RÉSUMÉ

The advent of new technology is breaking the boundaries of traditional teaching and learning patterns with virtual worlds (VW) creating new frontiers in education. Previous research has explored the use of VW within educational settings. However, limited studies have investigated the transition processes that educators experience by adopting VW based online tools during the COVID-19 pandemic. This qualitative exploratory study investigated 18 Chilean lecturers' teaching experiences using a three-dimensional computer-mediated environment: Second Life. Findings suggest that changing from traditional to virtual teaching context is a complex process, which (re)shaped the lecturers' various senses of identity and agency towards different instructional approaches resulting in the sense of in-betweenness with multiple digital competencies. These changes indicated that they taught in an 'in-between' mode mapped by different teaching mediations. The participants' teaching experiences of shaping a sense of in-betweenness could provide a unique theoretical lens to explore instructors' teaching experiences from traditional to a technology-mediated online setting.

RÉSUMÉ

OBJECTIVE: To evaluate the effectiveness of an online continuing education course on infant hearing health for primary care professionals. DESIGN: A prospective longitudinal study with interrupted time series pre-test/post-test design. The effectiveness of the online course was assessed by comparing pre- vs. post-training performance and analysing responses to evaluations of the quality of the course. STUDY SAMPLE: The sample comprised individuals enrolled between September 2018 and August 2019 in a Ministry of Health course, "Actions in primary care for the early identification of hearing impairment" offered on the AVASUS platform. RESULTS: Of the 2908 individuals registered, 1842 (63.3%) completed the course. Their ages ranged from 18 to 77 years, and 67.4% of them were females. Students and doctors were overrepresented among the enrollees. All Brazilian states were included in the sample. Comparisons of pre- and post-training performances showed a significant improvement in knowledge, and 99.3% of the participants provided positive comments regarding the quality of the course. CONCLUSIONS: The course is an effective distance learning tool on infant hearing health for primary care professionals. The online course circumvents the limitations posed by geographical barriers and also facilitates decentralisation.

RÉSUMÉ

In the post-truth era, one challenge facing science education is the circulation of fake news that distorts the information available for decision-making on issues that have a scientific basis and are controversial for society. In this work, we aimed at designing a learning environment with the objective of equipping students with skills that allow them to deal with socio-scientific issues (SSI) in an infodemic context. To this end, we proposed an educational innovation through design-based research, which was oriented to the treatment of information disseminated in the media and social networks related to COVID-19. We divided this information into four major constructs: virus and disease dynamics; pandemic and environmental crisis; hygiene and protocols; and vaccines, potential solutions, and pharmaceutical industry. On the basis of the activities of the didactic sequence, which included class discussion, interviews with the immediate environment, audiovisual productions, and a final plenary, we identified criteria that students applied to trust or not trust the circulating information and a series of strategies to corroborate the information. In addition, framing COVID-19 as an SSI allowed the discussion of curricular content in science and on sociocultural dimensions that cross the pandemic. Based on the implementation of the teaching-learning sequence, we conclude that the proposed activities favored reflection on critical thinking and awareness of the responsibilities they have as potential disseminators and/or generators of information.

RÉSUMÉ

The COVID-19 pandemic forced students and educators to shift from the traditional physical classrooms (face to face) to online platforms such as the Zoom to host live synchronous sessions. Although this video teleconferencing software facilitated communication between students and faculty around the world, technically the blank, silenced log-in screens removed the "connectedness" of the classroom. A medical educator plays music during the pre-class Zoom synchronous session as an approach to encourage a positive learning environment in a preclinical science course.

RÉSUMÉ

BACKGROUND: Attributes of physical learning spaces can facilitate or hinder learning. There are few studies about this topic in hospitals. The objective of this study was to explore the characteristics of physical learning spaces in a university hospital. METHODS: The setting was a large research-oriented public university hospital in Mexico City, affiliated with the National Autonomous University of Mexico. An intrinsic case study design was conducted with two instruments: a questionnaire to identify physical learning spaces and their attributes; the Learning Space Rating System (LSRS), an instrument used to evaluate spaces' characteristics that promote effective learning. RESULTS: 49 medical students and 60 internal medicine residents responded to the questionnaire. The attributes with the highest importance for students were: instructor physical availability, silence, comfort of the seating furniture, and Internet access. The study authors directly performed an evaluation of the learning spaces using the LSRS tool, the sections' scores were: clinical ward discussion room (74%), external consultation unit (71%), auditorium (70%), classroom (68%), library (66%), and the hospitalization room (61%). CONCLUSIONS: Physical learning spaces in medical training can be formally assessed to identify the attributes that students consider relevant for learning and provide needed information for redesign and reuse of spaces. Medical education scholars and trainees should be involved in the design and evaluation of university and hospital buildings.

