The objective of this systematic review (SR) with meta-analysis (MA) was to identify the dietary patterns of the population, regarding ethnicity and gender, and their association with the metabolic syndrome and its risk factors (MetS-RF). The literature search was performed using Medline, Scopus, Ebsco, SciELO, and BVS databases. Studies with adult participants that identified dietary patterns associated with MetS-RF were included. Pooled odds ratio (OR) and 95%CI were calculated using a random-effect, generic inverse variance method. Statistical heterogeneity and publication bias were explored. The dietary patterns were classified as healthy or unhealthy. Studies were categorized into three groups: Women (all ethnicities), Afro-descendant (men and women), and General Population (both genders and ethnicity). Among the articles found (n=8,496), 22 integrated the SR and 11 the MA. The adherence to the healthy dietary pattern was negatively associated (protective factor) with MetS-RF only in the General Population (OR=0.77; 95%CI: 0.61-0.98). Nevertheless, the unhealthy dietary pattern was associated with the higher prevalence of MetS-RF in all analyzed groups. It was concluded that an unhealthy eating pattern increases the chances of SM-RF in adults, regardless of gender and ethnicity.

BACKGROUND: Identifying factors associated with post-disaster youth substance use is a crucial element of developing evidence-based prevention and intervention efforts. Hurricane María struck Puerto Rico in September of 2017 and the wide-spread impact from this disaster, including exposure to trauma, displacement, and disrupted social supports had the potential to negatively impact levels of substance use among youth across the archipelago. However, post-disaster substance use remains under-investigated in this context. The current study sought to identify risk and protective factors associated with substance use among Puerto Rican youth in the aftermath of Hurricane Maria. METHODS: Cross-sectional, secondary data analyses were conducted using school-based survey data collected at all schools in Puerto Rico between February 1 and June 29, 2018 (5-9 months after Hurricane María). Social supports, substance use, and trauma symptoms were assessed. An ordinal regression analysis was conducted to identify student factors associated with greater likelihood of post-disaster substance use. RESULTS: A total of 36,485 participants (50.7% female, grades 7-12), were included in an ordinal regression analysis that compared the likelihood of respondents endorsing high, low, or no substance use after Hurricane María based on reported adult social support, counselor/teacher social support, peer social support, ptsd symptomatology, and gender. Findings showed that, when compared to students that endorsed low or no substance use, those who reported having adult social support demonstrated a 58% reduction in odds (OR = 0.42, 95% CI: 0.34-0.53) of reporting high substance use after Hurricane María, while students who reported having teacher/counselor social support demonstrated a 21% reduction in odds (OR = 0.79, 95% CI: 0.69-0.89) of reporting high substance use. Additionally, those that reported having peer social support demonstrated a 31% increase in odds (OR = 1.31, 95% CI: 1.10 to 1.58) of reporting higher substance use, compared to those that reported low or no substance use. CONCLUSIONS: While social support was generally protective, prevention efforts to build positive family and community connections may be indicated. Evidence-based school screenings of substance use and trauma may help direct intervention to those most at risk for co-occurring issues.

BACKGROUND AND AIMS: Breakfast consumption could have a synchronizer role in chronobiological functions. Across observational studies, the assessment of breakfast frequency consumption is heterogeneous, therefore consensus on the relation between of weekly frequency of breakfast consumption and the risk of diabetes is unclear. We examined the relation between weekly breakfast frequency consumption and the incidence of diabetes in middle-age women. METHODS AND RESULTS: Since baseline (2006-2008) we prospectively followed 71,373 women from the Mexican Teachers' Cohort. Participants were classified according to breakfast consumption frequency of 0, 1-3, 4-6, or 7 days/week. Diabetes was identified by self-report and clinical-administrative databases. We used Cox proportional hazards multivariable models to estimate hazard ratios (HR) and 95% confidence intervals (CI) for breakfast frequency and diabetes adjusting for covariates. Stratified analyses were performed for age, birth weight, ethnicity, and physical activity. We identified 3613 new diabetes cases between baseline and 2014. The prevalence of daily breakfast consumers was 25%. The median follow-up was 2.2 years, interquartile range 1.8-3.8 years. Relative to women who skipped breakfast, those who consumed breakfast every day had a 12% lower risk of diabetes (multivariable HR = 0.88; 95% CI 0.78, 0.99; p-trend = 0.0018). One additional day per week of breakfast was associated with a lower risk of diabetes (HR = 0.98; 95% CI 0.97, 0.99). In stratified analysis, the observed inverse relation appeared to be stronger in women aged ≥40 years and in indigenous women. CONCLUSIONS: Breakfast frequency was inversely associated with the incidence of diabetes independently of lifestyle factors. Regular breakfast consumption may be a potential component of diabetes prevention.

