ABSTRACT

Objetivo: Bolivia enfrenta serios problemas en la prevención secundaria del cáncer cervicouterino. Este estudio tiene por objetivo evaluar la eficacia y concordancia de los métodos de diagnóstico en la prevención secundaria del cáncer de cuello uterino para detectar lesiones cervicales intraepiteliales de alto grado. Métodos: sesenta y dos pacientes con una citología alterada o una prueba VPH-ar positiva complementada obligatoriamente con una citología, fueron sometidas a una colposcopia y biopsia dirigida. Aquellas pacientes con diagnósticos histopatológicos de NIC2+ en la biopsia colposcópica recibieron el tratamiento escisional correspondiente, obteniéndose muestras de tejido para su análisis histopatológico (biopsias escisionales). Los resultados de la citología e impresión colposcópica fueron comparados con los resultados histopatológicos de la biopsia colposcópica. Finalmente, los resultados histopatológicos de NIC2+ de la biopsia colposcópica fueron comparados con los resultados de la biopsia escisional. Resultados: la sensibilidad de la citología y la impresión colposcopia para detectar NIC 2+ fue de 31,43% y 80% respectivamente. La concordancia (Índice Kappa) de los resultados de la citología y la impresión colposcópica comparadas con los resultados NIC 2+ de la biopsia colposcópica fue 0,15 (leve) y 0,43 (moderado) respectivamente. Finalmente, la comparación entre los resultados histopatológicos de la biopsia colposcópica (NIC2+) y de la biopsia escisional dio una coincidencia del 68%. Conclusiones: la citología tuvo una baja eficacia y concordancia para detectar NIC 2+. La colposcopia mejora la identificación de lesiones subyacentes NIC 2+ en pacientes con citologías iguales o menores a LIE-BG.


Objectives: Bolivia faces serious problems in cervical cancer secondary prevention. This study aims to evaluate the effectiveness and concordance of diagnostic methods in the secondary prevention of cervical cancer to detect high-grade cervical intraepithelial lesions. Methods: sixty-two patients with altered cytology or a positive HR-HPV test, compulsorily complemented by cytology, underwent colposcopy and targeted biopsy. Those patients with histopathological diagnoses of CIN2+ in the colposcopic biopsy received the corresponding excisional treatment, obtaining tissue samples for histopathological analysis (excisional biopsies). The results of the cytology and colposcopy impression were compared with the histopathological results of the colposcopic biopsy. Finally, the histopathological results of CIN2+ from the colposcopic biopsy were compared with the results from the excisional biopsy. Results: the sensitivity of cytology and colposcopy impression to detect CIN 2+ was 31.43% and 80% respectively. The agreement (Kappa Index) of the results of cytology and colposcopic impression compared with the CIN 2+ results of colposcopic biopsy was 0.15 (mild) and 0.43 (moderate) respectively. Finally, the comparison between the histopathological result of the colposcopic biopsy and the excisional biopsy gave a simple percentage coincidence of 68%. Conclusions: cytology had low efficacy and concordance to detect CIN 2+. Colposcopy improves the identification of underlying CIN 2+ lesions in patients with cytology equal to or less than LIE-BG.

Previous studies by us demonstrated that the consumption of thermally oxidized oil diet adversely affects body growth, lipid metabolism, bone mass and femur biomechanical competence. AIM: The aim of this study was to evaluate the effects of a diet containing fried sunflower oil on the mandible of growing rats. MATERIALS AND METHOD: Male Wistar rats (21±1 day old) (n=21) were assigned at weaning to one of three diets for 8 weeks: a control diet (C), a diet containing sunflower oil (SFO) or a diet containing sunflower oil that had been repeatedly heated (SFOx); both SFO and SFOx were mixed with commercial rat chow at 13% (w/w). The consistency and viscosity of the 3 diets were similar. Zoometrics and food intake were recorded weekly. At wk=8, mandibular growth was assessed by measurements of anatomical points of cleaned bones, and mandible biomechanical competence was assessed to estimate the structural properties of the bone. Statistical analysis was performed by SPSS v. 20.0. RESULTS: Rats fed SFOx diet attained the lowest final body weight (P=0.0074), mandibular weight (P=0.0001) and mandibular \length (P=0.0002). Load bearing capacity (Wf;N), load of yielding (Wy;N) and stiffness (Wy/dy;N/mm) of the mandible were negatively affected by both sunflower oil diets (fresh and fried) (P=0.001; P=0.002; P=0.003, respectively) though SFOx induced the most significant reduction in Wy/dy (C:44.4(5.4) > SFO:36.1(2.1) > SFOx: 26.3(3.7) N/ mm; P=0.003). The deleterious effect of SFOx on mandibular growth was more accentuated on the posterior part of the bone (C:11.4(0.3)=SFO:11.2(0.2)>SFOx: 10.7(0.2) mm; p=0.0005); the anterior/ posterior ratio (C:1.25(0.02)=SFO:1.27(0.02)

