ABSTRACT

OBJECTIVE: To analyze the relationship and effect of alcohol use on Child-to-Parent Violence (CPV). METHOD: Cross-sectional, observational study with a quantitative approach, carried out through online data collection using the Conflict Tactics Scale (CTS2) and the Alcohol Use Disorders Identification Test (AUDIT) in 318 high school adolescents from southern Mexico. RESULTS: Moderate and significant relationships were found between alcohol use and verbal (rs = 0.408, p = 0.001) and economic violence against the mother (rs = 0.445, p = 0.001). A similar situation is presented in physical (rs = 0.473, p = 0.001), verbal (rs = 0.236, p = 0.001) and economic (rs = 0.477, p = 0.001) violence directed to the father. CONCLUSION: The relation among the variables was supported by Multiple Linear Regression models, with alcohol consumption in adolescents being a predictor of violence against mothers and fathers.

ABSTRACT

Victims of traumatic events that involve repeated interpersonal aggression and low or no chance of escape frequently report intense Self-Conscious Emotions (SCEs), such as Shame, Guilt, and Humiliation. Humiliation is the reaction to a forced loss of status and is hypothesized to have unique contributions to the development and maintenance of Posttraumatic Stress Disorder (PTSD) and Complex Posttraumatic Stress Disorder (CPTSD). However, previous studies did not include humiliation or did not simultaneously probe the relative contribution of each SCE to posttraumatic symptoms. This study aimed to investigate the dynamics between specific SCEs and trauma-related symptomatology in the general population who suffered a form of complex trauma. Four hundred forty-nine people (77.11% women) exposed to domestic violence and sexual abuse answered an online survey. We investigated whether each emotion would accurately predict probable PTSD and CPTSD levels above the proposed cutoff. We estimated a network model to understand the dynamics of their interactions and whether the traumatic event type would moderate relationships between SCEs and posttraumatic stress symptoms, comparing networks of two types of complex trauma. No SCE predicted PTSD, but humiliation was a predictor of CPTSD while controlling for Shame and Guilt. Humiliation was also the most central SCE domain in the networks of both traumatic events. Our results stress the relevance of Humiliation to understanding posttraumatic stress symptoms and the necessity to consider humiliation when studying the emotional processing in complex trauma.

ABSTRACT

PURPOSE: This study aimed to analyse parenting styles as a possible factor associated with caries in foster children and adolescents who had been victims of domestic violence. METHODS: The study was carried out in nine foster shelters in Recife, Northeast Brazil, with a sample of 88 participants aged 8 to 17 years old. Data were collected through intraoral examinations to verify deft/DMFT indices, applying the Demandingness and Responsiveness Scales to determine parenting styles. In turn, sociodemographic data were retrieved from records filed at the institutions. Descriptive analysis was performed, with the Mann-Whitney and Kruskal-Wallis tests being used to compare the categories of numerical variables. RESULTS: The mean deft/DMFT was of 3.78 in children (8 and 9 years old) and 1.93 in adolescents (10 years old and older). Overall, the index was of 2.31. Neglect was the main reason for the children's admission to foster care, while neglectful was the most prevalent parenting style. CONCLUSION: Children of indulgent parents had higher caries indices in both deciduous and permanent teeth, while children of authoritative (permanent teeth) and authoritarian parents (deciduous teeth) had the lowest caries indices. However, no significant associations were found between caries and parenting styles.

ABSTRACT

Domestic violence (DV) is a global prevalent health problem leading to adverse health consequences, yet health systems are often unprepared to address it. This article presents a comparative synthesis of the health system's pre-conditions necessary to enable integration of DV in health services in Brazil, Nepal, Sri Lanka and occupied Palestinian Territories (oPT). A cross-country, comparative analysis was conducted using a health systems readiness framework. Data collection involved multiple data sources, including qualitative interviews with various stakeholders; focus-group discussions with women; structured facility observations; and a survey with providers. Our findings highlight deficiencies in policy and practice that need to be addressed for an effective DV response. Common readiness gaps include unclear and limited guidance on DV, unsupportive leadership coupled with limited training and resources. Most providers felt unprepared, lacked guidance and felt unsupported and unprotected by managers and their health system. While in Brazil most providers felt they should respond to DV cases, many in Sri Lanka preferred not to. Such organizational and service delivery challenges, in turn, also affected how health providers responded to DV cases leaving them not confident, uncertain about their knowledge and unsure about their role. Furthermore, providers' personal beliefs and values on DV and gender norms also impacted their motivation and ability to respond, prompting some to become 'activists' while others were reluctant to intervene and prone to blame women. Our synthesis also pointed to a gap in women's use of health services for DV as they had low trust in providers. Our conceptual framework demonstrates the importance of having clear policies and highlights the need to engage leadership across every level of the system to reframe challenges and strengthen routine practices. Future research should also determine the ways in which women's understanding and needs related to DV help-seeking are addressed.

