Permanente en Salud - EPS y cumple con lo dispuesto en el artículo 14 de la Ley 8.080 / 1990, reglamentado por Decreto Presidencial N °. 7,508, del 28 de junio de 2011, y la Norma Básica Operativa de Recursos Humanos en Salud (NOB / RH-SUS).

PURPOSE: In low- and middle-income countries with limited resources, the success of a hematopoietic stem-cell transplantation (HSCT) program relies directly on its affordability while obtaining similar outcomes to developed regions. The objective of this study was to describe the experience of a tertiary/referral center in Mexico City performing HSCT with the subsidy of a nongovernmental organization (NGO). PATIENTS AND METHODS: We performed a retrospective analysis including 146 patients who underwent HSCT at the National Institutes of Health Sciences and Nutrition Salvador Zubiran and were subsidized by the NGO Unidos. RESULTS: Seventy-five patients (51%) and 71 patients (49%) underwent autologous and allogeneic HSCT, respectively. The median age was 30 years, 56% did not obtain a bachelor's degree, 79% had a low socioeconomic level, and 75% were unemployed. None had any health coverage. According to the real patient out-of-pocket expense, the subsidy by Unidos corresponded to 88% and 72% in autologous and allogeneic HSCT, respectively. CONCLUSION: Our results highlight that undergoing an HSCT was feasible for vulnerable patients because of the subsidy of medications and chemotherapy by Unidos. Therefore, creating NGOs in developing countries is important to provide complex medical procedures, such as HSCT, at limited-resource centers to underserved populations while obtaining good outcomes.

This article focuses on current trends in scholarly literature concerning the evaluation of short-term medical missions. The paucity of information on short-term medical missions in general has contributed to the lack of sufficient frameworks for evaluating them. While examples in the scholarly literature are sparse, in those rare cases where missions are evaluated, they tend to (1) produce their own criteria for evaluation, and (2) evaluate themselves based on metrics that emphasize their perceptions of accomplishments. I draw on interviews (n=31) as well as participant-observation regarding medical missions, to critique these trends. The data analyzed derive from an on-going ethnographic study began in Sololá, Guatemala in 1999, which since 2011 has been directly focused on short-term medical missions. More specifically, my data suggest potential conflict of interest inherent to both volunteering and hosting a short-term medical mission. NGO hosts, who maintain long-term residence in Sololá, may differ from short-term volunteers in both how they understand volunteer obligations as well what they consider helpful volunteer activity. These same organizations may remain financially tied to volunteer labour, limiting their own perceptions of what missions can or should do. I argue that these conflicts of interest have created an evaluation environment where critical questions are not asked. Unless these hard questions are addressed, short-term medical mission providers cannot be certain that their own activities are consonant with the moral imperatives that purportedly drive this particular humanitarian effort. This study demonstrates how ethnographic methods can be instrumental in attempts to evaluate humanitarian endeavours.

RÉSUMÉ

A bill for Public Sector Administrative Reform in Brazil introduces administrative strategies based on broad delegation of authority and "a posteriori" demands for results, thereby highlighting the issue of evaluation in public organizations. This article presents a methodological proposal to evaluate management capability in so-called social organizations, a kind of non-government public entity created within this Reform. Drafting of the bill included the definition of an underlying management concept and its dimensions, which oriented the construction of variables, indicators, parameters, and an analytical plan for evaluation. The final remarks include the argument that the conceptual choices and procedural decisions aim to produce an evaluation design that would guarantee the results' reliability and their sensitivity for approaching the phenomenon under evaluation, as well as the methodology's applicability and reproducibility in different contexts.

Estudo descritivo que teve como objetivos utilizar as diferentes visões construídas sobre as organizações de modo prático, a fim de efetuar a leitura e a compreensão de situações específicas da organização e ainda, para orientar, genericamente o planejamento e a administração da mesma. Utilizou-se como estratégia a leitura-diagnóstico do sistema organizacional de três Instituições do Estado de São Paulo, após a aplicação de um questionário em dois profissionais de cada instituição. O questionário constitui-se de questões que utilizam metáforas, para analise do sistema organizacional destas instituições, utilizando na seqüência “Metáfora da Máquina”, a “Metáfora Orgânica”, a “Metáfora do Cérebro”, a “Metáfora da Cultura”, “Metáfora Política” e a “Metáfora do Fluxo e Transformação”, abordadas por MORGAN (1996), em seu livro “Imagens da Organização”. Foi realizado um processo de avaliação critica. Esta avaliação critica foi feita sob o ponto de vista do funcionário de Instituições Particulares.

OBJECTIVE: To evaluate the effect of a quality-monitoring program on the occurrence of sentinel events and on attitudes and behaviors of personnel's responses in a Non-Governmental-Organization (NGO). MATERIAL AND METHODS: Between 1998 and 1999, a quasi-experimental design of the before-after type was conducted in 13 NGO clinics. Analysis of changes in attitudes and behaviors consisted in differences of means and analysis of variance between groups. RESULTS: The number of sentinel events decreased from 32 events detected before the quality-monitoring program to only 2 after it. Attitudes and behaviors improved, with differences of means of +1.1 and +1.2 (p < 0.05). CONCLUSIONS: The quality-monitoring program achieved the expected effects. It is noteworthy that attitudes and behaviors to prevent the occurrence of sentinel events were more prevalent after the intervention.

El objetivo del presente trabajo es mostrar los Potenciales Donantes Cadavéricos (PDC) que tienen los hospitales de Lima para promover el desarrollo de un Sistema de Organización Regional de Transplante. Se encontró 552 PDC, en 7 hospitales ubicados en las cuatro zonas en que Lima está dividido. El 58.5 por ciento de PDC fué aportado por la zona Lima-Ciudad, el 21 por ciento por Lima-Sur, el 11.6 por ciento por Lima norte y el 8.9 por ciento por Lima-Este. El 76.1 por ciento de los PDC fueron detectados en los servicios de Emergencia y el 13.2 por ciento en Neurocirugía. Si descontamos aproximadamente el 12 por ciento de contraindicaciones médicas y el 15 por ciento de PDC que se pierden por paro cardiaco y posteriormente el 40 por ciento de negatividad familiar, habría en Lima una tasa de donantes que alcanzaría los 40 PDC/por millón de habitantes (pmh); cantidad suficiente de PDC para que exista gran actividad trasplantadora; su aprovechamiento obliga al desarrollo de una organización que trabaje con cada hospital de Lima en coordinación con un centro único de registro de receptores el cuál distribuiría los órganos a los centros trasplantadores.
