Craniosacral treatment (CST) is an osteopathic technique grounded in the assumption that there is an intrinsic, fine movement of the cerebrospinal fluid. This rhythmic movement can be utilized for diagnostic and therapeutic purposes by palpation and manipulation of the skull, spine, and associated connective tissues. Therapeutic benefit is likely due to action on the autonomic nervous system (ANS), specifically through the vagus nerve. Current literature on the neurophysiological effects of CST is limited, which has contributed to controversy regarding its effectiveness. Heart rate variability (HRV) as a measure of cardiovascular stress and autonomic system activity is thus proposed as a tool to evaluate the neurophysiologic effects of CST. HRV can be analyzed in two different bands, high-frequency (HF) and low-frequency (LF) power associated with a parasympathetic and sympathetic response. In this meta-analysis, we provide a brief introduction to CST, analyze three primary studies, and summarize the therapeutic benefits and pitfalls of this alternative treatment on the ANS. A significant negative HF standardized mean difference after CST was observed; standardized mean difference = -0.46; 95% CI (-0.79,-0.14). No significant effect on LF power was observed. We conclude that CST does provide a moderate short-term increase in parasympathetic activity. These findings suggest that CST may be used to treat patients with an overactive sympathetic state. Further studies should be conducted for comparison against a control group to eliminate the possibility of a placebo effect and to elucidate long-term effects.

Osteopathic manipulative treatment (OMT) is a therapy used by osteopathic physicians in various medical settings. Postoperatively, OMT can be utilized to optimize the body's function and recovery. This meta-analysis examines the efficacy of OMT in reducing the length of postoperative hospital stays. Given the significant implications of prolonged hospitalization for both patients and healthcare resources, research strategies to safely shorten this period are crucial. This meta-analysis examined five select studies that measured the length of hospital stay in postoperative patients who received OMT compared with postoperative patients who did not. A random effects model was applied in our statistical analysis to account for heterogeneity due to variations in surgical procedures, hospitals, and patient populations. Individually, three studies reported statistically significant reductions in hospital stay for OMT patients, while two did not. This meta-analysis, comprising five studies and 519 patients, found a mean difference of -2.37 days in favor of OMT; however, this finding did not reach a statistical significance (P = 0.06). The substantial heterogeneity observed (heterogeneity tau2 = 6.75, chi2 = 34.6, df = 4, P < 0.00001, I2 = 88%) suggests that clinical dissimilarities among the five studies may have resulted in our inconclusive findings. While OMT shows promise in postoperative care, further research with standardized protocols and more homogenous patient populations is needed to assess its true impact.

Alligator bites in humans present a significant concern for public safety in the southern United States, especially in states like Florida with substantial alligator populations. Although these reptiles play a vital role in the local ecology, encounters with humans can lead to severe injuries and even fatalities. A case report is presented of a 58-year-old male who suffered an alligator bite while attempting to take a selfie with the reptile during a hunting trip in rural Florida. The patient's injuries included multiple lacerations on the dorsum of his right hand. Despite the incident, the patient hesitated in seeking medical attention due to a lack of insurance, emphasizing the need for public awareness of alligator bite management. The discussion highlights the potential complications of alligator bites, including hemorrhage and infection, as well as the importance of appropriate medical treatment, including wound irrigation, debridement, and antibiotic therapy. Moreover, preventive strategies are discussed, such as maintaining a safe distance from alligators and refraining from feeding them, to ensure coexistence between humans and these reptiles in their natural habitats. As knowledge of alligator bites remains limited, this case report contributes valuable information to promote public safety and guide future research in this area.

Post-Coronavirus disease 2019 (COVID-19) development of polymyositis is rare, with very few cases documented in the literature. In patients developing vague symptoms after recovery from COVID-19, it is important to investigate all avenues, including the possibility of polymyositis. Polymyositis is typically characterized as symmetrical muscle weakness and histological, electrical, and chemical evidence of muscle injury. Patients identified can be treated with immunosuppressants to return some quality of life and prevent disease progression. In this report, we describe a 58-year-old Caucasian male who presented with symptoms of weakness, myalgias, and arthralgias, six months after recovering from flu-like symptoms of COVID-19. The patient was tested for other autoimmune etiologies and myopathies without positive results. He was treated with prednisone and reported moderate improvement in symptoms. Unfortunately, the patient declined a muscle biopsy or electromyographic testing. According to the criteria for polymyositis set by the Myositis Association and the response to therapy, the patient's symptoms pointed to a likely diagnosis of post-COVID-19 polymyositis.

Sjögren's syndrome (SS) is the result of an abnormal immune response in the body that leads to the destruction of exocrine glands, primarily the ocular and salivary glands, resulting in xerophthalmia and xerostomia. We describe a case of a 55-year-old female who presented to the clinic with abnormal liver function tests, identified on routine blood work. After an extensive workup, the patient was diagnosed with autoimmune hepatitis (AIH) and SS. AIH occurs in a small number of patients with primary SS, and there is little research available on the overlap of these two autoimmune disorders. In this report, we will present some of the challenges in diagnosing AIH and identifying SS as the underlying diagnosis. Because of the ambiguous clinical picture, diagnosis of AIH can be difficult, especially in the setting of concomitant autoimmune disorders.

This case report details a rare case of Grover's disease in an 80-year-old Caucasian male complaining of a rash across his chest over the last three to four months. The patient has a past medical history of essential hypertension, hyperlipidemia, osteoarthritis of the knee, chronic gastroesophageal reflux disease (GERD), supraventricular tachycardia, status post prostate cancer, and restless legs syndrome. During his initial evaluation, he was found to have a pruritic, erythematous, papular rash most notably along his upper trunk and chest. The patient utilized multiple lotions, emollients, and anti-itch creams with minimal relief of his symptoms and presentation. Following a referral to Dermatology, a biopsy of the rash was conducted, which revealed intraepidermal acantholysis, the hallmark finding for a diagnosis of Grover's disease. Subsequently, he was treated with a topical triamcinolone acetonide 0.1% cream for 14 days. This study details a case of Grover's disease along with potential comorbidities and contributing factors in order to further understand the pathogenesis and etiology of this relatively rare condition.

UNLABELLED: To determine and compare the clinical performance of formulations containing 7.5% calcium sodium phosphosilicate (NovaMin), 5% potassium nitrate, and 0.4% stannous fluoride for the management of dentin hypersensitivity. METHODS: This was a single-center, randomized, double-blind, parallel-group design with a duration of 12 weeks. The study included a total of 120 subjects and measured sensitivity to cold water and air blast by the use of a visual analogue scale. Measurements were taken at baseline, two, four, and 12 weeks. RESULTS: All three products significantly reduced sensitivity versus baseline at each time point, although the calcium sodium phosphosilicate (NovaMin) dentifrice reduced sensitivity significantly more than the others at the two- and four-week time points. At the two-week time point, for air and water, respectively, the dentifrice containing NovaMin reduced sensitivity 45% and 49%, the stannous fluoride gel 30% and 26%, and the potassium nitrate dentifrice 35% and 34%. At the 12-week time point, the dentifrice containing NovaMin reduced sensitivity 87% and 91%, stannous fluoride gel 87% and 85%, and potassium nitrate dentifrice 84% and 79%. CONCLUSION: In this study, all three products were effective. Compared to the potassium nitrate and stannous fluoride formulations, the dentifrice containing NovaMin provided more substantial and significant improvements at the early time points.
