RÉSUMÉ

Guidelines suggest a thyrotropin (TSH) stimulation level ≥30 mIU/l for the administration of 131-iodine (I131) in patients with differentiated thyroid carcinoma (DTC). We present a patient with follicular thyroid carcinoma (FTC), with spinal metastasis as the initial manifestation, who after 6 weeks without levothyroxine did not present an elevation of ≥30 mIU/l of TSH. This situation was interpreted as secondary to the presence of functioning metastases and it was decided, regardless of the TSH level, to administer a therapeutic dose of I131, with iodine-uptake lesions in the liver and spine being confirmed.


Las guías sugieren un nivel de estimulación de tirotrofina (TSH) ≥30 mUI/l para la administración de yodo 131 (I131) en pacientes con carcinoma diferenciado de tiroides (CDT). Presentamos una paciente con carcinoma folicular de tiroides (CFT), con metástasis vertebral como manifestación inicial, que tras 6 semanas sin levotiroxina no presentó una elevación ≥30 mUI/l de TSH. Esta situación fue interpretada como secundaria a la presencia de metástasis funcionantes y se decidió, independientemente del valor de TSH, la administración de una dosis terapéutica de I131 constatándose lesiones captantes de I131 en hígado y columna vertebral.

INTRODUCTION: Several studies have shown the antiproliferative effect of iodine and 5­hydroxy-6 iodo-eicosatrienoic delta lactone (IL-δ) on diverse tissues. It was demonstrated that molecular iodine (I2) and IL-δ, but not iodide (I-), exerts anti-neoplastic actions in different cancers. The underlying mechanism through which IL-δ inhibits tumor growth remains unclear. The aim of this study was to analyze the effect of IL-δ on tumor growth and angiogenesis in human HT29 colorectal cancer xenografts. METHODOLOGY AND RESULTS: HT29 cells were injected subcutaneously into the flanks of nude mice and IL-δ was i.p. injected at a dose of 15 µg three days a week. IL-δ treatment in HT29 xenografts showed time-dependent inhibition of tumor growth, decrease of mitosis and PCNA expression (p < 0.05), increase of P27 expression and Caspase 3 activity after 18 days of treatment (p < 0.05). To assess tumor Microvessel Densities (MVD), CD31 staining by immunohistochemistry was analyzed. IL-δ treatment decreased MVD by 17% and 30% after 18 and 30 days respectively (p < 0.05), as well as it decreased VEGF and VEGF-R2 expression (p < 0.05). Additionally, our findings demonstrated that IL-δ increased VEGF-R1 and Ang-1 mRNA levels (p < 0.01). CONCLUSION: The antitumor efficacy of IL-δ in vivo involves inhibition of cell proliferation as well as induction of apoptosis. IL-δ has also anti-angiogenic effect associated with VEGF and VEGF-R2 downregulation followed by Ang-1 and VEGF-R1 increased expression. High levels of Ang-1 would contribute to mature vessel stabilization and maintenance while VEGF-R1 increase would produce anti-proliferative effect on endothelial cells.

El hipertiroidismo es el cuadro clínico resultante del exceso de hormonas tiroideas debido a hiperfunción glandular. Es una enfermedad rara en niños y adolescentes, pero con una alta morbilidad. La causa más frecuente es la enfermedad de Graves. El objetivo de esta publicación es realizar una revisión y actualización del hipertiroidismo infantojuvenil para guiar su detección y derivación temprana al endocrinólogo pediatra. Debe ser considerado cuando el niño o adolescente presente síntomas asociados a esta patología y bocio de grado variable. Se confirma con el perfil bioquímico característico.El tratamiento consiste, inicialmente, en bloquear los efectos del exceso de hormonas tiroideas con betabloqueantes y, además, disminuir su producción con drogas antitiroideas como primera elección. Ante efectos secundarios a su administración, recidivas o ausencia de remisión de la enfermedad, se optará por el tratamiento definitivo: yodo radioactivo o cirugía con el objetivo de lograr el hipotiroidismo o eutiroidismo


Hyperthyroidism is a serious and rare disorder in childhood characterized by the overproduction of thyroid hormones by the thyroid gland. Graves disease is the most common cause. The objective of this paper is to review and update hyperthyroidism in children and adolescents aiming to guide its early detection and referral to the pediatric endocrinologist. The disease should be suspected if typical symptoms and goiter are present and has to be confirmed with the characteristic biochemical profile. Initially, treatment to block the effect of the thyroid excess is needed. Antithyroid drugs are the recommended first-line treatment to diminish hormone production. Alternative treatments, such us radioactive iodine or thyroidectomy, are considered in cases of adverse effects to drugs, relapse or non-remission of the disease, in order to achieve hypothyroidism or euthyroidism.

