ABSTRACT

Introdução: As doenças musculoesqueléticas (DMSQ) são causas importantes de incapacidade física que tem como consequências a redução da qualidade de vida e o aumento dos custos em saúde. Objetivo: Este estudo teve o objetivo de descrever a prevalência de sintomas de DMSQ e da incapacidade física associada a esses sintomas em uma área de abrangência de uma Unidade Básica de Saúde (UBS) da família em área periurbana, além de analisar a associação da presença desses sintomas com características demográficas e doenças crônicas mais frequentes. Métodos: Estudo transversal realizado na área de cobertura de uma UBS periurbana entre agosto de 2018 e fevereiro de 2019 na qual pessoas com mais de 15 anos, selecionadas ao acaso, foram entrevistadas com o questionário da primeira fase do Community Oriented Program for Control of Rheumatic Diseases (COPOCORD), traduzido e validado para a língua portuguesa. Essas pessoas também foram questionadas quanto à presença de outras doenças diagnosticadas e a medicações em uso. Além disso, foram realizadas medidas de peso, altura e circunferência da cintura de cada participante ­ os dois primeiros foram utilizados para cálculo do índice de massa corporal (IMC). Resultados: Neste estudo foram entrevistadas 372 pessoas com média de idade de 46,5 (±18,3) anos, sendo 212 (57%) do sexo feminino. A prevalência de indivíduos que apresentaram sintomas de DMSQ nos últimos sete dias foi de 66,4% (IC95% 61,6­71,2). Cerca da metade (48,6%) e quase um quarto (24,2%) dos participantes relataram sintomas de intensidade moderada e severa, respectivamente. Os locais mais afetados foram as costas (50,27%), o pescoço (34,9%) e os joelhos (30,64%). A maioria, 209/247 (84,6%), relatou dor em mais de um local e 129/247 (52.2%) relataram limitação física para atividades da vida diária. Das 247 pessoas, 102 (41,3%) procuraram assistência médica, a maioria, 70 (68,6%), na UBS. Indivíduos com sintomas de DMSQ tinham média de idade e de IMC significativamente mais elevada que indivíduos sem esses sintomas, além de apresentarem maior frequência de diabetes e ansiedade e/ou depressão. No entanto, em análise multivariada, nenhuma variável foi preditora independente de sintomas de DMSQ. Conclusões: Sintomas de DMSQ são prevalentes nessa comunidade e a atenção básica deve estar preparada para manejo e reabilitação das pessoas com essas doenças.


Introduction: Musculoskeletal disorders are important causes of physical disability that result in reduced quality of life and increased health costs. Objective: To describe the prevalence of symptoms of musculoskeletal disorders and the physical disability associated with these symptoms in an area covered by a Health Center in a peri-urban area. In addition, we analyzed the association of the presence of these symptoms with demographic characteristics and the most frequent chronic diseases. Methods: This is a cross-sectional study carried out in the area covered by a peri-urban Health Center between August 2018 and February 2019 in which people over 15 years of age, randomly selected, were interviewed using the questionnaire from the first phase of the Community Oriented Program for Control of Rheumatic Diseases (COPOCORD), translated and validated into Brazilian Portuguese. They were also asked about the presence of other chronic diseases and medications in use. In addition, each participant's weight, height, and waist circumference were measured. The first two were used to calculate the body mass index. Results: A total of 372 people were interviewed with a mean age of 46.5 (±18.3) years, 212 (57%) of whom were women. The prevalence of individuals who presented symptoms of musculoskeletal disorders in the last seven days was 66.4% (95%CI: 61.6­71.2). About half (48.6%) and almost a quarter (24.2%) interviewees reported symptoms of moderate and severe intensity, respectively. The most affected sites were the back (50.27%), the neck (34.9%), and the knees (30.64%). The vast majority, 209/247 (84.6%), reported pain in more than one site and 129/247 (52.2%) reported physical impairments in activities of daily living. Of the 247 individuals, 102 (41.3%) sought medical assistance, the majority, 70/102 (68.6%), at the Health Center. Individuals with symptoms of musculoskeletal disorders had a significantly higher mean age and body mass index than individuals without these symptoms. They also had a higher frequency of diabetes and anxiety and/or depression. However, in multivariate analysis, no variable was an independent predictor of symptoms of musculoskeletal disorders. Conclusions: Symptoms of musculoskeletal disorders are prevalent in this community and primary health care must be prepared for the management and rehabilitation of people with these disorders.


