ABSTRACT

The aim of this report was to illustrate and to discuss a method capable of improving the person-oriented decision-making process during three years of gait rehabilitation based on the integration of: 1) the fundamental principles of motor learning and 2) the outcomes made available by both clinical standardized assessment tools (SAT) and measures made available by a gait analysis system (GA). The subject studied was a six-year-old child affected by hemiplegia after arterial ischemic stroke (AIS) who had limited upper and lower right-limb function but unaffected sensory and cognitive skills. Four different rehabilitative treatments were chosen when the child was inpatient or outpatient. Measurements of gait performance before and after selected treatments were evaluated using PEDI and GMFM (i.e., SAT) and kinematic and kinetic parameters (i.e., GA). Gait pattern and inter- and intralimb-joint coordination changed over time during the three examined years. However, after the first eight months of recovery, gait pattern modifications were detected by GA measures but not by SAT. The integration of SAT and GA findings, during the examined recovery evolution, resulted effective in the decision-making process for a person-oriented rehabilitative treatment.

ABSTRACT

El eritema elevatum diutinum (EED) es una dermatosis crónica, poco frecuente, de causa desconocida, que al comienzo presenta histopatológicamente una vasculitis leucocitoclásticade la piel y luego evoluciona hacia una fibrosis. Presentamos una paciente con lesiones dermatológicas características y generalizadas de esta rara entidad, en la que se obtuvo una buena respuesta con la sulfonoterapia.


Erythema Elevatum Diutinum (EED) is a rare chronic cutaneous condition of unknow origin, that initially presents histological as leucocytoclastic vasculitis of the skin and later resolves with fibrosis. We present a patient of this uncommon condition with typical cutaneous lesions and widespread involvement. The disease was initially misinterpreted on clinical examination. Dapsone therapy was followed by a remarkable recovery.

ABSTRACT

PRIMARY OBJECTIVE: To describe the outcome of near-drowning and rehabilitation contexts for recovery. METHODS AND PROCEDURES: Standardized measures were used to emphasize the functional impact of deficits over the first year post-injury in three children <2 years. Multimodal contexts for meaningful interplay were early adapted to the three cases. MAIN OUTCOMES AND RESULTS: The clinical pathways of recovery are identified. Initially all three cases manifested a generalized dystonia. Case 1 exhibited a good outcome with transient dyskinetic-dystonic syndrome; subsequently Bálint's syndrome emerged. In this case, the rehabilitation approach was organized on the pickup of direct perception of task-specific affordances. Cases 2 and 3 had poor outcomes presenting the worsening of torsion dystonia (status dystonicus) that hindered rehabilitation intervention. CONCLUSIONS: The dynamic reaggregation of spatial organization through meaningful interaction in specific ecological contexts is the principal goal of rehabilitation intervention. Status dystonicus represents the worst feature for recovery.
