Abstract Background Recently, a new heart failure (HF) classification was made considering the left ventricular ejection fraction (LVEF) phenotype. Comprehensive assessments of the groups are required to guide patient management. Objective To determine the differences in sociodemographic, clinical, functional aerobic capacity, and health-related quality of life (HRQOL) variables in patients with HF classified with different LVEFs and to explore the correlations between the variables. Methods This work is a cross-sectional descriptive and correlational study. Three groups of patients with HF (LVEF≥50%, LVEF<40%, and LVEF40-49%) were compared. Sociodemographic, clinical variables and functional aerobic capacity with Sit to Stand (STS), 6-minute walk test (6MWT), Duke Activity Status Index (DASI), Minnesota Living with HF Questionnaire (MLFHQ), and Patient Health Questionnaire 9 (PHQ-9) were considered. The Chi-square test, one-way analysis of variance (ANOVA) test, and Spearman's correlation were used for statistical analysis. The statistical significance level was set at 5%. Results A total of 209 patients were admitted with a diagnosis of HF, with a more significant number of men. Marital status was a predominantly stable union in the HF with preserved ejection fraction (HFpEF) and HF with mid-range ejection fraction (HFmrEF) groups. A sedentary lifestyle was lower in the HF with reduced ejection fraction (HFrEF) group 59 (84.3%), p-value = 0.033, and the angina pectoris was higher in the HFpEF 30 (42.9%). Systolic blood pressure at the end of the 6MWT evidenced a higher score in HFpEF 132.0±17.25 concerning HFrEF 128.0±16.57, p-value=0.043. The fat percentage was higher in HFpEF 30.20±8.80 regarding the HFmrEF group 26.51±7.60, p-value = 0.028. Conclusion There were significant differences according to the LVEF classification in marital status, angina symptoms, fat percentage, and blood pressure at rest.

Damage to the small intestine caused by non-steroidal anti-inflammatory drugs (NSAIDs) occurs more frequently than in the upper gastrointestinal tract, is more difficult to diagnose and no effective treatments exist. Hence, we investigated whether probiotics can control the onset of this severe condition in a murine model of intestinal inflammation induced by the NSAID, indomethacin. Probiotic supplementation to mice reduce the body weight loss, anemia, shortening of the small intestine, cell infiltration into the intestinal tissue and the loss of Paneth and Goblet cells associated with intestinal inflammation. Furthermore, a high antimicrobial activity in the intestinal fluids of mice fed with probiotics compared to animals on a conventional diet was elicited against several pathogens. Interestingly, probiotics dampened the oxidative stress and several local and systemic markers of an inflammatory process, as well as increased the secretion of IL-10 by regulatory T cells. Even more importantly, probiotics induced important changes in the large intestine microbiota characterized by an increase in anaerobes and lactobacilli, and a significant decrease in total enterobacteria. We conclude that oral probiotic supplementation in NSAID-induced inflammation increases intestinal antimicrobial activity and reinforces the intestinal epithelial barrier in order to avoid pathogens and commensal invasion and maintain intestinal homeostasis.

RÉSUMÉ

Psapharochrus auratus sp. nov., from Colombia, is described and illustrated. A new combination, Psapharochrus penrosei (Thomson, 1864) is proposed. The geographical distribution of seven species of Lamiinae is expanded to include them in the Colombian fauna, and eight new department records from Colombia are reported. Moreover, taxonomic notes in Lamiinae are provided.

RÉSUMÉ

Endovascular therapy has emerged as an essential part of the management we can offer patients suffering from peripheral arterial disease. The AHA/ACCF guidelines deemed ballon angioplasty as a reasonable alternative for patients with limb threatening lower extremity ischemia who are not candidates for an autologus venous graft. Endovascular treatment is most useful for the treatment of critical limb ischemia and should ensure adequate proximal flow before engaging in interventions of distal disease.To increase procedure success rate, a thorough diagnostic evaluation is fundamental. This evaluation must take into account amount of calcium, no flow occlusion, length of occlusion, and presence of collaterals. There are different tools and procedure techniques available. Among these are the medicated ballon angioplasty and atherectomy by laser or high-speed drill, among others. Further studies may consolidate endovascular intervention as a safe and effective management for patients with lower extremity arterial disease and possibly cause a change in the actual practice guidelines.

