ABSTRACT

The population of Nigerian older persons is expected to reach 26 million by 2050 from 9 million reported in 2016. This population change has several implications, thus the need to pay attention to healthy ageing. Hence, this report aims to compare ageing and its facilitators in Nigeria and the United Kingdom (UK). The life course theory was used to explore the influence of early life factors and experiences on ageing. Unlike the UK, little attention is given to the care of Nigerian older persons. Therefore, Nigerian stakeholders must design and implement a comprehensive policy on healthy ageing. Also, there is an urgent need for training nurses to meet this demand as it arises.

ABSTRACT

As the world's aging population is rising, so too is the prevalence of multimorbidity increasing among older adults. Multimorbidity is therefore a growing public health challenge among the older population. Researchers have reported fragmentation of care to be one of the major problems facing this population. The leading factors responsible for this issue are use of disease-centered approaches and specialism to manage people living with multimorbidity; poor communication between professionals and people with multimorbidity; and poor communication among the professionals caring for these people. Failure to address this problem leads to increased treatment burden, including polypharmacy. There is therefore a need for all healthcare professionals caring for older people living with multimorbidity to address this problem by providing continuous, coordinated person-centered care. For the person-centered care approach to be well-coordinated and continuous, there is a need for effective means of sharing information among healthcare providers, to facilitate inter-professional collaboration; extension of consultation time to better enable healthcare providers to understand the patient's needs; review of organizational frameworks and policies where necessary; and development of new guidelines for the management of multimorbidity.


À medida que aumenta a população mundial que envelhece, também aumenta a prevalência de multimorbidade entre os idosos. A multimorbidade é, portanto, um desafio crescente de saúde pública entre a população idosa. Pesquisadores relatam a fragmentação da assistência como um dos principais problemas dessa população. Os principais fatores responsáveis por esse problema são o uso de abordagens centradas na doença e especialização para lidar com pessoas que vivem com multimorbidade; a comunicação deficiente entre os profissionais e as pessoas com multimorbidade; e a falta de comunicação entre os profissionais que cuidam dessas pessoas. Falhas na abordagem a esse problema levam a um aumento da carga do tratamento, incluindo a polifarmácia. Portanto, é necessário que todos os profissionais de saúde que cuidam de pessoas idosas que vivem com multimorbidade abordem esse problema, fornecendo cuidados contínuos e coordenados centrados na pessoa. Para que a abordagem do cuidado centrado na pessoa seja bem coordenada e contínua, são necessários meios eficazes de compartilhamento de informações entre os provedores de saúde, para facilitar a colaboração interprofissional; a extensão do tempo de consulta para melhor capacitar os profissionais de saúde a compreender as necessidades do paciente; a revisão das estruturas e políticas organizacionais, quando necessário; e o desenvolvimento de novas diretrizes para o manejo da multimorbidade.
