RÉSUMÉ

BACKGROUND: The occurrence of an orofacial trauma can originate health, social, economic and professional problems. A 13-year boy suffered the avulsion of tooth 11 and 21, lost at the scenario. METHODS: Three intraoral appliances were manufactured: A Hawley appliance with a central expansion screw and two central incisors (1), trumpet edentulous anterior tooth appliance (2) and a customized splint (3) were designed as part of the rehabilitation procedure. Objectively assessing the sound quality of the trumpet player with these new devices in terms of its spectral, temporal, and spectro-temporal audio properties. A linear frequency response microphone was adopted for precision measurement of pitch, loudness, and timbre descriptors. RESULTS: Pitch deviations may result from the different intra-oral appliances due to the alteration of the mouth cavity, respectively, the area occupied and modification/interaction with the anatomy. This investigation supports the findings that the intra-oral appliance which occupies less volume is the best solution in terms of sound quality. CONCLUSIONS: Young wind instrumentalists should have dental impressions of their teeth made, so their dentist has the most reliable anatomy of the natural teeth in case of an orofacial trauma. Likewise, the registration of their sound quality should be done regularly to have standard parameters for comparison.

RÉSUMÉ

Severely traumatized incisors in young adults are a very common problem, and one that is difficult to treat because healing of these teeth is not always predictable and implant placement has to be delayed until adulthood. This case report of a horizontal root fracture in an avulsed central incisor illustrates the ability to maintain crucial tooth structure. A lateral socket augmentation procedure after extraction of a root fragment combined with an endodontic and regenerative periodontal treatment approach could preserve natural esthetics and function, which are directly related to quality of life. The preservation of alveolar bone volume following partial root removal will eventually facilitate later placement of a dental implant with an improved esthetic and functional prosthodontic result in a patient with a high-risk esthetic profile.

Traumatic injuries to anterior teeth in children range from minor chipping to total tooth loss and occur more often in boys than in girls. The treatment of permanent tooth loss in younger patients is complicated by the difficulty of doing restorative procedures which may be influenced by pulpal size, clinical crown height, and a dental profile which is constantly changing. This case report presented a situation involving a 9-year-old boy who had experienced traumatic loss of the maxillary central incisors. Innovative materials coupled with sound principles of removable partial denture design were utilized to fabricate a cast metal removable denture prosthesis to satisfy the esthetic, functional and psychological dental needs of the patient and his parents.

A 9-year-old boy presented with a missing upper front tooth after an accident involving kite flying. Diagnosis of avulsion of the upper right lateral incisor (22) was made after examination of his socket as well as by radiological investigations. A removable partial denture was given for the aesthetic purpose. The patient is under follow-up. The present case illustrates the management of avulsion.

Orthodontics is always the first choice treatment for aesthetic and functional rehabilitation of malaligned teeth but in some exceptionally rare cases, the unwillingness of the patient for orthodontic correction poses a tough challenge to the dentist. In such cases, the only other option for aesthetic restoration is the change in angulation with custom cast post and core followed by the placement of an aesthetic crown.

This case report describes the orthodontic treatment of a girl who lost her maxillary left central incisor from trauma. The patient had a concave profile, a Class I molar relationship on the right side and a Class II molar relationship on the left side, with a slight maxillary retrusion, an anterior edge-to-edge bite, and a posterior crossbite. Therefore, the treatment consisted of rapid palatal expansion to widen the maxilla in a transverse direction along with reverse headgear to improve the incisor relationship. The crown of the left canine was modified to resemble the left lateral incisor, and a porcelain veneer was placed to make the left lateral incisor resemble the left central incisor. This case report illustrates how orthodontics alone can be used to treat a missing central incisor, without a bridge or an implant.

Functional and esthetic results can improve significantly when a combined prosthodontic-orthodontic treatment approach is employed in cases requiring extensive oral rehabilitation. The patient presented in this case report was treated in his late teens with dental implants as a replacement for his maxillary incisors. Ten years later, the entire maxillary anterior segment was in infraocclusion compared to the rest of the dentition and lip line. Since prosthodontic follow-up treatment alone could not achieve an optimal functional and esthetic outcome, the patient was treated orthodontically prior to renewing the restoration. A fixed appliance was used to intrude the mandibular anterior teeth as well as vertically align the infrapositioned maxillary lateral incisors.