ABSTRACT

OBJECTIVE: To longitudinally assess domestic violence (DV) during the postpartum period, identifying types, patterns and determinants of DV, according to mothers' reports in Fortaleza, Brazil. METHODS: Data from the Iracema-COVID cohort study interviewed at home mothers who gave birth in the first wave of COVID-19, at 18 and 24 months after birth. Patterns of reported DV were classified as follows: no DV, interrupted DV, started DV and persistent DV. Adjusted multinomial logistic regressions were used to assess factors associated with persistent DV. RESULTS: DV was reported by 19 and 24% of the mothers at 18 and 24 months postpartum, respectively, a 5 percentage points increase. Persistent DV was present in 11% of the households in the period. The most frequent forms of DV were verbal aggression, reported by 17-20% of the mothers at 18 and 24 months, respectively; drunkenness or use of drugs at home, present in 3-5% of the households; physical aggression, reported by 1.2-1.6% of the mothers. Households with two or more forms of DV increased from 2 to 12% in the period. Adjusted factors associated with persistent DV were maternal common mental disorder, family headed by the mother and head of family's poor schooling. Food insecurity was associated with starting DV. CONCLUSION: Prevalence of DV was considerably high in the postpartum period. DV prevention policies should rely on improving care to women's mental health; preventing food insecurity; and fostering the educational level of young people of both sexes.

Introduction:The pandemic scenario has brought to light, and in a intesified manner, some concerning indicators regarding domestic violence against women. Thus, the question arises: did the COVID-19 pandemic make a difference in the number of domestic violence cases? Objective: To compare the occurrence of domestic violence cases against Brazilian women in the periods from 2015 to 2018 and 2019 to 2021 according to data from the state's Civil Police. Methodology: This is a documentary and descriptive study, conducted with secondary data. The information was collected through the Civil Police ombudsman on March 11, 2021, based on police reports of domestic violence against women from 2015 to 2021. Results: During the year of 2015 to 2021, a total of 26,671 police reports of violence against women were registered. From 2015 to 2018, the monthly average was 360.1 occurrences, and during the pandemic, from 2019 to 2021, the monthly average was 360.9 occurrences. There was a slight increase in the number of police reports during the COVID-19 pandemic, with a peak in 2020, which had a monthly average of 387.4 occurrences, the most intense period of the pandemic. The main violence perpetrated was intentional bodily harm, mainly against women aged 18 to 24, who were either working from home or unemployed. Conclusion: Despite the lack of significant differences between pre and post-pandemic years, the analysis reinforces the urgency of discussing the underlying factors of violence against women, especially domestic violence. The importance of preventive measures and support for victims to address this social problem is emphasized. It is essential to promote policies aimed at a more equal and safer society for all women (AU).

ABSTRACT

BACKGROUND/AIMS: Domestic violence (DV) encompasses a series of abusive behaviors, perpetrated in a family environment, against individuals of all ages and genders. Injuries to the head, neck, and face are frequent findings among victims of abuse, negatively impacting their quality of life. Although oral and maxillofacial injuries (OMFI) and traumatic dental injuries (TDI) are commonly diagnosed among DV victims, their prevalence is still unknown. This systematic review was aimed to assess the prevalence of OMFI and TDI among victims of DV. METHODS: The protocol of the review was registered in PROSPERO (CRD42023424235). Literature searches were performed in eight electronic databases, up to August 7th, 2023. Observational studies published in the Latin-roman alphabet and reporting the prevalence of OMFI and/or TDI were included. The Joanna Briggs Institute's critical appraisal tool, checklist for prevalence studies, was used for quality assessment. Results were presented as qualitative and quantitative syntheses. RESULTS: Seventeen studies, totaling 12,375 victims of domestic violence, were included. Meta-analyses showed an overall prevalence of 29% (95% CI: 15%-48%, I2 = 99%) and 4% (95% CI: 1%-10%, I2 = 98%) for OMFI and TDI, respectively. Higher pooled prevalence for OMFI (41%, 95% CI: 13%-46%, I2 = 99%) was demonstrated in samples with only women. OMFI was less prevalent (20%) among DV victims under 18, while TDI was lower among adults (1%). Hospital samples presented higher pooled prevalence of OMFI (32%), and forensic data from fatal victims presented higher prevalence of TDI (8%). CONCLUSION: The overall prevalence of OMFI and TDI in DV victims was 29% and 4%, respectively. Women victims of DV presented higher rates of OFMI (41%) and TDI (6%).

