ABSTRACT The objective of this report is to describe a case of meibomian gland dysfunction associated with keratoconus and to examine the importance of treatment for evaporative dry eye in cases of corneal ectasia. A 45-year-old man diagnosed as having keratoconus complained of burning, tearing, itching, and red eye. He had a history of penetrating corneal transplantation and wearing rigid contact lenses. The meibography revealed a severe meibomian gland dropout and normal tear meniscus height in both eyes. Objective propaedeutic tests are important tools for dry eye diagnosis and proper evaluation of ocular surface and tear film. In older patients, the classic signs of atopic conjunctivitis are not always present, and the causes of chronic rubbing must be further investigated. Treatment of underlying chronic inflammation such as dry eye, meibomian gland dysfunction, and blepharitis might be important to prevent keratoconus progression and guarantee symptom relief.


RESUMO O objetivo é relatar um caso de associação entre disfunção de glândulas de meibomius (DGM) e ceratocone, assim como ressaltar a importância do tratamento do olho seco evaporativo em casos de ectasia corneana. Paciente do sexo masculino de 45 anos com ceratocone e queixas de ardência, lacrimejamento, prurido e olho vermelho. O mesmo tem histórico de transplante de córnea penetrante e uso de lentes de contato rígidas. A meibografia revelou severa perda de glândulas de meibomius e menisco lacrimal normal em ambos os olhos. Testes propedêuticos objetivos são importantes ferramentas para diagnóstico de olho seco e apropriada avaliação da superfície ocular e filme lacrimal. Em pacientes mais velhos, os sinais clássicos de atopia não estão sempre presentes e investigações adicionais das causas de prurido crônico são necessárias. O tratamento da inflamação crônica subjacente como olho seco, disfunção de glândulas de meibomius e blefarite podem ser importantes para prevenir a progressão do ceratocone e garantir alívio dos sintomas.

