RÉSUMÉ

AIM: This study aimed to determine the effect of multidimensional training program on children with constipation and fecal incontinence and their mothers on anxiety and their constipation management. METHOD: The study had an experimental design with a pretest-posttest control group. The intervention group consisted of children diagnosed with functional constipation (n=20) and fecal incontinence (n=21) and their mothers in the Pediatric Surgery Clinic. The control group consisted of healthy (n=20) children who applied to two different Family Health Centers for any reason. Data were collected between February 2016 and June 2017 using a descriptive questionnaire, a nutrient consumption frequency form, the Rome III diagnostic criteria form, and the Trait Anxiety Scale for Children. RESULTS: After the training program, the mean score of the Trait Anxiety Scale for Children decreased in both intervention groups (p<0.01) and increased in the control group (p<0.05). In addition, the frequency of the consumption of milk, caffeinated drinks, and white bread was reduced in the children of both intervention groups after the training program. The consumption of dried legumes, vegetables, and whole wheat bread increased. According to the Rome III diagnostic criteria, it was determined that constipation symptoms were not observed in 50% of the children. CONCLUSION: The training program had a significant influence on the prevention and management of constipation and anxiety. Given that constipation is associated with both physical and mental health outcomes, the approach for the treatment should be holistic in nature, involving a multidisciplinary team.

RÉSUMÉ

Thiamine pyrophosphate (TPP) is the active metabolite of thiamine. This study aimed to investigate the effects of thiamine and TPP on cisplatin-induced peripheral neuropathic pain (PNP). Male albino Wistar type Rattus norvegicus were divided into six groups (n=6) that received 2 mg/kg cisplatin (CIS), 25 mg/kg thiamine (TM), 2 mg/kg cisplatin+25 mg/kg thiamine (CTM), 25 mg/kg TPP (TPP), 2 mg/kg cisplatin+25 mg/kg TPP (CTPP), or distilled water (healthy group; HG) for 8 days intraperitoneally. Analgesic effect was measured with a Basile Algesimeter. IL-1ß, malondialdehyde (MDA), total glutathione (tGSH), thiamine, and TPP were determined in blood samples. Histopathological examinations were performed on removed sciatic nerves. The percent analgesic effects of the CTM and CTPP groups were calculated to be 21.3% and 82.9%, respectively. Increased production of IL-1ß and MDA by cisplatin was inhibited by TPP, while it was not inhibited by thiamine. Conversion of thiamine to TPP significantly decreased in the CIS group. Histopathological and biochemical investigations demonstrated that hyperalgesia and sciatic nerve damage developed in the CIS and CTM groups with low TPP levels. These results indicate that cisplatin inhibits the formation of TPP from thiamine, leading to severe PNP. This finding suggests that TPP may be more beneficial than thiamine for the treatment of cisplatin-induced PNP.

RÉSUMÉ

In polycystic ovary syndrome (PCOS), myo-inositol (MI) supplements have shown many beneficial effects. In this study, therefore, we aimed to investigate the serum level of myo-inositol oxygenase (MIOX), which is the only enzyme catalyzing MI in vivo, in patients with PCOS. Serum MIOX enzyme levels and other laboratory parameters were compared between sixty patients, who were diagnosed with PCOS for the first time, and sixty healthy individuals at similar age and sex. MIOX serum levels were not different between two groups (p = 0.7428). MIOX median and 95% CI were 19.4 and 10.6-39.1 in the control group and 16.4 and 7.6-46.2 in the patient group respectively. Demographic data, biochemical and hematological parameters, hormone parameters were not different except from the lymphocyte count between the two groups. Lymphocyte count was higher in the patient group. Although the ratio of LH/FSH was higher in the patient group, it was not statistically significant. Our results suggest that serum MIOX levels do not change in PCOS. It was, therefore, concluded that MI deficiency observed in PCOS was not related to the level of MIOX enzyme which cleaves MI.

RÉSUMÉ

OBJECTIVE: This study was designed to examine, with the use of Berry's acculturation concept (2003), personal self-esteem and collective esteem in the acculturation of young Turks in Germany. Further, it was aimed to examine whether ethnic identity is linked to better mental health. METHOD: Consecutive psychiatric primary care patients with Turkish background were screened with the 5-item Acculturation Questionnaire, adopted from the Latin American Validated Acculturation Scale. The interviewed patients were also assessed for psychiatric disorders according to ICD-10. RESULTS: By using the acculturation questionnaire among 220 patients included to this study, 154 (70%) patients, (88 male- 57,14%, mean age:22,1 sd. +/- 3,26 and 66 female- 42,85%, mean age: 21,73 sd. +/- 1,19) were found to be relatively good adjusted (group 1), and of 66 (30%) patients (44 male- 66,6%, mean age:26,3 sd. +/-3,39 and 22 female- 33,3%, mean age:25,88 sd. +/- 3,41) were found to be relatively poor adjusted (group 2) to the host community. The psychiatric co-morbidity showed in these two groups two distinct patterns (p< 0.001). Endogenous depression and the depression caused by the conflicts in the family-milieu are significantly more common in the group I. Unlike, the depression triggered by the psycho-social maladjustment is clearly more prevalent in the group II. The predictors of good adaptation were shown to be: bilingualism, bicultural identity and Germany as country of birth, whereas poor adaptation was related to: dominance of ethnic culture and language, and being born outside of Germany. CONCLUSION: The research presented provides firm evidence for a bicultural identity through assessments of several domains of acculturation: language spoken most of the time, language thought, ethnic identity, birthplace and the degree of adjustment to the host community. Cultural identity is one of the key determinants of mental health in evaluating adaptation to the host culture among immigrants. It is vital for the mental health professionals to understand the roles, context, and therapeutic consequences that originate from culture.