ABSTRACT

Suicide is a major public health problem worldwide with far-reaching effects on families, communities, and societies. Influencing factors range from macro-level interventions like alcohol control policies and suicide prevention programs to individual contributors such as alcohol abuse and domestic violence. This study aimed to examine the relationship between Chile's suicide rate changes from 2002 to 2015 and the Alcohol Act of 2004, a national suicide prevention program implemented in 2007, alcohol abuse, and domestic violence. Assembling a unique longitudinal dataset from Chilean public institutions, the study employed an instrumental variable time-series cross-regional design. Results indicated that the Alcohol Act was not associated with suicide rates, domestic violence exhibited a significant association with increased suicide rates, and the national suicide prevention program was linked to reductions in suicide rates, especially among males. These findings align with research from neighbouring countries, showcasing the efficacy of suicide prevention programs in decreasing suicide rates in Chile. Results highlight the importance of integrating protocols to early-detect domestic violence in suicide prevention programs, as well as the need to further improving alcohol control policies to complement suicide prevention programs.

ABSTRACT

BACKGROUND: Adverse Childhood Experiences (ACEs) are potentially traumatic events that affect children and adolescents, increasing the risk for negative outcomes in several developmental domains. The literature suggests that ACEs affect boys and girls differently, and that protective factors such as social and emotional skills may favor resilience processes and promote Psychosocial Adjustment. OBJECTIVE: The present study aimed to verify gender differences in relation to ACEs and Empathy, and to identify whether the effect of ACEs on Psychosocial Adjustment is moderated by Empathy. PARTICIPANTS AND SETTING: A total of 1229 boys and girls aged 14 to 17 years from public schools in regions considered to be of high social vulnerability in two cities of Brazil participated in this study. METHODS: Comparisons between genders were carried out using a logistic regression model. We then fitted multiple regression models predicting Psychosocial Adjustment, where ACEs, Empathy, and an interaction term between ACEs and Empathy were employed as independent variables, controlled by gender. RESULTS: Logistic regression analysis showed that girls experience ACEs more often than boys (d = 0.26, p = 0.02; d = 0.36; p < 0.001), and score higher on the Empathy scale (d = 0.43, p < 0.001). The genders do not differ in Community Violence Exposure nor Psychosocial Adjustment levels. Interaction analysis showed that Empathy moderates the effect of ACEs on adolescents' level of Psychosocial Adjustment. In the context of close relationships, Empathy may counteract the effect of ACEs, while in cases involving domestic violence and/or psychological abuse, it may amplify the impact of ACEs. CONCLUSIONS: The results show the relevance of considering gender specificities and that the role played by Empathy is more complex and needs further investigation. We emphasize that programs and policies aiming to prevent ACEs and their harmful effects should consider its types, in addition to the diversity and the specificities of the target population.

ABSTRACT

Background: Addictions can modify the dynamics, communication, and establishment of assertive relationships in the family nucleus, situations that can cause different types of family violence. A phenomenon of special interest is child-to-parent violence or children's violence toward their parents. This type of violence can be exercised physically (hitting, kicking, shoving), verbally (shouting, blackmailing and insulting) and economically (using a card, stealing money or belongings from the parents). Although is generally supported that child-to-parent violence may be associated with alcohol-induced aggressiveness and lack of control, there is less evidence of a possible differentiation regarding the sex of the parents. Objective: Analyze the relationship and effect of alcohol on child-to-parent violence according to the parents' sex. Methods: This was a predictive study of 265 adolescents between 12 and 19 years of age. Data were collected from social networks using two self-applied instruments (the Alcohol Use Disorders Identification Test and the Conflict Tactics Scale: Parent-Child Version) programmed with the Survey Monkey® digital platform. Results: Of the adolescents studied, 66.8% had consumed alcohol at some time in their lives; of these, 6.6% had harmful consumption. A positive relationship was found between alcohol involvement scores and economic violence toward the mother and father. The former was supported by regression models where alcohol involvement predicted child-to-parent economic violence directed toward mothers and fathers. Conclusions: It is important to develop activities to prevent alcohol consumption as a risk factor for violence and to promote family integration in adolescents and their families.