Introducción: Las enfermedades musculoesqueléticas son causas importantes de discapacidad física que resultan en una reducción de la calidad de vida y un aumento de los costos de salud. Objetivo: Este estudio tuvo como objetivo describir la prevalencia de síntomas de enfermedades musculoesqueléticas (EME) y la discapacidad física asociada a estos síntomas en un área cubierta por una unidad básica de salud de la familia (UBS) en un área periurbana. También analizamos la asociación de la presencia de estos síntomas con características demográficas y enfermedades crónicas más frecuentes. Métodos: Estudio transversal realizado en el área de cobertura de una UBS periurbana entre agosto de 2018 y febrero de 2019 en el que se entrevistó a personas mayores de 15 años, seleccionadas al azar, mediante la primera fase del cuestionario Community Oriented Program for Control Of Rheumatic Diseases (COPOCORD) traducido y validado al portugués. También se les preguntó sobre la presencia de otras enfermedades diagnosticadas y medicamentos en uso. Además, se tomaron medidas del peso, la altura y la circunferencia de la cintura de cada participante. Los dos primeros se utilizaron para calcular el índice de masa corporal (IMC). Resultados: Fueron entrevistadas 372 personas con una edad media de 46,5 (±18,3) años, de las cuales 212 (57%) eran mujeres. La prevalencia de individuos que presentaron síntomas de enfermedades musculoesqueléticas en los últimos siete días fue del 66,4% (IC 95%: 61,6-71,2). Aproximadamente la mitad (48,6%) y casi una cuarta parte (24,2%) informaron síntomas de intensidad moderada y grave, respectivamente. Las zonas más afectadas fueron la espalda (50,27%), el cuello (34,9%) y las rodillas (30,64%). La gran mayoría, 209/247 (84,6%) refirieron dolor en más de una localización y 129/247 (52,2%) refirieron limitaciones físicas en las actividades de la vida diaria. 102 de los 247 (41,3%) buscaron asistencia médica, la mayoría, 70/102 (68,6%), en la UBS. Las personas con síntomas de DMSQ tenían una edad media y un IMC significativamente más altos que las personas sin estos síntomas. También tenían una mayor frecuencia de diabetes y ansiedad y/o depresión. Sin embargo, en el análisis multivariado, ninguna variable fue un predictor independiente de los síntomas del EMS. Conclusiones: Los síntomas de las enfermedades musculoesqueléticas son prevalentes en esta comunidad y la atención primaria debe estar preparada para el manejo y rehabilitación de las personas con estas enfermedades.

Introdução: A lombalgia é uma condição prevalente e que apresenta importante impacto na capacidade funcional e na qualidade de vida, sendo a sua correta abordagem na Atenção Primária à Saúde fundamental para a identificação e o estabelecimento de um diagnóstico etiológico precoce de possíveis patologias que possam estar relacionadas a desfechos mórbidos e a graves limitações funcionais. Apresentação do caso: Paciente de 56 anos, sexo masculino, hipertenso, foi encaminhado para serviço especializado de reumatologia com histórico de lombalgia havia mais de 20 anos. Ao exame físico foi constatada presença de deformidades da coluna vertebral e extensa limitação de movimentos. Exames radiográficos mostravam esclerose de articulações sacroilíacas, osteopenia difusa e coluna vertebral em aspecto de "bambu". Conclusões: Constata-se a importância de que na abordagem das lombalgias na atenção primária se busque o reconhecimento de possíveis etiologias graves e potencialmente incapacitantes que possam estar subjacentes à queixa de dor lombar. Com esse objetivo, é fundamental o reconhecimento das chamadas red flags relacionadas às lombalgias, além de sua caracterização como mecânica ou inflamatória. Perante a atuação da atenção primária no oferecimento de um cuidado pautado na integralidade e na prevenção de agravos, reafirma-se a importância de uma avaliação clínica pormenorizada das lombalgias nesse nível de atenção à saúde.


Introduction: Low back pain is a prevalent condition that has an important impact on functional capacity and quality of life, and its correct approach in Primary Care is fundamental to the identification and establishment of an early etiological diagnosis of possible pathologies that may be related to outcomes morbid conditions and serious functional limitations. Case presentation: 56-year-old male patient, hypertensive, referred to a specialized rheumatology service with a history of low back pain for over 20 years. Physical examination revealed the presence of spinal deformities and extensive movement limitations. Radiographic examinations showing sclerosis of the sacro-iliac joints, diffuse osteopenia and a "bamboo" appearance of the spine. Conclusions: It is important that in the approach of low back pain in Primary Care, we seek to recognize possible serious and potentially disabling etiologies that may underlie the complaint of low back pain. For that, it is essential to recognize the so-called "red flags" related to low back pain, in addition to its characterization as mechanical or inflammatory. Given the role of Primary Care in offering care based on integrality and in the prevention of injuries, the importance of a detailed clinical assessment of low back pain at this level of health care is reaffirmed.


Introducción: La lumbalgia es una patología prevalente que tiene un impacto importante en la capacidad funcional y la calidad de vida, y su correcto abordaje en Atención Primaria de Salud es fundamental para la identificación y establecimiento de un diagnóstico etiológico precoz de posibles patologías que puedan estar relacionadas con los resultados, condiciones morbosas y limitaciones funcionales graves. Presentación del caso: Paciente masculino de 56 años, hipertenso, remitido a servicio especializado de reumatología con antecedentes de dolor lumbar de más de 20 años. El examen físico reveló la presencia de deformidades de la columna y amplias limitaciones de movimiento. Los exámenes radiológicos muestran esclerosis de las articulaciones sacroilíacas, osteopenia difusa y una apariencia de "bambú" de la columna. Conclusiones: Es importante que al abordar la lumbalgia en Atención Primaria de Salud busquemos reconocer las posibles etiologías graves y potencialmente incapacitantes que pueden subyacer a la queja de lumbalgia. Con este objetivo, es fundamental reconocer las llamadas "banderas rojas" relacionadas con la lumbalgia, además de su caracterización como mecánica o inflamatoria. Dado el papel de Atención Primaria de Salud a la hora de ofrecer una atención basada en la integralidad y prevención de enfermedades, se reafirma la importancia de una evaluación clínica detallada de la lumbalgia en este nivel de atención sanitaria.

