AIM: The aim of this work was to evaluate the concordance between the low anterior resection syndrome (LARS) and preoperative LARS (POLARS) scores regarding the incidence of LARS in a Chilean population undergoing rectal surgery for cancer in a high-volume hospital. METHOD: The LARS score questionnaire, following telephone requests, was used to determine the presence and severity of LARS. The POLARS score was calculated based on variables described previously. Correlations and qualitative and quantitative concordance were evaluated using Spearman's correlation coefficient, the kappa coefficient and the Bland-Altman plot with Lin's concordance correlation coefficient. RESULTS: A total of 120 patients met the inclusion criteria: 37.5% underwent neoadjuvant radiotherapy, 61% underwent total mesorectal excision (TME) and 51.6% underwent ostomy. A total of 49% of patients did not present with LARS, whereas 28% had major LARS. The correlation between scales was poor, with a fair qualitative concordance to determine the presence/absence of LARS and a slight qualitative concordance to determine the degree of the intensity. The quantitative concordance was poor. CONCLUSION: In the Chilean population, concordance between the LARS and POLARS scores was qualitatively fair to determine the presence/absence of the disease and qualitatively slight to determine the degree of intensity. We do not suggest using the POLARS score in the perioperative period in the Chilean population deliberately, as the score may help to determine the presence/absence of LARS but cannot determine its degree of intensity. Additional evaluations are required to determine the factors contributing to the degree of agreement between the scales.

Although urinary crystals are habitual components, urolithiasis formation is always preceded by these concretions. We aimed to identify the change in the crystalline profile in sheep supplemented with ammonium chloride. Twenty-five male sheep aged three months, feedlot and randomly distributed into three groups were used: Control Group (CG) n = 5 did not receive Ammonium Chloride; G200 Group (n=10) (200mg/kg) of Ammonium Chloride for 56 consecutive days; G500 Group (n=10) (500mg/kg) of Ammonium Chloride for 56 consecutive days, administered daily orally. Sampling times and clinical evaluation were performed at seven days, with M0 (immediately before Ammonium Chloride), M1 (seven days after) until M8, totaling 70 days of feedlot. Urine samples were performed to identify the presence, type, and quantity of crystals. There was an increase in crystalluria in all groups in relation to time due to dietary influence, mainly in the CG, which presented more crystals of amorphous calcium phosphate and calcium oxalate. In addition, the G500 Group presented a higher presence of urate/uric acid crystals after urinary acidification, which are closely related to urinary pH.


Apesar de cristais urinários serem componentes habituais, a formação de urolitíase é sempre precedida dessas concreções. O presente estudo objetivou identificar a mudança do perfil cristalúrico em ovinos suplementados com cloreto de amônio. Foram utilizados 25 ovinos, machos, com idade de três meses, confinados e distribuídos aleatoriamente em três grupos: grupo controle (GC) (n=5) não recebeu cloreto de amônio; grupo G200 (n=10) (200mg/kg) recebeu cloreto de amônio por 56 dias consecutivos; grupo G500 (n=10) (500mg/kg) recebeu cloreto de amônio por 56 dias consecutivos, administrados diariamente por via oral. Os momentos (M) de colheita de amostras e avaliação clínica foram realizados com intervalo de sete dias, sendo M0 (imediatamente antes da administração do cloreto de amônio), M1 (sete dias após) até M8, totalizando 70 dias de confinamento. As amostras de urina foram analisadas para se identificar a presença, o tipo e a quantidade de cristais. Houve aumento da cristalúria em todos os grupos em relação ao tempo por influência dietética, principalmente no GC, que apresentou mais cristais de fosfato de cálcio amorfo e oxalato de cálcio. Além disso, o grupo G500 apresentou maior presença de cristais de urato/ácido úrico após acidificação urinária, estando esses intimamente relacionados ao pH urinário.