Con el objetivo de describir las recidivas de carcinoma papilar de tiroides (CPT) en pacientes tiroidectomizados atendidos en el Servicio Autónomo Oncológico del estado Lara, se realizó un estudio descriptivo transversal de recolección retrospectiva de datos de 140 historias clínicas de pacientes con diagnóstico de CPT registrados durante el lapso 2006-2017. Los pacientes incluidos se caracterizaron por un promedio de edad de 46,67 ± 13,94 años, siendo los más afectados los grupos de 41-50 años y 51-60 años; se observó un predominio del sexo femenino (86,33%). La intervención quirúrgica más realizada en estos pacientes fue la tiroidectomía total (44,29%) y tiroidectomía total con vaciamiento ganglionar (41,43%); 35,71% de los pacientes recibieron Iodo-131 como tratamiento adyuvante. El porcentaje de recidivas fue de 4,29%, siendo las recidivas más frecuentes la local y regional (33,33%, respectivamente); 66,67% de los casos de recidiva aparecieron en los primeros 5 años. De los pacientes con recidivas, 50% se les practicó tiroidectomía total y recibieron Iodo-131. Con estos resultados se aporta información epidemiológica actual sobre el CPT en nuestro centro con el fin de contribuir en la preparación de los servicios de cirugía y oncología para la atención de este grupo de pacientes(AU)


With the aim of describing the recurrences of papillary thyroid carcinoma in thyroidectomized patients treated at the Autonomous Oncology Service of Lara State, a cross-sectional descriptive retrospective study was conducted examining 140 medical records of patients with a diagnosis of papillary thyroid carcinoma registered during the period 2006-2017. Patients had an average age of 46.67 ± 13.94 years, being the most affected the 41-50 years and 51-60 years groups (26.43%, respectively); a predominance of females was observed (86.33%). The most performed surgical intervention was total thyroidectomy (44.29%) and total thyroidectomy with lymph node dissection (41.43%). Likewise, 35.71% received Iodine-131 as adjuvant treatment. The percentage of recurrences was 4.29%, being the most frequent local and regional with a prevalence of 33.33%, respectively; in addition, 66.67% of recurrences appeared during the first 5 years; 50% of these patients underwent total thyroidectomy and received Iodine-131. With these results, current information is provided in order to contribute to the preparation of surgical and oncological services for the care of cases of papillary thyroid carcinoma. It is important to maintain oncological follow-up in order to evaluate the evolution and prognosis of the different types of recurrences according to the place of appearance(AU)

The efficiency of the disinfectants used in the milking management is fundamental to the success in the dairy activity, being a critical point to the control of mastitis. The objective was to evaluate the in vitro efficacy of iodine used in pre and post-dipping against coagulase negative Staphylococcus (CNS). Thus, 53 CNS isolates were studied for the action of the 1.0% disinfectant and their serial dilutions of 0.5%, 0.375% and 0.25%, in addition to two commercial presentations of iodine in concentrations of 0.5% and 0.25%. The rate of CNS inhibition achieved by iodine at 0.375%, 0.5% and 1.0% for 60 seconds, was 60.4%. In 30 seconds, iodine at 0.5% and 1.0% showed a microbial inhibition rate of 52.8% and 56.6%, respectively. The other protocols tested were less efficient. It is concluded that the greatest in vitro disinfectant activity for CNS was demonstrated by iodine at 0.375%, 0.5% and 1.0%, for 60 seconds. Microbial susceptibility tests should be carried out periodically, as well as health education practices and corrective training on the property, in order to ensure udder health and mastitis control.(AU)


