ABSTRACT

INTRODUCTION: At present, cyclosporine (CsA) is the first-line treatment for Pure Red Cell Aplasia (PRCA), but CsA administration can be associated with a number of side effects due to its high toxicity. Therefore, it is urgent to explore a safe and effective treatment for elderly patients who cannot be treated with conventional doses of CsA, especially those with multiple complications. Allogeneic Stem Cell Transplantation (ASCT) for PRCA is a promising treatment, but reports of using umbilical cord blood (UCB) are very rare. CASE PRESENTATION: In this report, UCB and umbilical cord mesenchymal stem cells (UC-MSCs) combined with low-dose CsA (1-3mg/kg/d) were used to treat 3 elderly patients who were diagnosed with PRCA combined with multiple complications in heart, lung, and renal. The treatments were successful without complications, and 12 months after stem cell infusion, the blood tests of the patients came normal. Moreover, the function of the liver, heart, and kidney continued to be stable. CONCLUSION: This report provides an effective regimen of using UCB and UC-MSCs combined with low-dose CsA (1-3 mg/kg/d) to treat PRCA, especially for elderly patients with multiple complications who cannot use the conventional dosage.

ABSTRACT

Since quinolone is a kind of synthetic broad-spectrum antibacterial drugs, with the widespread use of this class of antibiotics, the risk and harm to human health have been attendant to the sewage containing quinolones which are discharged into the environment. Photocatalysis is considered as a promising technology for antibiotic degradation for its strong redox properties and reaction rate. As a metal oxidizing substance, Bismuth vanadate (BiVO4) is such a popular and hot material for the degradation of organic pollutants recently due to its good photocatalytic activity and chemical stability. Numerous studies have confirmed that BiVO4 composites can overcome the shortcomings of pure BiVO4 and cleave the main structure of quinolone under photocatalytic conditions. This paper mainly outlines the research progress on the preparation of BiVO4 composites and the degradation of quinolone antibiotics from the perspective of improving the catalysis and degrading the efficiency mechanism of BiVO4 composites.

ABSTRACT

OBJECTIVE: To investigate the clinical efficacy of Stanozolol combined with Cyclosporine A for treatment of aplastic anemia and its influence on cytokine levels. METHODS: This is a retrospective analysis of 90 patients with aplastic anemia treated in Department of Hematology, Shandong Provincial Third Hospital from July 2019 to July 2022. According to the different treatment methods, these patients were assigned into a control group and an observation group, with 45 cases in each group. Patients in the control group were treated with Stanozolol alone, while those in the observation group were treated with the combination of Stanozolol and Cyclosporine A. Patients in both groups were treated for six months continuously. The indicators in terms of therapeutic effect, drug onset time, cytokine levels, quality of life, and adverse reactions were recorded and compared between the two groups. RESULTS: After treatment, the total response rate in the observation group was significantly higher than in the control group (91.11% vs. 71.11%, P<0.05). The drug onset time in the observation group was shorter than that in control group (42.35±3.68 vs. 68.72±5.49, P<0.05). In contrast to the control group, the observation group exhibited significantly decreased levels of tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), and interleukin-2 (IL-2), and an increased level of vascular endothelial growth factor (VEGF) after treatment, with significant differences (all P<0.05). The QLQ-C30 scores in the observation group were significantly higher than that in the control group (P<0.05). Moreover, there was no statistical difference in the overall incidence of adverse reactions between the two groups (11.11% vs. 17.78%). CONCLUSION: Stanozolol combined with Cyclosporine A is more effective than Stanozolol alone in treatment of aplastic anemia.