Resumen Objetivo: El objetivo de esta revisión sistemática con metaanálisis es sintetizar la evidencia científica disponible sobre los efectos de la terapia manual aplicada de forma aislada en comparación a otras intervenciones en el dolor, la capacidad funcional, la impresión global de mejoría y severidad de síntomas autopercibida en personas con fibromialgia. Metodología: Se realizó una revisión sistemática y metaanálisis con búsqueda electrónica en MEDLINE, WoS, SCOPUS, PEDro, CENTRAL y LILACS. Resultados: Once estudios cumplieron con los criterios de selección y seis fueron incluidos en una síntesis cuantitativa. El dolor al terminar el tratamiento tuvo una diferencia de medias de -0.42 (IC al 95% -1.14, 0.30, p = 0.25), el estado funcional al terminar el tratamiento tuvo una diferencia de medias de -0.15 (IC al 95% -0.69, 0.40, p = 0.60), la impresión global de salud al terminar el tratamiento tuvo una diferencia de media de -1.13 (IC al 95% -1.68, -0.57 p = <0.00) y posterior a doce meses una diferencia de media de -0.48 (IC al 95% -0.78, -0.19 p = <0.00), la impresión global de severidad después de la intervención tuvo una diferencia de medias de -0.62 (IC al 95% -1.22, -0.03, p = 0.04) y posterior a doce meses una diferencia de medias de -0.43 (IC al 95% -1.18, 0.32, p = 0.26). Conclusiones: La terapia manual aislada es una intervención que podría mejorar significativamente la autopercepción del estado global de mejoría y severidad de síntomas a corto plazo en comparación a agentes físicos placebo. Sin embargo, no es mejor que otras intervenciones para disminuir el dolor y mejorar la capacidad funcional.


Abstract Objective: The aim of this systematic review with meta-analysis is to synthesize the available scientific evidence on the effects of manual therapy applied in isolation in comparison to other interventions on pain, functional capacity, global impression of improvement, and self-perceived severity of symptoms in people with fibromyalgia. Methodology: A systematic review with meta-analysis was conducted with electronic search of MEDLINE, WoS, SCOPUS, PEDro, CENTRAL, and LILACS. Results: Eleven studies met the selection criteria and six were included in a quantitative synthesis. Pain at the end of treatment had a mean difference of -0.42 (95% CI -1.14, 0.30, p = 0.25), functional status at the end of treatment had a mean difference of -0.15 (95% CI - 0.69, 0.40, p = 0.60), the global impression of health at the end of treatment had a mean difference of -1.13 (95% CI -1.68, -0.57 p = <0.00) and a mean difference of -0.48 after twelve months (95% CI -0.78, -0.19 p = <0.00), the global impression of severity after the intervention had a mean difference of -0.62 (95% CI -1.22, -0.03, p = 0.04 ) and a mean difference of -0.43 after twelve months (95% CI -1.18, 0.32, p = 0.26). Conclusions: Isolated manual therapy is an intervention that could significantly improve the self-perception of the global state of improvement and severity of symptoms in the short term compared to sham physical agents. However, it is not better than other interventions to reduce pain and improve functional capacity.

Introduction: The relationship of risk factors with musculoskeletal disorders and their intervention is a topic of interest, given their prevalence among workers. Thus, analyzing risk factors from approaches such as the gender perspective may be an alternative. Objectives: To analyze risk factors in a health entity with a high prevalence of musculoskeletal disorders in upper limbs and to describe possible intervention measures according to scientific evidence, from a gender perspective. Methods: This is an analytical study. A questionnaire was applied to 93 workers on demographic aspects, presence of factors related to the environment, the task, and the organization, extra-work activities related to musculoskeletal disorders in the upper limbs. Chi-square was used to identify significant relationships between the sex variable and individual, occupational, and non-occupational factors, corroborated by Fisher's test and prevalence ratio. According to the associations identified, a literature review was carried out to establish possible strategies. Results: Significant relationships were found between the sex variable and task-related factors such as the presence of repetitive or sudden movements (p < 0.05), supporting postulates of labor segregation. According to the literature consulted, the effectiveness of activities such as physical preparation and adaptation of the workplace under professional guidance, training activities, and breaks is discussed. It is important to review organizational factors. Conclusions: By identifying significant relationships between the sex variable and task-related factors, the present study contributes to the postulate of labor segregation, in terms of concentration of female labor in activities with particular working conditions. Regarding literature and actions, it is important to generate more studies from this perspective.


Introducción: La relación de factores de riesgo con desordenes musculoesqueléticos y su intervención es tema de interés, dada su prevalencia entre trabajadores. Así, analizar los factores de riesgo desde enfoques como la perspectiva de género puede ser una alternativa. Objetivos: Analizar factores de riesgo en una entidad sanitaria con alta prevalencia de desordenes musculoesqueléticos en miembros superiores y describir posibles medidas de intervención según evidencia científica, desde la perspectiva de género. Métodos: Éste es un estudio de tipo analítico. Se aplicó un cuestionario a 93 trabajadores sobre aspectos demográficos, presencia de factores relacionados con ambiente, tarea y organización, y actividades extralaborales vinculadas con desórdenes musculoesqueléticos en miembros superiores. Mediante chi cuadrado, se realizó la identificación de relaciones significativas entre la variable sexo y factores individuales, intralaborales y extralaborales, corroboradas por prueba de Fisher y razón de prevalencias. Según las asociaciones identificadas, se efectuó revisión de literatura para establecer posibles estrategias. Resultados: Se encontraron relaciones significativas entre la variable sexo y factores de la tarea como la presencia de movimientos repetitivos o súbitos (p < 0,05), apoyando postulados de segregación laboral. Según la bibliografia consultada, se discute la eficacia de actividades como preparación física y adecuación del puesto de trabajo bajo orientación profesional, actividades de capacitación y pausas. Es importante revisar factores organizacionales. Conclusiones: Al identificarse relaciones significativas entre la variable sexo y factores de la tarea, se aporta al postulado de segregación laboral en cuanto a concentración de mano de obra femenina en actividades con condiciones de trabajo particulares. Sobre la literatura y las acciones, es importante generar más estudios desde esta perspectiva.