Introducción: La pandemia por COVID-19 obligó a los gobiernos a implementar medidas de restricción social para proteger la salud de la población, afectando la calidad de vida de las personas, especialmente en grupos vulnerables como los niños, niñas y adolescentes (NNA). El objetivo de esta revisión sistemática (RS) fue evaluar el efecto de las medidas de restricción sobre la actividad física (AF) y conducta sedentaria (CS) de los NNA. Adicionalmente, se exploraron posibles factores determinantes de estos cambios. Métodos: Se realizó una RS, utilizando tres bases de datos. Se incluyeron estudios observacionales en donde se hubiera analizado la AF y CS de los participantes, utilizando cualquier método de evaluación. Dos investigadores analizaron los estudios, extrajeron los datos y evaluaron la calidad metodológica de los artículos primarios. El metaanálisis se realizó utilizando el modelo de efectos aleatorios, considerando un valor p < 0,05 como estadísticamente significativo. Resultados: Se incluyeron 19 artículos, con una muestra total de 15.095 NNA. La mayoría de los estudios reveló una reducción de la AF y un incremento de la CS en los sujetos, durante los confinamientos por COVID-19. El metaanálisis mostró una caída en la AF total, la AF moderada a vigorosa y un incremento del tiempo de sedentarismo. Diversos factores biodemográficos, familiares y ambientales exacerbaron las variaciones en la AF y la CS de los NNA. Conclusión: Las medidas de restricción aplicadas durante pandemia por COVID-19 redujo la AF e incrementó la CS de los NNA. Factores biodemográficos, familiares y ambientales determinaron estas variaciones.


Introduction: The COVID-19 pandemic forced governments to implement social restriction measures to protect the health of the population, affecting the quality of life of people, especially in vulnerable groups, such as children and adolescents (CA). The objective of this systematic review (SR) was to evaluate the effect of restriction measures on physical activity (PA) and sedentary behavior (SB) of CA. Additionally, possible determining factors of these changes were explored. Methods: An SR was carried out, using three databases. Observational studies were included in which the PA and SB of the participants were analyzed, using any evaluation method. Two investigators analyzed the studies, extracted data, and assessed the methodological quality of the primary articles. The meta-analysis was performed using the random effects model, considering a value of p < 0.05 as statistically significant. Results: 19 articles were included, with a total sample of 15,095 subjects. Most studies revealed a reduction in PA and an increase in SB in subjects during COVID-19 lockdowns. The meta-analysis showed a drop in total PA, moderate to vigorous PA, and an increase in sedentary time. Various biodemographic, family and environmental factors exacerbated the variations in the PA and SB of the CA. Conclusion: The restriction measures applied during the COVID-19 pandemic reduced the PA and increased the SB of the CA. Biodemographic, family and environmental factors determined these variations.

Urolithiasis has a high incidence among confined sheep. It is multifactorial and may cause economic damage. Our aim was to determine the capacity of urinary acidification using ammonium chloride in sheep. Twenty-five 3-month-old male sheep were confined and randomly divided into three groups; the G200 and G500 groups received 200mg/kg/GW and 500mg/kg/GW of ammonium chloride daily for 56 consecutive days, respectively, whereas the CG group did not receive ammonium chloride. Sampling times and clinical evaluation were performed weekly, starting from the 14th day of confinement (M1 or immediately before administering ammonium chloride) until the 17th day (M9) of the feedlot. Hemogasometry, biochemical examination of serum urea and creatinine concentration and ultrasound evaluation of the urinary tract were performed. The urinalysis indicated a higher incidence of ammonium magnesium phosphate crystals at the beginning of the study, showing a migration to urate crystal formation, mainly in the G500 group because of urinary acidification. No hemogasometric, serum biochemistry, ruminal fluid, or ultrasonographic changes were observed. Urinary acidification was achieved and maintained after M7 during the administration of ammonium chloride in the G500 group, but not in the other study groups.(AU)


A urolitíase apresenta alta incidência em ovinos confinados, etiologia multifatorial, e pode causar prejuízo econômico. O objetivo do presente estudo foi determinar a capacidade da acidificação urinária mediante o uso de cloreto de amônio em ovinos. Foram utilizados 25 ovinos de três meses de idade, confinados e divididos aleatoriamente em três grupos: grupo CG (controle) não recebeu cloreto de amônio; grupo G200 (200mg/kg/PV) recebeu cloreto de amônio por 56 dias consecutivos; grupo G500 (500mg/kg/PV) recebeu cloreto de amônio por 56 dias consecutivos, administrados diariamente por via oral. Os momentos (M) de colheita de amostras e de avaliação clínica foram realizados com intervalo de sete dias, sendo M1 (imediatamente antes do cloreto de amônio), M2 (sete dias após) até M9, totalizando 70 dias de confinamento. Foram realizadas hemogasometria, concentração sérica de ureia e creatinina e avaliação ultrassonográfica do trato urinário. Na urinálise, houve uma maior incidência de cristais de fosfato amônio magnesiano no início do estudo, com migração para formação de cristais de urato, principalmente no G500, devido à acidificação urinária. Não houve alterações hemogasométricas, na bioquímica sérica, no líquido ruminal, ou alterações ultrassonográficas. A acidificação urinária foi obtida e mantida a partir do M7 durante a administração do cloreto de amônio no grupo G500, não ocorrendo nos outros grupos de estudo.(AU)

