OBJECTIVE: To identify factors associated with hospitalization in the intensive care unit in children and adolescents with COVID-19. METHODS: This was a retrospective cohort study using secondary data of hospitalized children and adolescents (zero to 18 years old) with COVID-19 reported in Paraíba from April 2020 to July 2021, totaling 486 records. Descriptive analysis, logistic regression and multilevel regression were performed, utilizing a significance level of 5%. RESULTS: According to logistic regression without hierarchical levels, there was an increased chance of admission to the intensive care unit for male patients (OR = 1.98; 95%CI 1.18 - 3.32), patients with respiratory distress (OR = 2.43; 95%CI 1.29 - 4.56), patients with dyspnea (OR = 3.57; 95%CI 1.77 - 7.18) and patients living in large cities (OR = 2.70; 95%CI 1.07 - 6.77). The likelihood of requiring intensive care was observed to decrease with increasing age (OR = 0.94; 95%CI = 0.90 - 0.97), the presence of cough (OR = 0.32; 95%CI 0.18 - 0.59) or fever (OR = 0.42; 95%CI 0.23 - 0.74) and increasing Gini index (OR = 0.003; 95%CI 0.000 - 0.243). According to the multilevel analysis, the odds of admission to the intensive care unit increased in male patients (OR = 1.70; 95%CI = 1.68-1.71) and with increasing population size of the municipality per 100,000 inhabitants (OR = 1.01; 95%CI 1.01-1.03); additionally, the odds of admission to the intensive care unit decreased for mixed-race versus non-brown-skinned patients (OR = 0.981; 95%CI 0.97 - 0.99) and increasing Gini index (OR = 0.02; 95%CI 0.02 - 0.02). CONCLUSION: The effects of patient characteristics and social context on the need for intensive care in children and adolescents with SARS-CoV-2 infection were better estimated with the inclusion of a multilevel regression model.

La alteración aguda del estado mental en pediatría se refiere a un cambio repentino y significativo en la función cerebral y el nivel de conciencia de un niño. Puede manifestarse como confusión, desorientación, agitación, letargo o incluso pérdida de la conciencia. Esta condición es una emergencia médica, y requiere una evaluación y una atención inmediatas. Existen diversas causas de alteración aguda del estado mental en niños, algunas de las cuales incluyen infecciones del sistema nervioso central, como la meningitis o la encefalitis, los traumatismos craneoencefálicos, los trastornos metabólicos, las convulsiones o las intoxicaciones, entre otras. Este estudio tuvo como objetivo analizar, preparar y calificar la bibliografía actual para determinar las mejores recomendaciones sobre el tratamiento ante casos de alteración aguda del estado mental en pediatría de diferentes causas. El estudio se basó en la calificación de expertos en el campo para poder determinar la calificación de las recomendaciones, además de ser sometido a la revisión por parte del comité científico de la Academia Iberoamericana de Neurología Pediátrica. Nuestra guía representa una ayuda para el tratamiento de este síntoma inespecífico desde un enfoque básico y avanzado, aplicable por cualquier neurólogo pediatra.(AU)


In pediatric patients, an acute altered mental status refers to a sudden and significant change in a child’s brain function and level of consciousness. It may manifest as confusion, disorientation, agitation, lethargy or even a loss of consciousness. This condition is a medical emergency, and requires immediate evaluation and attention. There are several causes of acute altered mental status in children, including infections of the central nervous system such as meningitis or encephalitis, traumatic brain injury, metabolic disorders, seizures and poisoning, among others. The aim of this study was to analyse, prepare and classify the current literature in order to determine the best recommendations for the treatment of cases of acute altered mental status with various causes in pediatric patients. The study was based on opinions from experts in the field in order to classify the recommendations, and was submitted to the scientific committee of the Iberoamerican Academy of Pediatric Neurology for review. Our guide is an aid for the treatment of this non-specific symptom based on a basic and advanced approach, which can be applied by any pediatric neurologist.(AU)

