ABSTRACT

Low-income tropical regions, such as Haiti, grapple with environmental issues stemming from inadequate sanitation infrastructure for fecal sludge management. This study scrutinizes on-site sanitation systems in these regions, evaluating their environmental impacts and pinpointing improvement opportunities. The focus is specifically on systems integrating excreta valorization through composting and/or anaerobic digestion. Each system encompasses toilet access, evacuation, and sludge treatment. A comparative life cycle assessment was undertaken, with the functional unit managing one ton of excreta in Haiti over a year. Six scenarios representing autonomous sanitation systems were devised by combining three toilet types (container-based toilets (CBTs), ventilated improved pit (VIP) latrines, and flush toilets (WC)) with two sludge treatment processes (composting and biomethanization). Biodigester-based systems exhibited 1.05 times higher sanitary impacts and 1.03 times higher ecosystem impacts than those with composters. Among toilet types, CBTs had the lowest impacts, followed by VIP latrines, with WCs having the highest impacts. On average, WC scenarios were 3.85 times more impactful than VIP latrines and 4.04 times more impactful than those with CBTs regarding human health impact. Critical variables identified include the use of toilet paper, wood shavings, greenhouse gas emissions, and construction materials.

ABSTRACT

Despite advances in neonatology, neonatal mortality from preventable causes remains high in the North and Northeast regions of Brazil. This study aimed to analyze the determinants associated with neonatal and postneonatal mortality in newborns admitted to a neonatal intensive care unit. A cohort study was carried out in a capital in the Brazilian Northeast from 2013 to 2018. The outcome studied was death. Poisson regression was performed in the multivariate analysis of variables. Four hundred and eighty newborns were eligible, and 8.1% (39 newborns) died. Among them, 34 died in the neonatal period. The determinants that remained significantly associated with neonatal and postneonatal mortality in the final adjustment model (p < 0.05) were history of abortion, perinatal asphyxia, early neonatal sepsis and umbilical venous catheterization. All causes of this outcome were preventable. The neonatal mortality rate, although it did not include twins, neonates with malformations incompatible with life and other conditions, was 3.47 deaths per thousand live births (95% CI:1.10-8.03‱), well below the national average. In this study, pregnant women from different social classes had in common a private plan for direct access to health services, which provided them with excellent care throughout pregnancy and postnatal care. These results indicate that reducing neonatal mortality is possible through public policies with strategies that promote improvements in access to health services.

As falhas na higienização em um estabelecimento de alimentos podem refletir em problemas causando a contaminação ou deterioração do produto produzido. Esta pesquisa foi motivada por reclamações de consumidores informando que os queijos apresentaram fungos, mesmo estando dentro do prazo de validade e por solicitação do Serviço de Inspeção Municipal. O objetivo desta pesquisa foi avaliar a contaminação ambiental em uma agroindústria da agricultura familiar produtora de queijo colonial no Sudoeste Paranaense. Foram realizadas a contagem para aeróbios mesófilos em equipamentos e superfícies que entram em contato com o alimento e análise microbiológica ambiental de bolores e leveduras na sala de secagem dos queijos. A coleta foi realizada com método de esfregaço de suabe estéril para aeróbios mesófilos e semeadas em placas de Petri com Ágar Padrão de Contagem. Para a coleta ambiental foram expostas placas de Petri com ágar Saboraund durante 15 minutos. Os resultados demonstraram ausência de contaminação nas superfícies, mas foram encontrados bolores e leveduras de forma acentuada na sala de secagem dos queijos, o que pode contribuir para a deterioração do produto, diminuindo sua validade. Para minimizar as perdas por contaminação é necessário que o processo de higienização dos ambientes seja realizado de forma eficiente.