AIM: The purpose of this study was to review genitourinary (GU) and gastrointestinal (GI) toxicity associated with high-dose radiotherapy (RT) delivered with 3-dimensional conformal radiotherapy (3D-CRT) and intensity-modulated radiotherapy (IMRT) or volumetric arc therapy (VMAT) following radical prostatectomy (RP). BACKGROUND: RP is a therapeutic option for the management of prostate cancer (PrCa). When assessing postoperative RT techniques for PrCa, the published literature focuses on patients treated with 2-dimensional conventional methods without reflecting the implementation of 3D-CRT, IMRT, or VMAT. MATERIALS AND METHODS: A total of 83 patients were included in this analysis; 30 patients received 3D-CRT, and 53 patients received IMRT/VMAT. Acute and late symptoms of the GU and lower GI tract were retrospectively graded according to the Radiation Therapy Oncology Group and the European Organization for Research and Treatment of Cancer radiation toxicity grading systems. The relapse failure-free rate and overall survival were also evaluated. RESULTS: The rate of acute GU toxicity was 9.4% vs. 13.3% for the IMRT/VMAT and 3D-CRT groups (p = 0.583). The 5-year actuarial rates of late GI toxicity for IMRT/VMAT and 3D-CRT treatments were 1.9% and 6.7%, respectively. The rate of late GU toxicity for the IMRT/VMAT and 3D-CRT treatment groups was 7.5% and 16.6%, respectively (p = 0.199). We found no association between acute or late toxicity and the RT technique in univariate and multivariate analyses. CONCLUSION: Postprostatectomy IMRT/VMAT and 3D-CRT achieved similar morbidity and cancer control outcomes. The clinical benefit of highly conformal techniques in this setting is unclear although formal analysis is needed.

Hemibrycon iqueima sp. nov., is described from small streams in the Magdalena drainage at the foothills of the western slope of the Eastern Cordillera of the Colombian Andes, Suarez municipality, Tolima Department, Colombia. The new species is distinguished from its congeners in the Magdalena-Cauca River basin by a combination of characters related to snout-anal-fin origin length, head length, dorsal-pectoral fin distance, dorsal-fin-hypural distance, postorbital distance, orbital diameter, snout length, number of total vertebrae, pre-dorsal scales, scale rows between anal-fin origin and lateral line, number of branched rays of the anal fin, maxillary teeth number and number and arrangement of hooks on the branched rays of the pectoral and dorsal fins. In addition, the validity of this species is supported by previous molecular analyses that included specimens of the new species that had been erroneously identified. Phylogenetic relationships between the new species and congeners from Pacific coast basins are discussed.

PURPOSE: Breast cancer is the most common neoplasm in women and has the highest associated mortality rate. Rapid detection programmes can provide early diagnosis and increase the chances of survival. There are no specific tumor biomarkers for the early phase of the disease. The primary aim of this study was to search a blood biomarker with levels that exceeded the normal range established in the general population that could be used to screen breast cancer. METHODS/PATIENTS: Case-control study. Conventional as well as research (NGAL, EGFR and 8-OHdG) tumor biomarkers were analyzed. RESULTS: A total of 126 women were enrolled (cases: 63 patients with local breast cancer; Controls: 63 healthy women). Significant differences were found in patients with higher levels of the conventional markers, Ca15.3, CEA, Cyfra 21.1 and NSE. However, when commercial cut-off values were used, only Ca 15.13 was significant. In the group of research biomarkers, significantly higher levels of EGFR were found in the control group, and of 8-OHdG in the case group. Using logistic regression analysis and a ROC curve, an equation composed of five markers, Ca 15.3, NSE, NGAL, EGFR and 8-OHdG, which yielded a correct diagnostic probability of breast cancer of 91.8% was obtained. CONCLUSIONS: 8-OHdG has been identified as a new potential marker for screening early stage breast cancer. In addition, a model that combines five blood markers that can be used as a diagnostic test in certain groups of patients has been developed. New studies with a larger sample size are needed to verify the results obtained.