ABSTRACT

The objectives of this study were: 1) to estimate the prevalence of child abuse (CA), intimate partner violence (IPV), and elder abuse (EA) in a representative sample of older Mexican women by status, type, and cause of disability; and 2) to examine the associations of EA with CA, IPV, and disability status. We conducted a secondary data analysis of 21,718 women aged 60 years and older. Older women with disabilities had higher prevalences of CA, IPV, and EA than those with some difficulties or without disabilities. In older women whose disability was acquired at birth or due to aggression, this prevalence was especially high. Older women with disabilities and a history of CA and IPV were twice as likely to suffer EA. Policymakers and government leaders should incorporate the matter of violence against women with disabilities into their public agendas. This should be done while considering the women's various disabilities and causes of disability.

ABSTRACT

Objective: To analyze the level of knowledge of the dental professional about violence committed in children and adolescents, emphasizing the importance of their actions for the detection, care and referral of potential cases that allow, within a multidisciplinary context, an intervention timely and effective. Materials and methods: A survey was carried out with 132 dentists, 123 women and 9 men, with professional experience of 2 to 43 years, university professors in the specialty of pediatric dentistry, students of specialization course and related postgraduate courses or those who carry out their work activities in 2 public hospitals in the Autonomous City of Buenos Aires that care for minors. The survey included 15 questions distributed in 3 categories whose thematic axes delved into knowledge about violence committed against minors (Category 1), suspicion of alleged child abuses in the consultation (Category 2) and factors that condition the eventual referral of the case (Category 3). Each question included in the 3 categories is addressed according to whether the professional field of the surveyed subject was a specialist/graduate student (Group 1) or a hospitalist (Group 2). Results: In the first category, a significant lack of specific training is observed in the undergraduate stage with 89% for group 1 and 93.75% for group 2, although the latter have accessed courses and improvement activities aimed at to increase skills and abilities by 71.87%, in contrast to 24% in the other group. Likewise, 29% of group 1 and 50% of group 2 knew the protocols established in their professional environments. Both considered that the dentist is not trained to detect guiding behaviors towards possible cases (89%, group 1; 87.5%, group 2). For the second category, group 1 responded positively by 73%, while group 2 did so by 84.38%. In the third category, a high percentage stood out for group 1 in the questions related to the dentist's fear of aggravating the actions of family violence (64%) or retaliation (55%) against the child if he made the referral. In the same items, group 2 responded with dissimilar percentages (28.13% and 31.25%, respectively). Finally, the results were clearly differentiated in terms of lack of knowledge of the procedures to follow if a case warrants referral, with 71% for group 1 and 34.38% for group 2. Conclusion: It is imperative to instruct and train the professional dentist, raising awareness about the need to know current legislation and the detection and subsequent referral mechanisms. Although those who carry out their work at the hospital level seem to be better prepared to act, there is an evident widespread lack of knowledge that entails the risk of not announcing or acting inappropriately in cases of violence that threaten the safety of children and adolescents (AU)

ABSTRACT

Abstract Violence against women in domestic and family environments has its roots in the inequalities that exist between the genders. Studies on perpetrators of lethal violence are necessary for an expanded understanding of the phenomenon. This study aims to characterize the crimes of this nature and their perpetrators, with verbal autopsies carried out with acquaintances and relatives of women victims of homicide in Campinas. In total, we studied 38 cases of female homicide, of which 63.2% correspond to femicides. Most perpetrators were acquaintances (partners, spouses, and lovers) in the cases of feminicide, and not acquaintances (subjects whose identity was unknown) in the cases of female homicide. The deaths occurred mostly in domestic environments and were caused by the expressive use of violence and the use of sharp/perforating objects and firearms. Fleeing was the most frequent behavior after the perpetration of the murders. The cases are discussed articulating the interventions aimed at the reflexive groups of masculinities, educational and rehabilitation centers, or recovery programs, problematizing their difficulties and gains within the framework of Lei Maria da Penha.

