RÉSUMÉ

The present study aimed to determine the effectiveness of sclerotherapy using NBCA (Histoacryl Blue®; B. Braun, Melgungen, Germany), with or without hydrodissection, for the treatment of simple renal cysts. Materials and Methods: Patients who presented to an interventional radiology clinic for the diagnosis of symptomatic renal cysts which had previously been identified at an outpatient clinic were selected for inclusion in this study. A total of 28 patients were randomly divided into 2 groups, based on whether or not they underwent hydrodissection along with ultrasound-guided NBCA-based sclerotherapy. Sonographs were performed at 0, 7, and 180 days post-procedure to record the residual volume of the renal cysts and to determine the efficacy of the procedure. Results: A total of 32 cysts in 28 patients were treated with sclerotherapy, 18 (64%) females and 10 (36%) males. The average age of the patients was 61.8 years (range: 33-89 years). All patients reported an improvement in symptoms associated with the existing renal cysts at 7 and 180 days post-procedure, and at 7 days post-procedure a statistically significant reduction in cyst volume was observed (all patients: 96.8%; group A: 96%; group B: 97.6%). The reduced cyst volume was still observed 180 days post-procedure (all patients: 98.6%; group A: 98.2%; group B: 98.9%). There was no significant difference between the two treatment groups. Conclusion: There is a significant and persistent reduction in the volume of renal cysts, in addition to an improvement of the associated symptoms, after treatment with NBCA-based sclerotherapy, with or without hydrodissection.

BACKGROUND-OBJECTIVE: Prior studies have suggested a higher prevalence of simple renal cysts (SRC) among patients with aortic disease, including abdominal aortic aneurysms (AAA). Thus, the aim of this study was to systematically review all currently available literature and investigate whether patients with AAA are more likely to have SRC. METHODS: This study was performed according to the PRISMA guidelines. A meta-analysis was conducted with the use of random effects modeling and the I-square was used to assess heterogeneity. Odds ratios (OR) and the corresponding 95% confidence intervals (CI) were synthesized to compare the prevalence of several patients' characteristics between AAA vs. no-AAA cases. RESULTS: Eleven retrospective studies, 9 comparative (AAA vs. no-AAA groups) and 3 single-arm (AAA group), were included in this meta-analysis, enrolling patients (AAA: N = 2,297 vs. no-AAA: N = 35,873) who underwent computed tomography angiography as part of screening or preoperative evaluation for reasons other than AAA. The cumulative incidence of SRC among patients with AAA and no-AAA was 55% (95% CI: 49%-61%) and 32% (95% CI: 22%-42%) respectively, with a statistically higher odds of SRC among patients with AAA (OR: 3.02; 95% CI: 2.01-4.56; P< 0.001). The difference in SRC prevalence remained statistically significant in a sensitivity analysis, after excluding the study with the largest sample size (OR: 2.71; 95% CI: 1.91-3.84; P< 0.001). CONCLUSIONS: Our meta-analysis demonstrated a 3-fold increased prevalence of SRC in patients with AAA compared to no-AAA cases, indicating that the pathogenic processes underlying SRC and AAA could share a common pathophysiologic mechanism. Thus, patients with SRC could be considered at high risk for AAA formation, potentially warranting an earlier AAA screening.

RÉSUMÉ

We report the case of an 86-year-old adult man who, as a pedestrian, is hit by a motorcycle, suffering polytrauma; in initial care, he refers to thoraco-abdominal pain and subsequently neurological deterioration. Assessed by a neurosurgeon and general surgeon, a right chest tube is placed and a laparoscopy is performed where there is little bleeding from the abdominal cavity. It shows deterioration of its general state and dies in respiratory failure. During the necropsy procedure there is subarachnoid hemorrhage and cerebral herniation, rib fractures and pneumonic consolidation, a massive retroperitoneal hematoma is observed due to rupture of simple renal cyst.


Reportamos el caso de un hombre de 86 años que, siendo peatón, es atropellado por una motocicleta, sufriendo politraumatismo. En la atención inicial refiere a dolor toracoabdominal y posteriormente deterioro neurológico. Evaluado por un neurocirujano y un cirujano general, se coloca un tubo torácico derecho y se realiza una laparoscopia y observándose poco sangrado de la cavidad abdominal. El paciente muestra deterioro de su estado general y muere por insuficiencia respiratoria. Durante el procedimiento de necropsia se determina hemorragia subaracnoidea y hernia cerebral, fracturas costales y consolidación neumónica, se observa un hematoma retroperitoneal masivo por rotura de quiste renal simple.