ABSTRACT

OBJECTIVE: To determine the optimal dose and short-term effectiveness of kinesiotaping (KT) on pain intensity and disability in pregnant women with lumbo-pelvic pain. DATA SOURCES: MEDLINE (via PubMed Central), CINAHL, Epistemonikos, Scopus, and Web of Science from inception to 21st March 2023. STUDY SELECTION: We included randomized controlled trials (RCT) conducted on pregnant women with lumbo-pelvic pain treated with KT. DATA EXTRACTION: The outcomes included pain intensity and disability. ROB-2 and GRADE were used to assess the risk of bias and the certainty of the evidence, respectively. A random effects meta-analysis was performed using the standardized mean difference (SMD) and the corresponding 95% confidence interval (CI). The dose-response association was evaluated using a restricted cubic spline model. DATA SYNTHESIS: Seven RCTs involving 527 patients were included. Meta-analysis revealed a statistically significant effect in favor of KT on pain intensity (SMD = -1.71; 95% CI = -2.51 to -0.90; P = <0.001) and on disability (SMD = -1.15; 95% CI = -2.29 to -0.02; P = <0.001). The total duration of KT use ranged from 5 to 35 days. It was estimated that a dose of 5-10 days exceeded the minimal clinically important difference (MCID) for pain intensity (mean difference at 10 days = -2.63; 95% CI = -3.05 to -2.22). Low certainty of evidence was identified for both outcomes. CONCLUSIONS: In pregnant women with lumbo-pelvic pain, the use of KT for 5 to 10 days produces a short-term reduction in pain intensity that exceeds the MCID, with a low certainty of evidence. ABSTRACT

INTRODUCTION: Developmental dysplasia of the hip (DDH) includes a spectrum of clinical and imaging findings at birth or early infancy. The circumstances in which this condition is detected and managed may be heterogeneous in Brazil owing to its large territory and regional socio-economic differences. Mapping DDH perspectives in a country is fundamental for designing guidelines and strategies for public policy. This scoping review aims to map the available literature related to screening, diagnosis, treatment and outcomes of DDH in the Brazilian population to provide an overview of this condition and to describe regional variations in presentation and management across the country. METHODS AND ANALYSIS: This study will follow the methods outlined in the Joanna Briggs Institute Reviewers manual for conducting a scoping review. Relevant publications will be first searched in PubMed/MEDLINE, Scientific Electronic Library Online, Web of Science, Scopus, "Biblioteca virtual em saúde" and "Biblioteca Digital Brasileira de Teses e Dissertações" using search terms developed from a brief preliminary search of those databases. There were no language or date range limitations for study inclusion. Databases will be searched from their inception until February 2024. Titles and abstracts will be analysed by two or more independent reviewers to assess them against the inclusion criteria for the review. The search results and study inclusion process will be reported in full in the final version of the scoping review and presented in a Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for scoping review (PRISMA-ScR) flowchart. The resulting data will be recorded and organised to present the key information contained in all examined articles. ETHICS AND DISSEMINATION: This review will include existing available studies and does not require a specific ethical review or approval. The final study will be submitted for presentation at conferences that focus on Brazilian healthcare and publication in peer-reviewed journals. ABSTRACT

Aging is a universal and progressive process involving the deterioration of physiological functions and the accumulation of cellular damage. Gene regulation programs influence how phenotypes respond to environmental and intrinsic changes during aging. Although several factors, including sex, are known to impact this process, the underlying mechanisms remain incompletely understood. Here, we investigate the functional organization patterns of skeletal muscle genes across different sexes and ages using gene co-expression networks (GCNs) to explore their influence on aging. We constructed GCNs for three different age groups for male and female samples, analyzed topological similarities and differences, inferred significant associated processes for each network, and constructed null models to provide statistically robust results. We found that each network is topologically and functionally distinct, with young women having the most associated processes, likely due to reproductive tasks. The functional organization and modularity of genes decline with age, starting from middle age, potentially leading to age-related deterioration. Women maintain better gene functional organization throughout life compared to men, especially in processes like macroautophagy and sarcomere organization. The study suggests that the loss of gene co-expression could be a universal aging marker. This research offers insights into how gene organization changes with age and sex, providing a complementary method to analyze aging.