Failures in hygiene in a food establishment can result in problems causing contamination or deterioration of the product produced. This research was motivated by complaints from consumers reporting that the cheeses had mold, even though they were within their expiration date and at the request of the Municipal Inspection Service. This research was to evaluate environmental contamination in an agroindustry in the family farm producing colonial cheese in Southwest Paraná. For the microbiological assessment of environmental contamination, counting for mesophilic aerobes was carried out on equipment and surfaces that come into contact with food and, environmental microbiological analysis of molds and yeast in the cheese drying room. The collection was carried out using the sterile swab smear for mesophilic aerobes and seeded in Petri dishes with Counting Standard Agar. For environmental collection, sheets of Petri with Saboraund agar for 15 minutes. The results demonstrated absence of contamination on surfaces. But the presence of molds and yeasts in the drying room cheeses, which can contribute to the deterioration of the product and thus reduce the validity. To minimize losses due to contamination, it is It is necessary that the process of cleaning and disinfecting environments is carried out efficiently.

ABSTRACT

Schistosoma mansoni worms cause a waterborne parasitic disease called schistosomiasis. It commonly affects individuals in lack of sanitation structure. In Brazil, Pará state has Belém as one of the worst sanitation-ranking places in 2023, where schistosomiasis transmission was already documented. This study reports the occurrence of schistosomiasis in residents of Ilha das Onças, an island next to Belém. Stool samples were obtained from participants over 2 years old, all residents from Furo do Rio Grande, one of the rivers on the island. The Kato-Katz technique was performed for parasite investigation in the stool samples. Each participant responded to a sociodemographic and clinical questionnaire. The residences were georeferenced for map designing. Three out of 263 participants were S. mansoni positive, all men, ages ranging from 19 to 41 years old, with low parasitic load. Malacological surveys were carried out, but no Biomphalaria snails were found. Risk factors for schistosomiasis establishment are present on the island, and the lack of sanitation makes it a potential risk area. Malacological surveys are highly encouraged as preventive measures, as well as health surveillance for riverside populations, generating data that will help health authorities in the management and planning of preventive control actions.

BACKGROUND: Healthcare-associated infections are prevalent in low- and middle-income countries and may be reduced through proper hand hygiene (HH) adherence during patient care. AIM: We produced and distributed alcohol-based hand rub (ABHR) to 19 public primary- and secondary-level healthcare facilities in Quetzaltenango, Guatemala, and carried out HH observations to assess healthcare workers' (HCWs) HH adherence, and to identify factors associated with this practice. HH adherence was defined as washing hands with soap and water or using ABHR. METHODS: Observations were conducted before (2021, baseline) and after (2022, follow-up) ABHR distribution to evaluate the evolution of HH practices over time. Bivariate comparisons and mixed-effects logistic regression models were used to explore associations between HH adherence and the following independent variables: healthcare facility level, type of contact performed, timing of HH performance, occupational category of HCW and materials present (e.g., water, soap, ABHR). FINDINGS: We observed 243 and 300 patient interactions among 67 and 82 HCWs at each time point, respectively. HH adherence was low for both observation periods (40% at baseline and 35% at follow-up). HCWs were more likely to adhere to HH during invasive contacts, after patient contact, and if the HCW was a physician. CONCLUSION: HH adherence varied by scenario, which underscores the importance of addressing multiple determinants of behaviour change to improve adherence. This requires interventions implemented with a multi-modal approach that includes both increasing access to HH materials and infrastructure, as well as HH education and training, monitoring and feedback, reminders, and promoting a HH safety culture.

ABSTRACT

The banana tree circle (BTC) is a low-cost system for local greywater management, using a natural treatment and disposal process, providing additional resource recovery benefits. However, there are no standard design criteria for BTC that would allow for quality control of its efficiency and sustainability, and little is currently known about the full-scale performance of BTC. Based on the scoping literature review of 31 documents in the scientific database and eight documents from grey literature, a standard design model was proposed for the BTC technology based on the concept of water balance, greywater flows, rain, infiltration, and evapotranspiration. The first two steps of the BTC design were determining the areas required for infiltration and evapotranspiration. A cylindrical form trench, the soil percolation rate, and the hydraulic loading rate were considered for the infiltration area. The banana trees' evapotranspiration rate was taken into consideration for the evapotranspiration area. The proposed model was applied in a case study where we used a trench with 0.8 m depth and 1.5 m diameter. This study proposes a standard design criterion for the BTC based on environmental factors, and the scoping of the literature provides the basis for future studies to evaluate its environmental sustainability.