En estudios previos hemos demostrado los efectos adversos del consumo de una dieta rica en aceite termooxidado sobre el crecimiento corporal, el metabolismo de los lípidos, la masa ósea y la competencia biomecánica del fémur. OBJETIVO: El objetivo de este trabajo fue investigar el efecto de una dieta rica en aceite de girasol termooxidado (AGX) sobre los parámetros morfométricos y biomecánicos de la mandíbula de rata en crecimiento. Materiales y Método: Ratas macho Wistar de 22±1 días de edad (n=21) recibieron durante 8 semanas una de 3 dietas: control (C); dieta comercial, una dieta suplementada con aceite de girasol (AG) y una dieta suplementada con AGX. La consistencia y la viscosidad de las dietas fueron similares. Los parámetros zoométricos y el consumo de dieta se registraron semanalmente. A T=8, los animales se eutanasiaron y se obtuvieron las hemimandíbulas. El crecimiento mandibular se estimó por medidas morfométricas entre puntos anatómicos y las propiedades estructurales por biomecánica. El análisis estadístico se realizó por SPSS v. 20.0. RESULTADOS: Las ratas alimentadas con AGX presentaron menor peso corporal final (p=0.0074), peso mandibular (p=0.0001) y longitud mandibular (p=0.0002). Las propiedades estructurales de la mandíbula, Wf (p=0.001), Wy (p=0.002) y Wy/dy (p=0.003), se vieron afectadas negativamente en ratas alimentadas con AG o AGX, respecto a C; pero la rigidez ósea (Wy/dy) en AGX fue significativamente menor (C:44.4(5.4) > SFO:36.1(2.1) > SFOx: 26.3(3.7) N/mm; p=0.003). This case series included a tomographic, microbiological, and histopathological description of 15 secondary apical periodontitis (SAP) lesions obtained by apical microsurgery performed in 10 patients to better understand the etiology and pathogenesis of SAP. Preoperative tomographic analyses were performed through Cone beam computerized tomography - Periapical index (CBCT-PAI), and apical microsurgeries were then carried out. The removed apices were used for microbial culturing and for molecular identification using PCR for the detection of 5 strict anaerobic bacteria (P. gingivalis, P. intermedia, P. nigrescens, T. forsythia, and T.denticola) and 3 viruses Herpes simplex viruses (HSV), Cytomegalovirus (CMG) and Epstein-Barr Virus (EBV) by nested PCR. The removed apical lesions were histologically described. Univariate statistical analyses were performed by using STATA MP/16 (StataCorp LLC, College Station, TX, United States). CBCT-PAI analyses revealed PAI 4 and PAI 5 score lesions that involved cortical plate destruction. Eight SAPs were positive by culture, while nine SAP lesions were positive by PCR. Fusobacterium species were the most frequently cultured organisms in 7 SAP lesions, followed by D. pneumosintes in 3. In contrast, by single PCR, T. forsythia and P. nigrescens were detected in 5 lesions, T. denticola in 4 lesions, and P. gingivalis in 2 lesions. Twelve periapical lesions were granulomas, and the remaining three SAP lesions were radicular cysts. In conclusion, this case series study revealed that secondary apical lesions presented tomographic involvement of PAI 3 to 5, and that most SAP lesions were apical granulomas containing anaerobic and facultative microorganisms.

Resumo Esta série de casos incluiu uma descrição tomográfica, microbiológica e histopatológica de 15 lesões de periodontite apical secundária (SAP) obtidas por microcirurgia apical realizada em 10 pacientes para melhor compreender a etiologia e patogénese do SAP. As análises tomográficas pré-operatórias foram realizadas através de tomografia computadorizada de feixe cônico - índice Periapical (CBCT-PAI), e as microcirurgias apicais foram então realizadas. Os ápices removidos foram utilizados para a cultura microbiana e também para a identificação molecular por PCR para a detecção de 5 bactérias anaeróbias rigorosas (P. gingivalis, P. intermedia, P. nigrescens, T. forsythia, e T.denticola) e 3 vírus Herpes simplex (HSV), Cytomegalovirus (CMG) e Epstein-Barr Virus (EBV) por PCR aninhada. As lesões apicais removidas foram descritas histologicamente. Foram realizadas análises estatísticas univariadas utilizando STATA MP/16 (StataCorp LLC, College Station, TX, Estados Unidos da América). As análises CBCT-PAI revelaram lesões PAI 4 e PAI 5 que envolveram a destruição da placa cortical. Oito SAPs foram positivos por cultura, enquanto nove lesões de SAP foram positivas por PCR. As espécies de Fusobacterium foram os organismos mais frequentemente cultivados em 7 lesões SAP, seguidas por D. pneumosintes em 3. Em contraste, por PCR simples, T. forsythia e P. nigrescens foram detectados em 5 lesões, T. denticola em 4 lesões, e P. gingivalis em 2 lesões. Doze lesões periapicais foram granulomas, e as restantes três lesões SAP foram cistos. Em conclusão, este estudo de série de casos revelou que as lesões apicais secundárias apresentavam envolvimento tomográfico de PAI 3 a 5, e que a maioria das lesões de SAP eram granulomas apicais contendo microrganismos anaeróbios e facultativos.

