ABSTRACT

SUMMARY OBJECTIVE: The aim of this study was to evaluate the quality of life of patients with lifelong and acquired premature ejaculation and to examine its relationship with depression and anxiety. METHODS: Between February 2017 and January 2018, a total of 175 patients with premature ejaculation and 132 control men who applied to the urology department of the training and research hospital with the complaint of Premature Ejaculation were included. Patients were divided into three groups according to International Society for Sexual Medicine (ISSM) criteria as follows: Group 1, lifelong premature ejaculation; Group 2, acquired premature ejaculation, and Group 3, control group without premature ejaculation. A detailed medical history of patients was obtained and physical examinations were performed. Intravaginal ejaculation latency time (IELT) was recorded and patients were administered International Erectile Function Index-5 (IIEF-5), Premature Ejaculation Diagnostic Tool (PEDT), Sexual Health Inventory for Men (SHIM), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI)-1 and STAI-2, and Short Form-36 (SF-36). RESULTS: The mean mental component score (MCS) of the SF-36 was 51.65±6.57 in the lifelong premature ejaculation group, 49.33±8.65 in the acquired premature ejaculation group, and 61.12±11.09 in the control group (p<0.0001). The mean physical component score (PCS) was 50.99±7.43 in the lifelong premature ejaculation group, 48.32±11.58 in the acquired premature ejaculation group, and 55.17±8.10 in the control group (p<0.0001). Quality of life of premature ejaculation patients as assessed by SF-36 was lower in the subscales of physical functioning, general health perception, vitality, and role limitations due to emotional functioning, compared to the control group. CONCLUSIONS: Lifelong and acquired premature ejaculation patients deteriorate their quality of life: the deterioration in these patients' quality of life also negatively affects their depression and anxiety states.

ABSTRACT

Abstract: It has learned that soybean is being affected by a floral disorder known as floral malady where plants fail to develop pod and do notattendfull maturity. For this floral disorder, we present a new methylation-sensitive amplified polymorphism (MSAP) approach for the evaluation of relative quantitative characteristics of non-methylation, hyper-methylation, hemi-methylation, and full methylation status of CCGG sequences, which are recognized by the isoschizomers HpaII and MspI. We applied a technique to analyze alterations in the cytosine methylation a popular Indian soybean (Glycine max L.) genotype, JS-335.The result revealed that in the symptomatic plant, out of 392 MSAP sites, 281 (71.68%), 33 (8.41%),38 (9.69%), and 40(10.20%) found to beun-methylated, hemi-methylated, fully methylated and hyper-methylated, respectively. Whereas, the MSAP profile of asymptomatic plants revealedout of 402MSAP sites, 330 (81.28%) was un-methylated, 22(5.41%) hemi-methylated,29(7.14%) fully methylatedand 25 (6.15%) hypermethylated. In comparison with asymptomatic(18.71%) plant, approximately 10% increased methylation was noted in symptomatic(28.31%) plantprofiles. The increased levels of methylation was recorded in the symptomatic plants about 28.31%and18.71% in asymptomatic. The study showed a higher epigenetic influence on JS-335 genotype of floral malady symptomatic than same genotype of asymptomatic plant. No pod formation in symptomatic plant induce genome wide changes either in promoter or coding region of gene(s) and DNA fragments showing polymorphism related to differences in pattern and extent of methylation associated with floral malady.

ABSTRACT

The case of a 16-year old Jamaican girl who presented to the psychiatric service of a general hospital with features of Capgras syndrome is presented. Her history, treatment, progress and relevant psychodynamic and neurocognitive issues are explored. This is the first known published case of an adolescent with Capgras syndrome from the Caribbean. The case highlights that the syndrome may occur in different cultural contexts and that clinicians should be sensitive to its existence in order to avert under-diagnosis or misdiagnosis.


