RÉSUMÉ

Purpose: To compare best-corrected visual acuity (BCVA) before and after implantable collamer lens (ICL) surgery in patients with low, moderate and high myopia. Methods: A single-centre, prospective, registry-based study involving patients with myopia who received ICLs between October 2018 and August 2020. The study population was divided into three groups of subjects with low (from 0 to -6 D), moderate (-6 to -10 D) and high myopia (more than -10 D). We analysed uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), spherical equivalent (SE), differences between BCVA pre- and 1-month postoperative, and line gain in BCVA 1 month after surgery. Results: A total of 770 eyes belonging to 473 patients were surgically intervened during the study period, of which 692 eyes completed 1 month of postoperative follow-up and were included in the study population. At 1-month follow-up, 478 eyes (69%) achieved a BCVA of 20/20, 599 (87%) obtained 20/25 or better, and 663 (96%) had a BCVA of 20/40 or better. We identified a significant improvement in BCVA (baseline 0.15±0.2 logMAR vs follow-up 0.07±0.2 logMAR, p<0.0001) and a significant reduction in SE (baseline -9.23±4.1 D, vs follow-up -0.21±0.8 D, p<0.0001), with a significant relationship between preoperative SE and line gain (r = -0.46, p<0.0001). We identified a significantly higher line gain among eyes with higher degrees of myopia (low myopia 0.22±0.69 lines compared to moderate myopia 0.56±1.1 lines and high myopia 1.51±1.9 lines, p<0.0001). Notably, 99.6% of eyes with a high grade of myopia achieved improvement to a low degree (less than -6 D) at follow-up. The safety and efficacy indexes were 0.083±0.1 and -0.001±0.1, respectively. Conclusion: In this large patient cohort, ICL surgery was related to a significant line gain in BCVA, particularly among eyes with higher degrees of myopia.

AIM: To evaluate whether the Visian Implantable Collamer Lens with a central port (V4c ICL®; STAAR Surgical, Switzerland) affects the retinal nerve fibre layer (RNFL), macula and optic nerve head (ONH) measurements obtained by optical coherence tomography (OCT), and Heidelberg Retina Tomography (HRT). METHODS: This prospective study included myopic patients undergoing V4c ICL® implantation. RNFL thickness, macular thickness, ganglion cell analysis (GCA) and ONH main parameters were evaluated with RTVue OCT (Optovue Inc., USA) and Cirrus-HD OCT (Carl Zeiss Meditec, USA). ONH variables were also analysed with HRT-3 (Heidelberg Engineering, Germany). All measurements were performed before and 1 week and 12 months after the surgery. RESULTS: 31 eyes of 31 patients (mean age 30.1 ± 5.5 years) were included. Comparing with preoperative values, no significant differences in average RNFL thickness were found with RTVue, while a slight increase (4.3 µm) was detected with Cirrus-HD (85.2 ± 10.3 µm, preoperatively) at 1-week postoperatively (89.5 ± 8.3 µm; p < 0.05). Those changes were not observed at the last follow-up visit (86.6 ± 8.6 µm; p = 0.41). Cirrus-HD detected that macular thickness was slightly higher 1 week after surgery, compared with the preoperative examination (3.4% increase; p = 0.04). That difference remained stable at the 12-month postoperative visit (p = 0.01). GCA showed no changes. The ONH analysis with Cirrus-HD determined that rim area (p = 0.03) as well as disc area (p = 0.04) significantly increased. HRT-3 found no significant changes affecting those variables. CONCLUSIONS: The implantation of V4c ICL® did not induce a clinically significant impact on the results of the RNFL/ONH analysis with OCT and HRT.

INTRODUCTION: Fluctuating elevated intraocular pressure (IOP) is one of the complications of an implantable collamer lens (ICL), and its alteration is a predictive factor for the development of glaucoma. MATERIAL AND METHODS: A prospective, cross-sectional analytic study was conducted on patients suitable for ICL implantation. Complete clinical and biometric work-ups were performed, as well as night-time IOP curve, in supine position, with 4 determinations, in order to assess fluctuation, considering abnormal with a value higher than 5 mmHg. Patients underwent surgery with conventional technique and three months after the work-ups were repeated, including a night-time IOP curve to assess any changes in IOP fluctuations. RESULTS: A total of 31 eyes of 16 patients were studied. Mean IOP fluctuation in the preoperative assessment was 3.35 ± 2 mmHg, whereas the postoperative mean was 3.0 ± 2.2 mmHg, with the difference not being statistically significant. Visual acuity and capacity, as well as spheric equivalent did show a statistically significant improvement. There were 6 cases of complications, which were related to a higher vault and a greater ICL size. There was no relationship between these findings and the angle grade, pigment, and the level of training of the surgeon. CONCLUSIONS: The effect of an ICL on IOP fluctuations, has been studied for the first time, which was found to be not statistically significant. As in previous publications, the procedure was safe and reproducible, adding the fact that the level of training of the surgeon is not a determining factor in these findings.