ABSTRACT

About the neurological manifestations of COVID-19, smell impairment with some distortion or parosmia is currently one of the longest-lasting post-covid sequelae affecting many young adults around the world, we present a case of a 25yo patient that after the COVID-19 infection initiates with anosmia and dysgeusia of difficult control and some weeks later begins with parosmia associated to general discomfort and vomiting preceded by nausea, accompanied by poor oral intake, generating weight loss in 3 weeks, and depression probably related to the clinical profile and the confinement due to the pandemic. She was treated with prednisone, gabapentin, nasal irrigations and olfactory rehabilitation, with mild improvement, which was greater after the second dose of covid-19 vaccine.

ABSTRACT

Staphylococcus aureus y Staphylococcus epidermidis son los principales agentes etiológicos de las conjuntivitis bacterianas, que al tratarse con antibióticos de manera empírica, incrementan la resistencia antimicrobiana después de exposiciones repetidas. Se están buscando alternativas naturales para el tratamiento de infecciones bacterianas autolimitadas de la conjuntiva. Objetivo: determinar la actividad antimicrobiana de ocho extractos de las plantas frente a bacterias aisladas de pacientes con conjuntivitis bacterianas. Materiales y métodos: se tomaron muestras de 15 pacientes con conjuntivitis bacterianas. Se cultivaron en agar sangre y chocolate durante 24 h a 37 °C y se identificaron mediante el sistema automatizado vitek y pruebas de susceptibilidad antimicrobiana por el método de Kirby-Bauer. A cada aislamiento identificado con el género Staphylococcus se le evaluó su susceptibilidad frente a siete extractos: Ocimum basilicum, Sambucus nigra L., Delphinium elatum, Calendula officinalis, Bixa ore-llana (parte aérea y fruto independiente), Clinopodium brownei y Laurus nobilis, con un uso tradicional reportado para el tratamiento de infecciones oculares. Resultados: las bacterias aisladas con más frecuencia fueron S. epidermidis, S. hominis y S. aureus, las cuales presentaron resistencia antimicrobiana a oxacilina, tetraciclinas y eritromicina. Todos los aislamientos fueron inhibidos por los extractos de O. basilicum (cmi: >0.9 mg/mL) y L. nobilis (cmi: hasta 15 mg/mL). Conclusión: los extractos de C. officinalis y D. elatum tuvieron actividad antimicrobiana solo frente a los aislados con mayor sensibilidad antimi-crobiana. Los extractos etanólicos de O. basilicum y L. nobilis pueden ser una alternativa de tratamiento de las infecciones de la conjuntiva.


Staphylococcus aureus and Staphylococcus epidermidis are the primary etiological agents of bacterial conjunctivitis which are empirically treated with antibiotics. This results in an increase in antimicrobial resistance due to repeated exposure. Currently, natural treatment alternatives are being sought for self-limited bacterial infections of the conjunctiva. Objective: To determine the antimicrobial activity of eight extracts from Colombian plants against bacteria isolated from patients with bacterial conjunctivitis. Materials and methods: Samples were taken from 15 patients with bacterial conjunctivitis which were grown on blood and chocolate agar for 24 h at 37 °C. These samples were identified by the vitek automated system and antimicrobial susceptibility tests by the Kirby Bauer method. Each isolate identified with the genus Staphylococcus was evaluated for susceptibility to the following eight plant extracts of seven plant: Ocimum basilicum (basil), Sambucus nigra L. (elderberry), Delphinium elatum(belladonna), Calendula officinalis (marigold), Bixa orellana (annatto) (aerial part and independent fruit), Clinopodium brownei (pennyroyal), and Laurus nobilis (laurel), with traditional use previously reported for treating eye infections. Results: The most frequently isolated bacteria were S. epidermidis, S. hominis, and S. aureus, which exhibited antimicrobial resistance mainly to oxacillin, tetracyclines, and erythromycin. All isolates were inhibited by O. basilicum extracts (mic > 0.9 mg/mL) and L. nobilis (mic < 15 mg/mL). Conclusion: The extracts of C. officinalis y D. elatum showed antimicrobial activity only against isolates with higher antimicrobial sensitivity. Ethanolic extracts of O. basilicum y L. nobilis can be used as an alternative treatment for infections of the anterior segment of the eye.