Introduction: Surgical treatment of liver metastases is broadly accepted for colorectal and neuroendocrine tumors. However, the approach to liver metastases of other origins is not well established. The objective is the evaluation of factors that might be associated with better or worse perioperative outcomes (morbidity and mortality) and survival of patients with non-colorectal, non-neuroendocrine liver metastases (NCNNLM) treated surgically in a second level hospital. Material and Methods: A prospective study of patients with NCNNLM treated surgically or with radiofrequency was completed in San Cecilio University Hospital of Granada, between March 2003 and June 2013. The statistical analysis was made with IBM-SPSS version 21. Statistical significance was set at p ≤ 0.05. Survival data were analyzed by Kaplan-Meier's method. Results: Twenty-two patients with 26 surgeries were analyzed, 19 liver resections, 6 radiofrequency treatments and 1 combined case with surgery and radiofrequency. Operatory mortality was 0 percent and overage mortality was 23.1 percent. Univariate analysis only showed statistical significance in two of the factors, presence of positive lymphatic nodes during the surgery of the primary tumor and presence of extra-hepatic disease. Accumulated survival was 81 percent, 66 percent and 52 percent at 1, 2 and 3 years respectively. Conclusions: Despite the fact of treating together in the same group several different tumors, the surgical treatment of NCNNLM (with or without radiofrequency) has an acceptable survival rate. Thus, they can be treated with safety when we use a multi-disciplinary approach to these patients.


Introducción: Aunque el tratamiento quirúrgico de las metástasis hepáticas está bien establecido en el cáncer colorrectal y los tumores neuroendocrinos. el enfoque de las metástasis hepáticas de otras localizaciones no está bien definido. El objetivo es valorar aquellos factores que pudieran asociarse a unos mejores o peores resultados perioperatorios (morbimortalidad) y a la supervivencia de los pacientes sometidos a tratamiento quirúrgico de las metástasis hepáticas de tumores no colorrectales ni neuroendocrinos(MHNCNN) en un hospital de segundo nivel. Material y Métodos: Se ha realizado un estudio prospectivo de pacientes con MHNCNN tratadas mediante cirugía y/o radiofrecuencia en el Hospital Universitario San Cecilio de Granada, entre marzo de 2003 y junio de 2013. Para el análisis estadístico se utilizó el programa IBM-SPSS versión 21, considerándose significativo un valor de p ≤ 0,05. El estudio de supervivencia se realizó mediante análisis de Kaplan-Meier. Resultados: Se analizan 22 pacientes, con 26 intervenciones: 19 resecciones quirúrgicas, 6 radiofrecuencias y 1 caso combinando resección y radiofrecuencia. La mortalidad operatoria es de 0 por ciento y la morbilidad del 23 por ciento. El análisis univariante sólo muestra significación estadística en dos factores, presencia de ganglios positivos en la intervención del tumor primario y la existencia de enfermedad extrahepática. La supervivencia acumulada es del 81 por ciento, 66 por ciento y 52 por ciento al año, dos años y tres años. Conclusiones: Aunque con las limitaciones que presenta el agrupar un conjunto heterogéneo de tumores. el tratamiento quirúrgico de las MHNCNN, asociado o no a radiofrecuencia, presenta una tasa de supervivencia aceptable, pudiendo ser estas tratadas con seguridad, siendo fundamental un abordaje multidisciplinario de los pacientes.

Background: Chylous ascites is defined as the presence of thoracic or intestinal lymph in the abdominal cavity. Its association with acute pancreatitis is uncommon. Aim: To report three cases of chilous ascites related with acute pancreatitis. Material and Methods: Review of medical records of three patients with chylous ascites associated with acute pancreatitis. Results: We report three patients with chylous ascites out of 2,188 admissions for acute pancreatitis (0.13 percent). A 39 years old male with a pancreatic pseudocyst. During surgery, chylous ascites was found. He was successfully treated with octreotide and spironolactone. A 71 years male operated for an acute cholecystitis and a perivesicular abscess. Three days after surgery, a chylous fluid appears in the abdominal drainage, which disappears spontaneously. A 73 years old female operated for an acute pancreatitis. During surgery a chylous ascites is found. The patient died four days after surgery. Conclusions: Chylous ascites associated with pancreatitis is uncommon and octreotide may have a therapeutic role for it.