A trombose venosa profunda (TVP) e o tromboembolismo pulmonar (TEP) são as principais manifestações clínicas do tromboembolismo venoso (TEV). O TEV é uma complicação comum e potencialmente grave das cirurgias ortopédicas, uma vez que pode levar à limitação funcional e morte pós-operatória. O uso da tromboprofilaxia reduz acentuadamente a incidência de TEV relacionado às intervenções ortopédicas, e seu uso é recomendado por diversas diretrizes. Entretanto, a indicação e o tipo de tromboprofilaxia para algumas intervenções ortopédicas, assim como o uso de ferramentas de predição de risco, ainda não estão bem estabelecidos e constituem pontos de debate. Os objetivos deste trabalho foram identificar os fatores de risco para TEV e suas interações nas cirurgias ortopédicas e avaliar a aplicabilidade do escore de risco de Caprini na predição de TEV em pacientes ortopédicos. Além disso, devido à escassez de conhecimento e recomendações sobre tromboprofilaxia nas intervenções ortopédicas de tornozelo e pé, realizaram-se uma revisão narrativa e uma enquete internacional sobre o assunto. Para analisar a interação de fatores de risco de TEV e a aplicabilidade preditiva do escore de Caprini nas cirurgias ortopédicas, foram utilizados os dados do Multiple Environmental and Genetic Assessment of risk factors for venous thrombosis (MEGA), um estudo caso-controle holandês de base populacional cujo desfecho de interesse é o TEV. Neste, 263 indivíduos que tiveram TEV e 94 controles foram submetidos à cirurgia ortopédica. O risco de TEV foi maior nos primeiros 30 dias após a cirurgia [odds ratio ajustada (ORadj) 17,5; intervalo de confiança (IC) 95%, 9,2-33,4] e manteve-se aumentado até 90 dias (ORadj 11,5; IC 95%, 7,3-17,7). Houve interação entre cirurgia ortopédica e mutação fator V Leiden (ORadj 17,5; IC 95%, 4,1-73,6), tipo de sangue não-O (ORadj 11,2; 95% IC, 3,4-34,0) e níveis séricos de fator VIII maiores que 150 mg/dl (ORadj 18,6; IC, 7,4-46,9). Quanto ao escore de Caprini, foram avaliados 357 indivíduos com TEV submetidos à cirurgia ortopédica. Um total de 20,9% dos casos e 41,5% dos controles foram classificados no grupo de menor risco (Caprini < 5 pontos). Pacientes com um escore de Caprini maior que 11 pontos tiveram um risco de TEV seis vezes maior (OR 6,3; IC 95%, 1,7-22,9), e pacientes com um escore de 9 a 10 tiveram um risco três vezes maior (OR 3,5; IC 95% 1,2-10,3). A área sob a curva (AUC) do escore de Caprini foi de 0,64, evidenciando uma discriminação de predição de TEV moderada. Na enquete internacional sobre tromboprofilaxia de pé e tornozelo, 693 cirurgiões ortopédicos de pé e tornozelo de 49 países participaram. Aproximadamente 50% informaram prescrever tromboprofilaxia durante a imobilização do paciente. Quando utilizada, as escolhas preferidas em ordem decrescente foram ácido acetilsalicílico (AAS), heparina de baixo peso molecular (HBPM) e anticoagulantes orais diretos. AAS e HBPM foram predominantemente prescritos na América do Norte e Europa, respectivamente. TVP prévia, imobilidade, obesidade e trombofilia hereditária foram considerados os principais fatores de risco indicativos do uso de tromboprofilaxia. Concluiu-se que: houve risco aumentado de TEV e interação entre os fatores de risco fator V Leiden, níveis elevados de fator VIII e grupo sanguíneo não- O com cirurgia ortopédica; o escore de Caprini é uma ferramenta que pode auxiliar os cirurgiões ortopédicos a classificarem o risco de TEV no pós-operatório, embora seu desempenho preditivo discriminativo tenha sido moderado; apesar de a maioria dos cirurgiões ortopédicos de tornozelo e pé considerarem que a tromboprofilaxia está indicada para cirurgias de tornozelo e pé (principalmente na presença de fatores de risco como TVP prévia, imobilidade, obesidade e trombofilia hereditária), a prescrição, o tipo e a duração apresentam grande discrepância intercontinental. Assim, este trabalho contribui para a identificação de pacientes sob alto risco de TEV, ponto estratégico para o uso individualizado e seguro da tromboprofilaxia como ferramenta para redução do risco de TEV relacionado às cirurgias ortopédicas.