ABSTRACT

Introduction: Immunoglobulin A nephropathy (IgAN) presents various clinical manifestations and pathological phenotypes. Approximately 5% of patients with IgAN present with early onset nephrotic syndrome, mild mesangial lesions, and diffuse foot process effacement of podocytes, which resemble minimal change disease (MCD). These patients are defined as MCD-IgAN. Whether MCD-IgAN is a special type of IgAN or simply MCD accompanied by IgA deposition remains controversial. Methods: A total of 51 patients diagnosed with MCD-IgAN at Beijing Anzhen Hospital from January 2010 to September 2022 were recruited. The clinical and pathological characteristics of IgA-MCD were analyzed. Patients with IgAN but without MCD (non-MCD-IgAN) and healthy participants were enrolled as controls. Galactose-deficient immunoglobulin A1 (Gd-IgA1) and complement C3 were detected both in the circulation and in renal tissues. Results: We found that the levels of serum Gd-IgA1 were lower in participants with MCD-IgAN than in those with non-MCD-IgAN, but higher than in healthy participants. Gd-IgA1 was rarely deposited in the glomeruli of participants with MCD-IgAN, with a positive rate of only 13.7% (7/51); in contrast, the positive rate in participants with non-MCD-IgAN was 82.4% (42/51). Among renal Gd-IgA1-positive patients, Gd-IgA1 and immunoglobulin A (IgA) colocalized along the glomerular mesangial and capillary areas. Interestingly, we found that the circulating levels of complement C3 were significantly higher in participants with MCD-IgAN than in participants with non-MCD-IgAN. In addition, the intensity of C3c in glomeruli in participants with MCD-IgAN was significantly weaker than in participants with non-MCD-IgAN. Conclusions: Our study suggests that, in MCD-IgAN, most of the IgA that is deposited on glomeruli is not the same pathogenic Gd-IgA1 as found in general IgAN. Complement activation both in the circulation and in the renal locality was much weaker in MCD-IgAN than in non-MCD-IgAN. Our study suggests that IgAN with MCD might be MCD with coincidental IgA deposition.

ABSTRACT

To improve deep denitrification of black and odorous water and improve the quality of surface water, we studied the characteristics of nitrogen metabolism and denitrification of urban tailwater by microalgae (Z), Bacillus (Y), and Bacillus microalgae (ZY). The results showed that there was a good removal effect of ammonia nitrogen of group Z and group ZY in urban tailwater. The degradation rate of both groups reached 95%. The best effect on the removal of nitrite nitrogen was of group Z in urban tailwater. The combined action of Bacillus and Micrococcus played a relatively strong and stable effect on the conversion of nitrite nitrogen to nitrate nitrogen in the nitrogen cycle reaction. Bacillus could effectively remove nitrate nitrogen and improve the removal efficiency of nitrate nitrogen by microalgae. Best removal effect of nitrate nitrogen was observed in group ZY in urban tailwater, with a degradation rate as high as 99%, in which the nitrate nitrogen was removed almost completely. The Bacteria with high proportions in Z were Chroococcidiopsis_PCC_7203 (24.38%), uncultured_bacterium-g_norank_f_A4b (23.65%), Exiguobacteriu (7.09%), Leptolyngbya_PCC-6306(9.41%), and Bacillus (1.99%). The bacteria with high proportions in ZY were Brevibacillus (22.94%), Clostridium (8.78%), and Bacillus (4.88%), and the proportion of Chroococcidiopsis_PCC_7203 was only 7.84% in ZY, which was considerably lower than that in Z samples. The conclusions were as follows:microalgae could effectively remove ammonia nitrogen in the system. Bacillus and microalgae had very good removal effect of ammonia nitrogen and nitrate nitrogen. During the nitrogen removal of black and odorous water by algae, the Bacillus inhibited the excessive growth of microalgae and prevented eutrophication and black odor in water. This study can provide data support for the deep treatment of urban tail water and prevention of surface water eutrophication.