Dental practitioners, as part of their work, are exposed to a variety of hazards. This highlights the ongoing need for attention to occupational health in the dental field. A cross-sectional study was organised to investigate the range, prevalence, and associated factors for occupational health problems related to dental practice among Cambodian dentists. Participants underwent a face-to-face interview to explore dentists work-related health problems; 106 Cambodian dentists participated in this study, of which 68.9% were male. Ages ranged from 29 to 71 years, averaging 36.1 years, with the majority (77.4%) in the 29-40 age group. They had 5 to 18 years of practice experience, and worked an average of 52.2 h per week. Commonly reported health issues included back pain (88.7%), headaches (81.1%), shoulder pain (78.3%), arm/hand pain (57.5%), and eye problems (48.1%). Additionally, 38.7% of participants felt stressed and 19.8% depressed. Some reported suicidal thoughts and taking medication for depression. Despite these challenges, 91.5% enjoyed practicing dentistry. These findings highlight the need for interventions and strategies to address the physical and mental well-being of Cambodian dentists. By addressing these issues, steps can be taken to enhance the working conditions and professional satisfaction of dental professionals, ultimately benefiting both the practitioners and their patients.

ABSTRACT

For many women, house cleaning is an important way to participate in the labor market. In Brazil, there are 2 types of domestic workers: housekeepers have relatively secure employment and house cleaners are day laborers. The aim of this hypothesis-generating study was to describe the sociodemographic, occupational and health profile of a sample of domestic workers in Brazil. House cleaners received lower wages, had longer daily working hours and worked in a larger number of homes each week in comparison to housekeepers. About 51% of the domestic workers in this sample reported the use of pain medication and 34% reported spinal problems. Musculoskeletal symptoms were frequent in the lower back and upper limbs. Forty-seven percent reported high blood pressure. This study highlights the vulnerability of domestic workers, especially house cleaners, regarding workload, salary, and health conditions. ABSTRACT

INTRODUCTION: In low- and middle-income countries such as Haiti, musculoskeletal injuries are the leading cause of morbidity and mortality. Untreated injuries can contribute to decreased mobility, leading to disability and reduced productivity for individuals. The accessibility of timely fracture care poses a substantial challenge in Haiti, where socioeconomic instability and recent surges in gang violence exacerbate an already strained healthcare infrastructure. This manuscript delves into the intricate barriers to sustainable fracture care in Haiti, shedding light on the sociopolitical landscape and clinical challenges that influence the delivery of orthopedic services. ETHICAL DISCUSSION: The ethical considerations in providing fracture care in Haiti are multifaceted, including classic medical principles, self-preservation in the face of violence, issues of justice in resource and service allocation, and concerns of nonmaleficence in the context of international volunteers. These ethical dilemmas arise from the complex interplay of limited resources, the dangers posed by the current sociopolitical climate, and the involvement of international aid in a vulnerable healthcare system. CONCLUSION: To address the clinical and ethical conflicts of providing fracture care in Haiti, solutions include education and training of Haitian orthopedic surgeons, capacity building of healthcare facilities, and establishing ethical standards for international volunteers. This comprehensive approach is vital for advancing sustainable fracture care in Haiti and other resource-limited settings.

ABSTRACT

BACKGROUND: Electroencephalography (EEG) is a promising tool for identifying the physiological biomarkers of fibromyalgia (FM). Evidence suggests differences in power band and density between individuals with FM and healthy controls. EEG changes appear to be associated with pain intensity; however, their relationship with the quality of pain has not been examined. We aimed to investigate whether abnormal EEG in the frontal and central points of the 10-20 EEG system in individuals with FM is associated with pain's sensory-discriminative and affective-motivational dimensions. The association between EEG and the two dimensions of emotional disorders (depression and anxiety) was also investigated. METHODS: In this cross-sectional pilot study, pain experience (pain rating index [PRI]) and two dimensions of emotional disorders (depression and anxiety) were assessed using the McGill Pain Questionnaire (PRI-sensory and PRI-affective) and Hospital Anxiety and Depression Scale (HADS), respectively. In quantitative EEG analysis, the relative spectral power of each frequency band (delta, theta, alpha, and beta) was identified in the frontal and central points during rest. RESULTS: A negative correlation was found between the relative spectral power for the delta bands in the frontal (r= -0.656; p = 0.028) and central points (r= -0.624; p = 0.040) and the PRI-affective scores. A positive correlation was found between the alpha bands in the frontal (r = 0.642; p = 0.033) and central points (r = 0.642; p = 0.033) and the PRI-affective scores. A negative correlation between the delta bands in the central points and the anxiety subscale of the HADS (r = -0.648; p = 0.031) was detected. CONCLUSION: The affective-motivational dimension of pain and mood disorders may be related to abnormal patterns of electrical activity in patients with FM. ABSTRACT

INTRODUCTION: Conservative treatments such as physical therapies are usually the most indicated for the management of musculoskeletal pain; therefore, a detailed description of interventions enables the reproducibility of interventions in clinical practice and future research. The objective of this study is to evaluate the description of physical interventions for musculoskeletal pain in children and adolescents. METHODS: We considered randomized controlled trials that included children and adolescents between 4 and 19 years old with acute or chronic/persistent musculoskeletal pain. We included physical therapies related to all types of physical modalities aimed at reducing the intensity of pain or disability in children and adolescents with musculoskeletal pain. The description of interventions was assessed using the Template for Intervention Description and Replication (TIDieR) checklist. We performed electronic searches in the following databases: CENTRAL, MEDLINE, EMBASE, CINAHL, PsyINFO and PEDro up to April 2024. The description of physical interventions was presented using frequencies, percentages and 95% confidence intervals (CIs) of the TIDieR checklist items described in each study. We also calculated the total TIDieR score for each study and presented these data as mean and standard deviation. RESULTS: We included 17 randomized controlled trials. The description measured through the TIDieR checklist scored an average of 11 (5.2) points out of 24. The item of the TIDieR that was most described was item 1 (brief name) and most absent was item 10 (modifications). CONCLUSION: The descriptions of physical interventions for the treatment of musculoskeletal pain in children and adolescents are partially described, indicating the need for strategies to improve the quality of description to enable true clinical reproducibility.

