI: 0.782-0.832). Compared with the nomogram developed with only single-sequence MRI, the radiomics nomogram-MS had a higher discrimination power in both the training (C-index: 0.827, 95% CI: 0.809-0.845) and validation cohorts (C-index: 0.836, 95% CI: 0.815-0.857). Analysis of the calibration and decision curves confirmed the effectiveness and utility of the optimal radiomics nomogram-MS. Conclusions: The radiomics nomogram model that incorporates multiple-sequence MRI and clinical factors may be a useful tool for the early assessment of the long-term prognosis of patients diagnosed with NPC.

BACKGROUND: The feasibility of association liver partition and portal vein ligation for staged hepatectomy (ALPPS) for solitary huge hepatocellular carcinoma (HCC, maximal diameter ≥ 10 cm) remains uncertain. This study aims to evaluate the safety and the efficacy of ALPPS for patients with solitary huge HCC. METHODS: Twenty patients with solitary huge HCC who received ALPPS during January 2017 and December 2019 were retrospectively analyzed. The oncological characteristics of contemporaneous patients who underwent one-stage resection and transcatheter arterial chemoembolization (TACE) were compared using propensity score matching (PSM). RESULTS: All patients underwent complete two-staged ALPPS. The median future liver remnant from the ALPPS-I stage to the ALPPS-II stage increased by 64.5% (range = 22.3-221.9%) with a median interval of 18 days (range = 10-54 days). The 90-day mortality rate after the ALPPS-II stage was 5%. The 1- and 3-year overall survival (OS) rates were 70.0% and 57.4%, respectively, whereas the 1- and 3-year progression-free survival (PFS) rates were 60.0% and 43.0%, respectively. In the one-to-one PSM analysis, the long-term survival of patients who received ALPPS was significantly better than those who received TACE (OS, P = 0.007; PFS, P = 0.011) but comparable with those who underwent one-stage resection (OS, P = 0.463; PFS, P = 0.786). CONCLUSION: The surgical outcomes of ALPPS were superior to those of TACE and similar to those of one-stage resection. ALPPS is a safe and effective treatment strategy for patients with unresectable solitary huge HCC.

ABSTRACT

The purpose of this study was to evaluate the feasibility and benefits of using high-dose-rate three-dimensional conformal radiotherapy (3D-CRT) combined with active breathing control (ABC) for stereotactic body radiotherapy (SBRT) of patients with early-stage non-small-cell lung cancer (NSCLC). Eight patients with early-stage NSCLC underwent CT scans under standard free-breathing (FB) and moderately deep inspiration breath-hold (mDIBH) with ABC. Two high-dose-rate 3D-CRT plans (1000 Mu/min) were designed based on the CT scans with FB and mDIBH. The maximal dose (D1%), minimal dose (D99%), conformity index (CI), and homogeneity index (HI) of the planning target volume (PTV), and dose-volume indices of the organs at risk between each plan were compared. The mean PTV volume decreased from 158.04 cm(3) with FB to 76.90 cm(3) with mDIBH (p < 0.05). When mDIBH was used, increases in the affected lung volume (by 47%), contralateral lung volume (by 55%), and total lung volume (by 50%) were observed compared to FB (p < 0.05). The V5-V40 of the affected lung (Vx represented the percentage volume of organs receiving at least the x Gy), V5-V40 and the mean dose for the total lung, V5-V40 and mean dose of the chest wall, and the maximum dose of the spinal cord were less for mDIBH than FB (p < 0.05). There were no significant differences in CI, HI, D1%, or D99% for the PTV between the plans. In conclusion, high-dose-rate 3D-CRT combined with ABC reduced the radiation dose to the lungs and chest wall without affecting the dose distribution in SBRT of early-stage NSCLC patients.

ABSTRACT

OBJECTIVES: The objective of this study was to investigate the long-term outcomes and prognostic factors for nasopharyngeal carcinoma (NPC) in Han and Uyghur patients treated with intensity-modulated radiotherapy (IMRT) in the Xinjiang region of China. MATERIALS AND METHODS: One hundred twenty-one Han and 60 Uyghur patients with newly diagnosed NPC without distant metastasis received IMRT at the Affiliated Tumor Hospital of Xinjiang Medical University between 2005 and 2008. The Kaplan-Meier method was used to estimate survival rates, and the log-rank test was used to evaluate differences in survival. RESULTS: Comparing Han and Uyghur patients, the 5-year overall survival (OS), disease-free survival (DFS), local control (LC), regional control (RC), and distant metastasis-free survival (DMFS) rates were 81.9% vs 77.6% (P = 0.297), 72.1% vs 65.6% (P = 0.493), 88.3% vs 86.5% (P = 0.759), 95.0% vs 94.6% (P = 0.929), and 79.1% vs 75.2% (P = 0.613), respectively. Multivariate Cox proportional hazards regression identified the following independent prognostic factors in Han patients: N stage (P = 0.007) and age (P = 0.028) for OS, and age (P = 0.028) for DFS. OS differed significantly between Han and Uyghur patients >60 years old group (P = 0.036). Among Uyghur patients, the independent prognostic factors were age for OS (P = 0.033), as well as N stage (P = 0.037) and age (P = 0.021) for DFS. Additionally, Uyghur patients were less likely to experience mucositis and dermatitis than Han patients. CONCLUSION: Han and Uyghur patients with NPC had statistically significant differences in age, smoking history, and N staging. There was no significant difference in overall treatment outcomes with IMRT between these 2 ethnic populations in Xinjiang, China.