Atualmente, estão sendo estudadas alternativas naturais para o tratamento de infecções bacterianas autolimitadas da conjuntiva. Objetivo: determinar a atividade antimicrobiana de oito extratos de sete vegetais contra bactérias isoladas de pacientes com conjuntivite bacteriana. Materiais e métodos: foram retiradas amostras de 15 pacientes com conjuntivite bacteriana. As amostras foram cultivadas em ágar sangue e ágar chocolate por 24 horas a 37°C e os isolados foram identificados pelo sistema automatizado vitek, além de testes de susce-tibilidade antimicrobiana pelo método Kirby Bauer. Cada isolado identificado como sendo pertencente ao gênero Staphylococcus foi avaliado quanto à suscetibilidade a oito extratos vegetais: Ocimum basili-cum (manjericão), Sambucus nigra L. (sabugueiro), Delphinium elatum (belladona), Calendula officinalis(calêndula), Bixa orellana (urucum; parte aérea e fruto independente), Clinopodium brownei (poejo) e Laurus nobilis (louro), anteriormente relatados como uso tradicional para o tratamento de infecções ocu-lares. Resultados: as bactérias mais frequentemente isoladas foram S. epidermidis, S. hominis e S. aureus, que apresentaram resistência antimicrobiana principalmente à oxacilina, tetraciclinas e eritromicina. Todos os isolados foram inibidos por extratos de O. basilicum (cim: >0,9 mg/mL) e L. nobilis (cim: até 15 mg/mL). Conclusão: os extratos de C. officinalis e D. elatum apresentaram atividade antimicrobiana apenas contra os isolados com maior sensibilidade antimicrobiana. Os extratos etanólicos de O. basilicum e L. nobilis podem ser uma alternativa de tratamento para infecções conjuntivais.

ABSTRACT

OBJECTIVES: To describe the design process of a medical care program for adolescents with pediatric onset rheumatic diseases (PRD) during the transition from pediatric to adult care in a resource-constrained hospital. METHODS: The model of attention was developed in three steps: 1) the selection of a multidisciplinary team, 2) the evaluation of the state of readiness of patients and caregivers for the transition, and 3) the design of a strategy of attention according to local needs. The results of the first two steps were used in order to develop the strategy of attention. RESULTS: The transition process was structured in three stages: pretransition (at pediatric rheumatology clinic), Transition Clinic for Adolescents with Rheumatic Diseases (TCARD, the main intervention), and post-transition (at adult rheumatology clinic). Each stage was divided, in turn, into a variable number of phases (8 in total), which included activities and goals that patients and caregivers were to accomplish during the process. A multidisciplinary approach was planned by pediatric and adult rheumatologists, nutritionists, physiatrists, psychiatrist, psychologist, nurse, and social worker. During TCARD, counseling, education, nutritional, physical, and mental health interventions were considered. CONCLUSIONS: The proposed transition model for patients with rheumatic diseases can be a useful tool in developing countries.

La tercera causa de morbi-mortalidad en muchos países, es el accidente cerebrovascular (ACV), la incidencia es mayor en los pacientes con hemodiálisis. La presencia del ACV hemorrágico se vincula con peores resultados; el diagnóstico y tratamiento precoces son primordiales por la rápida expansión de la misma, que provoca un detrimento neurológico, hasta un desenlace fatal. Se presenta el caso de una mujer de 52 años, ingresa por servicio de emergencia en fecha 23/05/23 con antecedente de terapia de reemplazo renal tri-semanal, hipertensión arterial, diabetes mellitus; cuadro clínico con cefalea súbita, de moderada intensidad, acompañado de somnolencia, entumecimiento a nivel de columna cervicodorsal, paraparesia en extremidades inferiores e hipertensión; tomografía de cráneo con ACV hemorrágico pre tallo cerebral; ingresa a la Unidad de Terapia Intensiva (UTI) para monitorización y manejo, sin embargo a pesar del tratamiento evoluciono de manera tórpida y fallece en fecha 11/06/2023.


The third cause of morbidity and mortality in many countries is cerebrovascular accident (CVA), the incidence is higher in hemodialysis patients. The presence of hemorrhagic stroke is associated with worse results; early diagnosis and treatment are essential due to its rapid expansion, which causes neurological detriment, up to a fatal outcome. The case of a 52-year-old woman is presented, admitted for emergency service on 05/23/23 with a history of three-weekly renal replacement therapy, arterial hypertension, diabetes mellitus; clinical picture with sudden headache of moderate intensity, accompanied by drowsiness, numbness at the level of the cervicodorsal spine, paraparesis in the lower extremities and hypertension; skull tomography with pre-brain stem hemorrhagic stroke; he was admitted to the Intensive Care Unit (ICU) for monitoring and management, however despite the treatment he evolved torpidly and died on 06/11/2023.

Introducción: las radiaciones ionizantes (RI) pueden inducir la formación de micronúcleos (MN). La frecuencia de MN se utiliza como biomarcador de daño genético inducido por (RI). Objetivo: evaluar el daño al ADN resultante de la exposición ocupacional a RI en personal de clínicas veterinarias o afines. Metodología: se utilizó el ensayo de micronúcleos con bloqueo de la citocinesis (MNBC) para comparar la frecuencia observada del biomarcador en 40 individuos expuestos ocupacionalmente a RI con respecto a un grupo control de 32 participantes, ambos grupos pertenecen a personal veterinario. Además, se registraron variables demográficas, de estilo de vida y ocupacionales que pudieran influir en la formación de MN. Resultados: el análisis univariado no registró diferencias significativas en la frecuencia de MN entre los grupos de estudio (p=0,118). Mediante análisis multivariado se obtuvo que aproximadamente un 27% (R2 ajustado= 0,269) de la variabilidad de la frecuencia de MN puede explicarse por la influencia conjunta de la edad, el sexo y el número de radiografías realizadas por el individuo. La edad es la variable de mayor importancia relativa (β = 0,504), seguida del sexo del participante (β = -0,316) y el número de radiografías realizadas por día (β = 0,214). Conclusiones: La frecuencia de MN tiende a aumentar en mujeres, a medida que aumenta la edad del participante y a mayor número de radiografías realizadas.