BACKGROUND: Patterns of cognitive change and modifiable factors for cognitive decline versus stable cognitive trajectories have rarely been described in lower-educated older adults. OBJECTIVES: We aimed to identify long-term trajectories of cognitive functioning and possible factors associated with cognitive decline. DESIGN AND PARTICIPANTS: We used data from 1,042 adults aged ≥ 60 participating in the Health, Welfare and Aging Study (SABE), São Paulo, Brazil, without cognitive impairment at baseline. Data were collected across four waves (2000-2015). Group-based trajectory modelling was used to identify cognitive trajectories. Associations with socioeconomic variables, childhood background, lifestyle, and cardiovascular risk factors were explored using weighted multinomial logistic regressions. MEASUREMENTS: The abbreviated Mini-Mental State Examination was used to measure cognition. RESULTS: Three cognitive trajectories were identified: stable (n= 754, 68.6%), mild-decline (n= 183, 20.8%), and strong-decline (n= 105, 10.7%). At baseline, respondents in the strong-decline group were more likely to be older than those with stable and mild-decline trajectories. Furthermore, participants in both the mild and strong-decline groups were more likely to have no schooling, be divorced/separated, receive less than 4 monthly wages, and be underweight (BMI < 18.5) compared to the stable group. Finally, the mild-decline group was more likely to have lived in rural areas during childhood than participants located in a stable trajectory. CONCLUSIONS: Our findings suggest that interventions to reduce cognitive decline for low-educated older adults might include strategies addressing inequalities and improving modifiable risk factor burden.

This study aimed to test hypothesized effects of replacing sedentary behavior with moderate-to-vigorous physical activity, sleep, and different domains of physical activity by equivalent amounts on suggestive cognitive decline in an older adult population. This was a cross-sectional study including 473 older adults aged ≥ 60 years. Cognitive decline was assessed using the Mini-Mental Health Examination. Physical activity, its different domains and the time of exposure to sedentary behavior were assessed using the International Physical Activity Questionnaire. For data analysis, two isotemporal substitution models were constructed using Poisson regression. The first model tested the effect of sleep time, sedentary behavior, and moderate-to-vigorous physical activity on cognitive decline. The second model was used to determine the effect of physical activity domains (leisure, work, transport, and home), sleep time, and sedentary behavior on cognitive decline. Physical activity during leisure time was protective against cognitive decline among all domains tested, replacing sedentary behavior, sleep, and transport. Conversely, substitution of the leisure domain for sedentary behavior, sleep, and transport was considered a risk factor for cognitive decline. Leisure time proved to be a strong protective factor in reducing the risk of cognitive decline, and it is necessary to encourage and stimulate public policies that include it.

BACKGROUND: Olfactory dysfunction (OD) represents a frequent manifestation of the coronavirus disease 2019 (COVID-19). Apolipoprotein E (APOE) is a protein that interacts with the angiotensin-converting enzyme receptor, essential for viral entry into the cell. Previous publications have suggested a possible role of APOE in COVID-19 severity. As far as we know, no publications found significant associations between this disease's severity, OD, and APOE polymorphisms (E2, E3, and E4). OBJECTIVE: To analyze the epidemiology of OD and its relationship with APOE polymorphisms in a cohort of Long-COVID patients. METHODS: We conducted a prospective cohort study with patients followed in a post-COVID neurological outpatient clinic, with OD being defined as a subjective reduction of olfactory function after infection, and persistent OD being defined when the complaint lasted more than 3 months after the COVID-19 infection resolution. This cross-sectional study is part of a large research with previously reported data focusing on the cognitive performance of our sample. RESULTS: The final sample comprised 221 patients, among whom 186 collected blood samples for APOE genotyping. The persistent OD group was younger and had a lower hospitalization rate during the acute phase of the disease (p < 0.001). Furthermore, the APOE variant E4 allele frequency was lower in this group (p = 0.035). This study evaluated OD in an outpatient population with COVID-19. In the current literature on this disease, anosmia is associated with better clinical outcomes and the E4 allele is associated with worse outcomes. CONCLUSION: Our study provides new information to these correlations, suggesting APOE E4 as a protective factor for OD.