A eficiência dos desinfetantes empregados no manejo de ordenha é fundamental no sucesso na atividade leiteira, sendo um ponto crítico ao controle da mastite. Objetivou-se avaliar a eficácia in vitro do iodo utilizado no pré e pós-dipping frente à Staphylococcus coagulase negativa (SCN). Foram estudados 53 isolados de SCN quanto à ação do desinfetante a 1,0% e suas diluições seriadas de 0,5%, 0,375% e 0,25%, além de duas apresentações comerciais nas concentrações de 0,5% e 0,25%. A taxa de inibição de SCN alcançada pelo iodo a 0,375%, 0,5% e 1,0% durante 60 segundos, foi de 60,4%. Em 30 segundos, o iodo a 0,5% e 1,0% apresentaram taxa de inibição microbiana de 52,8% e 56,6%, respectivamente. Os demais protocolos testados foram menos eficientes. Conclui-se que a maior atividade desinfetante in vitro para SCN foi demonstrada pelo iodo a 0,375%, 0,5% e 1,0%, durante 60 segundos. Testes de susceptibilidade microbiana devem ser realizados periodicamente, assim como práticas de educação em saúde e treinamentos corretivos na propriedade, visando garantir a saúde do úbere e o controle da mastite.(AU)

A paralisia periódica hipocalêmica tireotóxica é uma complicação inusitada do hipertireoidismo, porém é considerada urgência endocrinológica e ainda frequentemente subdiagnosticada. Sua apresentação clínica consiste na tríade de défice de potássio, tireotoxicose e fraqueza muscular ­ sendo esse último sintoma comum em diversas patologias. Realizamos uma revisão bibliográfica e destacamos, por meio do relato de caso, a importância do diagnóstico precoce dessa doença, possibilitando uma evolução favorável ao paciente, independente de sua etnia, sexo ou região geográfica. Atentamos ainda ao tratamento da doença, que, apesar de sua simplicidade, acarreta muitos equívocos.


The thyrotoxic hypokalemic periodic paralysis is a rare complication of hyperthyroidism, but is considered an endocrinological urgency, and yet frequently underdiagnosed. Its clinical presentation consists of potassium deficit, thyrotoxicosis, and muscular weakness, with the latter symptom being very common in several pathologies. We performed a bibliographic review and highlight, through a case report, the importance of the early diagnosis of this disease to allow favorable progression to the patient, regardless of ethnicity, sex, or geographical region. We also reinforce the importance of the disease treatment which, despite its simplicity, leads to many mistakes.

Objective: To evaluate Di Cavalcanti's artworks in which goiters are represented before and after the introduction of iodized salt to the Brazilian population. Methods: One hundred and thirty paintings by Di Cavalcanti from the 20's to 70's demonstrating necks were evaluated. All the paintings were observed in reproductions. The neck circumference in the paintings was measured. Since there were no standard thresholds of neck circumference, cutoffs were based on the median. Baseline characteristics of artworks were compared based on high and normal neck circumference categories using Student's t-test, Mann-Whitney-Wilcoxon test, or chi square test. Results: We analyzed 29 artworks which portray the neck of 60 women (84.5%), 8 men (11.3%) and 3 children (4.2%). The analyses of the neck circumference showed 23.3% of women (14/60), 12.5% of men (1/8), and 33.3% of children (1/3) with an abnormal profile of the neck circumference. The neck circumference ratio in 29 paintings showed that the relative sizes of the necks painted between the 1920's and 1950's (r=0.45; p=0.03), and painted between the 1960's and 70's (r= 0.54; p=0.003) have linearly decreased. The decades in which the artworks were painted explained 40.0% of the variation in size of the neck circumference (p=0.002). Conclusion: Art imitates life. Di Cavalcanti was not a physician, and probably did not have the intention to illustrate a pathological condition, although the images observed in this study should be considered as goiter or enlarged neck.