INTRODUCTION: This article reports the use of an orthodontic mini-implant for a temporary crown restoration in a small edentulous space after limited orthodontic treatment. METHODS: Two clinical cases are presented: a 23-year-old woman and a 14-year-old boy. In the adult patient, a 2-piece orthodontic C-implant (Cimplant, Seoul, Korea) was placed in a 3-mm wide edentulous space to build up a temporary crown restoration after a short orthodontic treatment to regain space for a missing mandibular right permanent lateral incisor. In the boy, a C-implant was placed in the space resulting from an avulsed maxillary right permanent lateral incisor to prevent aggressive alveolar bone resorption after dental trauma. Both patients were followed for more than 4 years of retention to evaluate the stability of the temporary crown restoration built up on the orthodontic mini-implants. RESULTS: Both patients had successful long-term results, confirmed by clinical and radiographic examinations. Both were pleased with the results and plan to retain the orthodontic mini-implant temporary crown restoration until they are ready for a permanent restoration later. CONCLUSIONS: A 2-piece orthodontic C-implant system can be used to maintain edentulous space after active orthodontic treatment.

A history of traumatic dental injury to the maxillary central incisors during preadolescence or adolescence is common and may result in premature loss. Treatment options include prosthetic implant replacement, autotransplantation, and orthodontic space closure with direct composite recontouring. This case report describes the treatment of an adolescent girl who presented with a crowded Class I malocclusion complicated by a history of trauma to the maxillary central incisors. The treatment plan consisted of orthodontic space closure following loss of both maxillary central incisors and mandibular premolars. This case highlights that orthodontic space closure can be a valuable treatment option in selected Class I crowded and Class II uncrowded malocclusions, producing predictable and efficient results.

Significant maxillary anterior osseous defects are considered contraindications for fixed partial dentures. This clinical report discusses the surgical and restorative treatment protocol of a patient who sustained trauma to the premaxilla and was treated by distraction osteogenesis to provide an adequate restorative platform for an implant-retained fixed prosthesis.

Las causas de los traumatismos dentales son de naturaleza compleja y están influenciadas por diferentes factores, incluyendo la biología humana, comportamiento y el medio ambiente. Uno de los traumatismos dentales que produce gran consternación en el paciente afectado y en sus familiares es la avulsión o desarticulación dentaria que es el desplazamiento total del diente para fuera del alvéolo. Existen casos donde el espacio se cierra mesiodistalmente y es necesario un tratamiento ortodóncico previo a cualquier tratamiento rehabilitador con implantes. Se presentan los casos de dos pacientes femeninas tratadas en la Consulta de Implantología de la Clínica Estomatológica Ana Betancourt del municipio Playa. Las mismas se encontraban muy afectadas psicológicamente por presentar ausencia clínica de sus dientes. En el Caso 1 los había avulsionados en un accidente automovilístico. Los dientes ausentes eran el 22, 24, 25,14 y 15 con ausencia de espacio en zona del 22 y portadora de una prótesis parcial acrílica superior. El Caso 2 tenía ausente el 21 por un trauma a los 9 años durante la Educación Física, con pérdida también del espacio. Pasado 6 meses de tratamiento ortodóncico y recuperado dicho espacio, se colocaron implantes Kos en las zonas desdentadas y se rehabilitaron en la misma jornada de trabajo con prótesis fija implantosoportada provisionales; se logró así una excelente estética, ajuste seguro, función masticatoria adecuada y larga vida en cuanto a su duración en boca(AU)


The causes of dental trauma are complex in nature and are influenced by various factors, including human biology, behavior and environment. One of dental trauma producing great consternation in the affected patient and their family is avulsed tooth or disarticulation is the total displacement of tooth out of socket. There are cases where space is closed mesiodistally and orthodontic treatment is necessary before any rehabilitation treatment with implants. We report the cases of two female patients treated at the clinic Implantology Dental Clinic Ana Betancourt Playa municipality. They were very affected psychologically by presenting clinical absence of teeth. In case # 1 had avulsed in a car accident. The missing teeth were 22, 24, 25.14 and 15 with no space in area 22 and carry a higher acrylic partial denture. The case # 2 was absent on 21 by trauma during physical education, so from age 9 was a carrier of an upper acrylic partial denture. After 6 months of orthodontic treatment and recovered space Kos implants were placed in edentulous areas and rehabilitated in the same working day with implant fixed provisional prosthesis, thus achieving an excellent aesthetic, secure fit, proper chewing function and longevity in its duration in the mouth(AU)

Premature tooth loss in children may consist of single or multiple, primary or permanent, and anterior or posterior units of the dentition. This tooth loss may be due to either trauma or caries and, in some cases, to congenital or genetic defects. With anterior tooth loss cases, there are several problems the dental practitioner must consider. These are space maintenance, masticatory function, speech and esthetic appearance.

Autotransplantation is a safe and currently common procedure in cases of agenesis and tooth loss. Autotransplantation of premolars to replace missing maxillary central incisors has been shown to be an excellent treatment choice, especially in young patients in the 9-12-year-old age range. The choice for adequate restorative materials and techniques is also essential, direct composite resin being indicated in most cases. This paper reports the case of two premolars autotransplanted to replace both maxillary central incisors and addresses the esthetic issues involved in the reshaping of these teeth to incisor morphology.