ANTECEDENTES: A disfunção olfatória (DO) é uma manifestação frequente da doença do coronavírus 2019 (COVID-19). A apolipoproteína E (APOE) é uma proteína que interage com o receptor da enzima conversora de angiotensina, essencial para a entrada viral na célula. Publicações anteriores sugeriram um possível papel da APOE na gravidade da COVID-19. Até onde sabemos, nenhuma publicação encontrou associações significativas entre a gravidade dessa doença, DO e polimorfismos da APOE (E2, E3 e E4). OBJETIVO: Analisar a epidemiologia da DO e sua relação com os polimorfismos do gene APOE em uma coorte de pacientes com COVID longa. MéTODOS: Um estudo de coorte prospectiva com pacientes acompanhados em ambulatório neurológico pós-COVID, com DO sendo definida como uma redução subjetiva da função olfativa após a infecção e a DO persistente sendo definida quando a queixa durou mais de 3 meses após a resolução da infecção por COVID-19. Este estudo transversal é parte de uma pesquisa maior com dados anteriormente relatados, focando na performance cognitiva dos pacientes. RESULTADOS: Foram selecionados 221 pacientes para esse estudo, dos quais 186 haviam coletado amostras de sangue para genotipagem APOE. O grupo DO persistente foi mais jovem e apresentou menor taxa de internação na fase aguda da doença (p < 0,001). Além disso, a frequência do alelo E4 da APOE foi menor nesse grupo (p = 0,035). Este estudo avaliou a DO em uma população com COVID longa. Na literatura atual sobre essa doença, a anosmia está associada a melhores desfechos clínicos e o alelo E4 está associado a piores desfechos. CONCLUSãO: Nosso estudo acrescenta novas informações a essas correlações, sugerindo a APOE E4 como um fator de proteção para DO.

Background: Patients in serious condition due to COVID-19 often require special care in intensive care units (ICUs). This disease has affected over 758 million people and resulted in 6.8 million deaths worldwide. Additionally, the progression of the disease may vary from individual to individual, that is, it is essential to identify the clinical parameters that indicate a good prognosis for the patient. Machine learning (ML) algorithms have been used for analyzing complex medical data and identifying prognostic indicators. However, there is still an urgent need for a model to elucidate the predictors related to patient outcomes. Therefore, this research aimed to verify, through ML, the variables involved in the discharge of patients admitted to the ICU due to COVID-19. Methods: In this study, 126 variables were collected with information on demography, hospital length stay and outcome, chronic diseases and tumors, comorbidities and risk factors, complications and adverse events, health care, and vital indicators of patients admitted to an ICU in southern Brazil. These variables were filtered and then selected by a ML algorithm known as decision trees to identify the optimal set of variables for predicting patient discharge using logistic regression. Finally, a confusion matrix was performed to evaluate the model's performance for the selected variables. Results: Of the 532 patients evaluated, 180 were discharged: female (16.92%), with a central venous catheter (23.68%), with a bladder catheter (26.13%), and with an average of 8.46- and 23.65-days using bladder catheter and submitted to mechanical ventilation, respectively. In addition, the chances of discharge increase by 14% for each additional day in the hospital, by 136% for female patients, 716% when there is no bladder catheter, and 737% when no central venous catheter is used. However, the chances of discharge decrease by 3% for each additional year of age and by 9% for each other day of mechanical ventilation. The performance of the training data presented a balanced accuracy of 0.81, sensitivity of 0.74, specificity of 0.88, and the kappa value was 0.64. The test performance had a balanced accuracy of 0.85, sensitivity 0.75, specificity 0.95, and kappa value of 0.73. The McNemar test found that there were no significant differences in the error rates in the training and test data, suggesting good classification. This work showed that female, the absence of a central venous catheter and bladder catheter, shorter mechanical ventilation, and bladder catheter duration were associated with a greater chance of hospital discharge. These results may help develop measures that lead to a good prognosis for the patient.

Ocular toxoplasmosis (OT) is characterised by intraocular inflammation due to Toxoplasma gondii infection. Studies have found that interleukin 17 (IL-17) plays a central role in the pathology of OT. However, nucleotide variability in IL17 and interleukin 17 receptor (IL17R) genes has not been characterised in OT. As cytokine gene polymorphisms may influence the expression of these molecules, the aim of this study was to verify whether IL17A (rs2275913), IL17F (rs763780), IL17RA (rs4819554) and IL17RC (rs708567) polymorphisms are associated with OT in a Brazilian population. This study enrolled 214 patients seropositive for T. gondii (110 with OT and 104 without) and 107 controls. Polymorphisms were identified by PCR-restriction fragment length polymorphism analysis, validated by DNA sequencing with chi-square and multivariate analyses being used to assess possible associations between polymorphisms and OT. Logistic regression under the dominant model revealed a protection factor against OT of the C mutant allele of the IL17F (rs763780) polymorphism. The T/C-C/C genotypes were significantly more common in patients without OT compared to those with OT (p value = 0.0066) and controls (p value = 0.014). Findings from this study suggest that the IL17F polymorphism may have an influence in the immunopathology of OT in Brazilian individuals.