Objetivo: Avaliar as obras de Di Cavalcanti em que bócios estão representados, antes e após a introdução da iodação do sal para a população brasileira. Método: Foram avaliadas 130 pinturas de Di Cavalcanti entre os anos 1920 e 1970 demonstrando pescoços. Todas as pinturas foram observadas em reproduções. A circunferência do pescoço nas pinturas foi mensurada. Como não existia limite-padrão da circunferência do pescoço, os limites foram baseados na mediana. As características básicas das obras de arte foram comparadas por categorias da circunferência do pescoço em elevadas e normais, usando o teste t de Student, o teste de Mann-Whitney-Wilcoxon ou o teste qui-quadrado. Resultados: Analisamos 29 obras de arte que representavam o pescoço de 60 mulheres (84,5%), 8 homens (11,3%) e 3 crianças (4,2%). Ao analisar a circunferência do pescoço, 23,3% das mulheres (14/60), 12,5% dos homens (1/8) e 33,3% das crianças (1/3) demonstraram perfil anormal dela. A relação da circunferência do pescoço em 29 pinturas demonstrou que as circunferências do pescoço relativas aos pescoços pintados entre os anos 1920 e 1950 (r=0,45; p=0,03) e pintados entre os anos 1960 e 1970 (r=0,54; p=0,003) reduziram linearmente. As décadas em que as obra foram pintadas explicaram 40,0% da variação no tamanho da circunferência do pescoço (p=0,002). Conclusão: A arte imita a vida. Di Cavalcanti não era médico e, provavelmente, não tinha intenção de ilustrar uma condição patológica, embora as observações das imagens, neste estudo, tenham sido consideradas como bócio ou com aumento de volume do pescoço

Objetivo: Avaliar o perfil epidemiológico de pacientes com diagnóstico de carcinoma diferenciado de tireoide. Métodos: Estudo analítico descritivo, realizado por meio da análise e da coleta de dados de 34 prontuários de pacientes em seguimento em um ambulatório de endocrinologia de um hospital público em São Paulo. Foram incluídos todos os pacientes em acompanhamento no ambulatório com diagnóstico de tumores diferenciados; foram excluídos os menores de 18 anos e/ou com diagnóstico de outras neoplasias de tireoide (que não tumores diferenciados). Resultados: A idade dos pacientes variou de 25 a 84 anos ao diagnóstico, com média de 51 anos, tendo sido representados por 32 mulheres (94,11%) e 2 homens (5,88%). A maioria dos tumores (41,17%) possuía tamanho entre 1 e 2cm. A maior parte dos pacientes tinha baixo risco de recorrência, e todos foram tratados cirurgicamente. Conclusão: O tumor diferenciado mais frequente é o subtipo papilífero. Sua prevalência se dá em mulheres, com média de idade ao diagnóstico de 51 anos. (AU)


Objective: To evaluate the epidemiological profile of patients with differentiated thyroid carcinoma. Methods: Th is i s a d escriptive, analytical study performed through the analysis and collection of data from 34 medical records of patients being followed in an endocrinology clinic of a public hospital in the city of São Paulo. All patients being followed at the outpatients' department with a diagnosis of differentiated thyroid carcinoma were included, and those under 18 and/or with diagnosis of other cancer of the thyroid (other than differentiated tumors) were excluded. Results: the patients' ages ranged from 25 to 84 years at diagnosis, an average of 51 years represented by 32 women (94.11%) and 2 men (5.88%). Most tumors (41.17 %) were between 1 and 2cm. Most patients showed low risk of recurrence, and all were surgically treated. Conclusion: The most frequent differentiated tumor was the papillary one. Its prevalence was on women, with an average age at diagnosis of 51 years. There were only two patients with recurrent disease, and one with metastasis to the lung that died. (AU)

BACKGROUND: The immune system is a crucial component in cancer progression or regression. Molecular iodine (I2) exerts significant antineoplastic effects, acting as a differentiation inductor and immune modulator, but its effects in antitumor immune response are not elucidated. METHODS: The present work analyzed the effect of I2 in human breast cancer cell lines with low (MCF-7) and high (MDA-MB231) metastatic potential under both in vitro (cell proliferation and invasion assay) and in vivo (xenografts of athymic nude mice) conditions. RESULTS: In vitro analysis showed that the 200 µM I2 supplement decreases the proliferation rate in both cell lines and diminishes the epithelial-mesenchymal transition (EMT) profile and the invasive capacity in MDA-MB231. In immunosuppressed mice, the I2 supplement impairs implantation (incidence), tumoral growth, and proliferation of both types of cells. Xenografts of the animals treated with I2 decrease the expression of invasion markers like CD44, vimentin, urokinase plasminogen activator and its receptor, and vascular endothelial growth factor; and increase peroxisome proliferator-activated receptor gamma. Moreover, in mice with xenografts, the I2 supplement increases the circulating level of leukocytes and the number of intratumoral infiltrating lymphocytes, some of them activated as CD8+, suggesting the activation of antitumor immune responses. CONCLUSIONS: I2 decreases the invasive potential of a triple negative basal cancer cell line, and under in vivo conditions the oral supplement of this halogen activates the antitumor immune response, preventing progression of xenografts from laminal and basal mammary cancer cells. These effects allow us to propose iodine supplementation as a possible adjuvant in breast cancer therapy.