Introducción. Las lesiones musculoesqueléticas traen consecuencias en los profesionales quirúrgicos, y más aún en cirujanos de cabeza y cuello, que conllevan gran carga de discapacidad y ausentismo laboral, ocasionando retiro temprano, restricciones o modificaciones en la práctica quirúrgica. Métodos. Se seleccionaron publicaciones de las bases de datos Pubmed, Embase y LILACS. Se incluyeron todos los estudios que midieron la prevalencia de lesiones musculoesqueléticas y la presencia de riesgo ergonómico, síntomas asociados, uso de escalas de medición y estrategias de prevención. Los efectos estimados de los estudios se mostraron con intervalo de confianza del 95 %. Resultados. Se encontraron 438 estudios. Después de la revisión, se incluyeron 7 estudios, con un total de 868 pacientes. Se estimó mediante un metaanálisis de efectos aleatorios una prevalencia del 81 %. Conclusiones. Los problemas posturales en cirujanos de cabeza y cuello tienen una prevalencia de 81 %. Esto genera un gran impacto en su salud física y mental, aumentando las incapacidades y pérdida de años laborales. Las herramientas utilizadas para la evaluación son heterogéneas y algunos estudios no incluyen datos como los años de práctica y el nivel de entrenamiento.


Introduction. Musculoskeletal injuries have consequences for surgical professionals, and even more so for head and neck surgeons, which entail a large burden of disability and absenteeism from work, causing early retirement, restrictions or modifications in surgical practice. Methods. Publications were selected from the Pubmed, Embase and LILACS databases. All studies that measured the prevalence of musculoskeletal injuries and the presence of ergonomic risk, associated symptoms, use of measurement scales and prevention strategies were included. The estimated effects of the studies were shown with a 95% confidence interval. Results. 438 studies were found. After the review, seven studies were included, with a total of 868 patients. A prevalence of 81% was estimated through a random effects meta-analysis. Conclusions. Postural problems in head and neck surgeons have a prevalence of 81%. This generates a great impact on their physical and mental health, increasing disabilities and loss of working years. The tools used for evaluation are heterogeneous and some studies do not include data such as years of practice and level of training.

Loading both lateral and medial compartments is crucial to understanding the effect of muscle fatigue during sidestep cutting. The present study investigated the changes in tibiofemoral contact forces in the medial and lateral compartments and the muscle force contributions during the sidestep-cutting manoeuvre after a handball-specific fatigue protocol. Twenty female handball athletes performed three trials of the sidestep-cutting manoeuvre before (baseline) and after the fatigue protocol. Motion capture and ground reaction forces were measured, and the data were processed in OpenSim. The variables were compared using statistical parametric mapping (SPM), with a significance level of p < 0.05. The results showed a decreased knee flexion angle during fatigue in the early stance phase. In addition, the post-fatigue analysis demonstrated significantly reduced forces in vasti muscles. Similarly, during fatigue, the SPM analysis showed decreased tibiofemoral contact forces in the vertical and anterior directions. Vertical force applied to both medial and lateral condyles demonstrated a significant reduction after the fatigue protocol. These results indicated that forces applied to the tibiofemoral joint were reduced following the fatigue protocol compared to the baseline values. However, no consistent evidence exists that fatigue increases the risk of knee injuries.

BACKGROUND: Subjective pain intensity can be measured using instruments like the Faces Pain Scale-Revised (FPS-R), Verbal Rating Scale (VRS), Numerical Pain Rating Scale (NPRS), and Visual Analogue Scale (VAS). However, information on physiotherapists' and patients' knowledge and preference for these tools is scarce. OBJECTIVE: We investigated the knowledge and preference of physiotherapists and participants with musculoskeletal pain (MP) regarding the pain intensity measurement scales. METHODS: This cross-sectional study consisted of physiotherapists and participants with MP. Physiotherapists were recruited via social media for an online open survey, gathering sociodemographic, professional data, and their knowledge and preferences for pain intensity scales. Participants over 18 with MP, participated in interviews focusing on their familiarity and preferences for pain intensity scales. Data was analyzed descriptively, and Chi-squared test evaluated scale preferences. RESULTS: We included 352 physiotherapists (mean experience = 10.5 years) and 94 participants with MP. Of the physiotherapists, 94.3% were familiar with pain scales, but 30.4% struggled to differentiate them. The NPRS was the most used (56.3%) and preferred scale (52.4%). Among participants with MP, unfamiliarity was noted with all scales. After instruction, 46% preferred FPS-R, with preference varying by income and education levels (p < 0.001). CONCLUSION: We found a knowledge gap among physiotherapists in identifying pain intensity scales, with a preference for the NPRS among those familiar with the scales. Participants with MP had limited familiarity with these scales. After instructions, these participants preferred the FPS-R, particularly those with lower income and education levels.