Introduction: Ionizing radiation (RI) can induce the formation of micronuclei (MN). The formation of MN is used as a biomarker of radiation-induced genetic damage. Objective: assess DNA damage resulting from occupational exposure to RI in veterinary personnel. Methodology: the cytokinesis-block micronucleus assay (MNBC) was used to compare the observed frequency of MN in 40 individuals occupationally exposed to ionizing radiation with respect to a control group of 32 participants, both groups belonging to veterinary personnel. In addition, demographic, lifestyle and occupational variables that could influence the formation of MN were recorded. Results: univariate analysis did not show significant differences in the frequency of MN between the study groups (p=0.118). Using multivariate analysis, it was found that approximately 27% (adjusted R2= 0.269) of the variability in the frequency of MN can be explained by the joint influence of age, sex and the number of radiographic images performed by the participant. Age is the variable with the greatest relative importance (β = 0.504), followed by the sex of the participant (β = -0.316) and the number of X-rays performed per day (β = 0.214). Conclusions: the frequency of MN tends to increase in women, as the participant's age increases and as the number of radiographic images performed increases.

El hombro es una de las regiones anatómicas de mayor movilidad en la vida cotidiana, siendo una de las causas de consulta más frecuentes en el área de fisioterapia para su rehabilitación, pues la primera línea de acción es el tratamiento conservador del hombro; por ello, es de suma importancia conocer y evaluar el complejo articular del hombro, así como también de la región cervicotorácica, con los resultados de la valoración establecer una estrategia de tratamiento que pueden ir desde la terapia manual, el masaje terapéutico, cambios de temperatura hasta el empleo de otros agentes físicos. Paciente femenino de 42 años, con ocupación de asistente odontológica; no reporta discapacidad previa. Como antecedente médico se presenta accidente de tránsito en motocicleta sin producir fractura de hueso ni luxación, ocurrido ocho años antes de la consulta en fisiatría, la paciente tiene afectado el desarrollo de las actividades de la vida cotidiana. Recibió serie de tratamientos durante 10 sesiones, en las cuales se aplican agentes físicos, dado que anteriormente recibió tratamiento farmacológico sin resultados favorables. Se aplica protocolo de rehabilitación fundamentado en las técnicas de propiocepción y al finalizar la terapia la paciente reporta dolor leve, y mejora en la realización de actividades de la vida diaria.


The shoulder is one of the anatomical regions of greater mobility in daily life, being one of the most frequent causes of consultation in the area of physiotherapy for rehabilitation, since the first line of action is the conservative treatment of the shoulder; therefore, it is of utmost importance to know and evaluate the articular complex of the shoulder, as well as the cervicothoracic region, with the results of the assessment to establish a treatment strategy that can range from manual therapy, therapeutic massage, temperature changes to the use of other physical agents. Female patient, 42 years old, with occupation as a dental assistant; she reports no previous disability. As medical history, she had a traffic accident on a motorcycle without bone fracture or dislocation, which occurred eight years before the physiatry consultation, the patient has affected the development of activities of daily living. She received a series of treatments during 10 sessions, in which physical agents are applied, since she had previously received pharmacological treatment without favorable results. Rehabilitation protocol based on proprioception techniques is applied and at the end of therapy the patient reports mild pain and improvement in the performance of activities of daily living.


O ombro é uma das regiões anatômicas de maior mobilidade na vida diária, sendo uma das causas mais freqüentes de consulta na área de fisioterapia para reabilitação, pois a primeira linha de ação é o tratamento conservador do ombro; portanto, é extremamente importante conhecer e avaliar o complexo articular do ombro, bem como a região cervicotorácica, com os resultados da avaliação para estabelecer uma estratégia de tratamento que pode variar desde a terapia manual, massagem terapêutica, mudanças de temperatura até o uso de outros agentes físicos. Paciente do sexo feminino, 42 anos de idade, trabalhando como assistente odontológica; nenhuma deficiência anterior foi relatada. O histórico médico inclui um acidente de trânsito em uma motocicleta sem fratura ou deslocamento ósseo, que ocorreu oito anos antes da consulta ao fisiatra; as atividades de vida diária do paciente são afetadas. Ela recebeu uma série de tratamentos durante 10 sessões, nas quais foram aplicados agentes físicos, já que ela havia recebido anteriormente tratamento farmacológico sem resultados favoráveis. Um protocolo de reabilitação baseado em técnicas de propriocepção foi aplicado e ao final da terapia o paciente relatou dor leve e melhora no desempenho das atividades da vida diária.