Introducción: La ascitis quilosa (AQ) se define como la presencia de linfa de origen torácico o intestinal en la cavidad abdominal. Asociada a Pancreatitis Aguda (PA), sólo se han documentado 12 casos hasta 2013. El objetivo de este trabajo es presentar tres nuevos casos y realizar una revisión de la presentación, clínica, diagnóstico y tratamiento de esta entidad. Material y Métodos: Se recogen los datos clínicos y evolutivos de tres pacientes con AQ secundaria a pancreatitis aguda (AQPA); esta se ha definido como la presencia de linfa en cavidad abdominal en un paciente con episodio de pancreatitis aguda reciente y sin antecedente traumático, infeccioso, neoplásico o quirúrgico que pudiera ser el responsable de la misma. Se han recogido igualmente todos los casos documentados hasta 2013. Resultados: En los últimos 10 años han sido ingresados 2.188 pacientes por PA entre los cuales, se han diagnosticado 3 casos de AQPA (0,13 por ciento), dos hombres y una mujer, con PA de origen biliar. El diagnóstico ha sido "de visu" intra o postoperatorio. El débito inicial fue de 3.000, 300 y 1.500 cc. El tratamiento ha contado con octeótrido de entrada en todos los casos. En la revisión bibliográfica la AQPA ha sido diagnosticada también "de visu" y los pacientes se han resuelto con tratamiento conservador, tardando una media de 27,7 días si se utiliza octeótrido frente a 46,5 días si no se le ha prescrito. Conclusiones: La AQ asociada a PA sigue siendo infrecuente, su diagnóstico es "de visu" y se resuelve con tratamiento conservador, siendo el octeótrido un fármaco de primera línea.

BACKGROUND AND PURPOSE: The study aims to compare lipid profiles among ischemic stroke patients in a predominantly Caribbean-Hispanic population in Miami and a Mestizo Hispanic population in Mexico City. METHODS: We analyzed ischemic stroke Hispanic patients with complete baseline fasting lipid profile enrolled contemporaneously in the prospective registries of two tertiary care teaching hospitals in Mexico City and Miami. Demographic characteristics, risk factors, medications, ischemic stroke subtype, and first fasting lipid profile were compared. Vascular risk factor definitions were standardized. Multiple linear regression analysis was performed to compare lipid fractions. RESULTS: A total of 324 patients from Mexico and 236 from Miami were analyzed. Mexicans were significantly younger (58 · 1 vs. 67 · 4 years), had a lower frequency of hypertension (53 · 4% vs. 79 · 7%), and lower body mass index (27 vs. 28 · 5). There was a trend toward greater prevalence of diabetes in Mexicans (31 · 5 vs. 24 · 6%, P = 0 · 07). Statin use at the time of ischemic stroke was more common in Miami Hispanics (18 · 6 vs. 9 · 4%). Mexicans had lower total cholesterol levels (169 · 9 ± 46 · 1 vs. 179 · 9 ± 48 · 4 mg/dl), lower low-density lipoprotein (92 · 3 ± 37 · 1 vs. 108 · 2 ± 40 · 8 mg/dl), and higher triglyceride levels (166 · 9 ± 123 · 9 vs. 149 · 2 ± 115 · 2 mg/dl). These differences remained significant after adjusting for age, gender, hypertension, diabetes, body mass index, smoking, ischemic stroke subtype, and statin use. CONCLUSION: We found significant differences in lipid fractions in Hispanic ischemic stroke patients, with lower total cholesterol and low-density lipoprotein, and higher triglyceride levels in Mexicans. These findings highlight the heterogeneity of dyslipidemia among the Hispanic race-ethnic group and may lead to different secondary prevention strategies.

Objective. Assessment the total concentration of mercury in the liver and feathers of Pelecanus occidentalis of the Cispata bay, Colombia. Materials and methods. Mercury concentrations in liver and feather of Pelecanus occidentalis residents in the Cispata bay - Colombia were evaluated by digestion with an acidic mixture of H2SO4-HNO3 and KMnO4 to eliminate organic matter. The concentration of mercury was determined by the Atomic Absorption - Cold Vapor method (CVAAS). Results. Total mercury levels found in this study were higher in feathers (0.31-9.17 mgHg/kg) than in the liver (0.63-6.29 mgHg/kg), being higher than those reported in other seabirds studies. Conclusions. The high levels of total mercury in feathers and liver can be explained by the feeding habits of the organisms under study, showing the utility of feathers as a potential non-invasive tool for the monitoring of the ecosystem and thereby preventing the sacrifice of specimens.