Whole-body counters (WBC) are used in internal dosimetry forin vivomonitoring in radiation protection. The calibration processes of a WBC set-up include the measurement of a physical phantom filled with a certificate radioactive source that usually is referred to a standard set of individuals determined by the International Commission on Radiological Protection (ICRP). The aim of this study was to develop an anthropomorphic and anthropometric female physical phantom for the calibration of the WBC systems. The reference female computational phantom of the ICRP, now called RFPID (Reference Female Phantom for Internal Dosimetry) was printed using PLA filament and with an empty interior. The goal is to use the RFPID to reduce the uncertainties associated within vivomonitoring system. The images which generated the phantom were manipulated using ImageJ®, Amide®, GIMP®and the 3D Slicer®software. RFPID was split into several parts and printed using a 3D printer in order to print the whole-body phantom. The newly printed physical phantom RFPID was successfully fabricated, and it is suitable to mimic human tissue, anatomically similar to a human body i.e., size, shape, material composition, and density.

OBJECTIVE: There are conflicting data on whether fetoscopic laser photocoagulation of placental anastomoses (FLP) for treating twin-to-twin transfusion syndrome (TTTS) is associated with lower rates of overall survival. The objective of this study is to characterize survival and other associated morbidity after FLP across gestational ages of FLP. METHODS: This is a secondary analysis of prospectively collected data on patients with monochorionic-diamniotic twins that had FLP for TTTS at two centers between 2011 and 2022. Patients were divided into gestational age epochs for FLP before 18 wks, 18 0/7 - 19 6/7 wks, 20 0/7 - 21 6/7 wks, 22 0/7 - 23 6/7 wks, 24 0/7 - 25 6/7 wks and after 26 wks. Demographic characteristics, sonographic characteristics of TTTS and operative characteristics were compared across the gestational age epochs. Outcomes including overall survival, preterm delivery, preterm prelabor rupture of membranes (PPROM), intrauterine fetal demise (IUFD) and neonatal demise (NND) were also compared across gestational age epochs. Multivariate analysis was performed by fitting logistic regression models for these outcomes. Kaplan-Mejer curves were constructed to compare the interval from PPROM to delivery for each gestational age epoch. RESULTS: There were 768 patients that met inclusion criteria. The dual survival rate was 61.3% for FLP performed prior to 18 weeks compared to 78.0% - 86.7% across later gestational age epochs. This appears to be related to increased rates of donor IUFD following FLP performed before, versus after 18 weeks (28.0% vs. 9.3% - 14.1%). Rates of recipient IUFD/NND and donor NND were similar regardless of gestational age of FLP. Rates of PPROM were higher for earlier FLP, ranging from 45.6% for FLP before 18 weeks to 11.9% for FLP at 24 - 26 weeks gestational age. However, the gestational age of delivery was similar across gestational age epochs with a median of 31.7 weeks. In multivariate analysis, donor loss was independently associated with FLP before 18 weeks after adjusting for selective fetal growth restriction, Quintero stage and other covariates. PPROM and PTD were also associated with FLP before 18 weeks after adjusting for cervical length, placental location, trocar size, laser energy and amnioinfusion. CONCLUSION: FLP performed at earlier gestational ages is associated with lower overall survival, which is driven by higher risk of donor IUFD, as opposed to differences in PPROM or PTD. Baccharin is one of the major compounds found in Brazilian green propolis and its botanical source, Baccharis dracunculifolia. Considering the biological effects of propolis and B. dracunculifolia, this study aims to evaluate the analgesic and anti-inflammatory potential of baccharin. The neurodepressor potential was performed by the open field test, analgesia by mechanical stimulation with Dynamic Plantar Aesthesiometer, and by thermal stimulation with Hargreaves apparatus. In addition, the anti-inflammatory potential was achieved by the paw edema assay, histopathological evaluation, and NF-kB expression. Doses of 2.5, 5, and 10 mg/kg of baccharin were evaluated. After euthanasia, plantar tissue was collected and prepared for histology. As a result, analgesic activity was observed at a dose of 10 mg/kg of baccharin in thermal stimulation under an inflammatory process and anti-inflammatory potential at a dose of 5 mg/kg of baccharin from the second hour in the paw edema test. A decrease in cellular infiltrate and down-modulation of NF-kB, besides the reduction of edema in the histopathology was observed. There was no evidence of kidney and liver toxicity and neurodepressive potential at the doses tested. Thus, baccharin has a promising anti-inflammatory effect possibly associated with antiedematogenic activity by inhibiting mediators such as prostaglandins, inhibiting the migration of polymorphonuclear cells, and modulating NF-kB expression.

