RÉSUMÉ

The paper summarizes the clinical experience of professor ZHANG Ren in the staging treatment with characteristic acupuncture techniques for oculomotor paralysis. According to the symptoms of oculomotor paralysis, the staging treatment is given, in which acupuncture is dominant and the needling techniques are optioned in compliance with the symptoms. In the early, middle and late stages of illness, three different acupuncture therapies are delivered accordingly, i.e. the combination of the point-toward-point needling at the four acupoints located on the forehead and the electroacupuncture with disperse-dense wave, the surrounding needling and the triple needling at the acupoints around the eyeball, as well as the perpendicular needle insertion at the three acupoints within the orbit. Professor ZHANG Ren lays the stress on identifying the etiology and differentiating the symptoms, as well as the early intervention for the disease. For the intractable cases, the comprehensive regimen such as acupoint injection, dermal needling and auricular point sticking is supplemented. During treatment, the spirit harmonization is greatly considered to ensure the effectiveness. On the basis of the staging acupuncture therapy, the acupuncture technique for harmonizing the spirit and regulating qi is combined to obtain the favorable clinical effect on oculomotor paralysis.

BACKGROUND: Scalp acupuncture has been found to be effective at improving motor function after ischemic stroke, but few studies examining its central mechanisms of action have been carried out. The aim of this study was to investigate the clinical effects of scalp acupuncture on motor dysfunction and changes in spontaneous brain activity in patients with ischemic stroke. METHODS: This was an evaluator- and analyst-blinded, multi-center randomized controlled trial. A total of 108 convalescent-stage ischemic stroke patients with motor dysfunction were allocated to receive either scalp acupuncture combined with rehabilitation treatment (SR group) or rehabilitation treatment alone (RE group). Patients in both groups received treatment 5 times per week for 8 weeks. The primary outcome measure was the Fugl-Meyer assessment (FMA). Secondary outcome measures included the modified Barthel index (mBI), modified Rankin scale (mRS) and values of fractional amplitude of low frequency fluctuation (fALFF) acquired using a resting-state functional magnetic resonance imaging (rs-fMRI) technique. RESULTS: Both groups showed significant improvements in motor function, daily life ability and degree of disability, as measured by FMA, mRS and mBI (p < 0.05), and the SR group showed a significantly greater improvement (p < 0.05). Compared with the RE group, the areas where the fALFF values increased in the SR group were located in the cerebellum, praecuneus, precentral gyrus, superior frontal gyrus and parietal lobe. The improvement in FMA scores had the strongest correlation with the baseline fALFF values of the ipsilateral precentral gyrus. CONCLUSION: Scalp acupuncture improved motor function in convalescent-period ischemic stroke patients, and effects were correlated with regulation of motor-relevant brain regions. The fALFF value of the ipsilateral precentral and postcentral gyri could be potential clinical indices for prognostication of motor dysfunction. PURPOSE: To investigate the expression of receptor activator of nuclear factor-κB ligand (RANKL) in keratocystic odontogenic tumor (KCOT) before and after decompression. METHODS: Twenty two pairs of paraffin-embeded tissue specimens of KCOT were collected and subjected to immunohistochemical analysis of RANKL.The data were analyzed with SPSS17.0 software package for paired t test. RESULTS: RANKL positive cells were observed in the preoperative KCOT epithelium. The expression of RANKL was down-regulated after decompression(P<0.05). CONCLUSIONS: Decrease of RANKL expression after decompression may facilitate the lining epithelium of KCOT transforming to normal oral mucosal epithelium.

PURPOSE: To summarize the clinicopathologic characteristics, treatment and prognosis of salivary duct carcinoma. METHODS: Six cases of salivary duct carcinoma treated in our hospital were reported. And the literatures were reviewed. All cases were proved by operation and pathology. The clinical characteristics, histological features, therapeutic method and prognosis were analyzed. RESULTS: Salivary duct carcinoma predominantly influenced old male patients and mainly occurred in the parotid gland. Physical examination revealed a firm, immobile and poorly defined mass accompanied by nerve infiltrated symptom. The histological appearance was characterized by solid cell nests with ductal structures and central comedonecrosis. Treatment required extensive resection and radical neck dissection with postoperative radiotherapy. The clinical course was marked by development of both local disease recurrence as well as cervical lymph node and distant metastasis. CONCLUSIONS: Salivary duct carcinoma is a rare malignant salivary tumour with poor prognosis. Extensive resection is the key to improve the survival rate.

Bisphosphonate compounds are used to treat osteoporosis and malignant bone metastasis. Despite the benefits related to use of these medications, osteonecrosis of the jaws is a significant complication in a subset of patients receiving these drugs. This complication occurs either spontaneously or after a simple dento-alveolar surgery. Recently there was a patient who showed the features of bisphosphonate related osteonecrosis of the jaws (BRONJ) in our hospital. This article reported one typical case of osteonecrosis of the maxilla treated with Zoledronic acid and reviewed the relevant literature related to pathogenesis, clinical presentation as well as management and prevention of this new complication.

Two cases with spindle cell squamous carcinoma were reported in this paper. The clinical, histopathologic features, therapy and prognosis were discussed. These tumors probably represented a heterogeneous group of squamous cell carcinoma whose components required either an identifiable squamous cell carcinoma or evidence of epithelial differentiation in the spindle cell. The clinical features were as follow: presents a male predominance among 60 to 70 years and site predilection for the low lip, tongue and gingiva, more aggressive and easy lymphatic metastases. Wide surgical excision, alone or with radical neck dissection was the most successful therapeutic modality. Local recurrence, lymphatic metastases and the depth of tumor invasion into underlying structures were found to be reliable prognostic indicators, the prognosis was dismal.
