RÉSUMÉ

(1) Background: Reverse septal movement (RSM) often occurs after cardiac surgery, consisting of a paradoxical systolic movement of the interventricular septum. In this retrospective study, we aimed to investigate possible determinants of RSM after coronary artery bypass surgery (CABG). (2) Methods: Patients who underwent CABG with on- or off-pump techniques at our center from March 2019 to October 2021 were retrospectively included. Exclusion criteria were: exposure to combined procedures (e.g., valve implantation), prior cardiac surgery, intraventricular conduction delays, and previous pacemaker implantation. Laboratory tests and echocardiographic and cardiopulmonary bypass (CPB) duration data were collected. (3) Results: We enrolled 138 patients, of whom 32 (23.2%) underwent off-pump CABG. Approximately 89.1% of the population was male; the mean age was 70 ± 11 years. There was no difference in RSM incidence in patients undergoing the off-pump and on-pump techniques (71.9% vs. 62.3%; p = 0.319). In patients undergoing on-pump surgery, the incidence of RSM was slightly higher in longer CPB procedures (OR 1.02 (1.00-1.03) p = 0.012), and clamping aortic time was also greater (OR 1.02 (1.00-1.03) p = 0.042). (4) Conclusions: CPB length seems to be correlated with a higher RSM appearance. This better knowledge of RSM reinforces the safety of CABG and its neutral effect on global biventricular function.

RÉSUMÉ

Geostatistics is a tool that can be used to produce maps with the distribution of nutrients essential for the development of plants. Therefore, the present study aimed to analyze the spatial variation in chemical attributes of soils under oil palm cultivation in agroforestry systems in the eastern Brazilian Amazon, and their spatial dependence pattern. Sixty spatially standardized and georeferenced soil samples were collected at each of three sampling sites (DU1, DU2, and DU3) at 0-20 cm depth. Evaluated soil chemical attributes were pH, Al3+, H+Al, K+, Ca2+, Mg2+, cation exchange capacity (CEC), P, and organic matter (OM). The spatial dependence of these variables was evaluated with a semivariogram analysis, adjusting three theoretical models (spherical, exponential, and Gaussian). Following analysis for spatial dependence structure, ordinary kriging was used to estimate the value of each attribute at non-sampled sites. Spatial correlation among the attributes was tested using cokriging of data spatial distribution. All variables showed spatial dependence, with the exception of pH, in one sampling site (DU3). Highest K+, Ca2+, Mg2+, and OM levels were found in the lower region of two sampling sites (DU1 and DU2). Highest levels of Al3+ and H+Al levels were observed in the lower region of sampling site DU3. Some variables were correlated, therefore cokriging proved to be efficient in estimating primary variables as a function of secondary variables. The evaluated attributes showed spatial dependence and correlation, indicating that geostatistics may contribute to the effective management of agroforestry systems with oil palm in the Amazon region.

The soil carbon dioxide (CO2) efflux dynamics and its controlling factors of Amazonian agroforestry systems are poorly understood. The objective of this study was to evaluate the temporal variation of soil CO2 efflux in oil palm-based agroforestry systems and the relation between efflux and biotic (microbial and total soil carbon, microbial respiration, fine roots, individual components of agroforestry systems (AFS)) and abiotic factors (soil moisture and temperature). The measurements were taken during the less rainy (December 2010) and rainy (May 2011) periods. The soil CO2 efflux was highest during the rainy season, probably due to increased microbial activity influenced by climatic factors coupled with biotic factors. The soil CO2 efflux correlated positively with soil moisture and microbial biomass carbon and negatively with soil temperature and metabolic quotient, but these correlations were weak. The soil CO2 efflux was sensitive to the type of agroforestry system and to rainfall seasonality.

The success of oil palm (Elaeis guineensis Jacq.)-based agroforestry systems (oil palm-AFS) depends on sustainable soil management, especially of soil chemical and microbiological characteristics. Our objective was to evaluate the impact of oil palm-AFS on soil mineral and microbial nitrogen (N) in contrasting rainfall seasons. We evaluated different soil nitrogen (N) forms (microbial-N, nitrate, ammonium) and soil carbon concentration in oil palm-AFS with low and high diversity of species planted, which were compared with an adjacent 13-yr-old secondary forest. Most variables (total N, C:N ratio, microbial-N, microbial-N:total N ratio, ammonium, and net nitrification rate) varied only in response to rainfall seasonality. Soil C was significantly higher in the high diversity AFS (15.6 mg g-1) than in the secondary forest (13.0 mg g-1). In the rainy season, nitrate concentration (5.1 mg N kg-1 soil) was higher in the high diversity AFS than in other vegetation types; consequently, the average soil ammonium concentration (9.6 mg N kg-1 soil) was significantly lower in the high diversity AFS. Net N mineralization in the low diversity AFS (0.1 mg N kg-1 soil day-1) in the dry season was significantly lower than in other vegetation types. The soil variables were more sensitive to the rainfall seasonality than to the conversion of secondary forest to oil palm-based agroforestry systems.

BACKGROUND: Takotsubo cardiomyopathy is an acute cardiac syndrome characterized by transient LV regional wall motion abnormalities (with peculiar apical ballooning appearance), chest pain or dyspnea, ST-segment elevation and minor elevations of cardiac enzyme levels CASE PRESENTATION: A 68-year-old woman was admitted to the Emergency Department because of sudden onset chest pain occurred while transferring her daughter, who had earlier suffered a major seizure, to the hospital. The EKG showed sinus tachycardia with ST-segment elevation in leads V2-V3 and ST-segment depression in leads V5-V6, she was, thus, referred for emergency coronary angiography. A pre-procedural transthoracic echocardiogram revealed regional systolic dysfunction of the LV walls with hypokinesis of the mid-apical segments and hyperkinesis of the basal segments. Coronary angiography showed patent epicardial coronary arteries; LV angiography demonstrated the characteristic morphology of apical ballooning with hyperkinesis of the basal segments and hypokinesis of the mid-apical segments. The post-procedural course was uneventful; on day 5 after admission the echocardiogram revealed full recovery of apical and mid-ventricular regional wall-motion abnormalities. CONCLUSION: Takotsubo cardiomyopathy is a relatively rare, unique entity that has only recently been widely appreciated. Acute stress has been indicated as a common trigger for the transient LV apical ballooning syndrome, especially in postmenopausal women. The present report is a typical example of stress-induced takotsubo cardiomyopathy in a Caucasian Italian postmenopausal woman.