ABSTRACT

Existing salient object detection methods often adopt deeper and wider networks for better performance, resulting in heavy computational burden and slow inference speed. This inspires us to rethink saliency detection to achieve a favorable balance between efficiency and accuracy. To this end, we design a lightweight framework while maintaining satisfying competitive accuracy. Specifically, we propose a novel trilateral decoder framework by decoupling the U-shape structure into three complementary branches, which are devised to confront the dilution of semantic context, loss of spatial structure and absence of boundary detail, respectively. Along with the fusion of three branches, the coarse segmentation results are gradually refined in structure details and boundary quality. Without adding additional learnable parameters, we further propose Scale-Adaptive Pooling Module to obtain multi-scale receptive field. In particular, on the premise of inheriting this framework, we rethink the relationship among accuracy, parameters and speed via network depth-width tradeoff. With these insightful considerations, we comprehensively design shallower and narrower models to explore the maximum potential of lightweight SOD. Our models are proposed for different application environments: 1) a tiny version CTD-S (1.7M, 125FPS) for resource constrained devices, 2) a fast version CTD-M (12.6M, 158FPS) for speed-demanding scenarios, 3) a standard version CTD-L (26.5M, 84FPS) for high-performance platforms. Extensive experiments validate the superiority of our method, which achieves better efficiency-accuracy balance across five benchmarks.

BACKGROUND: Janagliflozin is a novel sodium-glucose cotransport-2 inhibitor. Despite its remarkable effect in glycemic control, no systematic research has evaluated the effect of renal impairment (RI) on its pharmacokinetics and pharmacodynamics. METHODS: Here, patients with T2DM (n = 30) were divided into normal renal function (eGFR ≥ 90 mL/min/1.73 m2), mild RI (eGFR between 60 and 89 mL/min/1.73 m2), moderate RI-I (eGFR between 45 and 59 mL/min/1.73 m2), and moderate RI-II (eGFR between 30 and 44 mL/min/1.73 m2) groups. They were administered 50 mg janagliflozin orally, and plasma and urine samples were collected for the determination of janagliflozin concentration. RESULTS: Following oral administration, janagliflozin was rapidly absorbed, with the time to Cmax of 2-6 h for janagliflozin and 3-6 h for its metabolite XZP-5185. Plasma exposure levels were similar for janagliflozin in T2DM patients with or without RI but decreased for the metabolite XZP-5185 in T2DM patients with eGFR between 45 and 89 mL/min/1.73 m2. Janagliflozin significantly promoted the excretion of urinary glucose, even in patients with reduced eGFR. Janagliflozin was well tolerated in patients with T2DM with or without RI, and no serious adverse events (SAEs) occurred during this trial. CONCLUSIONS: The exposure levels of janagliflozin in T2DM patients were slightly increased with worsening of RI (i.e., 11% increase in the AUC in patients with moderate RI compared with the normal renal function group). Despite worsening of renal function, janagliflozin exerted a significant pharmacologic effect and was well tolerated, even in patients with moderate RI, implying a promising role in the treatment of patients with in T2DM. ABSTRACT

In order to denitrify the urban tail water deeply and control the eutrophication of surface water, the molecular biology methods were used to study the nitrogen metabolism performance of the denitrification complex flora and the algal-bacteria symbiotic system. The results showed that the nitrogen metabolism complex flora was high ammonification and denitrification performance. The removal effect of ammonia nitrogen of group JZ was very well in urban tailwater, and the degradation rate was as high as 95%. The removal effect of total nitrogen of group JZ was better than that of group J in the experimental water distribution. High-throughput sequencing showed that the main dominant flora and proportion of group J were Firmicutes 44.53%, Proteobacteria 43.41%, Actinobacteria 5.37%, Bacteroidetes 3.04%, and Chloroflexi 1.35%. The main dominant bacterial groups in the group JZ were 33.89% Cyanobacteria, 25.34% Chloroflexi, 19.38% Proteobacteria, 10.02% Firmicutes, and 4.20% Acidobacteria. The dominant species in group J were compared with those in group JZ; the proportions were 82% and 18% in Firmicutes, 69% and 31% in Proteobacteria, 1% and 99% in Cyanobacteria, 5.1% and 95% in Chloroflexi, 73% and 27% in Actinobacteria. It was concluded that the removal effect of ammonia nitrogen of group JZ was high in the urban tailwater. With the addition and growth of Micrococcus in group J, the nitrogen metabolism flora in group JZ changed accordingly, so as to adapt to the environment in which the dominant algae formed. It forms a new nitrogen metabolism system of bacteria and algae with Micrococcus. This research provides a theoretical and data basis for the application of algal-bacterial co-metabolism systems.