INTRODUCCIÓN: En el Hospital Nacional Edgardo Rebagliati Martins ­ ESSALUD nacen aproximadamente 7 mil niños anualmente, de los cuales el 10 % requiere de una atención especial en la unidad de cuidados intensivos neonatales y donde también el 2.5 % son bebes prematuros que nacen con pesos menores de 1500 gramos y que requieren cuidados extremos como el de recibir soporte nutricional vía parenteral. La nutrición parenteral (NP) es una técnica de soporte vital y nutricional artificial, en la que los nutrientes se administran por vía endovenosa para cubrir las necesidades energéticas y mantener un estado nutricional adecuado en aquellos pacientes donde la vía enteral es insuficiente, inadecuada o está contraindicada. Existen diversas circunstancias o condiciones clínicas que hacen que un niño no pueda alimentarse de manera normal (vía oral) y deba recibir, vía parenteral, el soporte nutricional que necesita. Los elementos claves de una óptima nutrición, según la Organización Mundial de la Salud (OMS), están conformados por los macro y micronutrientes, estos últimos representados por las vitaminas, minerales y oligoelementos. Los oligoelementos son componentes químicos conformados principalmente por zinc, cobre, iodo, selenio, manganeso, entre otros, y que cumplen diversas funciones dentro del organismo entre las que destacan su participación en diversos sistemas enzimáticos. Actualmente, en EsSalud no se cuenta con soluciones de administración endovenosa que aporten oligoelementos en la NP para pacientes pediátricos. De este modo, los especialistas manifiestan la necesidad de contar con un suplemento nutricional que brinde estos elementos de acuerdo a las necesidades de cada paciente, y que, además, éste sea específicamente de uso pediátrico. TECNOLOGÍA SANITARIA DE INTERÉS: Los oligoelementos son micronutrientes o elementos químicos que en cantidades muy pequeñas resultan indispensables para diversas funciones dentro del organismo. Estos compuestos participan principalmente como catalizadores en sistemas enzimáticos (WHO, 1996). Estos son principalmente zinc, selenio, cobre, iodo y manganeso (NCBI, 1989), importantes para el presente dictamen. METODOLOGÍA: Se llevó a cabo una búsqueda sistemática de la literatura con respecto a la eficacia y seguridad del uso de oligoelementos pediátricos endovenosos que aporten zinc, cobre, iodo, manganeso y selenio a recién nacidos o lactantes que reciben nutrición parenteral. La búsqueda se inició revisando la información sobre el uso del medicamento de acuerdo con entidades reguladoras como: Food and Drug Administration (FDA); European Medicines Agency (EMA); Dirección General de Medicamentos y Drogas (DIGEMID); Organización Mundial de la Salud (OMS). Se realizó tanto una búsqueda sistemática como una búsqueda manual en las páginas web de grupos dedicados a la investigación y educación en salud que elaboran guías de práctica clínica descritas a continuación: National Guideline Clearinghouse (NGC); National Institute for Health and Care Excellence (NICE); Canadian Agency for Drugs and Technologies in Health (CADTH); Scottish Medicines Consortium (SMC). RESULTADOS: De acuerdo con la pregunta PICO, se llevó a cabo una búsqueda de evidencia científica relacionada al uso de oligoelementos pediátricos endovenosos que aporten zinc, cobre, iodo, manganeso y selenio a recién nacidos o lactantes que reciben nutrición parenteral. En la presente sinopsis se describe la evidencia disponible según el tipo de publicación, siguiendo lo indicado en los criterios de elegibilidad (GPC, ETS, RS, MA y ECA fase III). CONCLUSIONES: El presente dictamen tuvo como objetivo evaluar la mejor evidencia científica disponible hasta setiembre del 2019 en relación a la eficacia y seguridad del uso de oligoelementos pediátricos que aporten zinc, cobre, iodo, manganeso y selenio a pacientes pediátricos que reciben nutrición parenteral. La evidencia proveniente de dos RS y dos ECA, la cual también ha sido utilizada como evidencia en las GPC identificadas, sugieren de manera conjunta que el aporte de oligoelementos en la NP en neonatos y lactantes es necesario a fin de dar el soporte nutricional adecuado, tomando en consideración las concentraciones específicas para cada etapa de la vida, así como la edad gestacional al nacimiento, el peso al nacer y las condiciones fisiológicas de fondo. El aporte de cada oligoelemento trae beneficios específicos en cuanto a la función biológica que desempeñan cada uno dentro del organismo, entre los que se encuentran, optimización de las funciones bioquímicas relacionadas al crecimiento y desarrollo, mejora del sistema inmunológico, mejora del desarrollo neurológico. No obstante, el exceso o la falta de cada uno de los oligoelementos trae consigo consecuencias biológicas asociadas a toxicidad tales como, principalmente, problemas neurológicos (exceso de manganeso), mortalidad y problemas tiroideos (deficiencia de iodo), incremento de infecciones (deficiencia de zinc), y mayor de dependencia de oxígeno (deficiencia de selenio). Es necesario brindar el aporte específico requerido de oligoelementos a los neonatos y lactantes utilizando la mejor opción disponible comercialmente, que se ajuste a las necesidades recomendadas. Por lo expuesto, el Instituto de Evaluaciones de Tecnologías en Salud e Investigación - IETSI, aprueba el uso de oligoelementos pediátricos que aporten zinc, cobre, iodo, manganeso y selenio en neonatos y lactantes que reciben nutrición parenteral, según lo establecido en el Anexo N° 1. La vigencia del presente dictamen preliminar es de dos años a partir de la fecha de publicación. Así, la continuación de dicha aprobación estará sujeta a los resultados obtenidos de los pacientes que reciban este tratamiento y de nueva evidencia que pueda surgir en el tiempo.