OBJECTIVES: To determine the association between elevated (> 1.5 multiples of the median (MoM)) middle cerebral artery (MCA) peak systolic velocity (PSV) and fetal demise of the donor twin in pregnancies complicated by twin-twin transfusion syndrome (TTTS) in the absence of twin anemia-polycythemia sequence (TAPS). Secondary objectives were to evaluate if donor or recipient MCA-PSV is associated with a risk for their corresponding fetal death, and to compare the proportion of donor fetuses with low MCA pulsatility index (PI) among donor twins with high MCA-PSV and those with normal MCA-PSV to evaluate the contribution of blood-flow redistribution to the fetal brain in donor twins with high MCA-PSV. METHODS: This prospective cohort study included TTTS cases that underwent laser surgery between 2011 and 2022 at a single center. TAPS cases were excluded from the study. Multivariable and Poisson regression analysis were performed to explore the association between isolated elevated donor MCA-PSV and fetal demise, adjusted for TTTS stage, selective fetal growth restriction (sFGR) and other confounders. RESULTS: Of 660 TTTS cases, donor MCA-PSV was not recorded in 48 (7.3%) cases. Of the remaining 612 patients, nine (1.5%) were lost to follow-up and 96 TAPS cases were excluded; thus, 507 cases were included in the study. High donor MCA-PSV was seen in 6.5% (33/507) of cases and was an independent risk factor for donor fetal demise (adjusted relative risk (aRR), 4.52 (95% CI, 2.72-7.50)), after adjusting for confounders. Regression analysis restricted to each Quintero TTTS stage demonstrated that high donor MCA-PSV was an independent risk factor for fetal demise of the donor in Quintero Stage II (aRR, 14.21 (95% CI, 1.09-186.2)) and Quintero Stage III (aRR, 3.41 (95% CI, 1.82-6.41)). Donor MCA-PSV in MoM was associated with fetal demise of the donor (area under the receiver-operating-characteristics curve (AUC), 0.69; P < 0.001), but recipient MCA-PSV in MoM was not associated with fetal demise of the recipient (AUC, 0.54; P = 0.44). A higher proportion of donor twins in the group with high MCA-PSV had a low MCA-PI compared to the group with normal MCA-PSV (33.3% vs 15.5%; P = 0.016). CONCLUSIONS: Elevated donor MCA-PSV without TAPS prior to laser surgery for TTTS is associated with a 4-fold increased risk for donor fetal demise, adjusted for sFGR, TTTS stage and other confounders. Doppler evaluation of donor MCA-PSV prior to laser surgery may help stratify TTTS staging to evaluate the risk of donor fetal demise. © 2024 International Society of Ultrasound in Obstetrics and Gynecology.

RÉSUMÉ

CRISPR interference (CRISPRi) is the leading technique to silence gene expression in bacteria; however, design rules remain poorly defined. We develop a best-in-class prediction algorithm for guide silencing efficiency by systematically investigating factors influencing guide depletion in genome-wide essentiality screens, with the surprising discovery that gene-specific features substantially impact prediction. We develop a mixed-effect random forest regression model that provides better estimates of guide efficiency. We further apply methods from explainable AI to extract interpretable design rules from the model. This study provides a blueprint for predictive models for CRISPR technologies where only indirect measurements of guide activity are available.

This research aims to identify critical contamination points by nutrients, their possible origin (point and nonpoint sources), their spatial distribution, and possible attenuation by natural and anthropogenic processes. The study area is the Velhas River Basin, located in the Southeast Region of Brazil (17.0°-20.5° S; 43.5°-45.0°W). A historical series of water quality monitoring, land cover map, demographic and agricultural censuses, sewage treatment diagnostics, and local hydrographic networks were used to achieve the objectives. In addition, the regions were divided into incremental areas, enabling individualized analyses of each sub-basin. Descriptive statistics, seasonality, categorized data tests, agglomerative hierarchical cluster analysis, and principal component analysis were used. There was a significant contribution of nutrients in the most important urban agglomeration of the basin, resulting in peak concentrations measured at that place. Although the values were reduced by the mouth (650 km), the percentage of legislation violations remained high. The effects of punctual contamination were intensified by the low percentage of treated sewage in the basin, the absence of adequate treatment technologies to remove nutrients, and the disorderly urbanization. Furthermore, it was estimated that the nutrient load from animal husbandry is approximately 75% of the load from domestic effluents due to the high number of cattle in the basin and the low percentage of forests.