Objetivo. Evaluar las concentraciones de mercurio total en hígado y plumas de Pelecanus occidentalis de la Bahía de Cispatá, Colombia. Materiales y métodos. Se evaluó la concentración de mercurio en hígado y plumas de Pelecanus occidentalis pertenecientes a la bahía de Cispatá - Colombia, por digestión con una mezcla ácida de H2SO4-HNO3 y KMnO4 para eliminar la materia orgánica. La concentración de mercurio fue determinada por el método de Absorción Atómica - Vapor Frio (CVAAS). Resultados. Los valores de mercurio total fueron más altos en plumas (0.31-9.17 mgHg/kg) que en hígado (0.63-6.29 mgHg/kg), siendo mayores a los reportados en otros estudios de aves marinas. Conclusiones. Las altas concentraciones de mercurio total en plumas e hígado pueden ser atribuidas a la dieta de los organismos estudiados; demostrando la utilidad de las plumas como herramienta potencial no invasiva de monitoreo del ecosistema y evitando con ello el sacrificio de los individuos.

A fitocosmética é um setor em nítido crescimento por se ter verificado reais vantagens na incorporação de produtos vegetais em relação a certos produtos sintéticos em produtos cosméticos. O proposto trabalho foi desenvolvido enfocando-se o uso de xampu com a utilização de microesferas de biopolímero e extrato hidroalcoólico de Capsicum frutescens L., mais popularmente conhecida como pimenta-malagueta, para o tratamento de queda capilar simples (alopecia androgênica). As análises físico-químicas envolveram determinação de cinzas totais, pH, identificação de capsaicina e densidade do xampu, entre outros. Considerando-se os diversos aspectos exigidos para a garantia da qualidade do material botânico, que englobam não somente os aspectos físico-químicos, mas também o microbiológico, realizou-se, tanto no extrato quanto no produto final, a contagem total de micro-organismos, pesquisa de Salmonella sp e Escherichia coli, Staphylococcus aureus e Pseudomonas aeruginosa. Foram realizadas análises sensoriais em voluntários, visando avaliar a qualidade sensorial do produto. Os voluntários avaliaram a amostra segundo questionário aplicado. Para as propriedades avaliadas, as microesferas esfoliantes incorporadas ao xampu mostraram-se ser bem aceitas pelos voluntários, principalmente devido à sensação transmitida pela esfoliação, fazendo com que o extrato hidroalcoólico promova uma maior oxigenação local.


Phytocosmetics is a sector of the market in rapid growth, since real advantages have been demonstrated in the incorporation of plant products in cosmetics, in place of certain synthetic products. The focus of this study was on the use of shampoo containing microspheres of biopolymer and a hydroalcoholic extract of the fruit of Capsicum frutescens L., more commonly known as chili pepper, for the treatment of hair loss (androgenic alopecia). The physical and chemical analysis included the determination of total ash, pH and density of the shampoo and the identification of capsaicin. Considering the various requirements for quality assurance of the plant material, encompassing not only physicochemical but also microbiological criteria, both the extract and the final product were subjected to a total count of microorganisms and tests for Salmonella sp, Escherichia coli, Staphylococcus aureus and Pseudomonas aeruginosa. The sensory quality of the product was assessed by a panel of volunteers, who rated the sample of shampoo in response to a questionnaire. For the properties assessed, the exfoliating microspheres incorporated into the shampoo proved to be well tolerated by the volunteers, mainly on account of the feeling conveyed by exfoliation, causing the hydroalcoholic extract to promote greater local oxygenation.