ABSTRACT

The aims were (i) to determine the effects of Cognitive behavioral therapy for insomnia (CBT-I) on sleep disturbances, pain intensity and disability in patients with chronic musculoskeletal pain (CMP), and (ii) to determine the dose-response association between CBT-I dose (total minutes) and improvements in sleep disorders, pain intensity and disability in patients with CMP. A comprehensive search was conducted in PubMed/MEDLINE, Web of Science, CINAHL, and SCOPUS until December 17, 2023. Randomized clinical trials (RCTs) using CBT-I without co-interventions in people with CMP and sleep disorders were eligible. Two reviewers independently extracted data and assessed risk of bias and certainty of the evidence. A random effects meta-analysis was applied to determine the effects on the variables of interest. The dose-response association was assessed using a restricted cubic spline model. Eleven RCTs (n = 1801 participants) were included. We found a significant effect in favor of CBT-I for insomnia (SMD: -1.34; 95%CI: -2.12 to -0.56), with a peak effect size at 450 min of CBT-I (-1.65, 95%CI: -1.89 to -1.40). A non-significant effect was found for pain intensity. A meta-analysis of disability was not possible due to the lack of data. This review found benefits of CBT-I for insomnia compared to control interventions, with a large effect size. In addition, it was estimated that a 250-min dose of CBT-I had a large effect on reducing insomnia and that the peak effect was reached at 450 min. These novel findings may guide clinicians in optimizing the use of CBT-I in people with CMP and insomnia.

BACKGROUND: Knee ligament rupture is one of the most common injuries, but the diagnosis of its severity tends to require the use of complex methods and analyses that are not always available to patients. AIM: The objective of this research is the investigation and development of a diagnostic aid system to analyze and determine patterns that characterize the presence of the injury and its degree of severity. METHODS: Implement a novel proposal of a framework based on stacked auto-encoder (SAE) for ground reaction force (GRF) signals analysis, coming from the GaitRec database. Analysis of the raw data is used to determine the main features that allow us to diagnose the presence of a knee ligament rupture and classify its severity as high, mid or mild. RESULTS: The process is divided into two stages to determine the presence of the lesion and, if necessary, evaluate variations in features to classify the degree of severity as high, mid, and mild. The framework presents an accuracy of 87 % and a F1-Score of 90 % for detecting ligament rupture and an accuracy of 86.5 % and a F1-Score of 87 % for classifying severity. CONCLUSION: This new methodology aims to demonstrate the potential of SAE in physiotherapy applications as an evaluation and diagnostic tool, identifying irregularities associated with ligament rupture and its degree of severity, thus providing updated information to the specialist during the rehabilitation process.

BACKGROUND: Pathologies of the locomotor system are frequent and can cause disability and impact the quality of life of the people affected. In recent years, online training and feedback have emerged as learning tools in many fields of medicine. OBJECTIVE: This study aims to evaluate medical interns' musculoskeletal examination performance after completing an online training and feedback module. METHODS: This study employed a quasi-experimental design. Medical interns were invited to complete a 4-week musculoskeletal physical examination training and feedback module via an e-learning platform. The course included written and audiovisual content pertaining to medical history, physical examination, and specific tests for the diagnosis of the most common knee, spine, shoulder, ankle, and foot conditions. Before and after completing the module, their ability to perform the physical examination was evaluated using an objective structured clinical examination (OSCE) with simulated patients that took place face-to-face. A control group of experts was assessed using the OSCE, and their performance was compared to that of the interns before and after the training. At the end of the module feedback on the OSCE was provided to participants through the platform asynchronously and two evaluation questions about the user experience were conducted at the end of the study. RESULTS: A total of 35 subjects were assessed using the OSCE, including 29 interns and 6 experts. At the beginning of the training module, the group of interns obtained an average score of 50.6 ± 15.1. At the end of the module, 18 interns retook the OSCE, and their performance increased significantly to an average of 76.6 ± 12.8 (p < 0.01). Prior to the training, the experts performed significantly better than the interns (71.2 vs. 50.6; p = 0.01). After the interns received the training and feedback, there were no significant differences between the two groups (71.2 vs. 76.6; p = 0.43). Two evaluation questions were conducted at the end of the study, revealing that 93% of the participants affirm that the training module will be useful in their clinical practice, and 100% of the participants would recommend the training module to a colleague. CONCLUSION: The online training and feedback module enhances the musculoskeletal examination performance of medical interns.

Los trastornos músculo-esqueléticos relacionados con el trabajo constituyen motivo de preocupación para la comunidad científica, avalado por su efecto negativo en la vida de los trabajadores y la productividad de las empresas. En el proceso de fundición de metales, los moldeadores manuales con pisón neumático exteriorizan manifestaciones de estas afecciones. Ello indujo a realizar una investigación que tuvo como objetivo elaborar un programa de Gimnasia Laboral para contribuir a la atención de los trastornos músculo-esqueléticos en los moldeadores manuales metalúrgicos. Para recopilar, analizar y procesar los datos se emplearon métodos teóricos, empíricos y matemático-estadísticos, entre ellos el histórico-lógico, analítico-sintético, inductivo-deductivo, sistémico-estructural-funcional, la observación participante, encuesta, entrevista, revisión documental, el experimento, criterio de usuarios y la estadística descriptiva e inferencial. La población objeto de estudio estuvo conformada por 19 moldeadores manuales metalúrgicos, seleccionados de forma intencional. El diagnóstico y la sistematización teórica alcanzada posibilitaron elaborar un programa de Gimnasia Laboral, en correspondencia con los requerimientos del proceso de moldeo, cuya efectividad fue constatada.