Deep vein thrombosis (DVT) and pulmonary embolism are the main clinical manifestations of venous thromboembolism (VTE). VTE is a common and potentially serious complication of orthopedic surgeries, as it can lead to functional limitation and postoperative death. The use of thromboprophylaxis markedly reduces the incidence of VTE related to orthopedic interventions, and its use is recommended by several guidelines. However, the indication and type of thromboprophylaxis for some orthopedic interventions, as well as the use of risk prediction tools are still not well established and is debatable. The objectives of this study were to identify risk factors for VTE and their interactions in orthopedic surgeries and to evaluate the applicability of the Caprini risk score in predicting VTE in orthopedic patients. In addition, due to the lack of knowledge and recommendations on thromboprophylaxis in foot and ankle surgeries, a narrative review and an international survey on the subject were carried out. Data from the MEGA Study (Multiple Environmental and Genetic Assessment of risk factors for venous thrombosis) which is a Dutch population-based case-control study whose outcome of interest is VTE were used. A total of 263 subjects who had VTE and 94 controls underwent orthopedic surgery. The risk of VTE was the highest in the first 30 days after surgery (adjusted odds ratio [ORadj] 17.5; 95% CI [confidence interval], 9.2-33.4) and remained increased up to 90 days (ORadj, 11.5; 95% CI, 7.3- 17.7). There was interaction between orthopedic surgery and factor V Leiden mutation (ORadj 17.5, 95%CI 4.1-73.6), non-O blood type (ORadj 11.2; 95%CI 3.4-34.0) and factor VIII plasma levels greater than 150 mg/dl (ORadj 18.6; CI 7.4-46.9). To assess the applicability of the Caprini Score, 357 individuals with VTE undergoing orthopedic surgery in the MEGA were evaluated. A total of 20.9% of cases and 41.5% of controls were classified in the lowest risk group (Caprini <5 points). Patients with a Caprini score greater than 11 points had a six-fold increased risk of VTE (OR 6.3, 95% CI 1.7- 22.9) and patients with a score of 9-10 had about three-fold increased risk (OR 3.5, 95% CI 1.2-10.3). The area under curve for the Caprini score was 0.64, evidencing a moderate VTE prediction discrimination. In the international survey on foot and ankle thromboprophylaxis, 693 orthopedic foot and ankle surgeons from 49 countries participated. Approximately 50% reported prescribing thromboprophylaxis during patient immobilization that, when used, the preferred choice, in descending order, was acetylsalicylic acid (ASA), low molecular weight heparin (LMWH) and direct oral anticoagulants. ASA and LMWH were predominantly prescribed in North America and Europe, respectively. Previous DVT, immobility, obesity and hereditary thrombophilia were considered the main risk factors indicative of the use of thromboprophylaxis. It is concluded that there was increased risk of VTE and interaction between factor V Leiden, high plasma levels of factor VIII or non-O blood group and orthopedic surgery; that the Caprini score is a tool that can help orthopedic surgeons to classify the risk of postoperative VTE, although its discriminative predictive performance was moderate and that although most ankle and foot orthopedic surgeons consider that thromboprophylaxis is indicated for ankle and foot surgeries (especially in the presence of risk factors such as previous DVT, immobility, obesity and hereditary thrombophilia), the prescription, type and duration present a large intercontinental discrepancy. Thus, this work contributed to the identification of patients at high risk of VTE, a strategic point for the individualized and safe use of thromboprophylaxis as a tool to reduce the risk of VTE related to orthopedic surgeries.

ABSTRACT Purpose: The purpose of this study is to translate and validate a Portuguese version of the Keratoconus Outcomes Research Questionnaire. The Keratoconus Outcomes Research Questionnaire is a psychometrically robust and valid instrument used to assess the impact of keratoconus on activity limitations and symptoms. Methods: We performed a translation, cross-cultural adaptation, and validation of the Portuguese version of the Keratoconus Outcomes Research Questionnaire. The initial translation of the English version to the Portuguese language was performed by two independent native speaker translators, followed by an interdisciplinary panel evaluation of the translated version. The Portuguese version was then back-translated into English by two independent native speakers, followed by evaluation and comparison with the original English version by the same interdisciplinary panel. For subsequent validation, the translated questionnaire was administered at two different times to a population of 30 subjects, and the results were compared in a concordance analysis. Results: The translation into Portuguese and back-translation were determined to be correct. Thirty participants were enrolled in the study (mean age, 29.23 ± 7.56 years). Nine questions (31%) had almost perfect agreement (questions 3, 4, 5, 8, 18, 22, 27, 28, and 29), 15 questions (51.7%) had substantial agreement (questions 1, 2, 6, 7, 9, 12, 14, 15, 16, 17, 20, 21, 23, 25, and 26), 4 questions (13.8%) had moderate agreement (questions 10, 11, 19, and 24) and 1 question (3.5%) had reasonable agreement (question 13). High-correlation coefficients were obtained when comparing results of the initial application and second application of this questionnaire to a sample of 30 individuals, which indicated excellent concordance with regard to results, repeatability, and reliability. Conclusions: This translated and validated questionnaire can be applied to a larger population with the intent to assess quality of life in keratoconus patients in the overall Brazilian population as well as in distinct regions of the country.