BACKGROUND: Metabolic syndrome (MetS) may represent risk factor for long-term renal function of kidneys from living donors. The aim of this study was to evaluate the impact of MetS on renal function in donors. METHODS: Data regarding the presence or absence of MetS and renal function, as assessed by estimated glomerular filtration rate (eGFR) were obtained from 140 kidney donors before nephrectomy (BN) and at follow-up (AF). Donors were divided into those with (group 1; n =28) versus without MetS (group 2; n = 112). RESULTS: Comparing the groups, we observed a significantly greater reduction in eGFR among the group with MetS BN versus AF 27.5% (19.3-33.0) versus 21.4% (9.6-34.1 P = .02) respectively using a Cox regression model, including age, gender, serum uric acid, body mass index (BMI), and basal eGFR, MetS BN (hazard ratio = 2.2; 95% confidence interval [CI], 1.21-4.01; p = .01) was an independent factor associated with a greater risk of a-eGFR <70 mL/min/1.73 m(2) at follow-up (P < .001). Additionally, age (hazard ratio = 1.03%; 95% CI, 1.01-1.06; P < .001), and female gender (hazard ratio = 1.86; 95% CI, 1.03-3.36; P = .03) were associated with a greater decrease in eGFR. Individuals with MetS BN showed a GFR <70 mL/min/1.73 m(2) at significantly shorter follow-up time (5.6 ± 0.8 years) versus persons without MetS (12.8 ± 1.0 years; P = .001) CONCLUSION: Kidney donors with MetS BN experiment a significantly greater decrease in eGFR at follow-up.

RÉSUMÉ

Collecting duct carcinoma (CDC) of Bellini is a rare variant of renal cell carcinoma. It tends to be more aggressive when locally advanced or when having distant spread on diagnosis. The only favorable prognosis factor is low-stage, low-grade, with disease-free survival reported up to 5 years. We reviewed our renal cell carcinoma database and found 2 cases of CDC, namely 1 young female with locally advanced disease with supraclavicular metastasis and a dismissal prognosis, and 1 male with localized disease with 10-year disease-free survival.

OBJECTIVE: To evaluate the effect of carbohydrate supplementation upon some aspects of the immune function in athletes during intense indoor cycling. METHODS: Twelve male athletes cycled for 20 min at a velocity corresponding to 90% of that obtained at the anaerobic threshold and rested for 20 min. This protocol was repeated six times. The athletes received, during the trial, water ad libitum, or a solution of carbohydrate (95% glucose polymers and 5% fructose) at 10% (w/v), 1 g kg h every 20 min, starting at the 10th minute of the first exercise period, plus extra water ad libitum. RESULTS: Exercise induced a reduction in peripheral blood mononuclear cell proliferation (37%) as well as in the production of cytokines by cultured cells (interleukin-1 (IL-1), interleukin-2 (IL-2), tumor necrosis factor-alpha (TNF-alpha) and interferon-gamma (IFN-gamma), by 37%, 35%, 26% and 16%, respectively). All of these changes were prevented by the ingestion of a carbohydrate drink by the athletes, except that in IFN-gamma production, which was equally decreased (17%) after the second trial. The concentration of plasma glutamine, an important fuel for immune cells, was decreased in the placebo group but maintained in the group that received carbohydrate. CONCLUSION: Carbohydrate supplementation affects positively the immune response of cyclists by avoiding or minimizing changes in plasma glutamine concentration.

The phlebotomine sandfly fauna of traditional (shaded) and intensified (unshaded) coffee plantations in Colombia was sampled by a variety of methods and the species composition and density under the two systems compared. Twenty species of Lutzomyia sandflies (Diptera: Psychodidae: Phlebotominae) were collected, of which eight were found only in the 'Coffee Axis' ('Eje Cafetero') of the departments of Caldas, Risaralda and Quindio, six were exclusive to the department of Norte de Santander and six occurred in both regions. Four species were collected only in traditional plantations and two exclusively in intensified ones. At least 13 species occurred in both plantation types. Fifteen species are opportunistic man-biters and eight are suspected vectors of leishmaniasis caused by Le. braziliensis, Le. panamensis or Le. mexicana. Seven species were collected inside houses and may be involved in intradomiciliary transmission of Leishmania. The dominant species in Norte de Santander was Lu. spinicrassa, which made up 93.8% of all the sandflies collected in this department. This species was absent from the Eje Cafetero and a number of others among the 15 recorded there might be responsible for Leishmania transmission in this region, including Lu. trapidoi, Lu. yuilli, Lu. gomezi, L. hartmanni and Lu. ovallesi. Sandfly population densities were significantly higher in traditional plantations than in intensified ones. Residents of traditional plantations were able to describe sandflies in significantly more detail than those of intensified plantations, based on seven basic characteristics related to the appearance and biting behaviour of the insects.