SUMMARY Short bowel syndrome (SBS) is the leading cause of intestinal failure in children, a condition of absence of sufficient bowel to meet the nutritional and metabolic needs of a growing individual. The treatment of patients in this situation is based on the association of parenteral and enteral nutrition for prolonged periods of time until intestinal rehabilitation occurs with complete enteral nutrition autonomy. Six consecutive cases of children with SBS (residual intestinal length of 5 cm to 75 cm) were managed with this program and were diagnosed with associated hypothyroidism during the treatment (ages at the diagnosis 5 months to 12 years). All patients were successfully treated with oral hormone reposition therapy and in one patient, the replacement was performed via rectal enemas due to a complete absence of small bowel. Although iodine deficiency associated to long-term parenteral nutrition is a well-known condition, this is the first report in the literature about an expressive number of patients with hypothyroidism detected in patients with SBS during the prolonged treatment for intestinal rehabilitation.

BACKGROUND: Mammary cancer has a high incidence in canines and is an excellent model of spontaneous carcinogenesis. Molecular iodine (I2) exerts antineoplastic effects on different cancer cells activating re-differentiation pathways. In co-administration with anthracyclines, I2 impairs chemoresistance installation and prevents the severity of side effects generated by these antineoplastic drugs. This study is a random and double-blind protocol that analyzes the impact of I2 (10 mg/day) in two administration schemes of Doxorubicin (DOX; 30 mg/m2) in 27 canine patients with cancer of the mammary gland. The standard scheme (sDOX) includes four cycles of DOX administered intravenously for 20 min every 21 days, while the modified scheme (mDOX) consists of more frequent chemotherapy (four cycles every 15 days) with slow infusion (60 min). In both schemes, I2 or placebo (colored water) was supplemented daily throughout the treatment. RESULTS: mDOX attenuated the severity of adverse events (VCOG-CTCAE) in comparison with the sDOX group. The overall tumor response rate (RECIST criteria) for all dogs was 18% (interval of reduction 48-125%), and no significant difference was found between groups. I2 supplementation enhances the antineoplastic effect in mDOX, exhibiting a significant decrease in the tumor epithelial fraction, diminished expression of chemoresistance (MDR1 and Survivin) and invasion (uPA) markers and enhanced expression of the differentiation factor known as peroxisome proliferator-activated receptors type gamma (PPARγ). Significant tumor lymphocytic infiltration was also observed in both I2-supplemented groups. The ten-month survival analysis showed that the entire I2 supplementation (before and after surgery) induced 67-73% of disease-free survival, whereas supplementation in the last period (only after surgery) produced 50% in both schemes. CONCLUSIONS: The mDOX+I2 scheme improves the therapeutic outcome, diminishes the invasive capacity, attenuates the adverse events and increases disease-free survival. These data led us to propose mDOX+I2 as an effective treatment for canine mammary cancer.