RESUMO Objetivo: Desenvolver a versão em Português do Keratoconus Outcomes Research Questionnaire (KORQ). O Keratoconus Outcomes Research Questionnaire é um instrumento psicometricamente válido e robusto para avaliação do impacto do ceratocone na limitação de atividades e sintomas. Métodos: Foi realizado no estudo a tradução, adaptação transcultural e validação em Português do Keratoconus Outcomes Research Questionnaire. A tradução inicial da versão em inglês para o idioma português foi realizada por dois tradutores de língua nativa inglesa independentes, seguida de uma avaliação interdisciplinar da versão traduzida. Após isso, a versão em Português foi traduzida novamente para o inglês por dois tradutores nativos de língua portuguesa independentes, seguida de avaliação e comparação com a versão original em inglês pelo mesmo painel interdisciplinar. Para a subsequente validação, o questionário traduzido foi aplicado em dois tempos diferentes em uma população de 30 indivíduos, e os resultados foram comparados em uma análise de concordância. Resultados: O processo de tradução para a língua portuguesa e tradução reversa do questionario Keratoconus Outcomes Research Questionnaire foi conduzido de maneira satisfatória. Trinta participantes foram incluídos no estudo (média idade, 29.23 ± 7.56 anos). Nove questões (31%) com concordância quase perfeita (questões 3, 4, 5, 8, 18, 22, 27, 28 e 29), cinco questões (51.7%) com concordância substancial (questões 1, 2, 6, 7, 9, 12, 14, 15, 16, 17, 20, 21, 23, 25 e 26), quatro questões (13.8%) com concordância moderada (questões 10, 11, 19 e 24) e uma questão (3.5%) com concordância razoável (questão 13). Os altos coeficientes de correlação obtidos ao comparar os resultados da aplicação inicial com a re-aplicação do questionário em uma amostra de 30 indivíduos indicam a excelente concordância em relação aos resultados, repetibilidade e confiabilidade. Conclusão: Esse questionário traduzido e validado pode ser aplicado em populações maiores com o objetivo de avaliar a qualidade de vida em pacientes com ceratocone na população brasileira em geral, assim como em regiões distintas do país.

To assess the quality of life of keratoconus patients using the Keratoconus Outcomes Research Questionnaire (KORQ), translated and validated in Portuguese language. The KORQ is the only validated keratoconus specific questionnaire and has a high rating for its psychometric properties. This cross-sectional study enrolled 100 keratoconus patients from a tertiary referral eye hospital between April 2018 and June 2019. Associations between age, sex, allergic conjunctivitis, keratoconus stage, best-corrected visual acuity (BCVA), maximum simulated keratometry (Kmax), steep keratometry (K2), pachymetry, treatments performed, hydrops, and KORQ scores were evaluated using univariate (Wilcoxon test and the Kruskal Wallis test) and multivariate linear regression with stepwise backward modeling. Lower KORQ scores are associated with better quality of life, whereas, higher scores are associated with greater impairment of functional activities and symptoms. Among the 100 patients, mild, moderate, and severe keratoconus were observed in 15%, 46% and 39% of participants, respectively. Univariate analysis showed lower function scores values, with male sex (p < 0.05) and both functional and symptom scores were significantly associated with BCVA < 0.3 (LogMAR) (p < 0.05). Multivariate analysis indicated significantly lower functional scores in individuals with BCVA < 0.3 (LogMAR) (p < 0.001) and those with a history of crosslinking treatment (p = 0.022), while symptom scores were only significantly associated with only BCVA < 0.3 (LogMAR) (p < 0.001). In patients with keratoconus, BCVA in the better eye and history of crosslinkig were factors associated with better quality of life scores using the KORQ.

