ABSTRACT

OBJECTIVE: This study examined the morphological changes in the colonic mucosa and the presence of inflammation in rats induced with 1,2-dimethylhydrazine (DMH) 30 mg/kg BW over 9, 11, and 13 weeks without a latency period. METHODS: Hematoxylin and eosin staining was performed to assess the morphology and characteristic alteration of the epitheliocytes in the colon. Immunohistochemistry was employed to assess the expression of tumor necrosis factor (TNF)-α and cyclooxygenase-2 (COX-2). The difference in the severity of inflammation and COX-2 expression was examined using one-way analysis of variance. The correlation of COX-2 expression with the severity of inflammation was analyzed using Spearman's rank correlation test. RESULT: Until week 13, chronic inflammation and non-hyperplastic and hyperplastic aberrant crypt foci occurred. The severity of inflammation gradually shifted from high moderate to low moderate. TNF-α expression was high in all groups; however, COX-2 expression was gradually lower with longer duration of induction, which corresponded with the severity of inflammation. CONCLUSION: DMH induction until week 13 without a latency period caused chronic inflammation without the formation of adenoma or adenocarcinoma. A very strong correlation was established between COX-2 expression and inflammation.

ABSTRACT

BACKGROUND: Older adults are at risk of decreasing functional status due to their condition and many factors. Although many studies have been conducted about declining in functional status, based on the author's knowledge, only this study that has conducted about functional status changes in the elderly involving the frailty status which undergoing surgery in Indonesia. There are many factor was postulated, some of that was checked routine and applicable in clinical practice. Furthermore, identification of these risk factors can be used a basis for decision making to perform surgeries in older adults because poor functional status causes declining quality of life in the elderly patients. The aim of this research was to determine the risk factors for declined functional status within 30 days after elective surgeries in elderly patients. METHODS: We conducted a prospective cohort study from July 2021 to December 2021 at Dr. Kariadi Hospital, Semarang, Indonesia. We included patients aged 60 or older who underwent elective surgery under general anesthesia. We excluded those who underwent emergency surgery, day care surgery, or were unwillingness to participate. The functional status were assessed using the ADL (Activity of Daily Living) Barthel index. To identify risk factors of  declined ADL scores, a logistic regression analysis was performed on the age variable, gender, body mass index, frailty status, postoperative complications, as well as haemoglobin, and albumin levels. RESULTS: This study included 191 participants, with 97 women (50.79%) and 94 men (42.21%). Declined in functional status within 30-days after surgery occurred in 54 participants (28.2%). There was a significant changed of functional status before and after surgery. Multivariate analysis showed that independently significant variables for declined functional status were male sex (OR 4.48, p value < 0.001), hypoalbuminemia (OR 2.59, p value 0.02), preoperative functional status (OR 2.37; p value 0.05), and postoperative complications (OR 24.885; p value < 0.001). CONCLUSION: Risk factors for declined functional status within 30 days after elective surgery in older patients are postoperative complications, preoperative functional status, hypoalbuminemia, and male gender.

ABSTRACT

Previous reports have suggested that high frequent tofu consumption is associated with worse cognitive function in East Asian elderly. Some studies also found an increased risk of dementia with high tofu consumption in those older than 65years of age. Tofu and other soy products, such as tempeh, contain high levels of plant estrogens or isoflavones. This study revisited a rural Central Javanese population (56-97 years of age) who were covered by the Borobudur District Health Centers. Data on cognitive performance were available for n=142 participants. Results showed positive linear associations of weekly tofu (beta=.22, p<0.05) and tempeh (beta=.23, p<0.01) consumption with immediate recall, which were significant in those with an average age of 67 years. In those with an average age of 80 years, the earlier reported negative association of tofu with immediate recall was no longer significant. Lifestyle changes (reduction of tofu consumption after dissemination of results) or "healthy survivor effects" may have been responsible for this finding. These findings may be reminiscent of the "Window of Opportunity" theory, which suggests that estrogenic compounds can exert positive effects on verbal memory, but not in older men and women, when no or negative effects of these compounds on brain cells and cognition have been found. Long-term, placebo-controlled treatment studies should investigate whether tempeh, a fermented soybean product that also contains folate, can maintain cognitive function in middle-aged and elderly participants.