Several potential risk factors have been identified in the etiopathogenesis of febrile seizures (FS), including the type and extent of breastfeeding (BF). Given the lack of conclusive data, this study aims to systematically evaluate the evidence on the association between BF and FS. We conducted a systematic review and meta-analysis according to PRISMA guidelines. The search was conducted using descriptors for FS, BF, and formula feeding in MEDLINE, Embase, and Web of Science databases. We included observational studies that compared the incidence of FS between those who had ever breastfed and those who were formula fed. The study protocol was registered on the PROSPERO platform under the number CRD42023474906. A total of 1,893,079 participants from 8 datasets were included. Our main analysis showed no significant association of any type of BF on the incidence of FS compared with formula-fed children (OR: 0.84; CI: 0.67-1.04; I2 = 78%; Cochran's Q = 0.0001), although meta-regression showed that BF was associated with a lower incidence of FS in preterm infants. Our secondary outcome showed a significantly reduced incidence of FS in children who received BF exclusively (OR: 0.80; CI: 0.65-0.99; I2 = 70%; Cochran's Q = 0.02).    Conclusion: There was no significant reduction in the incidence of FS in those who were breastfed compared to formula feeding. However, our meta-regression analysis indicated an association between BF and a lower incidence of FS in preterm infants. Additionally, children who exclusively received BF had a significantly reduced incidence of FS. These findings should be further investigated in prospective cohorts. What is Known: • Breastfeeding can modify risk factors for febrile seizures, such as susceptibility to viral and bacterial infections, micronutrient deficiencies, and low birth weight. • However, studies have shown conflicting results regarding the impact of breastfeeding on febrile seizures. What is New: • When comparing any breastfeeding pattern with no breastfeeding, there is no significant difference in the incidence of febrile seizures. • When comparing exclusive breastfeeding with no breastfeeding, there may be a decrease in the occurrence of febrile seizures.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is responsible for causing coronavirus disease 2019 (COVID-19). The development and severity of this infectious disease is influenced by a combination of environmental and genetic factors. Angiotensin-converting enzyme 2 (ACE2) facilitates SARS-CoV-2 entry into human cells, with transmembrane serine protease 2 (TMPRSS2) playing a crucial role in S protein priming. Other proteases, such as cathepsin L and elastase, neutrophil-expressed (ELANE), have the capability to prime the S protein and contribute to SARS-CoV-2 infection. ELANE variants have not been previously examined in COVID-19 patients. We aimed to assess the association of single nucleotide variants (SNVs) within ELANE with COVID-19 and biochemical markers. The study included 319 SARS-CoV-2-infected patients and 288 controls. Genotyping of ELANE rs17216663C/T (Pro257Leu), rs17223045C/T (As1n30Asn), and rs3761007G/A was conducted using a 5'-nuclease allelic discrimination assay (TaqMan assay). Our findings indicate that ELANE rs17223045C/T (C vs T: odds ratio [OR] 0.08, P = 0.005, and CC vs CT: OR 0.08, P = 0.005) and rs3761007G/A (G vs A: OR 0.38, P = 0.009, and GG vs GA: OR 0.40, P = 0.008) confer protection against COVID-19. However, these variants were not associated with biochemical markers. In conclusion, our data suggests that ELANE rs17223045C/T and rs3761007G/A SNVs may play a protective role against COVID-19.

OBJECTIVE: This systematic review aimed to answer the question: "What are the risk and protective factors for shoulder complaints (pain, injury, or problem) in indoor volleyball players?". METHODS: Searches were conducted in electronic databases: PubMed/MEDLINE, Embase, CINAHL, and SPORTDiscus, as well as reference lists of the included studies. We included studies evaluating potential intrinsic and extrinsic factors associated with shoulder complaints in indoor volleyball players of any country, age, sex, and competitive level. The Quality in Prognosis Studies (QUIPS) tool was used to assess the risk of bias. RESULTS: The risk factors for shoulder complaints in volleyball players were identified through four prospective studies, which exhibited a moderate to low risk of bias. These factors included previous shoulder pain or injury, playing in outside and opposite positions, subacromial bursa thickening, and having an average external rotator (ER) isokinetic eccentric torque lower than the average internal rotator (IR) concentric torque. Protective factors were enhancing shoulder IR and ER isokinetic eccentric strength, being male sex, being older, and maintaining a concentric strength ratio ER/IR within the 0.60-0.75 range. CONCLUSIONS: This systematic review highlights risk factors that clinicians and researchers should consider when assessing and tracking indoor volleyball players.