(1) Background: The interaction between single nucleotide variants (SNVs) associated with congenital heart diseases (CHDs) and their gene methylation status has not been well researched. The aim of the present study was to determine if there is a relationship between the methy lation status (MS) of genes and the allelic variants associated with CHDs. (2) Methods: Seven SNVs of the genes AXIN1, TBX1, TBX20, and MTHFR were selected from the literature. DNA extraction, genotyping, and a methylation analysis were performed on healthy subjects and subjects with CHDs. (3) Results: Twenty-two subjects with CHDs were selected as the case group (15 with ventricular septal defects (VSDs) and 7 with atrial septal defects (ASDs)), and 44 healthy subjects comprised the control group. The MTHFR and AXIN1 genes were hypermethylated in the control group when compared to the case group. When analyzed separately, those with atrial septum defects exhibited greater methylation, except for the gene MTHFR where there were no differences. Only the alternate alleles of MTHFR showed a significantly different methylation status in those without cardiopathy. (4) Conclusions: The MTHFR and AXIN genes were hypermethylated in the control group; however, only the alternate alleles of MTHFR (rs1801133 and rs1801131) showed a significantly different methylation status.

ABSTRACT

Fluorescence microscopy is an important tool for disease diagnosis, often requiring costly optical components, such as fluorescence filter cubes and high-power light sources. Due to its high cost, conventional fluorescence microscopy cannot be fully exploited in low-income settings. Smartphone-based fluorescence microscopy becomes an interesting low-cost alternative, but raises challenges in the optical system. We present the development of a low-cost inverted laser fluorescence microscope that uses a smartphone to visualize the fluorescence image of biological samples. Our fluorescence microscope uses a laser-based simplified optical filter system that provides analog optical filtering capabilities of a fluorescence filter cube. Firstly, we validated our inverted optical filtering by visualizing microbeads labeled with three different fluorescent compounds or fluorophores commonly used for disease diagnosis. Secondly, we validated the disease diagnosis capabilities by comparing the results of our device with those of a commercial fluorescence microscope. We successfully detected and visualized Trypanosoma cruzi parasites, responsible for the Chagas infectious disease and the presence of Antineutrophil cytoplasmic antibodies of the ANCA non-communicable autoimmune disease. The samples were labeled with the fluorescein isothiocyanate (FITC) fluorophore, one of the most commonly used fluorophores for disease diagnosis. Our device provides a 400× magnification and is at least one order of magnitude cheaper than conventional commercial fluorescence microscopes.

BACKGROUND: Timely detection of cervical cells infected with high-risk human papillomavirus (HPV) improves cervical cancer prevention. In Bolivia, actual screening coverage only reaches 33.3% of the target population aged between 25 and 64 years despite free cytology screening. Furthermore, 50% to 80% screened women are lost during follow-up. This study aimed at identifying factors explaining this lack of follow-up care. METHOD: During the first phase, face-to-face semi-structured interviews were conducted with HPV-positive women. Secondly, we explored the reasons for the non-adherence to the follow-up care: knowledge, perceptions and beliefs about HPV, as well as barriers to healthcare access, using a structured survey on Cochabamba women and healthcare professionals. RESULTS: Barriers to effective follow-up of the targeted populations were associated with health system shortcomings, including poor service delivery at the front- and second-line, health providers shortage, inadequate training, waiting time, high direct and indirect costs of care seeking and care, complex procedures to obtain HPV screening results and poor patient-provider communication. The follow-up was perceived as extremely stressful by the participants. CONCLUSION: Improved communication on HPV and HPV-related cancers in terms of representation in the general population and among the health professional's population is vital to improve access for HPV infection follow-up care.

Objetivo: Describir las aberraciones citogenéticas que pueden ser observadas por medio de la técnica Giemsa en fluorescencia y encontradas en pacientes con cáncer antes y después de ser sometidos a tratamiento con radioterapia. Métodos: Se analizó un mínimo de 200 metafases en primera división mitótica antes y después del tratamiento de radioterapia en nueve pacientes que asistieron a la sección de radioterapia del Hospital San Juan de Dios Costa Rica. En cada caso se contabilizó cada tipo de cromosomopatía por medio de la prueba de Giemsa en fluorescencia y utilizando bromodeoxiuridina y naranja de acridina. Resultados: Las cromosomopatías producidas por radioterapia se observaron tanto antes como después del tratamiento sin embargo destacó el incremento en la frecuencia de los cromosomas dicéntricos y anillos céntricos una vez finalizada la terapia. La frecuencia de fracturas cromatídicas de asociaciones satelíticas y de alteraciones morfológicas no se ve afectada por la radioterapia. Uno de los participantes presentó un recuento mitótico bajo. Conclusión: La radioterapia aumenta significativamente la frecuencia de los cromosomas dicéntricos y dicéntricos más anillos en la muestra en estudio. Este trabajo es relevante por ser el primer estudio en Costa Rica en el que se analizan los cromosomas dicéntricos como biomarcadores de exposición a radiaciones ionizantes mediante la prueba de Giemsa en fluorescencia y utilizando bromodeoxiuridina y naranja de acridina.