RÉSUMÉ

Objectives: To investigate the most common sources of stress faced by medical students and the degree to which different stressors impact their lives. As a secondary objective, a new instrument, Medical Student Stress Factor Scale (MSSF), was developed based on these stressors. Methods: The MSSF was applied to 431 medical students. The Depression, Anxiety and Stress Scale was administered to investigate the relationship between these stressors and the students' mental health. Results: The five greatest stressors were the extensive content, lack of time to study, sleep deprivation, excessive self-pressure for good grades, and lack of leisure time. The MSSF showed good psychometric characteristics, resulting in a 28-item scale with five subscales. The Relationships and Health subscales were associated with depressive symptoms; Relationships, Health, and Learning Environment/Academic Performance were associated with anxiety symptoms, while Health, and Learning Environment/Academic Performance were associated with stress symptoms. Conclusions: Our study presents important data about the most important stressors that affect medical students' lives and cause mental health issues. The factors are generally related to academic performance and the learning environment and should be targeted in future interventions.

RÉSUMÉ

BACKGROUND: Home-based interventions have potential for improving early child development (ECD) in low-resource settings. The design of locally acceptable strategies requires an in-depth understanding of the household context. In this formative research study, we aimed to characterize the home play and learning environments of children 6-23 months of age from low-income households in peri-urban Lima, Peru. METHODS: Drawing on the developmental niche framework, we used quantitative and qualitative methods to understand children's physical and social settings, childcare practices, and caregiver perspectives. We conducted interviews, unstructured video-recorded observations, and spot-checks with 30 randomly selected caregiver-child dyads, 10 from each child age group of 6-11, 12-17, and 18-23 months of age, as well as key informant interviews with 12 daycare instructors. We analyzed the data for key trends and themes using Stata and ATLAS.ti and employed an adapted version of the Indicator of Parent-Child Interaction to evaluate the observations. RESULTS: Children's social settings were characterized by multi-generational homes and the presence of siblings and cousins as play partners. Access to books and complex hand-eye coordination toys (e.g., puzzles, building blocks) in the home was limited (30.0 and 40.0%, respectively). Caregivers generally demonstrated low or inconsistent levels of interaction with their children; they rarely communicated using descriptive language or introduced novel, stimulating activities during play. Reading and telling stories to children were uncommon, yet 93.3% of caregivers reported singing to children daily. On average, caregivers ascribed a high learning value to reading books and playing with electronic toys (rated 9.7 and 9.1 out of 10, respectively), and perceived playing with everyday objects in the home as less beneficial (rated 6.8/10). Daycare instructors reinforced the problems posed by limited caregiver-child interaction and supported the use of songs for promoting ECD. CONCLUSIONS: The features of the home learning environments highlighted here indicate several opportunities for intervention development to improve ECD. These include encouraging caregivers to communicate with children using full sentences and enhancing the use of everyday objects as toys. There is also great potential for leveraging song and music to encourage responsive caregiver-child interactions within the home setting.

RÉSUMÉ

BACKGROUND: Although learning environment (LE) is an important component of medical training, there are few instruments to investigate LE in Latin American and Brazilian medical schools. Therefore, this study aims to translate, adapt transculturally, and validate the Medical School Learning Environment Scale (MSLES) and the Johns Hopkins Learning Environment Scale (JHLES) to the Brazilian Portuguese language. METHOD: This study was carried out between June 2016 and October 2017. Both scales have been translated and cross-culturally adapted to Brazilian Portuguese Language and then back translated and approved by the original authors. A principal components analysis (PCA) was performed for both the MSLES and the JHLES. Test-retest reliability was assessed by comparing the first administration of the MSLES and the JHLES with a second administration 45 days later. Validity was assessed by comparing the MSLES and the JHLES with 2 overall LE perception questions; a sociodemographic questionnaire; and the Depression, Anxiety, and Stress Scale (DASS-21). RESULTS: A total of 248 out of 334 (74.2%) first- to third-year medical students from a Brazilian public university were included. Principal component analysis generated 4 factors for MSLES and 7 factors for JHLES. Both showed good reliability for the total scale (MSLES α = .809; JHLES α = .901), as well as for each subdomain. Concurrent and convergent validity were observed by the strong correlations found between both scale totals (r = 0.749), as well as with both general LE questions: recommend the school to a friend (MSLES: r = 0.321; JHLES: r = 0.457) and overall LE rating (MSLES: r = 0.505; JHLES: r = 0.579). The 45-day test-retest comparison resulted in a Pearson correlation coefficient of 0.697 for the JHLES and 0.757 for the MSLES. CONCLUSIONS: Reliability and validity have been demonstrated for both the MSLES and the JHLES. Thus, both represent feasible options for measuring LE in Brazilian medical students.

