Objective To compare the functional outcomes of two circular external fixation techniques to treat complex fractures of the proximal end of the tibia. Materials and Methods The present is a retrospective cohort study with 51 patients who underwent surgical treatment for complex fractures of the tibial plateau with a circular external fixator. There were two groups of patients: 12 subjects underwent treatment with the classic assembly technique, and 39 subjects underwent treatment with the simplified technique. The variables analyzed included age, sex, injury mechanism, trauma energy, associated injuries, fixator type, time of fixator use, and clinical-radiographic outcomes. The classic technique mainly uses transfixing Kirschner wires, while the simplified one replaces the Kirschner wires with Schanz pins in the distal block of the circular external fixator. Result There were no statistically significant differences ( p > 0.05) between the two groups concerning the clinical-radiographic outcomes, including fracture consolidation, quality of joint fracture reduction, range of motion, lower limbs residual discrepancy, and postoperative pain. Conclusion We suggest that the simplified technique, using Schanz pins instead of Kirschner wires, can be a viable and effective alternative to treat complex fractures of the proximal end of the tibia with a circular external fixator. This simplified approach can offer benefits, such as a lower infection rate and greater patient comfort, without compromising clinical and radiographic outcomes, thus justifying its use.
RÉSUMÉ
INTRODUCTION: The reported incidence of infection related with tibial plateau fractures (IRTPF) ranges from 2 to 23%. This complication can result in catastrophic consequences such as deformity, post-traumatic osteoarthritis, chronic pain, loss of function, and substantial economic burdens on healthcare systems due to extended hospital stays and the resources required for treatment. Consequently, it is imperative to emphasize the identification of infection risk factors. METHODS: A retrospective case-control study was designed, encompassing patients who underwent surgery for tibial plateau fractures between 2015 and 2020. Frequencies and measures of central tendency were compared between infected patients (cases) and non-infected patients (controls) using rank-based statistical tests. Subsequently, two logistic regression models were employed to control for potential confounding variables. RESULTS: A total of 314 patients were included, predominantly male (71.15%). Average age of 44.41 years. IRTPF were observed in 7.64% of the patients. In the univariate inferential statistical analysis, high-energy fractures (OR 6.35, p < 0.001), fractures with compartment syndrome (OR 7.10, p < 0.001), two-stage management with temporary external fixation (OR 8.18, p < 0.001), the use of 2 or more approaches in definitive surgery (OR 2.93, p = 0.011), and the use of two or more plates (OR 9.17, p < 0.001) were identified as risk factors for infection. On average, the duration of surgery in infected patients was 201.2 min, compared to 148.4 min in non-infected patients (p < 0.001). When performing two logistic regression models, the following independent risk factors were identified: high-energy fractures (OR 5.04, p = 0.012), the presence of compartment syndrome (OR 4.53, p = 0.007), and the use of two or more plates in definitive surgery (OR 5.04, p = 0.023). CONCLUSIONS: High-energy tibial plateau fractures (Schatzker IV, V, and VI), the presence of concomitant compartment syndrome, and the use of 2 or more plates in definitive surgery are associated with a higher risk of infection related to fracture following open reduction and osteosynthesis treatment. Resumo Objetivo Comparar os resultados funcionais entre duas técnicas de fixação externa circular utilizadas no tratamento de fraturas complexas da extremidade proximal da tíbia. Materiais e Métodos Trata-se de um estudo de coorte retrospectivo, com 51 pacientes submetidos ao tratamento cirúrgico de fraturas complexas do planalto tibial com fixador externo circular. Os pacientes foram divididos em dois grupos: 12 pacientes tratados com a técnica clássica e 39 pacientes tratados com a técnica simplificada. As variáveis analisadas incluíram idade, sexo, mecanismo da lesão, energia do trauma, lesões associadas, tipo de fixador, tempo de uso do fixador e resultados clínico-radiográficos. A técnica clássica lança mão principalmente de fios de Kirschner transfixantes, e a simplificada substitui os fios de Kirschner por pinos de Schanz no bloco distal do fixador externo circular. Resultados A partir da comparação das montagens, não encontramos diferenças estatisticamente significativas (p> 00,5) entre os dois grupos em relação aos resultados clínico-radiográficos, incluindo a consolidação da fratura, a qualidade da redução da fratura articular, a amplitude de movimento, a discrepância residual na medida dos membros inferiores e a dor do paciente no pós-operatório. Conclusão Sugerimos que a técnica simplificada, utilizando pinos de Schanz no lugar dos fios de Kirschner, pode ser uma alternativa viável e eficaz no tratamento de fraturas complexas da extremidade proximal da tíbia com fixador externo circular. Essa abordagem simplificada pode oferecer benefícios, como menor taxa de infecção e maior conforto para o paciente, sem comprometer os resultados clínicos e radiográficos, o que justifica, o seu uso.