BACKGROUND: High cannabis use rates among American Indian (AI) adolescents necessitate the identification of factors that protect against early cannabis initiation. METHODS: Data collected from 279 AI middle school students attending reservation-based schools in 2018 and 2019 were analyzed. Three waves of data, with approximately 6 months between each, were used. A repeated measures latent class analysis examined a school-related protective factor index over three waves. The predictive power of lifetime cannabis use on school protection class membership was estimated, along with differences in past month cannabis use at follow-up 2 across school protection classes while holding baseline use constant. RESULTS: Four school protection classes were identified: high, moderate, low, and declining protection. Abstinence at baseline was associated with an increased odds of membership in the high protection class compared to the moderate and low protection classes. Students with consistent and high school protection throughout middle school were significantly less likely to report past month cannabis use at follow-up 2 compared to other classes. IMPLICATIONS FOR SCHOOL HEALTH POLICY, PRACTICE, AND EQUITY: Maintaining a high level of school protection throughout middle school substantially lowers the odds of cannabis use among AI adolescents. CONCLUSIONS: Interventions to promote school-related protective factors in this population are essential and should be designed and tested.

El consumo de alcohol se considera un fenómeno complejo y multifactorial que puede estudiarse a través de diversas posturas teóricas, sin embargo, algunas teorías existentes carecen de sufi-cientes elementos para explorar y comprender dicho fenómeno. Por lo anterior, se propone una Teoría de Rango Medio que aborde los factores personales, ambientales y representaciones so-ciales del consumo de alcohol en jóvenes universitarios a partir de la Teoría Cognitiva Social de Bandura y de la conceptualización de la representación social de Moscovici. Para ello, se utilizó el método de Estructura Conceptual-Teórico-Empírico propuesto por Fawcett, compuesto por cinco pasos. La teoría derivada de este método contribuye en la identificación de un fenómeno específico para la disciplina mediante la determinación de conceptos y su relación con el con-sumo de alcohol en jóvenes universitarios. Además, de la incorporación de las representaciones sociales como elemento innovador para la profundización del fenómeno. Esta propuesta puede ser considerada en futuras investigaciones para ser probada y contribuir significativamente en el desarrollo de conocimiento sobre el consumo de alcohol


Alcohol consumption is considered a complex and multifactorial phenomenon that can be studied through various theoretical positions; however, some existing theories lack sufficient elements to explore and understand this phenomenon. Therefore, a Middle Range Theory is proposed that addresses personal, environmental, and social representations of alcohol consumption in young university students based on Bandura's Social Cognitive Theory and Moscovici's conceptualiza-tion of social representation. For this, the method of Conceptual-Theoretical-Empirical Structure proposed by Fawcett was used, consisting of five steps. The theory derived from this method con-tributes to the identification of a specific phenomenon for the discipline by determining concepts and their relationship with alcohol consumption in young university students. In addition, the incorporation of social representations as an innovative element for the deepening of the phenom-enon. This proposal can be considered in future research to be tested and contribute significantly to the development of knowledge about alcohol consumption


O consumo de álcool é considerado um fenômeno complexo e multifatorial que pode ser estu-dado por meio de vários posicionamentos teóricos, porém, algumas teorias existentes carecem de elementos suficientes para explorar e compreender esse fenômeno. Assim, propõe-se uma Teoria de Médio Alcance que aborda as representações pessoais, ambientais e sociais do consu-mo de álcool em universitários com base na Teoria Social Cognitiva de Bandura e na conceitu-ação de representação social de Moscovici. Para isso, foi utilizado o método da Estrutura Con-ceitual-Teórico-Empírica proposto por Fawcett, composto por cinco etapas. A teoria derivada desse método contribui para a identificação de um fenômeno específico da disciplina ao deter-minar conceitos e sua relação com o consumo de álcool em jovens universitários. Além disso, a incorporação das representações sociais como elemento inovador para o aprofundamento do fenômeno. Essa proposta pode ser considerada em pesquisas futuras a serem testadas e contribuir significativamente para o desenvolvimento do conhecimento sobre o consumo de álcool

Objetivo: analisar os fatores que influenciam na interação mãe-filho-ambiente no processo de amamentação. Métodos: trata-se de estudo transversal realizado na Rede de Atenção Básica de Saúde de um município do interior do Maranhão, Brasil, com mães de crianças até um ano de idade em processo de amamentação. A coleta de dados foi realizada na Unidade Básica de Saúde ou domicílio, entre abril e julho de 2021, utilizando a Escala Interativa de Amamentação (EINA). Utilizaram-se testes t de Studente ANOVA a um fator, com p-valor < 0,05. Resultados: a média do total de pontuação na escala foi de 210,75 (Desvio Padrão - DP ±16,95), indicando adequada interação entre mãe-filho-ambiente no processo de amamentação. Ter maior escolaridade (ensino superior completo), ser casada e desejar amamentar apresentaram associação com a interação mãe-filho-ambiente no processo de amamentação. Conclusões: maior escolaridade, ser casada e desejar amamentar estão associadas à maior interação mãe-filho-ambiente no processo de amamentação. Quanto maior a vontade de amamentar, maior será a interação mãe-filho-ambiente no processo de amamentação. Estes achados permitem planejar estratégias para melhorar a identificação de binômios com maiores riscos de desmame precoce; e desenvolver ações específicas para o cuidado frente aos preditores que interferem no processo de amamentação.