Neglected tropical diseases are significant causes of death and temporary or permanent disability for millions living in developing countries. Unfortunately, there is no effective treatment for these diseases. Thus, this work aimed to conduct a chemical analysis using HPLC/UV and GC/MS to identify the major constituents of the hydroalcoholic extracts of Capsicum frutescens and Capsicum baccatum fruits, evaluating these extracts and their constituents' schistosomicidal, leishmanicidal and trypanocidal activities. The results obtained for the extracts of C. frutescens are better when compared to those obtained for C. baccatum, which can be related to the different concentrations of capsaicin (1) present in the extracts. The lysis of trypomastigote forms results for capsaicin (1) led to a significant value of IC50 = 6.23 µM. Thus, the results point to capsaicin (1) as a possible active constituent in these extracts.

The use of various herbs and their compounds has been a strategy widely used in the fight against various human diseases. For example, rosmarinic acid, a bioactive phenolic compound commonly found in Rosemary plants (Rosmarinus officinalis Labiatae), has multiple therapeutic benefits in different diseases, such as cancer. Therefore, the study aimed to evaluate in silico and in vitro the inhibition potential of the enzyme Elastase from the porcine pancreas by rosmarinic acid isolated from the plant species R. officinalis Linn. Through Molecular Docking, the mechanism of action was investigated. In addition, rosmarinic acid presented a range of 5-60 µg/mL and significantly inhibited Elastase. At 60 µg/mL, there was an inhibition of 55% on the enzymatic activity. The results demonstrate the inhibition of Elastase by rosmarinic acid, which can lead to the development of new enzyme inhibitors that can be an inspiration for developing various drugs, including anticancer drugs.

Abstract Objective Autism spectrum disorder (ASD) is characterized by persistent deficits in social communication and social interaction and by restricted and repetitive patterns of behavior. Some studies have shown that substances derived from Cannabis sativa improve the quality of life of children with ASD without causing serious adverse effects, thus providing an alternative therapeutic option. The objective of this study was to evaluate the efficacy and safety of a cannabis extract rich in cannabidiol (CBD) in children with ASD. Methods In this randomized, double-blind, placebo-controlled clinical trial, 60 children, aged from 5 to 11 years, were selected and divided into two groups: the treatment group, which received the CBD-rich cannabis extract, and the control group, which received the placebo. They both used their respective products for a period of 12 weeks. Statistical analysis was done by two-factor mixed analysis of variance (two-way ANOVA). Results Significant results were found for social interaction (F1,116 = 14.13, p = 0.0002), anxiety (F1,116 = 5.99, p = 0.016), psychomotor agitation (F1,116 = 9.22, p = 0.003), number of meals a day (F1,116 = 4.11, p = 0.04), and concentration (F1,48 = 6.75, p = 0.01), the last of which was only significant in mild ASD cases. Regarding safety, it was found that only three children in the treatment group (9.7%) had adverse effects, namely dizziness, insomnia, colic, and weight gain. Conclusion CBD-rich cannabis extract was found to improve one of the diagnostic criteria for ASD (social interaction), as well as features that often co-exist with ASD, and to have few serious adverse effects.

According to the multilevel analysis, the odds of admission to the intensive care unit increased in male patients (OR = 1.70; 95%CI = 1.68-1.71) and with increasing population size of the municipality per 100,000 inhabitants (OR = 1.01; 95%CI 1.01-1.03); additionally, the odds of admission to the intensive care unit decreased for mixed-race versus non-brown-skinned patients (OR = 0.981; 95%CI 0.97 - 0.99) and increasing Gini index (OR = 0.02; 95%CI 0.02 - 0.02). Conclusion The effects of patient characteristics and social context on the need for intensive care in children and adolescents with SARS-CoV-2 infection were better estimated with the inclusion of a multilevel regression model.