BACKGROUND: Posterior tibial plateau fractures, including avulsion fractures of the posterior cruciate ligament (PCL) insertion, represent a challenge for the orthopedic trauma surgeon. These injuries have gained a new perspective both diagnostically and therapeutically after the regular use of multiplanar computed tomography. In the herein study, we describe the outcome of patients sustaining a tibial plateau fracture with posterior articular involvement treated by open reduction and internal fixation (ORIF) using our modified gastrocnemius splitting anatomic approach. METHODS: This observational retrospective descriptive study was conducted at Complejo Medico de la Policia Federal Argentina Churruca-Visca. All patients were treated by ORIF by our team through our modified gastrocnemius splitting anatomic approach, and followed-up for a minimum of 12 months. RESULTS: A total of 18 patients sustaining plateau fracture were treated by this approach. Satisfactory tomographic reduction with articular gap and/or step-off <2 mm was achieved in 16. The mean time to the return to activities of daily living was 192.2 days. CONCLUSION: The modified gastrocnemius splitting anatomic approach represents a good alternative for the management of tibial plateau fractures involving the posterior quadrants.

Tibial plateau fractures with significant joint depression and metaphyseal comminution pose a challenge. In order to prevent the collapse of the articular surface, some authors propose filling the subchondral void created during reduction with bone graft/substitute, which can add further complications. We present two cases of tibial plateau fractures with severe joint depression of the lateral condyle; both treated with a periarticular rafting construct, in one caseadditional bone substitute was used and in the other case no bone graft/substitute was used; their final outcomes were reported. The treatment of joint depression in tibial plateau fractures using periarticular rafting constructs without bone graft, may be also a valid option, to achieve good final results without the morbidity associated with the use of bone graft/substitutes.

Objetivo: comparar si existe diferencia en resultados clínicos, radiológicos y funcionales con el uso de diferentes tipos de injertos óseos o sustitutos sintéticos, así como tiempo quirúrgico y consolidación para el tratamiento de fracturas de platillo tibial con hundimiento articular en pacientes mayores de 18 años. Material y método: se realizó una búsqueda sistematizada en bases de datos de literatura médica, PubMed, Lilacs, Scielo, Cochrane y portal Timbó. Se utilizaron términos "tibial plateau fracture", "bone grafting", "bone substitutes". Se incluyeron estudios comparativos en seres humanos, pacientes mayores de 18 años, fracturas de platillo tibial que asociaron hundimiento articular, en los cuales se realizó aporte de injerto óseo o material sintético. Artículos en español, inglés, portugués. Publicaciones enero 1980 y diciembre 2021. Se obtuvieron 10 artículos. Resultados: los tipos de injertos y sustitutos óseos encontrados fueron 10. Las muestras en sumatoria total 524 pacientes. La edad promedio fue 49 años. El promedio de seguimiento fue de 12 meses. Se dividieron los estudios en tres grupos: comparación entre injerto autólogo (IOA) e injerto alogénico, IOA con sustitutos sintéticos, y los que comparan sustitutos sintéticos entre sí. El aloinjerto y los sustitutos sintéticos demostraron no ser inferiores en resultados clínicos, funcionales e imagenológicos, mejorando los tiempos intraoperatorios y disminuyendo complicaciones en el sitio donante con respecto al IOA. Conclusiones: el IOA continúa siendo el gold standard a pesar de sus posibles complicaciones vinculadas al sitio donante. El aloinjerto y los sustitutos sintéticos representan una opción válida para tratar estas lesiones.
Objective: to find out whether there are clinical, radiological and functional differences when using different types of bone grafts or synthetic substitutes, as well as surgical times and consolidation to treat depressed tibial-plateau fractures in patients older than 18 years old. Method: a systematized search was conducted in medical literature, PubMed, Lilacs, Scielo, Cochrane and Timbó portal databases using the following terms: "tibial plateau fracture", "bone grafting", "bone substitutes". The study included comparative studies in human patients older than 18 years old with depressed tibial-plateau fractures who were treated with bone grafts or synthetic materials. Publications in Spanish, English and Portuguese, between January, 1980 and December, 2021 were included in the search, what resulted in 10 articles found. Results: there were 10 kinds of bone grafts and bone substitutes found. Samples added up to 524 patients. Average age was 49 years old. Average follow up was 12 months. Studies were divided into 3 groups: comparison between autologous bone grafts and allogenic bone grafts, comparison between autologous grafts and synthetic substitutes and studies comparing synthetic substitutes with one another. Allogenic grafts and synthetic grafts proved at least equivalent in terms of clinical, functional and imaging studies results, improving intraoperative times and reducing complications in donor site when compared to autologous grafts. Conclusions: autologous grafts continue to be the gold standard despite possible complications associated to the donor site, and allogenic grafts and synthetic substitutes constitute a valid option to treat these lesions.