Objective: analyze the factors that influence the mother-child-environment interaction in the breastfeeding process. Methods: this is a cross-sectional study carried out in the Primary Health Care Network of a municipality in the interior of Maranhão, Brazil, with mothers of children up to one year old who were breastfeeding. Data was collected at the Basic Health Unit or in their households, between April and July 2021, using the Interactive Breastfeeding Scale (EINA, as per its acronym in Portuguese). Student's t and one-way ANOVA tests were used, with a p-value < 0.05. Results: the average total score on the scale was 210.75 (Standard Deviation - SD ± 16.95), indicating adequate interaction between mother-child-environment in the breastfeeding process. Having a higher level of education (complete higher education), being married and wanting to breastfeed were associated with mother-child-environment interaction in the breastfeeding process. Conclusions: higher levels of education, being married and wanting to breastfeed are associated with greater mother-child interaction in the breastfeeding process. The greater the desire to breastfeed, the greater the mother-child-environment interaction in the breastfeeding process. These findings make it possible to plan strategies to improve the identification of pairs at greater risk of early weaning, and to develop specific actions for caring for the predictors that interfere with the breastfeeding process.


Objetivo: analizar los factores que influyen en la interacción madre-hijo-entorno en el proceso de lactancia materna. Métodos: Se trata de un estudio transversal realizado en la Red de Atención Primaria de Salud de un municipio del interior de Maranhão, Brasil, con madres de niños de hasta un año que estaban amamantando. Los datos fueron recolectados en la Unidad Básica de Salud o en el domicilio, entre abril y julio de 2021, utilizando la Escala Interactiva de Lactancia Materna (EINA, por sus siglas en portugués). Se utilizaron pruebas t de Student y ANOVA unidireccional, con un valor p < 0,05. Resultados: la puntuación media total de la escala fue de 210,75 (desviación estándar - DE ± 16,95), lo que indica una interacción adecuada entre madre-hijo-entorno en el proceso de lactancia. Tener un mayor nivel de educación (educación superior completa), estar casada y querer amamantar se asociaron con la interacción madre-hijo-ambiente en el proceso de lactancia. Conclusiones: mayores niveles de educación, estar casada y querer amamantar se asocian a una mayor interacción madre-hijo-entorno en el proceso de lactancia. Cuanto mayor es el deseo de amamantar, mayor es la interacción madre-hijo-entorno en el proceso de lactancia. Estos hallazgos permiten planificar estrategias para mejorar la identificación de las parejas con mayor riesgo de destete precoz y desarrollar acciones específicas para el cuidado de los predictores que interfieren en el proceso de lactancia.

Esta pesquisa teve como objetivo investigar as perspectivas dos psicólogos dos Centros de Referência da Assistência Social (CRAS) que compõem a equipe de Proteção e Atenção Integral à Família (PAIF) a respeito do seu trabalho no Acompanhamento Familiar oferecido para famílias com membros com transtornos mentais. Oito psicólogos que atuavam nos CRAS de um município no interior de Minas Gerais participaram do estudo. O instrumento utilizado foi um roteiro de entrevista semiestruturada, com a subsequente análise de conteúdo temática. As categorias temáticas foram analisadas à luz da literatura específica da área. De maneira geral, os resultados indicaram que os psicólogos se sentem despreparados para o exercício de sua função no CRAS, uma vez que a formação específica e continuada em Psicologia não ofereceu subsídios adequados para o conhecimento da atuação no campo da Assistência Social. A natureza (psico)terapêutica do trabalho é discutida, assim como a necessidade de formações continuadas para a atuação. Ressalta-se a necessidade de mais pesquisas que abordem a formação em Psicologia e suas relações com a Assistência Social, bem como os impactos desse despreparo na prática dos profissionais, de maneira a fomentar maior satisfação pessoal/profissional e, consequentemente, aprimorar a assistência oferecida à comunidade.(AU)


This study aims to investigate the views of psychologists who worked at Social Assistance Reference Centers (CRAS) associated with the Comprehensive Family Care Program (PAIF) on their work with the aforementioned program. Overall, eight psychologists who worked at CRAS units in small municipalities in Minas Gerais for at least one year participated in this research. The instrument used was a semi-structured interview script, and the data were analyzed under the content analysis (thematic) method. Thematic categories were analyzed based on the specific literature. Results indicate that the psychologists generally felt unprepared to work at CRAS since their degree in Psychology provided inadequate knowledge to deal with Social Assistance issues. This study discusses the (psycho)therapeutic nature of their practice and the need for ongoing training for their proper performance. This study highlights the need for further research that addresses the links between education in Psychology and Social Assistance and the impacts of said unpreparedness on the performance of those professionals. Such research might provide more professional/personal satisfaction and, in turn, improve the quality of the offered service.(AU)