Typically, the onset of candidiasis is characterised by the appearance of a biofilm of Candida albicans, which is associated with several diseases including oral candidiasis in young and elderly people. The objective of this work was to investigate the in vitro fungicidal activity as well as the antibiofilm activity of ambroxol (AMB) against C. albicans growth. In the present investigation, the fungicidal activity of AMB was established using the cell viability 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Also the minimum inhibitory concentration (MIC) of AMB required to inhibit the fungal growth was determined. Simultaneously, the antibiofilm activity of AMB was evaluated using fluorescence microscopy. The study revealed that 2 mg ml(-1) of AMB exhibited higher fungicidal activity than 3.3 mg ml(-1) of terbinafine, one of most common commercial antifungals. A MIC of 1 mg ml(-1) was determined for AMB to interfere with C. albicans growth. Furthermore, AMB was found to be effective in inhibiting the biofilm formation of C. albicans and exerted its fungicidal activity against the fungal cells interspersed in the preformed biofilm. The study suggests a potential role of the mucolytic agent, AMB, as an interesting therapeutic alternative in the treatment of oral candidiasis.

Peach (Prunus persica L.) is the most important stone fruit tree grown in Spain and is the second most important fruit crop in Europe. The influence of eight Prunus rootstocks (GF-677, Krymsk® 86, PADAC 97-36, PADAC 99-05, PADAC 9912-03, PADAC 0024-01, PAC 0021-01 and PAC 0022-01) on vigor, yield and fruit quality traits of 'UFO 3' flat peach cultivar was studied. The highest trunk cross sectional area was exhibited by GF-677 and the lowest by PADAC 99-05, while intermediate values were found on the other rootstocks. The highest yield efficiency was found on PADAC 99-05, PAC 0021-01, PAC 0022-01 and PADAC 0024-01 and the lowest was shown on Krymsk® 86. The fruit quality parameters measured were color, fruit and stone weights, equatorial diameter, pulp thickness, pulp yield, firmness, pH, soluble solids content and titratable acidity. 'UFO 3' grafted on GF-677 resulted in the largest fruit weight, while the smallest was on PADAC 99-05. Fruits of 'UFO 3' showed a tendency to have higher firmness, higher red colored skin and RI when grafted on PADAC 99-05.

La insuficiencia renal crónica es el resultado del deterioro progresivo e irreversible de la función renal, que genera incapacidad del riñón para remover los productos de desecho y mantener el equilibrio ácido básico. El objetivo de este estudio fue identificar las causas de insuficiencia renal crónica en nuestros pacientes y su importancia relativa en este desenlace. Se hizo un estudio descriptivo retrospectivo de corte transversal, basado en la revisión de 367 registros pertenecientes al Servicio de Nefrología Infantil, de pacientes con diagnóstico de insuficiencia renal crónica (IRC), que acudieron al Servicio de Consulta Externa del Hospital Universitario San Vicente de Paúl, de Medellín, entre el 01 de enero de 1960 y el 30 de agosto de 2010. De los 367 pacientes, 199 (54,2%) fueron hombres y 168 (45,8%), mujeres; las enfermedades predominantes como causa de la IRC fueron: reflujo vésico-ureteral (37,1%), hidronefrosis (24,0%), valvas de la uretra posterior (13,4%), glomerulonefritis aguda (12,3%), síndrome nefrótico (11,2%) e hipoplasia renal (9,3%).


Chronic renal failure (CRF) results from progressive and irreversible deterioration of kidney function; it manifests as the inability to remove waste products and to maintain acid-base balance. The objectives of this study were to identify the diseases responsible for CRF in our patients and their relative importance in such outcome. A descriptive retrospective study was carried out, based on the review of the charts of 367 children with the diagnosis of CRF treated at the Pediatric Nephrology Service (Hospital Universitario San Vicente de Paúl, Medellín, Colombia) between 01 January 1960 and August 30, 2010. One hundred and ninety nine patients (54,2%) were males, and 168 (45,8), females. The predominant diseases as cause of CRF were: vesico-ureteral reflux (37.1%), hydronephrosis (24.0%), posterior urethral valves (13.4%), acute glomerulonephritis (12.3%), nephrotic syndrome (11.2%), and kidney hypoplasia (9.3%).