Aim: The objective of this study was to describe the before and after cytogenetic aberrations found in current patients of radiotherapy. This can be observed through the technique called "Giemsa in fluorescence" Methods: A minimum of 200 metaphases were analyzed in the first mitotic division in 9 patients. The patients where observed before and after radiotherapy treatment at the San Juan de Dios Hospital in Costa Rica. In each case any type of chromosomopathy was counted using the "Giemsa in fluorescence" test as well as Bromodeoxyuridine and acridine orange. Results: The chromosomopathies are observed before and after treatment with radiotherapy. The treatment seems to change the frecuency increasing the dicentric chromosomes and centric rings after the treatment. The frequency of chromatid fractures satellite associations and morphological alterations were not affected by radiotherapy. Conclusion: The chromosomopathies produced by radiotherapy were observed both before and after treatment with variations in their frequency. After radiotherapy dicentric chromosomes and dicentric chromosomes plus rings frequencies increased significantly. A low mitotic count was present this could have been the result of radiation on the bone marrow or by the cell repair and apoptosis system. The standardized " Fluorescence Plus Giemsa" test using Bromodesoxyuridine and acridine orange was used for the fiesta time in Costa Reica. This allowed for the measurement of radiation exposure used in the treatment or detection of diseases and cancer in pacients.

RESUMEN Las plantas medicinales son de interés farmacéutico, debido a que tienen diferentes propiedades biológicas, entre las que se destaca, la actividad antioxidante y, de la cual, se conoce que existe una relación con la concentración de los compuestos fenólicos, por su capacidad de reducir los radicales libres. Es así, que se propuso evaluar la actividad antioxidante y la cuantificación de fenoles y flavonoides totales, de 11 extractos etanólicos de plantas, empleadas en la medicina tradicional, para el tratamiento de infecciones urinarias (IU). Se evaluó la actividad antioxidante, por los métodos de DPPH y FRAP y se cuantificó el contenido fenólico y de flavonoides, por el método Folin-Ciocalteu y tricloruro de aluminio, respectivamente. Se encontró que, de los 11 extractos evaluados, los que presentaron el mayor contenido de estos metabolitos y de potencial antioxidante fueron: Uncaria tomentosa, Sedum praealtum y Kohleria hirsuta. Este trabajo permitió contribuir al conocimiento fitoquímico y antioxidante de plantas, usadas para tratar IU y se sugiere que U. tomentosa, S. praealtum y K. hirsuta sean objeto de futuros estudios biodirigidos, así como la evaluación de otras actividades biológicas.


ABSTRACT Medicinal plants are of pharmaceutical interest because they possess different biological properties, among which their antioxidant activity stands out, and therefore it is known that there is a relationship with the concentration of phenolic compounds due to their ability to reduce free radicals. Therefore, it was proposed to evaluate the antioxidant activity and the quantification of phenols and flavonoids of 11 ethanolic extracts of plants used in traditional medicine for the treatment of urinary infections (UI). The antioxidant activity was evaluated by the DPPH and FRAP methods and the phenolic and flavonoid content was quantified by the Folin-Ciocalteu and aluminum trichloride method, respectively. It was found that of the 11 extracts evaluated, those with the highest content of these metabolites and antioxidant potential were three: Uncaria tomentosa, Sedum praealtum and Kohleria hirsuta. This work made it possible to contribute to the phytochemical and antioxidant knowledge of the plants used for the treatment of UI and it is suggested that U. tomentosa, S. praealtum and K. hirsuta be the object of future bio-directed studies, as well as the evaluation of other activities biological.

ABSTRACT

Antimicrobial Resistance (AMR) is one of the main challenges of today's medicine because it has become a global problem that affects the treatment of multiple infections and impacts public health. This resistance is caused as the bacteria generate selective pressure-promoting mechanisms to evade the action of conventional drugs, which are also associated with adverse effects. Infections caused by these multi-resistant bacteria potentially reduce the possibility of effective therapy; this situation increases morbidity and mortality and treatment costs. Therefore, to establish combined therapy as a strategy for the control of infections caused by multi-resistant bacteria, a bibliographic search was carried out between 2015 and 2020 in databases such as PubMed, Scopus and Science Direct. The exhaustive review of the articles allowed a critical analysis of the information. Mechanisms were identified for obtaining drugs with antimicrobial potential, their biological activity and the possible effect of their combination against multidrug-resistant bacteria as an alternative for infectious disease control and as a response to reduce the use of antibiotics. Combined therapy is presented as an innovative therapeutic alternative, which uses non-antibiotic substances that can be obtained by three routes: the repositioning of drugs, synthetic substances and natural products. In this way, important elements are provided to guide researches that seek to reduce antimicrobial resistance.

BACKGROUND: Optic neuritis (ON) causes several sequela. Aggressive treatment with plasma exchange (TPE) is an option. This study describes improvement and safety outcomes with TPE. METHODS: We recruited adults with ON in neuromyelitis optica spectrum disorders (NMOSD) patients treated with TPE. The primary outcome was an improvement in the visual acuity scale (VOS). We described the data and used multivariate logistic regression to identify factors associated with response. RESULTS: Eighty-three patients received 558 TPE sessions. Mean age was 40.9 years (±13.7 years); 73.5% were women, 50.1% were first attack, and 10.7% were bilateral. Median VOS: 5 (range [R], 2-7). Median time between onset and TPE was 8 days (R, 1-32). By Keegan's criteria, 82.4% experience improvement and 78.3% improve in at least 1 point in VOS. Age and pre-TPE VOS were related to improvement. Low fibrinogen occurs in 26% sessions. CONCLUSION: TPE is effective and safety for ON in NMOSD patients. There is a need for a clinical trial using a therapeutic equivalent.