Este estudio tuvo la intención de conocer las perspectivas de los psicólogos de los Centros de Referencia de Asistencia Social (CRAS) que forman parte de los equipos del Protección y Atención Integral a la Familia (PAIF) acerca del seguimiento de familias con miembros portadores de trastornos mentales. Ocho psicólogos que actuaban en los CRAS de un municipio del interior del estado de Minas Gerais (Brasil) participaron en el estudio. El instrumento utilizado fue un guion de entrevistas semiestructuradas; y, para análisis de datos, se utilizó el análisis de contenido temático. Las categorías temáticas se analizaron a la luz de la literatura específica del campo. De modo general, los resultados indicaron que los psicólogos no se sienten preparados para desempeñar la función en el CRAS, puesto que la formación en Psicología no ofreció conocimientos adecuados para actuar en el campo de la asistencia social. Se discuten la naturaleza (psico)terapéutica del trabajo y la necesidad de formación continua para esta actuación. Se destaca la necesidad de más investigaciones que tratan de la formación en Psicología y sus relaciones con la asistencia social, y los impactos de esta falta de preparo en la práctica de los profesionales, de modo a promover una satisfacción personal/profesional y, consecuentemente, optimizar la asistencia ofrecida a la comunidad.(AU)

Objetivo: mapear as evidências científicas sobre a cultura de segurança e segurança do profissional de saúde no enfrentamento da pandemia de COVID-19 nos serviços de urgência e emergência móveis e de pronto atendimento. Métodos: Scoping Review, desenvolvida segundo diretrizes de JBI. Realizou-se a busca em sete bases de dados e os achados foram encaminhados para uma pasta de arquivos da plataforma Rayyan. Resultados: 7.357 publicações das quais foram selecionados 19 artigos e 03 documentos da literatura cinzenta. Conclusões: houve elevação exponencial dos casos de infecção entre os profissionais de saúde nos momentos críticos da pandemia, repercussões na saúde mental dos profissionais e elevadas cargas físicas e sociais. A cultura de segurança foi afetada pela escassez de infraestrutura, desproteção e abandono pelas instituições de saúde e governos. Proteger os profissionais da urgência/emergência deve ser prioridade no enfrentamento de pandemias.


Objective: to map the scientific evidence on the safety and security culture of healthcare professionals when facing the COVID-19 pandemic in mobile emergency and emergency services and emergency care. Methods: Scoping Review, developed according to JBI guidelines. A search was carried out in seven databases and the findings were sent to a file folder on the Rayyan platform. Results: 7,357 publications from which 19 articles and 03 documents from gray literature were selected. Conclusions: there was an exponential increase in cases of infection among health professionals at critical moments of the pandemic, repercussions on the mental health of professionals and high physical and social burdens. The security culture was affected by the lack of infrastructure, lack of protection and abandonment by health institutions and governments. Protecting urgency/emergency professionals must be a priority when fighting pandemics.

Métodos: Scoping Review, desarrollado según lineamientos del JBI. Se realizó una búsqueda en siete bases de datos y los hallazgos se enviaron a una carpeta de archivos en la plataforma Rayyan. Resultados: 7.357 publicaciones de las cuales se seleccionaron 19 artículos y 03 documentos de literatura gris. Conclusiones: hubo un aumento exponencial de casos de infección entre profesionales de la salud en momentos críticos de la pandemia, repercusiones en la salud mental de los profesionales y altas cargas físicas y sociales. La cultura de seguridad se vio afectada por la falta de infraestructura, desprotección y abandono por parte de las instituciones de salud y los gobiernos. Proteger a los profesionales de urgencias/emergencias debe ser una prioridad en la lucha contra las pandemias.

Objetivo: analizar los factores asociados con el uso inconsistente del preservativo masculino en hombres VIH negativos que tienen sexo con hombres. Método: estudio transversal, analítico, nacional realizado de forma online en todas las regiones de Brasil, en 2020, a través de redes sociales y sitios de citas. El uso inconsistente del preservativo se definió como el uso ocasional o no usarlo nunca. Se realizaron análisis estadísticos descriptivos, pruebas de asociación y regresión logística binaria. Resultados: 1222 (85%) de los 1438 participantes informaron uso inconsistente del preservativo. Las variables "homosexuales" (ORA: 2,03; IC 95%: 1,14-3,59; p = 0,016), "tener pareja estable" (ORA: 2,19; IC 95%: 1,55-3,09; p<0,001), "sexo oral" (ORA: 2,41; IC 95%: 1,31-4,43; p = 0,005), "anal insertivo" (ORA: 1,98; IC 95%: 1,10-3,58; p = 0,023) y "diagnóstico de ITS" (ORA: 1,59; IC 95%: 1,13-2,24; p = 0,007) se asociaron de forma independiente con el uso inconsistente del preservativo masculino. Las variables "recibió consejo de un amigo sobre la prueba del VIH" (ORA: 0,71; IC 95%: 0,52-0,96; p = 0,028) y "trabajador sexual" (ORA: 0,26; IC 95%: 0,11-0,60; p = 0,002) fueron factores protectores. Conclusión: las variables estudiadas indicaron que hay una fuerte relación entre las parejas estables y el aumento de la confianza y la baja adherencia al uso del preservativo, lo que coincide con otros estudios.