Los quistes renales simples son poco frecuentes en recién nacidos, niños y adolescentes. Suelen ser únicos, y ocasionalmente múltiples. No existen evidencias de riesgo familiar. El objetivo de la presentación fue resaltar la importancia del diagnóstico precoz de malformaciones congénitas y/o defectos estructurales del feto, por ultrasonografía, en la atención primaria de salud. Presentamos las imágenes ecográficas de un caso de afectación fetal por un quiste renal simple a las 36 semanas, sin otros hallazgos ultrasonográficos. Previo asesoramiento genético y dado la avanzada edad gestacional y el buen pronòstico con que cursan estos casos además de no tener otro defecto genético asociado, la pareja decidió la continuación del embarazo. El caso fue confirmado por ecografía posnatal(AU)


Simple renal cysts are rare in newborns, children and adolescents. They tend to be unique, and occasionally multiple. There is no evidence of family risk. The objective of the presentation was to highlight the importance of early diagnosis of congenital malformations and / or structural defects of the fetus, by ultrasonography, in primary health care. We present the echography of a case of fetal involvement by a simple renal cyst at 36 weeks, with no other ultrasonography findings. The parents decided to continue the pregnancy previous genetic counseling, given the advanced gestational age and good prognosis with these cases, and considering there is no other genetic defect associated. The case was confirmed by postnatal ultrasound(AU)

Introducción: El divérticulo calicial se detecta en 0,21 a 0.60 por ciento de los urogramas excretores. Objetivos: Describir una paciente con diagnóstico incidental de esta anomalía y su seguimiento durante 11 años. Presentación del caso: Durante la realización de un urograma excretor para el estudio de una hidronefrosis en una niña de siete años de edad, se detectó un divertículo calicial en el riñón contralateral y después de 11 años de seguimiento ultrasonográfico no se ha demostrado modificación ni complicación del divertículo. Conclusiones: El divertículo calicial es una anomalía congénita que puede mantenerse sin complicaciones durante años por lo que debe tratarse conservadoramente(AU)


ABSTRACT Introduction: Calyceal diverticulum is detected in 0,21 percent to 0.60 percent of excretory urogram. Objectives: To describe a patient with an incidental diagnosis of this anomaly and her follow up during 11 years. Case presentation: During the performance of an excretory urogram for studying a hydronefrosis in a seven years old girl, a calyceal diverticulum was detected in the contralateral kidney; and after 11 years of ultrasonographic follow-up there has been no modifications or complication related with the diverticulum. Conclusions: Calycial diverticulum is a congenital anomaly that can last years without presenting complications. That is why it must be treated conservatively(AU)

RÉSUMÉ

OBJECTIVE: The objective of our study was to systematically summarize the published evidence of demographic, clinical, diagnostic imaging, and therapeutic characteristics of patients with multilocular cystic nephroma (MLCN). CONCLUSION: Cross-sectional imaging evaluation is important for suggesting the diagnosis of MLCN but has several limitations. The number of radical nephrectomies reported for MLCN encourages discussion concerning the utility of percutaneous presurgical biopsy and frozen-section intraoperative biopsy as a more conservative diagnostic approach.

RÉSUMÉ

Se describe el caso clínico de un paciente de 59 años de edad, con quistes renales, tratado en la Unidad de Diálisis del Hospital "Alberto Fernández Monte de Oca" del municipio de San Luis, en la provincia de Santiago de Cuba, quien durante el tratamiento hemodialítico presentó dolor abdominal de aparición brusca, difuso, de gran intensidad y se complicó tan gravemente que falleció. El resultado de la necropsia confirmó la presencia de riñones poliquísticos (con 5 kg de peso el derecho y 6 el izquierdo), insuficiencia renal crónica terminal y trombosis mesentérica superior.


The case report of a 59 year-old patient with renal cysts, treated in the Dialysis Unit of "Alberto Fernández Monte de Oca" Hospital in San Luis municipality, from Santiago de Cuba is described. He presented with an intense and abrupt, diffused abdominal pain during the hemodialysis treatment, he had serious complications and so he died. The result of the autopsy confirmed the presence of polycystic kidneys (5 kg weight the right one and 6 the left one), terminal chronic renal failure and superior mesenteric thrombosis.

Relatamos un caso de gran quiste peripiélico causando obstrucción de la pelvis renal y episodios recurrentes de dolor lumbar. Se utilizó ureterorrenoscopia flexible para la marsupialización interna del cisto en el sistema colector renal, con electrocoagulación del interior del cisto. El caso y la técnica se describen con detalles.


We report a case of a large peripelvic renal cyst causing obstruction of renal pelvis and episodes of flank pain. Flexible ureteronephoscopy has been used for the internal marsupialization of cyst into the renal collecting system with eletrocoagulation of the inner part of the cyst. The case and technique are described in detail.