This study investigated the systemic inflammatory response and mechanism of pulmonary lesions induced by Crotalus durissus cascavella venom in murine in the state of Bahia. In order to investigate T helper Th1, Th2 and Th17 lymphocyte profiles, we measured interleukin (IL) -2, IL-4, IL-6, IL-10, IL-17, tumor necrosis factor (TNF) and interferon gamma (IFN-γ) levels in the peritoneal fluid and macerated lungs of mice and histopathological alterations at the specific time windows of 1h, 3h, 6h, 12h, 24h and 48h after inoculation with Crotalus durissus cascavella venom. The data demonstrated an increase of acute-phase cytokines (IL-6 and TNF) in the first hours after inoculation, with a subsequent increase in IL-10 and IL-4, suggesting immune response modulation for the Th2 profile. The histopathological analysis showed significant morphological alterations, compatible with acute pulmonary lesions, with polymorphonuclear leukocyte (PMN) infiltration, intra-alveolar edema, congestion, hemorrhage and atelectasis. These findings advance our understanding of the dynamics of envenomation and contribute to improve clinical management and antiophidic therapy for individuals exposed to venom.

PURPOSE: The goal of this study was to compare the effect of early corneal collagen cross-linking (CXL) intervention (before 17 years of age) with that of late intervention (after 17 years of age) on the characteristics and progression of keratoconus. PATIENTS AND METHODS: One hundred five eyes of 94 patients with keratoconus undergoing treatment with CXL were included. The patients were divided into 2 groups by age: group 1 (mean age of 13.8 yrs; range 10-16) and group 2 (mean age of 21.5 yrs; range 17-36). Eyes were evaluated regarding best-corrected visual acuity (BCVA), refractive error, corneal endothelial cell density, and central corneal thickness, as well as using slit-lamp biomicroscopy, Goldmann tonometry, and the keratometry (Kmax, Ksteep, and Kflat parameters) test before CXL and at 1, 3, 6, and 12 months thereafter. RESULTS: The mean (SD) BCVA of group 1 was 0.45 (±0.25) before CXL and 0.56 (±0.29) 1 year after CXL (P = 0.030); mean (SD) Kmax, Ksteep, and Kflat were 58.47 (±7.2), 52.93 (±5.4), 47.22 (±4.2) before CXL respectively, and 58.21 (±7.7), 52.25 (±5.5), and 46.56 (±4.6) 1 year after CXL, respectively (P = 0.897, 0.481, and 0.491). The mean (SD) BCVA of group 2 was 0.50 (±0.30) before CXL and 0.56 (±0.32) 1 year thereafter (P = 0.346); mean (SD) Kmax, Ksteep, and Kflat were, respectively, 57.64 (±7.1), 54.02 (±6.2), and 48.60 (±4.1) before CXL and 56.46 (±8.0), 52.46 (±5.8), and 47.85 (±4.9) 1 year after CXL, respectively (P = 0.553, 0.258, and 0.640). CONCLUSIONS: The study showed no statistical differences between younger and older patients. These findings support the indication of CXL treatment in pediatric patients for early stabilization of the disease and better progress regarding BCVA and keratometry parameters.

A cárie dentária na atualidade continua sendo um problema de saúde pública, apesar do grande avanço tecnológico e de novas técnicas para preveni-la. Esta, se caracteriza como uma doença multifatorial, infecciosa e dieta dependente. Além disso, se apresenta mais prevalente em crianças devido a maior ingestão de carboidratos e sacarose, além de falta de controle motor durante higienização. O tratamento é dividido em quatro fases: sistêmica, preparatória, reabilitadora e manutenção. Assim, este artigo tem como objetivo relatar o caso clínico de criança de 6 anos de idade acometida por cárie precoce, descrevendo o tratamento em fases, compreendendo a reabilitação com resina composta nas unidades acometidas pela cárie e mantenedor de espaço com intuito de substituir as unidades na qual foram perdidas. Ao final do tratamento proposto, a paciente demonstrou satisfeita com o resultado, o que foi possível devolver a qualidade de vida da mesma. (AU).