Objective: to analyze the factors associated with inconsistent use of male condoms among HIV-negative men who have sex with other men. Method: a cross-sectional, analytical and nationwide study conducted online in all the Brazilian regions in 2020, via networks and in dating websites. Inconsistent condom use was defined as occasional use or as never using it. Descriptive statistical analyses were performed, as well as association and binary logistic regression tests. Results: inconsistent condom use was reported by 1,222 (85%) of all 1,438 participants. The "homosexuals" (ORAdj: 2.03; 95% CI: 1.14-3.59; p=0.016), "having a fixed partner" (ORAdj: 2.19; 95% CI: 1.55-3.09; p<0.001), "oral sex" (ORAdj: 2.41; 95% CI: 1.31-4.43; p=0.005), "insertive anal" (ORAdj: 1.98; 95% CI: 1.10-3.58; p=0.023) and "STI diagnosis" (ORAdj: 1.59; 95% CI: 1.13-2.24; p=0.007) variables were independently associated with inconsistent use of male condoms. The "receiving advice on HIV test from a friend" (ORAdj: 0.71; 95% CI: 0.52-0.96; p=0.028) and "sex worker" (ORAdj: 0.26; 95% CI: 0.11-0.60; p=0.002) variables were protective factors. Conclusion: the variables under study pointed to a strong relationship between steady partners and increased trust and low adherence to condom use, corroborating other studies.

Background: The COVID-19 pandemic has changed our habits and lifestyle due to social isolation and mobility restrictions. This new scenario, together with the fear of contracting the coronavirus, influenced mental health, especially among older adults, who presented reductions in social contact and physical activity (PA). Thus, the objective of the study was to analyze the impact caused by social isolation during the COVID-19 pandemic on the mood states of active and sedentary older adults. Methods: This is an observational study conducted during the COVID-19 pandemic. Older adults aged over 60 years, who were registered in the database of the Secretariat for the Promotion of Citizenship from a city in southern Brazil, participated in the research. An online questionnaire was applied with questions about sociodemographic characteristics, level of PA, confinement, and mood states in two periods: May 2020 and June 2021. The Mann-Whitney U test was used to compare the mood states of active and inactive individuals during the pandemic. Results: One hundred and fifty participants answered the questionnaire, of which 80 (53.3%) reported practicing PA. More active older adults suffered fewer changes in mood when compared to inactive older adults, with lower levels of confusion (p = 0.035), depression (p = 0.002), and fatigue (p = 0.000). Older adults confined for more than 50 days were more likely to develop depression. In addition, the mood state correlated with the variable fear of contracting the coronavirus; the greater the fear, the greater the mental confusion, depression, fatigue, and tension, and the lower the vigor in the older adults. The practice of PA is also correlated with the mood state; the greater the number of hours dedicated to PA, the lower the confusion, depression, fatigue, and tension of the older adults. Conclusion: The practice of PA positively influenced the mental health of older adults during periods of isolation and social restrictions. PA has a protective factor for the development of mental health problems and improves mood states, with greater time performing PA leading to more benefits.

RÉSUMÉ

ABSTRACT: This article aimed to describe the perspective of people who make harmful use of illicit substances on the contributions of risk and protective factors to the development of problems related to drug use. One hundred eleven participants were recruited from a health service for drug users in a city in the interior of the state of São Paulo through posters with information, distribution of brochures, and snowball sampling technique. The sample consists of 51.9% men and 48.1% women, with 81.3% single and 10.3% married. Among them, 54.2% of participants completed high school, 64.1% were Catholic, and 34% rarely practiced their religion. The results on risk factors were classified into three domains, namely, "personal characteristics and behaviors," "family circumstances," and "other social pressures," and those on protective factors were also classified into three domains, namely, "personal characteristics and behaviors," "family circumstances," and "circumstances in the community." The data make it possible to guide the incorporation of multiple strategies to protect the consumption of illicit drugs in human biopsychosocial development, especially among children and adolescents.