ABSTRACT

Silicone rubber insulation material is widely used for the external insulation of power systems. During the continuous service of a power grid, it will be seriously aged due to the influence of high voltage electric fields and harsh climate environments, which will reduce its insulation performance and service life and cause transmission line failure. How to evaluate the aging performance of silicone rubber insulation materials scientifically and accurately is a hot and difficult issue in the industry. Starting from the composite insulator, which is the most widely used insulating device of silicone rubber insulation materials, this paper expounds the aging mechanism of silicone rubber materials, analyzes the applicability and effectiveness of various existing aging tests and evaluation methods, especially discusses the magnetic resonance detection methods emerging in recent years, and finally summarizes the characterization and evaluation technology of the aging state of silicone rubber insulation materials.

Biodegradable mulch films are widely used to replace conventional plastic films in agricultural fields. However, their ecological effects on different microbial communities that naturally inhabit agricultural fields are scarcely explored. Herein, differences in bacterial communities recovered from biofilms, bulk soil, and rhizosphere soil were comparatively assessed for polyethylene film (PE) and biodegradable mulch film (BDM) application in peanut planted fields. The results showed that the plastic film type significantly influenced the bacterial community in different ecological niches of agricultural fields (P < 0.001). Specifically, BDMs significantly increased the diversity and abundance of bacteria in the rhizosphere soil. The bacterial communities in each ecological niche were distinguishable from each other; bacterial communities in the rhizosphere soil showed the most pronounced response among different treatments. Acidobacteria and Pseudomonas were significantly enriched in the rhizosphere soil when BDMs were used. BDMs also increased the rhizosphere soil bacterial network complexity and stability. The enrichment of beneficial bacteria in the rhizosphere soil under BDMs may also have implications for the observed increase in peanut yield. Deepening analyses indicated that Pseudoxanthomonas and Glutamicibacter are biomarkers in biofilms of PE and BDMs respectively. Our study provides new insights into the consequences of the application of different types of plastic films on microbial communities in different ecological niches of agricultural fields.

ABSTRACT

Malignant hematological diseases (MHD) are a kind of bone marrow hematopoietic system disease caused by malignant clonal proliferation, including leukemia, lymphoma, myelodysplastic syndrome, etc. At present, there are too few meta-analyses on the effect of decitabine pretreatment-based allogeneic hematopoietic stem cell transplantation (allo-HSCT) on transplantation-related complications and prognosis of MHD patients. A systematic search of PubMed, MEDLINE, Science Direct, The Cochrane Library, CNKI, and CBM was performed. The relevant literature on decitabine pretreated allo-HSCT for the treatment of MHD was screened, and the screening time was up to May 1, 2023. After extensive investigation, only 5 articles met the criteria. Result: The results of this article showed that the odds ratio of mortality after decitabine conditioning treatment was 2.35, the incidence of complications after transplantation was 1.07, and the survival rate following transplantation was 0.82. Decitabine-based conditioning regimen can reduce the incidence of transplantation-related complications and improve the prognosis of MHD patients receiving allo-HSCT. Although it has limitations, it still has very important clinical implications.