Material e método: foi realizada busca sistemática nas bases de dados da literatura médica, PubMed, Lilacs, SciELO, Cochrane e portal Timbó. Utilizaram-se os termos "tibial plateau fracture", "bone grafting", "bone substitutes". Foram incluídos estudos comparativos incluindo seres humanos maiores de 18 anos e fraturas do platô tibial associadas a colapso articular, nas quais foi realizado enxerto ósseo ou sintético, publicados entre janeiro de 1980 e dezembro de 2021 em espanhol, inglês e português. Foram obtidos 10 artigos. Resultados: foram identificados 10 tipos de enxertos e substitutos ósseos. 524 pacientes com idade média de 49 anos foram estudados. O seguimento médio foi de 12 meses. Os estudos foram divididos em 3 grupos: comparação entre enxerto autólogo (IOA) e enxerto alogênico, IOA com substitutos sintéticos e substitutos sintéticos entre si. O aloenxerto e os substitutos sintéticos mostraram-se não inferiores nos resultados clínicos, funcionais e de imagem, melhorando os tempos intraoperatórios e reduzindo as complicações da área doadora em relação à IOA. Conclusões: o IOA continua a ser o padrão ouro apesar de suas possíveis complicações relacionadas ao local doador; tanto o aloenxerto como os substitutos sintéticos representam uma opção válida para tratar essas lesões.

BACKGROUND: Knee stiffness is a challenging complication following complex fractures around the knee. Several treatment strategies have been described in the last decades, but clinical results and complication rates still remain as potential drawbacks. The aim of this study was to access the clinical outcomes and complications of the modified Judet quadricepsplasty following knee stiffness secondary to complex fractures around the knee. METHODS: A total of 11 patients presenting post-traumatic knee stiffness underwent modified Judet quadricepsplasty from 2014 to 2017. All procedures were performed by the same surgical team, and all patients followed the same postoperative pain control and rehabilitation protocols. No patients underwent medial approach for medial release. When necessary, medial release was performed through the lateral approach. Patients were evaluated using the Judet criteria for final range of motion after 1-year minimum follow-up. RESULTS: According to the Judet criteria, 4 patients (36.4%) presented excellent, 6 (54.5%) good, and 1 (9.1%) poor clinical outcomes. Blood transfusion was required in 5 patients (45.4%). No patients presented infection or wound dehiscence. CONCLUSIONS: Although quadricepsplasty is considered a high morbidity surgical procedure, our favorable functional outcomes with very low complication rates using this modified Judet quadricepsplasty confirmed safety and efficacy of this helpful surgical procedure for the challenge of post-traumatic knee stiffness. Determinar prevalência de fraturas de platô tibial em pacientes internos em um Hospital Público do Piauí. MATERIAIS E MÉTODOS: Foram analisados 50 prontuários com Fraturas de Platô Tibial de Janeiro de 2017 a Janeiro de 2018. O instrumento de análise foi um questionário composto por informações relevantes, como: sexo, idade, raça, mecanismo de lesão, tipos de fratura e lado, para a coleta dos dados. Para análise dos dados utilizou-se a estatística descritiva com cálculos de médias, desvio padrão e frequências absoluta e relativa. RESULTADOS: A maior incidência das fraturas foi no sexo masculino (72%). A média de idade dos pacientes foi de 38 ± 13,86 anos, vítimas principalmente de acidentes automobilísticos (66%), sendo o lado mais acometido o direito (70%) com a incidência maior de fratura oblíqua (48%). CONCLUSÃO: há maior prevalência de fraturas do platô tibial no sexo masculino, faixa etária de 19-29 anos, vítima de acidente automobilísticos, com fraturas oblíquas.
To determine the prevalence of tibial plateau fractures in internal patients at a Public Hospital of Piauí. MATERIALS AND METHODS: Fifty medical records with Tibial Plateau Fractures from January 2017 to January 2018 were analyzed. The analysis instrument was a questionnaire composed of relevant information, such as: sex, age, race, injury mechanism, types of fracture and affected side, for data collection. For data analysis we used descriptive statistics with calculations of means, standard deviation and absolute and relative frequencies. RESULTS: The highest incidence of fractures was in males (72%). The mean age of the patients was 38 ± 13.86 years, mainly victims of automobile accidents (66%), with the right side most affected (70%) with the highest incidence of oblique fracture (48%). CONCLUSION: there is a higher prevalence of fractures of the tibial plateau in males, aged 19-29 years, victim of automobile accidents, with oblique fractures.