Currently, caries is still a major problem in public healthy, despite the great technological development and new prevention methods. The caries is a multifactorial, infectious, transmissible and diet dependent disease. It is more prevalent in kids because of their higher intake of carbohydrates and sucrose, and the lack of motor control during the toothbrushing. The treatment is divided into four stages: systemic, preparatory, rehabilitative and maintenance. Thus, this article aims to report a case about a six-year-old child affected by early childhood caries, describing the treatment phases, comprising rehabilitation with composite created in units and space maintainer to replace the missing units. At the end of the treatment, the patient showed satisfaction with the outcome, restoring the quality of her life. (AU).

OBJECTIVES: Viral conjunctivitis is a common, highly contagious disease that is often caused by an adenovirus. The aim of this study was to evaluate the prevalence of adenoviral conjunctivitis by analyzing data from a prospective clinical study of 122 consecutively enrolled patients who were treated at the Clinical Hospital of the State University of Campinas (UNICAMP) after a clinical diagnosis of infectious conjunctivitis between November 2011 and June 2012. The prevalence of adenoviral conjunctivitis in the study population was similar to its prevalence in other regions of the world.

A exposição exagerada da gengiva é um dos problemas que afeta, negativamente, a estética do sorriso. Apresentando-se em diversos graus de extensão, sua etiologia pode estar relacionada com excesso vertical maxilar, protrusão dentoalveolar superior, extrusão e/ou erupção passiva alterada dos dentes anaterossuperiores, e hiperatividade dos músculos elevados do lábio superior. Em grande parte dos casos, porém, alguns desses fatores ou mesmo todos eles encontram-se associados. O Ortodontista parece ser o profissional mais indicado a diagnosticar e planejar o tratamento desses pacientes, por ter condições de avaliar a participação de cada um dos fatores etiológicos no problema e, numa proposta de abordagem transdisciplinar, encaminhá-los para profissionais das áreas afins.

Species of fish of Marajó Island, State of Pará, Brazil, were examined to identify the trematodes parasitizing 102 Hoplerytrinus unitaeniatus (gold wolf fish) and 104 Hoplias malabaricus (thraira). Metacercariae of two species of trematodes, 170 specimens of Clinostomatopsis sorbens and 10 Ithyoclinostomum dimorphum were found and identified. The parasitary indices of C. sorbens from H. unitaeniatus and H. malabaricus, were 43.14% and 30.77% for prevalence, 2.52 and 1.84 for mean intensity, 1.09 and 0.57 for mean abundance and 1 to 9 and 1 to 7 for range of infection, respectively, on both fish the site of infection was the mesentery. The parasitary indices of I. dimorphum from H. unitaeniatus were 2.94% for prevalence, 2.66 for mean intensity, 0.08 for mean abundance, 1 to 4 for range of infection, and the sites of infection were the mesentery and the muscle. Metacercariae of I. dimorphum were collected in muscles of a specimen of H. malabaricus, with 0.96% of prevalence, intensity of infection of 2 parasites and 0.02 of abundance. New morphological data of external and internal structures are presented. This is the first record of metacercariae of C. sorbens and I. dimorphum in Amazonian fish.