RÉSUMÉ

Nowadays, online or distance learning postgraduate schools program the academic period in different sequentially developed modules. This article assesses the impact of the programming of short-term university courses (30-60 days) on the frequency and quality of interaction when communication takes place through mobile technologies. Frequency corresponded to the messages generated by actors in learning environments, and quality was quantified using a scale of interaction levels operated by assessing the content of the messages issued by the actors. Results revealed that there is no correlation between interaction and academic performance in short-term courses. Additionally, a downward trend was observed in the number of individuals involved and in the frequency and quality of interaction throughout the course.

RÉSUMÉ

SUMMARY OBJECTIVE: We sought to understand the landscape of published articles regarding medical schools' learning environments (LE) worldwide, with an explicit focus on potentially harmful aspects of the LE as an effort to identify areas specifically in need of remediation or intervention that could prevent future unprofessional behaviors, burnout, violence and mistreatment among students and physicians. METHODS: A bibliometric analysis was conducted in six electronic databases (PubMed/Medline, Web of Science, Cochrane Library, SCOPUS, ERIC-ProQuest, and PsycINFO) up to December 31, 2016, including 12 themes: learning environment - general, hidden curriculum (harmful), unethical behaviors, bullying/hazing, violence, sexual discrimination, homophobia, racism, social discrimination, minorities discrimination, professional misconduct, and other negative aspects. RESULTS: Of the 9,338 articles found, 710 met the inclusion criteria. The most common themes were general LE (233 articles), unprofessional behaviors (91 articles), and sexual discrimination (80 articles). Approximately 80% of articles were published in the 21st century. CONCLUSION: There is a definite increase in scientific articles on negative aspects of the medical school LE in high-quality journals, especially in the 21st century. However, more studies are needed to investigate negative LE aspects with greater attention to experimental, longitudinal, and cross-cultural study designs.

This article was migrated. The article was marked as recommended. Introduction: The educational environment (EE) of medical education is the set of factors of a material and affective nature that permeates the tripod formed by the educational institution, by the teacher and by the student. A healthy environment results in quality learning and, consequently, a professional with a higher level of competence and satisfaction. Objectives: To analyze the educational environment of a medical school public university in Brazil in the view of its students and to investigate the presence of association between the sociodemographic variables of the students and their perceptions about the EE. Methods: A cross-sectional observational study with medical students from four different periods of the course, using the questionnaire DREEM and a sociodemographic questionnaire. Results: 210 students participated. Although DREEM's overall score shows that students have a more positive rather than negative view of EE in general, there was a large discrepancy in results among the groups surveyed, indicating that the more students progress in the medical course, the more negative your view on the educational environment. The perception about the general teaching environment and social relations was negative in the four groups. Female students, white, and those who lived outside Rio de Janeiro before attending in medical school perceived EE more negatively. The perception of EE by students who entered the system of racial or social quotas and those who did not join the quota system was similar. Conclusion: the analysis of the educational environment the students' dissatisfaction with the traditional Flexnerian models. Low-income, self-declared non-white and female students experience more problems and negative situations in the medical school. That teacher-centered teaching is an unfavorable factor in EE and discourages the progressive intellectual and professional autonomy advocated by the current National Curricular Guidelines of the Brazil.

The management of hospitals has been increasingly difficult. The problems are caused, mainly, by the role of financial resources, resulting in long lines to start treatments, lack of places in hospital beds, some increase in operating costs and restriction in physical space. In the hospital set, the cost of hospitalization is one of the largest, being appointed as one of the main responsible for the increment in medical and hospital that grow above the general inflation. Beginning form this difficulty, it is necessary to adopt a new method capable to eliminate waste and to generate value for the patient. The lean healthcare proved to be a capable method of solving problems in hospital institutions, being a methodology focused on generating of value for the client and, therefore, reducing the waste in the activities of the institutions. However, the managers find difficulties to implement it, because it is a very complex methodology that demands professionals who can complete the mastery of the subject in order to achieve the results. In this context, the technologies of information and communication are presented as a strategic resource in the teaching-learning process, being the virtual learning environment (VLE) one of the most important. This study has as aim to develop a prototype of a VLE that look on the steps for the implementation of the better tools and practices of the methodology of lean healthcare for the hospitalization sector in hospital institute. For the development of VLE, this research was based on the Institutional Design Model. As the first step, an integrative review of literature was fulfilled objecting to identify in the literature problems in the processes of the work of the hospitalization sector of hospital institutions, as well the tools used in the implementations of the lean healthcare methodology. Among the problems identified, stands out the length of stay of the patient, leading to an overload of hospital beds, delays in hospital discharge, and the errors during the care installment. From this result, the VLE prototype was developed and it is available at the electronic address: < https://diangelessilva.wixsite.com/website>. As a future proposal, experts in lean methodology, by computer experts, will evaluate the virtual environment and new demands for hospitalization related to lean healthcare will be identified and added