ResumenLas fracturas de plato tibial representan el 1,2% de las fracturas, se observan predominantemente en dos grupos: en pacientes jóvenes que sufren lesiones de alta energía y en adultos mayores con osteopenia debido a traumas de baja energía. 1,2En el paciente joven es más frecuente la lesión de tejidos blandos debido a la resistencia ósea, mientras en el paciente anciano suele asociarse mayor frecuencia de depresión de la superficie articular.2Las causas asociadas son la compresión axial asociada a varo o valgo, precipitaciones, accidentes de tránsito y actividades deportivas. Las fracturas del plato tibial lateral son más frecuentes que las mediales, debido al valgo fisiológico.1,2 .Se dividen en VI tipos. Los tipos de fracturas V y VI involucran los dos platos tibiales, medial y lateral, por lo que en este tipo de fracturas se observan peores resultados postquirúrgicos. Las consecuencias se deben al tiempo requerido para la recuperación y las secuelas residuales.El presente estudio pretende documentar cuáles son los tratamientos de elección en el Hospital San Juan de Dios y sentar un precedente epidemiológico con datos costarricenses que sirva de base para futuras investigaciones y/o protocolos de tratamiento.Los resultados quirúrgicos se miden de acuerdo a la satisfacción de los pacientes con el proceso quirúrgico y la funcionalidad postquirúrgica, esto debido a que dichas medidas han demostrado poseer mayor utilidad para monitorizar las intervenciones terapéuticas.6,7
AbstractTibialplateau fractures represent 1,2% of all fractures, they are predominant in two groups: young patients who suffer of high-energy trauma and in elderly with osteopenia due to low energy trauma. In young patients, soft tissue trauma is more frequent due to bone resistance, in the elderly it is associated with articular surface depression. Fractures causes are associated with valgus or varus axial compression, precipitation, car accidents and sports. Lateral tibialplateau fractures are more frequent than medial fractures due to physiological valgus.These fractures are divided in VI types, V and VI include both tibialplateau, medial and lateral, having worst post surgical results due to recovery time and residual sequels.This study will evaluate treatment election in Hospital San Juan de Dios and present epidemiological background with costarrican data that will be useful for future investigations and/or treatment protocols.Surgical results are assessed with patient satisfaction with the surgical process and post surgical functionality, because these variables have proven best effect in therapeutical intervention evaluation.

OBJECTIVE: Evaluate complication rates and functional outcomes of fibular neck osteotomy for posterolateral tibial plateau fractures. DESIGN: Retrospective case series. SETTING: University hospital. PATIENTS: From January 2013 to October 2014, 11 patients underwent transfibular approach for posterolateral fractures of the tibial plateau and were enrolled in the study. All patients who underwent transfibular approach were invited the return to the hospital for another clinical and imaging evaluation. INTERVENTION: Transfibular approach (fibular neck osteotomy) with open reduction and internal fixation for posterolateral fractures of the tibial plateau. MAIN OUTCOME MEASUREMENTS: Complications exclusively related to the transfibular approach: peroneal nerve palsy; knee instability; loss of reduction; nonunion and malunion of fibular osteotomy; and functional outcomes related to knee function. RESULTS: Two patients failed to follow-up and were excluded from the study. Of the 9 patients included in the study, no patients demonstrated evidence of a peroneal nerve palsy. One patient presented loss of fracture reduction and fixation of the fibular neck osteotomy, requiring revision screw fixation. There were no malunions of the fibular osteotomy. None of the patients demonstrated clinically detectable posterolateral instability of the knee following surgery. American Knee Society Score was good in 7 patients (77.8%), fair in 1 (11.1%), and poor in 1 (11.1%). American Knee Society Score/Function showed 80 points average (60-100, S.D:11). CONCLUSION: The transfibular approach for posterolateral fractures is safe and useful for visualizing posterolateral articular injury. The surgeon must gently protect the peroneal nerve during the entire procedure and fix the osteotomy with long screws to prevent loss of reduction. LEVEL OF EVIDENCE: Therapeutic level IV.