RESUMO Objetivo Identificar fatores associados ao internamento na unidade de terapia intensiva de crianças e adolescentes com COVID-19. Método Estudo de coorte retrospectiva, com dados secundários, de crianças e adolescentes hospitalizados (zero a 18 anos), notificados com COVID-19 na Paraíba, de abril de 2020 a julho de 2021, totalizando 486 registros. Foram realizadas análise descritiva, regressão logística e regressão multinível, considerando o nível de significância de 5%. Resultados Na regressão logística sem níveis hierárquicos, ocorreu aumento da chance de internamento na unidade de terapia intensiva em pacientes do sexo masculino (RC = 1,98; IC95% 1,18 - 3,32), com desconforto respiratório (RC = 2,43; IC95% 1,29 - 4,56), dispneia (RC = 3,57; IC95% 1,77 - 7,18) e residentes em cidades com grande porte populacional (RC = 2,70; IC95% 1,07 - 6,77). Foi observada diminuição da chance de cuidados intensivos com aumento da idade em anos (RC = 0,94; IC95%=0,90 - 0,97), presença de tosse (RC = 0,32; IC95% 0,18 - 0,59), febre (RC = 0,42; IC95% 0,23 - 0,74) e aumento no Índice de Gini (RC = 0,003; IC95% 0,000 - 0,243). Na análise multinível, a chance de internamento na unidade de terapia intensiva aumentou no sexo masculino (RC = 1,70; IC95%=1,68-1,71) e por conta do aumento no porte populacional do município a cada 100 mil habitantes (RC = 1,01; IC95% 1,01 - 1,03); a chance de internamento na unidade de terapia intensiva diminuiu em pacientes pardos versus não pardos (RC = 0,981; IC95% 0,97 - 0,99) e por conta do aumento a cada pontuação do Índice de Gini (RC = 0,02; IC95% 0,02 - 0,02). Conclusão Os efeitos das condições próprias do paciente e do contexto social na necessidade de cuidados intensivos em crianças e adolescentes com infecção pelo SARS-CoV-2 são mais bem estimados com a inclusão de um modelo de regressão multinível nas análises.

RÉSUMÉ

Resumo Trata-se de um Estudo de Avaliabilidade (EA) do Programa de Vigilância e Controle da Leishmaniose Visceral (PVCLV), com vistas à construção de bases para uma futura avaliação. Utilizou-se uma abordagem descritivo-qualitativa em quatro etapas: (1) Delimitação do programa; (2) Construção do modelo lógico, com a explicitação dos recursos necessários, atividades a serem desempenhadas e resultados esperados, bem como das matrizes avaliativas, com as dimensões de análise, categorias e indicadores; (3) Identificação de perguntas avaliativas e escolha do desenho da avaliação; (4) Recomendações dos autores sobre o PVCLV e a pertinência da realização da avaliação do programa. Este EA concluiu que a intervenção está apta a ser avaliada, porque foi possível identificar o delineamento da intervenção; o consenso entre os especialistas sobre ela; e a identificação de perguntas avaliativas que subsidiam uma posterior avaliação. Destaca-se a importância da participação de atores-chave em todas as etapas de desenvolvimento do estudo, os quais têm papel central no processo de desenvolvimento do programa nos diversos níveis de gestão. Espera-se que esta pré-avaliação contribua para identificação de prioridades e redirecionamento das ações, de modo a assegurar factibilidade na avaliação do PVCLV, gerando ações de intervenção em tempo oportuno.


Abstract This is an Evaluability Study (ES) of the Visceral Leishmaniasis Surveillance and Control Program, with a view to building bases for a future evaluation. A descriptive-qualitative approach was used in four steps: (1) Delimitation of the program; (2) Construction of the logical model, with the explanation of the necessary resources, the activities to be performed and the expected results, as well as the evaluative matrices, with the dimensions of analysis, categories and indicators; (3) Identification of evaluative questions and choice of evaluation design; (4) Authors' recommendations on Visceral Leishmaniasis Surveillance and Control Program and the pertinence of carrying out an evaluation of the program. This ES concluded that the intervention was able to be evaluated, because it was possible to identify the intervention design; the consensus among specialists about it; and the identification of evaluative questions that support a subsequent evaluation. It highlights the importance of the participation of key actors in all stages of the study's development, which have a central role in the program's development process at the various management levels. It is expected that this pre-assessment will contribute to identifying priorities and redirecting actions, to ensure feasibility in the assessment of Visceral Leishmaniasis Surveillance and Control Program, generating intervention actions promptly