ABSTRACT

Over a hundred years ago after the discovery of Chagas disease (CD) in Brazil, the World Health Organization estimates a number of 6 to 7 million people infected by Trypanosoma cruzi worldwide. Therefore, the goal of this work was to identify variables related to the spread of infection by T. cruzi in humans living in rural areas, seeking predictor variables. A systematic review of the literature has been conducted, with a search in the Scopus platform, using the search string "Chagas disease" and "rural", resulting in 85 valid and analyzed scientific studies (1977 and 2022). Twenty-seven predictor variables have been acquired, and 19 of them have been grouped, such as: socioeconomic and educational, housing, environmental, sanitary, and cultural; and 8 variables related to T. cruzi seropositive individuals. The predictor variables yielded significant results (p-value < 0.05) in 59.5% of the cases (195/328), with a median of 66.7%. In other words, studies relating to 50% of the 27 variables showed significance equal to or greater than 66.7% of the time. The independent variables with the highest proportion of significant data (p-value < 0.05) were Education (87.6%), Intradomicile building (70%), Domestic animals (69.6%), and Triatomines (69.2%) in the households. Some variables reached 100%; however, few articles were found, indicating the need for further research, especially for Sanitation and Culture. It has been concluded that, in the several contexts found, the social vulnerability and lack of information led the individual to living in environments where inhabitability is inadequate, to perform limited work activity and develop habits and behaviors which impair them in an environmental insalubrity situation, favorable to the access of vectors and pathogens of anthropozoonoses such as CD.

This ongoing exploration contributes significantly to the body of knowledge disseminated within the scientific community. Nevertheless, the advent of the global era has presented nursing with novel challenges that persist from the dawn of history parallel to a lingering shadow obscuring the true essence of the profession. An example is environmental nursing, often overlooked from the lens of social sustainability­a neglect that researchers, as minority groups in a relentless struggle, have sidelined. These challenges align with the dynamic demands imposed by society and the mandates of state and/or governmental parliaments.

ABSTRACT

BACKGROUNDS: This study assesses the impact of Water, Sanitation, and Hygiene (WASH) interventions on cholera understanding and hygiene practices in La Gonâve Island, Haiti. It examines the changes after implementing interventions in seven villages across the Downtown, Mountain, and Seaside regions. METHODS: A retrospective investigation surveyed 210 school students from each region using a validated questionnaire. It assessed knowledge, attitudes, practices (KAP), and environmental aspects related to cholera and hygiene. Data analysis involved descriptive statistics and chi-square tests. RESULTS: The study highlights significant disparities in education levels, toilet ownership, and healthcare access. Challenges in finding public toilets (86.67%) and accessing water sources (67.78%) are consistent across regions, with Seaside facing financial constraints (85.00%) and water cost concerns (91.67%). Attitudes toward hygiene vary, with the Mountain region having the highest 'Never' responses for handwashing (38.89%), and Downtown leading in water treatment practices (11.67%). There is a strong willingness to share health knowledge, particularly in Downtown (100.00%). Seaside (83.33%) and Downtown (73.33%) revealed a higher cholera awareness, while nearly half of Mountain students lacked knowledge (54.44%). CONCLUSIONS: This study highlights significant disparities in WASH practices among La Gonâve's adolescents in Downtown, Mountain, and Seaside regions. Urgent interventions are crucial for improving sanitation, ensuring clean water access, and implementing targeted hygiene education, especially in the resource-constrained Mountain and Seaside areas. The findings underscore the vital roles of adolescents and schools in disseminating knowledge, with further research needed to explore intervention differences.