ABSTRACT

Violence against women is currently considered a violation of human rights and a global public health problem. It is associated with domestic dominance and linked to hierarchical gender norms, such as male control over women's belongings and behaviors. Power and gender inequality are convincing explanations for the causes of violence against women. In violent environments, the woman, concerned for her and her children's safety is more likely to neglect their needs, having her parental capacity for protection and care altered. This study aimed to understand the meaning of motherhood for women living in a shelter in the city of São Paulo, by understanding their perception of being a mother. A qualitative research method was used, with individual interviews: ten residents and five workers, all fully transcribed and analyzed for content, resulting in six categories among the women: Motherhood, Motherhood in Violence, Relationship with the mother, Relationship with the children, Consequences of Violence, and Life Changes after being in the shelter. Five categories among the workers: Motherhood, Difficulty with Motherhood, Perception of the shelter, Perception of violence, and Identification and prejudice about violence. The results show that the participating women understand motherhood according to social gender norms that assign all responsibilities and obligations regarding children to the mother. These norms also attribute to women the responsibility for everything that happens to their children. Regarding the shelter workers, the results demonstrate a lack of training for dealing with violence. Continued training of the CAMVV team is crucial and must be implemented immediately.

ABSTRACT

This study addresses the relation between subjectivity and contemporary enslaved labor from the enslaved workers' narratives in Brazil. A qualitative social research was carried out based on a constructionist perspective. We sought interaction with rescued workers and used (a) participant observation of workers' routine in an institutional project that supports them and a field diary, (b) semi-structured and open individual interviews with workers and a member of the team project. The fieldwork lasted a year and a half and the analysis followed Content Analysis. Freudian theory and Foucault's thought were used for interpretation, which managed to understand aspects of workers' experiences, exploitation characteristics, parental abandonment, as well as the tensions in self-classification as enslaved. The narratives pointed to a dramatic reality manifested in body exploitation, authoritarian abuses, violence, and negligence. At the same time, these narratives showed forms of worker resistance that calls for further investigations to increase knowledge on the subjective experiences of those who were enslaved.(AU)

ABSTRACT

Abstract Objective To describe suspected/confirmed cases of child maltreatment related to fractures in a pediatric hospital in southern Brazil. Method Study of the Information System of Notifiable Diseases notifications and the victims' medical records between January/2016 and December/2020. Variables related to the victim, the perpetrator, the type of abuse, the presence of fractures, and their anatomical location and death were evaluated. Logistic regression was performed to identify fracture-related variables, adjusted for sex and age. The results were expressed in odds ratios and their respective 95% confidence intervals. It was considered significant p < 0.05. Results There were 276 cases, 73 infants (26.4%), male predominance (151, 54.7%), with authorship of the mistreatment by relatives (245, 96,0%), 85 (31,5%), they presented fractures, with five deaths (1.9%). Factors related to the presence of fracture: age of the victim (less than two years old; n = 82; or 2.48; 95% CI: 1.45 - 4.25), having more than two aggressors involved (n = 144; or 2.09; 95% CI: 1.16-3.75), the medium being traffic/automobile accident, (n = 52; or 2.65; 95% CI: 1.04-6.75), consult an orthopedist (n = 91; or 6.77 / 95% CI: 3.66-12.51), and the need for surgical intervention (n = 15; OR 36.72; 95% CI: 8.22-164.03). Conclusions The importance of suspicion, early identification of aggression, and the correct completion of notifications for activating the system of guaranteeing rights and removal of the aggressor was emphasized.

ABSTRACT

This study aims to investigate the views of psychologists who worked at Social Assistance Reference Centers (CRAS) associated with the Comprehensive Family Care Program (PAIF) on their work with the aforementioned program. Overall, eight psychologists who worked at CRAS units in small municipalities in Minas Gerais for at least one year participated in this research. The instrument used was a semi-structured interview script, and the data were analyzed under the content analysis (thematic) method. Thematic categories were analyzed based on the specific literature. Results indicate that the psychologists generally felt unprepared to work at CRAS since their degree in Psychology provided inadequate knowledge to deal with Social Assistance issues. This study discusses the (psycho)therapeutic nature of their practice and the need for ongoing training for their proper performance. This study highlights the need for further research that addresses the links between education in Psychology and Social Assistance and the impacts of said unpreparedness on the performance of those professionals. Such research might provide more professional/personal satisfaction and, in turn, improve the quality of the offered service.(AU)