ABSTRACT

Noble metal nanomaterials have many excellent optical properties due to localized surface plasmon resonance induced by external electric and magnetic fields. The plasmon-enhanced optical properties of nanomaterials can be controlled by changing their shapes or compositions. Here, we use a gentle approach to synthesize Au/PbS/Au nanostars with multiple tips and explore the surface-enhanced Raman scattering (SERS) activity, the second harmonic generation (SHG), and photocatalytic performance. The Au/PbS/Au nanostars have ultrabroad spectral responses and significantly enhanced local electric fields near the sharp tips. The size and tip length of the Au/PbS/Au nanostars can be adjusted by changing the amount of HAuCl4. The Au/PbS/Au nanostars exhibit largely enhanced SERS activity and photocatalytic degradation efficiency compared with the Au bipyramids and the Au BPs@PbS nanocrystals. In addition, the SHG of Au/PbS/Au nanostars is also significantly enhanced due to asymmetry and local field enhancement. This research shows potential in many applications ranging from photophysics to photochemistry.

Global agricultural intensification leads to a decline in soil quality; however, the extent to which long-term rice cultivation adversely impacts soil, based on chemical and microbial perspectives, remains unclear. The present study was conducted on a seed multiplication farm in Wuchang, Heilongjiang Province, China, to quantify changes in the nutrient properties and microbial profiles of meadow soil in cultivated (rhizosphere and bulk soil) and uncultivated paddy plots from spring to winter. A non-parametric method was used to compare carbon metabolism characteristics among the three groups of soil samples. Principal component analysis was used to distinguish soil chemical properties and carbon source utilization profiles among the soil samples across different seasons. Under rice cultivation, pH, organic matter, total nitrogen, and alkali-hydrolyzed nitrogen concentrations were generally higher in rhizosphere soils than in bulk or uncultivated soils. However, microbial biomass in cultivated soils was consistently lower than in uncultivated soils. There was a discernible difference in carbon substrate preference between summer and other seasons in the three sample groups. In conclusion, agricultural activities in rice cultivation could reshape soil microbial communities in the long term. Notably, specific cultivation activity may induce distinct soil microbial responses, which are more sensitive than chemical responses.

Silicone rubber insulators (SRIs) are widely used in high-voltage power grids. Due to high-voltage fields and harsh environmental conditions, SRIs eventually deteriorate with use in the power grid, decreasing their insulating performance and operational life and contributing to transmission line failures. Therefore, quantitatively assessing the aging status of SRIs is crucial. In this study, we evaluated the viability of the magnetic resonance method for assessing the age of SRIs at the level of chemical structure; we built and made a portable magnetic resonance sensor, and evaluated the sensor's functionality. By measuring the SRI sheds at various service times, it was discovered that the equivalent transverse relaxation time, T2eff, can describe the degree of aging of the SRIs. The results of the magnetic resonance measurements were also compared with those of the static contact angle method, and the two measurement methods yielded the same conclusions. However, the magnetic resonance method was more sensitive than the one using the static contact angle method.

Biochar has been widely used in the fields of environment and energy, and green preparation can make biochar-based materials more environmentally friendly. Particularly, in the low-temperature pyrolysis of biochar, labile C with low biological toxicity is the main influencing factor of bacteria in soil. Therefore, it is worth studying to develop the fabrication technology of low-temperature pyrolysis biochar with rich pore structure. The mechanical effect of ultrasonic cavitation is considered to be an effective strategy for the preparation of biochar. However, the sonochemical effects on biochar remain to be studied. In this review, ultrasonic modification and ultrasonic-chemical modification on biochar has been reviewed. Metal oxide/biochar composites can also be obtained by an ultrasonic-chemical method. It is worth mentioning that there have been some reports on the regeneration of biochar by ultrasound. In addition to ultrasonic preparation of biochar, ultrasound can also trigger the sonocatalytic performance and promote the adsorption ability of biochar for the removal of harmful substances. The catalytic mechanism of ultrasound/biochar needs to be further investigated. For application, biochar prepared by ultrasound has been used for the removal of heavy metals in water, the adsorption of carbon dioxide, and soil remediation.