O peixe sapo-pescador é muito apreciado e representa um valioso recurso pesqueiro em nível mundial. O objetivo deste estudo foi o de identificar larvas de Anisakidae pela sua importância higiênico-sanitária e na saúde pública, cujos parasitos infectam espécimes do peixe sapo-pescador, Lophius gastrophysus Miranda-Ribeiro, 1915 (Lophiidae), obtidos em mercados dos municípios de Cabo Frio, Niterói, Duque de Caxias e Rio de Janeiro; são apresentados os índices parasitológicos e sítios de infecção dos helmintos encontrados. Foram investigados 87 espécimes de Lophius gastrophysus no período de março de 2000 a dezembro de 2005. Desses, 17 (19,5%) estavam parasitados por larvas de anisaquídeos. Anisakis simplex, Hysterothylacium sp. e Raphidascaris sp. apareceram com prevalências de 1,14, 12,6 e 12,6%, intensidade e intensidade média de infecção de 1, 2,81 e 10,5, abundância média de 0,01, 0,35 e 1,33 respectivamente. A 33-year-old woman complained of unilateral eyelid edema and blurred vision. Initial ophthalmic examination disclosed anterior chamber reaction with keratic precipitates on the cornea, without posterior abnormalities. Anterior uveitis was treated. Despite that, patient showed rapidly progressive unilateral vision loss with optic nerve swelling. Systemic workup was inconclusive, as well as cranial magnetic resonance imaging and cerebrospinal fluid examination. Based on the hypothesis of optic neuritis, intravenous methylprednisolone pulse was performed with no success. During the following days, the patient presented pericardial effusion and cardiac tamponade, progressing to death. Necropsy was performed and diagnosis of extranodal natural killers/T-cell lymphoma, nasal type with ocular involvement was confirmed by immunohistochemistry.


Paciente feminina de 33 anos apresentando edema palpebral unilateral com baixa acuidade visual. Ao exame oftalmológico inicial apresentava reação de câmara anterior com precipitados ceráticos corneano, sem alterações no fundo de olho. Foi instituído tratamento para uveíte anterior e solicitado exames sistêmicos para investigação. Após alguns dias, paciente apresentou piora da acuidade visual, com edema de disco óptico unilateral. Sendo então solicitada ressonância nuclear magnética de crânio. Ambos exames de imagem e investigação sistêmica foram inconclusivos. Em vista da piora progressiva da acuidade visual e sob hipótese diagnóstica de neurite óptica, foi iniciado pulso intravenoso de metilprednisolona por 3 dias sem sucesso. A paciente apresentou dos dias seguintes, derrame pericárdico e tamponamento cardíaco, evoluindo para óbito. A necrópsia confirmou o diagnóstico de linfoma extranodal de células T e natural killers do tipo nasal com envolvimento ocular, através de imunoistoquímica.

INTRODUCTION: Currently, people's esthetic requirements and expectations have increased substantially. Therefore, dentists have been seeking ways to provide excellent treatment results which, consequently, increasingly require a well organized transdisciplinary approach. The link between orthodontics and periodontics became evident from the moment professionals began to understand the biology of tooth movement. As regards smile esthetics, however, such cooperation is now essential. OBJECTIVE: To show clinically how and when orthodontists and periodontists should work jointly to enhance smile esthetics based on the display and harmony of the gingival contour.


INTRODUÇÃO: nos dias atuais, as expectativas e exigências estéticas das pessoas têm aumentado substancialmente. Por isso, a Odontologia vem buscando maneiras de oferecer tratamentos com resultados excelentes e, consequentemente, necessita trabalhar, cada vez mais, de modo transdisciplinar. A inter-relação da Ortodontia com a Periodontia é evidente desde quando se compreendeu a biologia da movimentação dentária; contudo, em se tratando de estética do sorriso, tornou-se fundamental. OBJETIVO: evidenciar, clinicamente, como e quando a Ortodontia e a Periodontia devem trabalhar de modo integrado para a obtenção de sorrisos mais estéticos, considerando-se a exposição e a harmonia do contorno gengival.

OBJECTIVE: To identify clinical and laboratory data which differentiate Klinefelter syndrome (KS) patients according to age group. METHODS: The study included all cases of hypogonadism, gynecomastia and/or infertility whose karyotype was performed at a university hospital from January 1989 to December 2011, in a total of 105 subjects. The following data were retrospectively analyzed: age at first visit, ratio of arm span to height, pubic hair, gynecomastia, testicular volume, luteinizing hormone (LH), follicle stimulating hormone (FSH), total testosterone (T), and spermiogram. RESULTS: During the study period, 33 patients were diagnosed with Klinefelter syndrome (KS+) and 72 were not (KS-). Out of all KS cases, only seven (21.2%) were diagnosed before 20 years old and two (6.1%) before 10 years old. Age at first consultation (in years) was similar in both groups (KS+ = 31.3±12.9 and KS- = 27.6±12.1), as were ratio of arm span to height and frequency of gynecomastia. However, in KS+ patients, pubic hair was less developed, testicular volume was smaller and testosterone levels were lower, while LH and FSH levels and frequency of azoospermia were higher. CONCLUSIONS: Klinefelter syndrome is both an under and late diagnosed condition. The most important data for diagnosis are testicular volume, hormone levels and presence of azoospermia in spermiogram, especially in puberty and adult life.