Leptospirosis is a zoonosis caused by Leptospiraspp., a spirochete that presents serovars diversity, whose prevalence, mode of transmission and circulation depend on the ecology and complex interrelation between humans, animals, and the environment in which they coexist. In animals, it affects reproduction causing economic losses and in terms of the environment, the evidence is limited. However, the survival of the bacteria is water favors; is for this, cases of leptospirosis increase with floods and rainfall. Despite its global distribution, epidemic potential, high human mortality rate, and socioeconomic burden, this zoonosis is neglected. Furthermore, worldwide zoonoses prioritization exercises based on the impact on agriculture, human and animal health have led to leptospirosis ranking among the most important zoonoses associated with poverty. This situation reflects the need for an integral management from the regulatory institutions of human, animal, and environmental health; but one main barrier of intersectionality is how the administration is designed for these events control. The current structures lead us to reflect and tend towards a holistic approach, seeking new forms of organization, new strategies to study, control and treat leptospirosis, the control of which is the responsibility of different sectors and disciplines. The comprehensive management of leptospirosis implies a higher level of understanding of the agent and of the biological, socioeconomic, and cultural risk factors in the regions and from a practical perspective, it is necessary to promote jointwork initiatives; as well as present evidence of the need for work from a "one health" perspective for a zoonosis that has become an emerging problem in public health.(AU)


A leptospirose é uma zoonose causada por Leptospiraspp., uma espiroqueta que apresenta diversidade de sorvares, cuja prevalência, modo de transmissão e circulação dependem da ecologia e da complexa inter-relação entre humanos, animais e o meio em que convivem. Em animais, afeta a reprodução causando prejuízos econômicos e em termos de meio ambiente as evidências são limitadas. Porém, a água favorece a sobrevivência da bactéria; portanto, enchentes e aumento das chuvas são fatores que têm sido associados ao aumento dos casos de leptospirose. Apesar de sua distribuição global, potencial epidêmico, alta taxa de mortalidade humana e carga socioeconômica, essa zoonose é negligenciada. Além disso, exercícios de priorização de zoonoses em todo o mundo com base no impactona agricultura, saúde humana e animal levaram a leptospirose a ser classificada entre as zoonoses mais importantes associadas à pobreza. Esta situação reflete a necessidade de uma gestão integral por parte dos órgãos reguladores da saúde humana, animal e ambiental; mas uma das principais barreiras da intersetorialidade é a forma como a administração é projetada para o controle desses eventos. As estruturas atuais levam-nos a refletir e a tender para uma abordagem holística, buscando novas formas de organização, novas estratégias para estudar, controlar e tratar a leptospirose, cujo controle é responsabilidade de diferentes setores e disciplinas. A gestão integral da leptospirose implica um maior conhecimento do agente e dos fatores de risco biológicos, socioeconômicos e culturais das regiões e, do ponto de vista prático, é necessário promover iniciativas de trabalho conjunto; bem como, apresentar evidências da necessidade de trabalhar a partir de uma perspectiva de "uma saúde" para uma zoonose que se tornou um problema emergente de saúde pública.(AU)

Abstract In recent months, rare cases of thrombosis at unusual sites associated with thrombocytopenia, occurring within a typical risk window (i.e., 4-28 days) after receiving SARS CoV2 vaccines, have been reported. Healthcare professionals should be prepared to detect these cases on time. The Expert Panel of the Knowledge Management and Transfer Network conducted a free search of the related literature. With the available information and the clinical expertise of the working group, we formulated, reviewed, and endorsed recommendations for the timely suspicion, diagnosis (case definitions, the use of initial laboratory and imaging tests, specific tests), and management of these thrombotic conditions. This document is considered a living document that will be updated as new evidence emerges, and recommendations may change over time.


Resumen En meses recientes se han reportado casos raros de trombocitopenia y trombosis en sitios inusuales, que ocurren dentro de una ventana de riesgo típica ( por ejemplo de 4 a 28 días) luego de recibir vacunas de SARS CoV 2. Los profesionales de la salud deben estar preparados para detectar estos casos a tiempo. Un panel de expertos y una red de transferencia de conocimiento realizó una búsqueda libre de literatura seleccionada. Con la información disponible y la experticia clínica del grupo de trabajo revisamos y dimos recomendaciones para la sospecha temprana, el diagnostico (definición de caso, el uso de pruebas de laboratorio especificas y de imágenes diagnósticas) para le manejo de estas condiciones tromboticas. Este documento es considerado un documento vivo que debe ser actualizado a medida que surja nueva evidencia y las recomendaciones vayan cambiando con el tiempo