Introduction: IgA nephropathy (IgAN) encompasses a wide range of clinical and histology features. Some patients present without hematuria, with or without hypertension, still rapidly progress in renal function. Renal pathology of this part of patients were predominant intrarenal arteriolar lesions, rarely presented glomerular proliferative lesions. We aim to investigate the clinical and pathological characteristics and prognosis of these IgAN patients and initially explore whether the abnormal activation of complement is involved in the intrarenal arteriolar lesions of IgAN. Methods: A total of 866 patients with renal biopsy-proven IgAN diagnosed at Beijing Anzhen Hospital were recruited. IgAN patients without intrarenal arteriolar lesions and proliferative lesions were excluded (n = 115), the rest were divided into arteriolar lesions group (n = 202) and proliferative lesions group (n = 549). Among them, 255 patients were regularly followed up for at least 1 year. Renal biopsy tissues of 104 IgAN patients were stained for complement components by immunohistochemistry and immunofluorescence. Results: Compared with proliferative lesions group, the arteriolar lesions group experienced high percentage of hypertension (p = 0.004), low percentage of gross hematuria (p = 0.001), microscopic hematuria (p < 0.001) and less initial proteinuria (p = 0.033). Renal survival between the two groups was not significantly different (p = 0.133). MBL, C4d, FH and FHR5, C3c, and MAC deposited on intrarenal arteriole in arteriolar lesions group. Compare with the proliferative lesion group, the arteriolar lesions group exhibited a higher intensity of C3c deposition on the intrarenal arterioles (p = 0.048). C3c and CD31 co-deposited on intrarenal arterioles area in patients with intrarenal arteriolar lesions. Conclusion: Renal survival of the IgAN patients in arteriolar lesions group was not better than those in proliferative lesions group. Abnormal activation of complement may be involved in the pathogenesis of arteriolar damage through the injury of endothelial cells in this clinical phenotype of IgAN.

ABSTRACT

This paper concentrates on a simple and robust control method for the discrete time nonlinear systems to fulfill the requirement of predefined accuracy. A sliding mode control method is designed by introducing equivalent dynamic linearization technique according to the input/output (I/O) information merely. A square-root type error transformation method is presented for the tracking error to be restricted within a preassigned zone. The performance of presented control method is demonstrated through experiments on a nonlinear system. Experiment results show that the presented control method has a superior tracking accuracy compared with PID controller and model-free adaptive control (MFAC).

ABSTRACT

The cooperation of magnetic and electric plasmon resonances in cup-shaped metallic nanostructures exhibits significant capability for second-harmonic generation (SHG) enhancement. Herein, we report an approach for synthesizing Au open nanoshells with tunable numbers and sizes of openings on a template of six-pointed PbS nanostars. The morphology of Au nanoshells is controlled by adjusting the amount of HAuCl4, and the characteristic shapes of pointed nanocaps, open nanoshells, and hollow nanostars are obtained. Owing to the collaboration of electric and magnetic plasmon resonance modes, the Au nanoshells exhibit significantly broadened and highly tunable optical responses. Furthermore, the morphology-dependent SHG of the Au nanoshells shows two maximal SHG intensities, corresponding to four-opening and one-opening Au nanoshells with appropriate opening sizes. Ag/Au and AuAg/Ag/Au open nanoshells were further investigated to achieve enhanced SHG. By adjusting the thickness of the Ag shell, the SHG intensity of Ag/Au open nanoshells reaches a maximum due to the gradient field at the AuAg bimetallic interface. After replacing the Ag shells with Au shells, the SHG intensity of AuAg/Ag/Au open nanoshells reaches a maximum due to further symmetry breaking. These findings provide a strategy to prepare colloidal metal nanocrystals with prospective applications ranging from nonlinear photonic nanodevices to biospectroscopy and photocatalysis.