OBJETIVO: Identificar dados clínicos e laboratoriais que diferenciam os casos com síndrome de Klinefelter de acordo com a faixa etária. CASUÍSTICA E MÉTODOS: Foram incluídos todos os casos de hipogonadismo, ginecomastia e/ou infertilidade avaliados em hospital universitário cujo cariótipo foi realizado entre janeiro de 1989 e dezembro de 2011, totalizando 105 pacientes. Foram avaliados: idade na primeira consulta, relação entre envergadura e altura, pilificação pubiana, ginecomastia, tamanho testicular, hormônio luteinizante (LH), hormônio folículo-estimulante (FSH), testosterona e espermograma. RESULTADOS: Foram diagnosticados três casos com síndrome de Klinefelter (SK+) e 72 sem a síndrome (SK-). Dos casos com síndrome de Klinefelter, apenas sete (21,2%) foram diagnosticados antes dos 20 anos e dois (6,1%) antes dos 10 anos de idade. A idade na primeira consulta (em anos) foi semelhante nos dois grupos (SK+ = 31,3±12,9 e SK- = 27,6±12,1), o mesmo ocorrendo com a relação entre envergadura e altura e a presença de ginecomastia. No entanto, a pilificação pubiana foi menor no grupo SK+, o mesmo ocorrendo com a média do volume bitesticular e a testosterona, enquanto que o LH e o FSH foram mais elevados neste grupo, o mesmo ocorrendo com a frequência de azoospermia. CONCLUSÕES: A síndrome de Klinefelter ainda é pouco e tardiamente diagnosticada em nosso meio, sendo os dados de tamanho testicular, LH, FSH, testosterona e presença de azoospermia no espermograma os mais importantes para o seu diagnóstico, principalmente na puberdade e na vida adulta.

The aim of this study was to evaluate the tegument, musculature and mesentery of 102 specimens of Hoplerytrinus unitaeniatus, 104 of Hoplias malabaricus and 101 of Pygocentrus nattereri, from Arari Lake, Marajó Island, State of Pará, Brazil. Were identified the nematodes Contracaecum sp., Eustrongylides sp. and Procamallanus sp. Contracaecum sp. was the most prevalent, with rates of 84.31% (H. unitaeniatus), 95.19% (H. malabaricus), and 89.11% (P. nattereri). The highest prevalences of Eustrongylides sp. occurred in H. unitaeniatus (56.86%) and H. malabaricus (53.84%). Procamallanus sp. was only collected in the mesentery. Specimens of Eustrongylides sp. collected from the musculature were 91.9% of its population. Among the nematodes found in the mesentery, 98.34% were Contracaecum sp. with a mean intensity (MI) of 7.92 ± 8.11 (H. unitaeniatus), 8.49 ± 8.34 (H. malabaricus) and 7 ± 6.40 (P. nattereri). Contracaecum sp. presented the highest MI (8.49 ± 8.34) and mean abundance (8.09 ± 8.34). The highest MI values were observed in the mesentery. Eustrongylides sp. presented MI of 2.65 ± 3.21 (H. unitaeniatus), 3.41 ± 3.27 (H. malabaricus) and 2.17 ± 1.18 (P. nattereri). Nematodes with zoonotic potential that were found with high prevalence, shows the importance of actions by the health authorities.