Se presenta el caso de una joven jamaicana de 16 años que acudió al servicio de psiquiatría de un hospital general con los rasgos del síndrome de Capgras. Se explora su historia, tratamiento, progreso, problemas neurocognitivos y psicodinámicos relevantes. Éste es el primer caso publicado de que se tenga noticias, de una adolescente con el síndrome de Capgras en la región del Caribe. El caso pone de relieve que el síndrome puede ocurrir en diferentes contextos culturales, y que los médicos deben de estar sensibilizados con su existencia a fin de evitar un subdiagnóstico o un diagnóstico erróneo.

OBJECTIVE: Current epidemiological research indicates that HIV/AIDS endures and continues to be a significant vulnerability among adolescents and youths despite the increased access to antiretroviral drugs and the reduction in the global progression of the disease. This study examined the association between substance use and psychological distress within the Jamaican population of youths coping with the illness. METHOD: This is a cross-sectional survey that utilized a correlational design. The sample population consisted of 62 youths, age range 15-25 years, living with HIV/AIDS. Sociodemographic information was gathered through interviews and self-report scales were used to measure depression, anxiety, stress and substance use. Chi-square was used to assess the relationship between the variables under study: psychological distress and substance use. RESULTS: More than half the sample were heterosexuals who contracted the virus through consensual intercourse. The average age of respondents was 21.29 years and slightly more than half were female (56.5%). The majority of respondents were single (54.8%), unemployed (73%), heterosexual (69.4%) youths with a secondary level education (63%). There was a statistically significant relationship between psychological distress and substance use (χ2 = 7.3959, df = 3, p = 0.047). CONCLUSION: The emotional needs of youths living with HIV/AIDS are just as important as their medical needs.

OBJETIVO: La investigación epidemiológica actual indica que el VIH/SIDA perdura y sigue siendo un aspecto de vulnerabilidad significativa entre los adolescentes y jóvenes, a pesar del aumento del acceso a medicamentos antirretrovirales y la reducción de la progresión global de la enfermedad. Este estudio examinó la asociación entre el uso de sustancias y el distrés psicológico dentro de la población jamaicana de jóvenes que luchan con la enfermedad. MÉTODO: Este es un estudio transversal que utilizó un diseño correlacional. La muestra poblacional consistió de 62 jóvenes, años rango 15-25 años viven con el VIH/SIDA. Se recogió información sociodemográfica a través de entrevistas y se usaron escalas de autoreporte para medir la depresión, la ansiedad, el estrés, y el uso de sustancias. El chi-cuadrado fue utilizado para evaluar la relación entre las variables objeto de estudio: el distrés psicológico y el uso de sustancias. RESULTADOS: Más de la mitad de la muestra estuvo formada por heterosexuales que contrajeron el virus a través de relaciones sexuales consensuales. La edad promedio de los encuestados fue 21.29 años, y poco más de la mitad eran mujeres (56.5%). La mayoría de los encuestados eran solteros (54.8%), desempleados (73%), jóvenes heterosexuales (69.4%) con una educación de nivel secundario (63%). Hubo una relación estadísticamente significativa entre el distrés psicológico y el uso de sustancias (χ2 = 7.3959, df = 3, p = 0.047). CONCLUSIÓN: Las necesidades emocionales de los jóvenes que viven con el VIH/SIDA son tan importantes como sus necesidades médicas.

ABSTRACT

The goals of this study are to evaluate the impact of a distressed environment on self-esteem in Jamaican adolescent girls and to assess the impact of behavioural patterns on self-esteem. Participants were African-Jamaicans ages 13-17 years from a Place of Safety (experimental group) and local high schools (control group). All participants completed the Rosenberg Self-Esteem Scale (RSES) while the experimental group also completed the Child Behaviour Checklist (CBCL). Participants in the experimental group were classified as either internalizers (depressed, anxious and somatizing), externalizers (exhibiting aggressive, disruptive behaviours), both (externalizers and internalizers) or other (reporting no or very few symptoms). It was hypothesized that the participants residing in the Place of Safety would report lower levels of self-esteem in comparison to the control group. Secondly, the 'both' group would report lower levels of self-esteem than any other group. Results indicated no differences in self-esteem scores; however, statistical significance was found between 'externalizers' and 'both' groups (p = 0.006). This study challenges the assumptions of self-esteem and its relationship to behavioural and emotional problems.