Oxidative stress and inflammation are involved in the development and/or progression of benign prostatic hyperplasia (BPH). Molecular iodine (I2) induces antiproliferative and apoptotic effects in prostate cancer cells, but it is unknown if I2 regulates oxidative stress in the normal and/or tumoral prostate. The purpose of this study was to analyze the effects of I2 and celecoxib (Cxb) on oxidative stress and inflammation in a model of prostatic hyperplasia. Cxb was used as positive control of cyclooxygenase-2 (COX-2) inhibition. Prostatic hyperplasia was induced in male Wistar rats (170g) with testosterone (5mg/kg/week, for three weeks). One week before hyperplasia induction, I2 (25mg/day/rat) or Cxb (1.25mg/day/rat) was supplied for four weeks in the drinking water. Prostatic hyperplasia was evaluated by histological analysis, DNA content, and/or proliferating cell nuclear antigen (PCNA) expression. Lipoperoxidation (malondialdehyde) and nitrite (NO2-) levels were analyzed by colorimetric methods, while nitric oxide synthase (NOS), COX, and myeloperoxidase (MPO) enzymes were analyzed using RT-PCR, immunoblotting, and/or enzymatic assays. Levels of 15-F2t-isoprostanes, prostaglandins (PGE2), leukotrienes (LTB4), and tumor necrosis factor alpha (TNFα) were measured by ELISA. Control testosterone-treated animals exhibited hyperplasia in the dorsolateral prostate, as well as increments in almost all oxidative parameters except for COX-1, TNFα, or MPO. I2 and Cxb prevented epithelial hyperplasia (DNA content) and oxidative stress induction generated by testosterone in almost the same intensity, and the minimum I2 dose required was 2.5mg/rat. The antioxidant capacity of I2 was also analyzed in a cell-free system, showing that this element inhibited the conversion of nitrate (NO3-) to NO2-. I2 did not modify the prostatic oxidative state in testosterone untreated rats. In summary, our data showed that antiproliferative and antioxidant effects of I2 involve the inhibition of NOS and the COX-2 pathway. Further studies are necessary to analyze the therapeutic and/or adjuvant effects of I2 with first-line medications used to treat BPH.

La presencia de metástasis a distancia en el cáncer diferenciado de tiroides es un hecho infrecuente que ocurre en menos del 10% de los pacientes. Cuando sucede, la supervivencia a 10 años disminuye significativamente. La curación es el objetivo primario, pero dado que 2 tercios de los tumores metastásicos se volverán radiorrefractarios en su evolución, la finalidad terapéutica es estabilizar la enfermedad y tratar sus síntomas. En la última década, el manejo de estos pacientes cambió radicalmente con el advenimiento de las terapias con blancos moleculares. El objetivo de esta revisión es describir las características de los pacientes con cáncer diferenciado de tiroides que presenten metástasis a distancia, la supervivencia global, los métodos diagnósticos utilizados, y los tratamientos locales y sistémicos disponibles.