La emergencia de las lentes intraoculares fáquicas en las pasadas décadas mostró una nueva alternativa para las ametropías extremas. Desde el año 1950 han sido diseñados diferentes modelos de lentes intraoculares fáquicas con distintos diseños y complicaciones. Se presenta un caso con antecedente de miopía elevada, quien posterior a la colocación de una lente intraocular fáquica de cámara anterior de fijación angular presentó como complicación en el posoperatorio abundante pigmentación iridiana sobre el lente y sinequias anteriores en 360 grados. Nos motivamos a publicar este caso y de esta forma proporcionar una actualización sobre las complicaciones de la colocación de estos lentes, así como ofrecer algunas recomendaciones para detectar a tiempo los posibles riesgos de este procedimiento(AU)


The emergence of the phakic intraocular lenses in the last decades showed a new alternative for the extreme ametropias. Since1950 different models of phakic intraoduclar lenses have been designed with different designs and complications. Here is a patient with a history of high myopia who, after having undergone the implantation of an anterior chamber phakic intraocular lens with angle fixation, presented with abundant iridian pigmentation over the lens and anterior sinechia at 360 degrees. The objectives of this report were to provide an update on the complications of the implantation of these lenses as well as making some recommendations to timely detect the possible risks of this procedure(AU)

Objetivo: determinar la variación que evidencia el endotelio corneal, en pacientes operados de alta miopía con la utilización de lentes intraoculares fáquicas de cámara anterior con soporte iridiano. Método: se realizó un estudio longitudinal, prospectivo y descriptivo en una muestra conformada por 26 ojos de 15 pacientes, operados de alta miopía con la utilización de lentes intraoculares fáquicas Artisan y Artiflex de cámara anterior con soporte iridiano, que se atendieron en el Servicio de Córnea y Cirugía Refractiva del Instituto Cubano de Oftalmología Ramón Pando Ferrer de septiembre de 2009 a mayo de 2010. Resultados: se evidenció una pérdida celular endotelial posterior a la cirugía seis meses de 6,9 por ciento. En el posoperatorio la hexagonalidad y el coeficiente de variación disminuyeron, mientras que la paquimetría no mostró diferencias significativas. Conclusiones: en el presente estudio se observó la presencia de un daño mínimo en el endotelio corneal con el uso de lentes fáquicas de cámara anterior, en la corrección quirúrgica de las altas miópias


Objective: to determine the variation of the corneal endothelium in those patients operated on from high myopia, with the use of iris claw anterior chamber phakic intraocular lenses. Methods: a prospective, longitudinal and descriptive study was carried out in a sample of 26 eyes from 15 patients. They had been operated on from high myopia with the use of iris claw anterior chamber phakic intraocular lenses Artisan and Artiflex at the Cornea and Refractive Surgery Service of Ramon Pando Ferrer Cuban Institute of Ophthalmology from September 2009 through May 2010. Results: it was evinced that there was endothelial cell loss after surgery six months of 6.9 percent. In the postoperative phase, hexagonality and variation coefficient diminished whereas pakimetry did not show significant differences. Conclusions: the present study indicated the existence of minimal damage in the corneal endothelium with the use of the anterior chamber phakic lenses for the surgical correction of high myopias

OBJETIVO: Determinar la efectividad de los lentes intraoculares fáquicos Artisan en la corrección de la alta miopía en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer" en el período de enero del 2006 a enero del 2009. MÉTODOS: Se realizó un estudio prospectivo, descriptivo. El universo estuvo constituido por el total de ojos (120) a los cuales se les implantó estos lentes. RESULTADOS: Se encontró un componente esférico de 0,5, no astigmatismo inducido y una mejoría de la agudeza visual corregida de 0,9. No se presentaron complicaciones transoperatorias y entre las posoperatorias mediatas predominó la iritis o uveítis (3,4 por ciento) así como la atrofia iridiana (5 por ciento) entre las tardías. CONCLUSIONES: Los resultados refractivos fueron favorables. Predominaron los pacientes sin complicaciones.

METHODS: A prospective descriptive study was made in which the universe of study was 120 eyes implanted with this type of lenses. RESULTS: Spheral component of 0.5; the induced astigmatism was not found and the corrected visual acuity improvement was 0.9. There were no transoperative complications; the predominant postoperative mediate complications were iritis or uveitis (3.4 percent) and iridic atrophy (5 percent) prevailed in the late ones. CONCLUSIONS: The refractive results were favourable. The uncomplicated patients predominated.

RESULTS: The achieved refractive outcomes in patients were the following: spheral component was 0,25; no case of induced astigmatism, and the improvement of corrected visual acuity was 0,9. There were not transoperative complications whereas the predominant complication in the postoperative phase was iritis or uveitis (40 percent) between the inmediate and mediate postoperative periods. Identificar los efectos sobre el endotelio corneal al implantar lentes intraoculares fáquicas Artisan en la corrección de la alta miopía en el Instituto Cubano de Oftalmología Ramón Pando Ferrer en el período de enero de 2006 a julio de 2007. Se realizó un estudio prospectivo, descriptivo. El universo estuvo constituido por 17 ojos, a los cuales se les implantó lente intraocular fáquica Artisan para la corrección de la alta miopía. La pérdida celular endotelial fue de 4,6 por ciento y no hubo variaciones significativas en el coeficiente de variación y la hexagonalidad. La pérdida celular endotelial fue baja al año de la cirugía. El coeficiente de variación y la hexagonalidad no mostraron cambios significativos.


To identify the effects on the corneal endothelium of Artisan phakic intraocular lens implantation for the high myopia correction at Ramón Pando Ferrer Cuban Institute of Ophthalmology from January 2006 to July 2007. A prospective descriptive study was performed on 17 eyes which underwent Artisan phakic intraocular lens implantation for high myopia correction. Endotelial cell loss was 4,6 percent and no significant variations were found in variation coefficient and hexagonality. Endotelial cell loss was low after one year of surgery. The variation coefficient and hexagonality did not show any significant changes.