Introducción: El Herpes Labial Recurrente supone una condición inmunológica alterada, tal como una hiperactividad de células T-reguladoras CD4+CD25+ (Treg). Éstas ejercen control sobre la tolerancia periférica y reducen el riesgo inmunopatológico, suprimiendo otras líneas celulares. Por ende, la supresión ejercida sobre la reacción inmune antiviral podría afectar negativamente el curso de la infección. Este contexto ha impulsado la búsqueda de nuevas alternativas inmunomoduladoras como la Equinácea purpúrea. Dada su propiedad inmunosupresora, se propone en el tratamiento del Herpes Labial Recurrente. Metodología: Estudio clínico prospectivo que analiza las subpoblaciones linfocitarias en 12 pacientes con Herpes Labial Recurrente, antes y después de recibir Equinácea purpúrea (30 gotas tres veces al día durante siete días).Resultados: En comparación con individuos sanos, los pacientes presentan una respuesta aumentada de células Treg. Esta condición se reduce significativamente tras recibir Equinácea purpúrea (515 + 145 y 432 + 113 cel/mm3 antes y después del tratamiento, respectivamente, p < 0,005). Conclusión: La hiperactividad de células Treg podría explicar el estado de inmunosupresión de estos pacientes y favorecería la persistencia viral. Se propone esta fitomedicina como una alternativa inmunoterapéutica beneficiosa.


Background: Recurrent Herpes Labialis patients may suffer from immunological alterations, such as CD4+CD25+Regulatory-T Cell (Treg) hyperactivity. These cells control peripheral tolerance and reduce immunopathology risk by suppressing other immunological cells. Hence, the Treg cell suppression on the antiviral immune reaction may perturb adversely the herpes infection outcome. This scenario has forced physicians to explore new immunomodulatory alternatives in Phytomedicine, such as Echinacea purpurea. Regarding the immunosuppressive property, it has been challenged to be employed in the Recurrent Herpes Labialis management. Methods: Clinical prospective study that analyzed lymphocytic subpopulation profile in twelve patients with Recurrent Herpes Labialis, before and after receiving E. purpurea (30 drops three times a day during seven days). Results: Comparing to healthy subjects, patients presented an enlarged Treg cell response. This condition became significantly reduced after receiving E. purpurea. (515 + 145 and 432 + 113 cel, before and after treatment respectively, p < 0.005). Conclusion: The intensified Treg cell activity may elucidate the immune suppression these patients undergo, aiding the viral persistence and survival. This proposes E. purpurea asa beneficial immunotherapeutic alternative.

BACKGROUND: No information is available about the financial impact of central venous catheter (CVC)-associated bloodstream infection (BSI) in Mexico. OBJECTIVE: To calculate the costs associated with BSI in intensive care units (ICUs) in Mexico City. DESIGN: An 18-month (June 2002 through November 2003), prospective, nested case-control study of patients with and patients without BSI. SETTING: Adult ICUs in 3 hospitals in Mexico City. PATIENTS AND METHODS: A total of 55 patients with BSI (case patients) and 55 patients without BSI (control patients) were compared with respect to hospital, type of ICU, year of hospital admission, length of ICU stay, sex, age, and mean severity of illness score. Information about the length of ICU stay was obtained prospectively during daily rounds. The daily cost of ICU stay was provided by the finance department of each hospital. The cost of antibiotics prescribed for BSI was provided by the hospitals' pharmacy departments. RESULTS: For case patients, the mean extra length of stay was 6.1 days, the mean extra cost of antibiotics was US dollars 598, the mean extra hospital cost was US dollars 11,591, and the attributable extra mortality was 20%. CONCLUSIONS: In this study, the duration of ICU stay for patients with central venous catheter-associated BSI was significantly longer than that for control patients, resulting in increased healthcare costs and a higher attributable mortality. These conclusions support the need to implement preventive measures for hospitalized patients with central venous catheters in Mexico.

We studied the kinetic roughening dynamics of drying wet paper. The configurations of dry paper sheets are found to be self-similar, rater than self-affine. Accordingly, the paper roughening dynamics corresponds to the new class of anomalous kinetic roughening [J. J. Ramasco, J. M. López, and M. A. Rodríguez, Phys. Rev. Lett. 84, 2199 (2000)], characterized by the equal local and global roughness exponents zeta = alpha = 1 and the dynamic exponent z = 1.0+/-0.2, whereas the spectral roughness exponent alpha(s) > 1 is determined by the long-range correlations characterized by the fractal dimension of D crumpled sheet.