This clinical report describes the indirect restoration of anterior teeth in a young patient after a severe dental trauma using all-ceramic coronal posts. Step-by-step clinical procedures and their rationale are described. The learning objective of this case report is to outline the principles for the management of traumatic injuries of permanent anterior teeth in young patients to re-establish function and provide good esthetic results.

A 16 year-old female patient was introduced to the Department of Restorative Dentistry after having completed orthodontic treatment. She suffered from the missing of the lateral incisivi. Aim of the treatment was to replace the missing teeth of the growing patient with atraumatic and minimal-invasive technique. With a pre-impregnated fibreglass ribbon and with the aid of a special matrix technique the teeth were replaced as fibreglass ribbon resin-bonded bridges with a direct laminating technique using an "Esthetic Composite", including opakdentine, dentine and enamel compounds as well as special compounds for effects like lustre. An esthetically satisfying result could be achieved with the tooth-preserving methods available at the moment.

The prevalence of traumatisms with avulsion of deciduous teeth varies from 7 to 13%, usually involving one tooth only. In this case report, a trauma with multiple losses of deciduous teeth and laceration of soft tissue is described as a consequence of a horse backward kick. After suture of extra-oral tissue wounds and application of anti-tetanic vaccine, the patient was referred to the emergency ambulatory of Sacred Heart University. Treatment procedures included radiographic analysis, removal of bone fragments and of a residual root of the tooth involved, remodeling and suture of intra-oral tissues. The patient was observed periodically and after full recovery, a prosthetic appliance was installed for functional and esthetic rehabilitation.

Avulsion is a complex injury affecting the pulp, periodontal ligament and the alveolar bone. Avulsed permanent teeth can survive following replantation. However, post-traumatic external root resorption eventually resulting in loss of the traumatized tooth is a frequent finding. After replantation of the avulsed teeth, esthetic requirements can be needed. Treatment options include porcelain laminate veneers, metal-ceramic restorations and all-ceramic crowns as well as minimally invasive procedures such as direct resin composite bonding. This article describes the restoration of avulsed and replanted teeth with direct resin composite laminate veneers. Because of the fact that lost fragments were recovered with the loss of anterior esthetic, and bearing in mind the patient's psychologically affected, we considered direct resin composite laminate veneer restoration of the avulsed and replanted teeth as the best therapeutic option. The patient was satisfied with the final result.

UNLABELLED: This article describes the restoration of an intruded root using a custom-made metal dowel and metal-ceramic veneer core restoration. The treatment plan for this patient consisted of restoring the missing esthetics and eliminating psychological trauma by utilizing the root of the intruded maxillary left central incisor to replace both missing central incisor crowns. This treatment will preserve space and bone until the patient is old enough for another prosthodontic restoration to be considered. CLINICAL SIGNIFICANCE: As the lost fragments were not recovered, we considered this restoration of the intruded root to be the best therapeutic option, considering the effect on the patient's psyche. The patient was satisfied with the final result.

This case report describes the multidisciplinary management of an adolescent girl with traumatic loss of the maxillary right central lateral incisors, and a unilateral Class II crossbite malocclusion. A rapid palatal expansion appliance was used initially to correct the transverse deficiency and secondarily as an anchorage unit for unilateral molar distalization to correct the Class II molar relationship. Temporary incisors were extended from a Nance stabilizing appliance during premolar and canine retraction. A staged approach was used with bone grafting before the placement of 2 single-tooth implants. A favorable outcome was achieved through proper coordination among the orthodontist, the surgeon, and the restorative dentist.

Avaliar o nível de informação de estudantes, comidade média de 12 anos, seus pais e professores, de escolas públicas e particulares, quanto à conduta de urgência frente ao traumatismo dental com avulsão. Método: A amostra foi constituída de 990 indivíduos, aos quais foi aplicado um questionário fechado, devidamente orientado, que, após análise e avaliação por dois endodontistas e uma patologista bucal, foram classificados em quatro níveis alto, razoável,baixo e muito baixo. Resultados: Do total de entrevistados,apenas 6,77 por cento apresentaram um alto nível de conhecimento;16,26 por cento apresentaram um nível razoável; 51,13 por cento um nível baixo, enquanto 23,84 por cento apresentaram um nível deconhecimento muito baixo. Não foi estabelecida uma correlação significativa entre escolas públicas e particulares.Conclusão: É baixo o nível de conhecimento sobre a condutade urgência frente ao traumatismo dental, tornando necessária a inclusão do tema nos programas de prevenção à saúde bucal.