ABSTRACT

To clarify the impact of water quality and social activity in the Baiyangdian wetland on the biological community, the change characteristics of bacterial, fungal, and archaeal communities in different areas of the Dian District were studied. Samples were collected at the entrance of Fuhe District (NBB), tourist areas with frequent human social activities (NBD), residential breeding areas (NBX), and village sparse areas (NBN). The physical and chemical characteristics and biological communities of the samples were evaluated. The results of the study show that the COD concentration of organic pollutants in the NBB was 12.35 mg·L-1, and the total nitrogen concentration was 10.12 mg·L-1, that the concentration was highest. Moreover, the water quality in NBD and NBX was better than that of NBB. The NBN area exhibited the best water quality, with COD and total nitrogen concentration values of 6.9 mg·L-1 and 1.82 mg·L-1, respectively. Many types of NBB bacteria were recorded, with a diversity index of 5.86, and NBN diversity index exceeding 4.78. The dominant bacterial flora in all samples was the Proteobacteria, which accounts for 68.8% of the total bacterial communities in NBN samples. The diversity index of fungi in NBB was only 2.14. There were many types of fungi in NBN, with a diversity index of 3.23. Chytridiomycota was found in the NBD and NBN, accounting for 5.4% and 9.8% of the total number of fungi, respectively. The Chytridiomycota was main decomposer of hard to degrade organic carbon. The diversity of archaea of NBN was the lowest among all the samples. Crenarchaeota was the dominant phylum, which accounts for 39.0%, 51.9%, 47.3%, and 30.1% of NBB, NBD, NBX, and NBN samples, respectively. The number of Halobacterota was lower than Crenarchaeota. The main factor of eutrophication and microbial community changes in Baiyangdian wetland was the results of the combined action of external and internal pollution. Both external and internal pollution increased the organic matter, nitrogen and phosphorus content in the water, and the microbial community structure has changed significantly. The contents of organic matter, nitrogen, and phosphorus in water were increased and the microbial community structures were changed significantly by the increase of both external and internal pollution.

ABSTRACT

BACKGROUND AND OBJECTIVES: The neural EGF-like 1 (NELL-1) protein is a novel antigen in primary membranous nephropathy. The prevalence and clinical characteristics of NELL-1-positive membranous nephropathy in Chinese individuals with primary membranous nephropathy are unclear. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A total of 832 consecutive patients with biopsy-proven primary membranous nephropathy were enrolled. The glomerular expression of phospholipase A2 receptor (PLA2R) and thrombospondin type 1 domain-containing 7A (THSD7A) was screened. Glomerular immunohistochemistry staining for NELL-1 was performed in 43 patients with PLA2R- and THSD7A-negative membranous nephropathy, 31 patients with PLA2R-positive membranous nephropathy, and two patients with PLA2R and THSD7A double positivity. The NELL-1 antibody was also detected in the sera of patients with NELL-1-positive membranous nephropathy by western blot. Clinical and pathologic features were comparable between patients with isolated NELL-1-positive, isolated PLA2R/THSD7A-positive, and triple antigen-negative membranous nephropathy. RESULTS: Among the 832 patients with primary membranous nephropathy, 11 of 54 (20%) patients with PLA2R-negative membranous nephropathy had THSD7A-positive membranous nephropathy. NELL-1-positive membranous nephropathy accounted for 35% (15 of 43) of all patients with PLA2R- and THSD7A-negative membranous nephropathy. One patient was double positive for NELL-1 and PLA2R in glomerular deposits and positive for only the PLA2R antibody in the serum. Most patients with NELL-1-positive membranous nephropathy were women. No tumors were found. There were significant differences in the prevalence of IgG subtypes between patients with different antigen positivity. Among patients with isolated NELL-1-positive membranous nephropathy, although 80% (12 of 15) were IgG4 staining positive, the proportion of IgG4 dominance was only 67% (ten of 15). CONCLUSIONS: About one third of patients who were PLA2R and THSD7A negative were NELL-1 positive in Chinese patients with primary membranous nephropathy. NELL-1-positive membranous nephropathy was more common than THSD7A-positive membranous nephropathy in PLA2R-negative membranous nephropathy.