As lesões músculo-esqueléticas relacionadas com o trabalho são motivo de preocupação para a comunidade científica, sustentadas pelos seus efeitos negativos na vida dos trabalhadores e na produtividade das empresas. No processo de fundição do metal, os moldadores manuais com compactador pneumático externalizam as manifestações dessas condições. Isso originou uma pesquisa que teve como objetivo desenvolver um programa de Ginástica Laboral para contribuir no cuidado de lesões osteomusculares em moldadores manuais metalúrgicos. Para coletar, analisar e tratar os dados foram utilizados métodos teóricos, empíricos e matemático-estatísticos, incluindo histórico-lógico, analítico-sintético, indutivo-dedutivo, sistêmico-estrutural-funcional, observação participante, levantamento, entrevista, revisão documental, o experimento, critérios de usuário e estatísticas descritivas e inferenciais. A população em estudo foi composta por 19 modeladores manuais metalúrgicos, selecionados intencionalmente. O diagnóstico e a sistematização teórica conseguidos permitiram desenvolver um programa de Ginástica Laboral, em correspondência com as exigências do processo de moldagem, cuja eficácia foi verificada.


Work-related musculoskeletal disorders are a cause of concern for the scientific community, supported by their negative effect on the lives of workers and the productivity of companies. In the metal casting process, manual molders with pneumatic tamper externalize manifestations of these conditions. This led to carrying out a research that aimed to develop a Labor Gymnastics program to contribute to the care of musculoskeletal disorders in metallurgical manual shapers. To collect, analyze and process the data, theoretical, empirical and mathematical-statistical methods were used, including historical-logical, analytical-synthetic, inductive-deductive, systemic-structural-functional, participant observation, survey, interview, documentary review, the experiment, user criteria and descriptive and inferential statistics. The population under study was made up of 19 metallurgical manual shapers, intentionally selected. The diagnosis and theoretical systematization achieved made it possible to develop a Labor Gymnastics program, in correspondence with the requirements of the molding process, whose effectiveness was verified.

Os dentistas são um grupo de alto risco para o desenvolvimento de doenças de desordens musculoesqueléticas e tendo em vista que o sistema de produção industrial desenvolve produtos que atendem a maioria da população destra, os estudantes canhotos precisam se adequar a uma formação acadêmica, usando instrumentais, cadeiras odontológicas eoutros objetos projetados para destros. O presente trabalho teve como objetivo a coletade informações sobre os canhotos nos cursos de Odontologia da cidade de Uberlândia ­Minas Gerais. Foram incluídos todos os alunos canhotos matriculados no ano de 2022 e que estavam cursando ou já cursaram disciplinas com atividades laboratoriais ou clínicas. Questionários foram aplicados para identificação do perfil, das dificuldades, da ergonomia e das dores osteomusculares dos alunos canhotos em suas atividades. Os dados foram em seguida tabulados e passaram por análise estatística. Da quantidade total de alunos das três instituições (n=1.578), foram entrevistados 45 (2,8%) alunos canhotos, sendo a maioria feminina (80%), na qual identificou-se um posicionamento inadequado do operador canhoto quando comparado ao preconizado pela ISO-FDI, além da limitação de movimento na presença de auxiliar (82,2%). Os locais com maior frequência de dor/desconforto foram: pescoço (79%), costas superior esquerda (63%) e inferior esquerda (61%) e punhos/ mãos esquerda (56%). A intensidade da dor variou entre alguma, moderada e bastante. O impedimento de realizar atividades diárias foi relatado por 17% dos alunos (n=7) e destes somente 1 buscou atendimento médico. Não houve diferença estatística na comparação entre instituição pública e privada. Diante dos resultados, concluiu-se que os canhotos representam minoria dos alunos de Odontologia e apresentam várias regiões de dor/ desconforto devido às adaptações e posturas erradas durante os atendimentos. Apesar de grande parte apresentar dor, poucos tiveram impedimento de atividades rotineiras ou procuraram ajuda médica(AU)


Dentists are a high risk group for the development of musculoskeletal disorders and considering that the industrial production system develops products that serve the majority of the right-handed population, lefthanded students need to adapt to an academic training, using instruments, dental chairs and other objects designed for right-handers. This study aimed to collect information about left-handers in Dentistry courses in the city of Uberlândia - Minas Gerais. All left- handed students enrolled in the year 2022 and who were taking or had taken courses with laboratory or clinical activities were included. Questionnaires were applied to identify the profile, difficulties, ergonomics and musculoskeletal pain of left-handed students in their activities. The data were tabulated and then undergo statistical analysis. Of the total number of students from the three institutions (n=1,578), 45 (2.8%) left-handed students were interviewed, the majority being female (80%), in which an inadequate positioning of the left-handed operator was identified when compared to the recommended one by ISO-FDI, in addition to limitation of movement in the presence of an assistant (82.2%). The places with the highest frequency of pain/discomfort were: neck (79%), upper left back (63%) and lower left back (61%) and left wrists/hands (56%). The intensity of pain varied between some, moderate and a lot. The impediment to carrying out daily activities was reported by 17% of the students (n=7) and of these, only 1 sought medical attention. There was no statistical difference when comparing public and private institutions. In view of the results, it was concluded that left-handers represent a minority of dentistry students and have several regions of pain/discomfort due to adaptations and wrong postures during consultations.