El objetivo de este estudio fue evaluar el impacto de un medio de estrechez sobre la autoestima de las adolescentes jamaicanas y examinar el impacto de los patrones de conducta sobre su autoestima. Las participantes fueron afro-jamaicanas de 13-17 años de edad de un Lugar de Seguridad (grupo experimental) y de escuelas secundarias locales (grupo control). Todas las participantes completaron la Escala de Autoestima de Rosenberg (RSES), mientras que el grupo experimental también completó el Inventario de Comportamiento Escolar (IDCE). Las participantes en el grupo experimental fueron clasificadas bien como internalizadoras (las deprimidas, ansiosas y somatizantes), bien como externalizadoras (las que presentaban conductas agresivas, disociadoras), como ambas (internalizadoras y externalizadoras) u otras (aquellas que reportaban muy pocos o ningún síntoma). Se planteó la hipótesis de que las participantes que residían en el Lugar de Seguridad reportarían niveles más bajos de autoestima en comparación con el grupo control. En segundo lugar, los grupos de "ambas" reportarían niveles más bajos de autoestima que cualquier otro grupo. Los resultados no indicaron ninguna diferencia en las puntuaciones de autoestima. Sin embargo, se halló un dato estadístico significativo entre los grupos de "externalizadoras" y los de "ambas" (p = 0.006). Este estudio cuestiona las suposiciones sobre la autoestima y su relación con los problemas conductuales y emocionales.

ABSTRACT

Este trabajo presenta el caso clínico de una paciente de 21 años, quien consultó por dolor abdominal y edema en miembros inferiores, 3 meses de evolución; se acompaño de perdida de peso (10 Kg). Al ingreso se presentó en regulares condiciones generales, disneícas, con palidez muco-cutánea moderada y tumoración lumbo-abdominal izquierda. El diagnóstico de ingreso fue tumor suprarenal izquierdo invadiendo la vena cava inferior y la aurícular derecha. El tratamiento fue nefroadrenalectomía izquierda y trombectomía de la vena cava inferior y la aurícula derecha (con bomba extracorpórea) que murió en su post-operatorio inmediato durante su reintervención quirúrgica. Es importante resaltar que la presencia de un trombo tumoral en la vena inferior, es un factor que no debería ser considerado como contraindicación para la resección tumoral con fines curativos

ABSTRACT

En pacientes inmunocomprometidos especialmente diabéticos o trasplantados renales; la presencia de imágenes de defecto en el tracto urinario superior y obstrucción de defecto en el tracto urinario superior y obstrucción del mismo nos debe llamar la atención, pues se trata de un cuadro de acúmulos de hongos (fungus ball). De hecho los urólogos debemos esperar más de estos casos, dado que cada vez atendemos más pacientes críticamente enfermos. La fragilidad de los mismos requieren intervención médica y endourológica adecuada. El éxito depende del abordaje combinado ya que se ha demostrado que ni el tratamiento médico o el quirúrgico por sí sólo son suficientes

La enfermedad oclusiva hepática es una patología poco frecuente con una alta mortalidad y de tratamiento médico quirúrgico, dependiendo de su etiología; con manifestaciones clínicas del dolor abdominal, ascitis y hepatomegalia. Presentamos el caso de una paciente de 34 años, con las manifestaciones clínicas antes descritas, su evolución, exámenes realizados para determinar el sitio de la lesión y su etiología, con la finalidad de distinguir entre síndrome de Budd-Chiari (SBC) y enfermedad veno oclusiva hepática.

Continuación describiremos un paciente de 68 años quien posterior a trauma espinal presenta lumbalgia de muy fuerte intensidad; el examen físico y estudios paraclínicos orientaron hacia una LOE, a nivel de L4-L5 y el hallazgo operatorio fue el de un Hematoma Subdural Subagudo, siendo su evolución posterior buena. A continuación haremos una revisión de la bibliografía por ser una patología poco frecuente