Distant metastases occur in less than 10% of patients with differentiated thyroid carcinoma. In these patients, overall survival at 10 years is considerably reduced. Whereas cure is the initial goal of treatment, stabilisation of the disease and management of symptoms have become the primary objective in many patients with persistent radio-iodine refractory progressive disease. In the last decade, several targeted therapies have shown encouraging results in patients with advanced disease. The objective of this review is to describe the characteristics, diagnosis, overall survival, and the local and systemic available treatments for patients with distant metastases from differentiated thyroid cancer.

Iodinated contrast (IC) is clinically used in diagnostic and interventional procedures, but its use can result in contrast-induced acute kidney injury (CI-AKI). Chronic kidney disease (CKD) and chronic hyperglycemia (CH) are important predisposing factors to CI-AKI. The aim of this study was to investigate the impact of iodinated contrast on the renal function and hemodynamics in rats with chronic hyperglycemia and chronic kidney disease. A total of 30 rats were divided into six groups; Sham: control of chronic renal disease; Citrate: control of chronic hyperglycemia (CH); Nx5/6: rats with 5/6 nephrectomy; Chronic Hyperglycemia: rats receiving Streptozotocin 65 mg/kg; Nx5/6 + IC: rats Nx5/6 received 6 mL/kg of IC; CH + IC: Chronic hyperglycemia rats receiving 6 mL/kg of IC. Renal function (inulin clearance; urinary neutrophil gelatinase-associated lipocalin, NGAL) and hemodynamics (arterial blood pressure; renal blood flow; renal vascular resistance) were evaluated. Iodinated contrast significantly increased urinary NGAL and reduced inulin clearance, while the hemodynamics parameters showed changes in arterial blood pressure, renal blood flow, and renal vascular resistance in both CKD and CH groups. The results suggest that the iodinated contrast in risk factors models has important impact on renal function and hemodynamics. NGAL was confirmed to play a role of highlight in diagnosis of CI-AKI.

Relata-se neste artigo o caso de um paciente de ascendência negra com quadro agudo e progressivo de paralisia flácida ascendente, sem comprometimento de musculatura respiratória ou facial, em vigência de hipertireoidismo em tratamento e hipocalemia desencadeados por quadro infeccioso. Foi realizado o diagnóstico de paralisia periódica hipocalêmica tireotóxica e controle inicial dos sintomas por adequação sérica do potássio e posterior resolução do quadro após tratamento com iodoterapia.


It is reported in this paper the case of a black ancestry patient with acute and progressive framework ascending flaccid paralysis without impairment of respiratory and facial muscles, in effect in treatment of hyperthyroidism and hypokalemia triggered by infection. The diagnosis of thyrotoxic hypokalemic periodic paralysis was accomplished and initial symptoms were controlled for adequacy of serum potassium and subsequent resolution after treatment with radioiodine.

Staphylococcus spp. são reconhecidos como importantes causadores de mastites em rebanhos leiteiros. Esses micro-organismos têm a capacidade de produzir uma estrutura denominada biofilme, que é responsável pela sobrevivência e muitas vezes pela resistência a ação de produtos desinfetantes e as demais condições adversas. Neste trabalho avaliou-se a ação de dois produtos pós-dipping a base de iodo (0,7%) e clorexidine (2,0%) sobre a adesão de Staphylococcus coagulase positiva (SCP) e Staphy-lococcus coagulase negativa (SCN) isolados de casos de mastite subclínica, e também sobre biofilmes pré formados a partir destes isolados. Os produtos testados apresentaram uma alta redução na taxa de adesão de todos os isolados. No entanto, a ação sobre os biofilmes consolidados só foi estatisticamente significativa sobre os SCN. Assim, ressalta-se a importância dos programas sanitários a fim de prevenir a formação de biofilmes e diminuir as fontes de contaminação da glândula mamaria em sistemas de produção leiteira.(AU)

In this paper we evaluate the action of two teat dip products based on iodine (0,7%) and chlorhexidine (2,0%) on the adhesion of coagulase-positive staphylococci (CPS) and coagulase negative staphylococci (CNS) from subclinical mastitis cases, and also on its consolidate biofilm. The products tested showed a high reduction on the adhesion of all isolates. However, action on the consolidate biofilm was significant for CNS only, and unsatisfactory for CPS. Thus, it emphasizes the importance of health programs to prevent biofilm formation and reduce the sources of contamination of the mammary gland in dairy production systems.(AU)