ABSTRACT

Las resistencias bacterianas a antimicrobianos representan uno de los principales problemas en la actualidad, encontrándose dentro de las principales causas de muerte en todo el mundo. Latinoamérica y Argentina, lejos de ser una excepción,presentan incidencias crecientes de infecciones por gérmenes resistentes. Cada día, se conocen mejor los mecanismos de resistencia que presentan los bacilos gram negativos y algunos cocos positivos. El problema no surge sólo por el sobreuso de antimicrobianos en la medicina clínica. Su sobreutilización para maximizar los beneficios productivos en la pesca, la ganadería y la agricultura contribuyen a esta situación. Desde la perspectiva de la atención primaria de la salud,consideramos fundamental conocer todos los aspectos que forman parte de esta problemática para intentar mitigar el daño que las resistencias bacterianas generan a nivel global. Argentina se transformó en el primer país de la región y del continente en contar con una ley para prevenir y controlar la resistencia a los Antimicrobianos. Consideramos de vital importancia que se fomenten más y mejores políticas sanitarias de orden público para enfrentar este creciente desafío. (AU)


Nowadays, bacterial resistance to antimicrobials is one of the main problems, being one of the leading causes of death worldwide. Latin America and Argentina, far from being an exception, have an increasing incidence of infections by resistant germs. Every day, the resistance mechanisms of gram-negative bacilli and some positive cocci are better known. The problem does not arise only because of the overuse of antimicrobials in clinical medicine. Its overuse to maximize productive benefits in fishing, livestock, and agriculture also contributes to this issue. From the perspective of primary health care,it is essential to know all the aspects of this problem to mitigate the damage that bacterial resistance generates at a global level. Argentina became the first country in the region and the continent to have a law to prevent and control antimicrobial resistance. We consider it vitally important that more and better public health policies are promoted to face this growing challenge. (AU)

ABSTRACT

Sewage sludge contains plant nutrients and organic matter in its composition, making it a potential partial substitute for mineral fertilizers if it meets environmental, agronomic, and sanitary standards. The objective was to evaluate the content of nutrients and heavy metals in the sludge generated in four wastewater treatment stations (WWTPs) in Rio de Janeiro state and assess its potential value and usefulness. The samples of 19 batches from the WTTPs Alegria, Barra da Tijuca, Ilha do Governador, and Sarapuí were analyzed. The WWTPs differ in methods and processes used for treating sewage and sludge. The total contents of C, N, P, K, Ca, Mg, Fe, Al, Na, Co, Mn, As, Ba, Cd, Cr, Cu, Ni, Se, Pb, and Zn were evaluated, as well as the ratio C/N, pH, organic matter content, and electrical conductivity. The grouping of sludge samples was assessed using principal components (PCA) and cluster analysis. The economic valuation of sludge was conducted utilizing the substitute goods method, which compared the sludge's N-P-K contents with the prices of consolidated nutrient sources. All the evaluated sludge batches exhibited concentrations of heavy metals below the limits allowed by Brazilian law, along with high levels of nutrients and organic matter. Considering the chemical characteristics, all evaluated materials showed potential for agricultural use, but it is crucial to evaluate the microbiological characteristics of sludge batches before agriculture application. PCA and cluster analysis demonstrated that sludge samples from the same WWTP clustered close to each other, demonstrating higher similarity among themselves than with samples from other WWTPs. The sludge had an average added value of U$ 88.46 per megagram, considering the total contents of N, P, and K in its composition. Land application of sewage sludge can reduce the need to purchase mineral fertilizers, thereby supporting the feasibility of reusing this material in the agricultural sector.

ABSTRACT

Ciprofloxacin hydrochloride (CIP) is a broad-spectrum synthetic antibiotic often found in domestic sewage and industrial waste due to the inefficiency of conventional treatments. Given the potential risk of drug accumulation, this study presents coatings of titanium dioxide nanotubes (TiO2) doped with different bismuth (Bi) concentrations to degrade CIP through photocatalytic and photoelectrochemical processes. Characterization studies revealed that bismuth (Bi) doping affected the morphology of the materials, with concentrations of 0.01 and 0.05 mol L-1, resulting in collapsed materials with a smaller active surface area. Photocatalysis tests for all the materials exhibited a similar degree of efficiency to photolysis, approximately 33%. Ecotoxicity tests using the biomarkers Lactuca sativa L., Lemna minor, and Artemia salina indicated that, although they were similar to photolysis in terms of efficiency, the effluents generated when employing the doped catalysts showed lower levels of toxicity, with the best results achieved for the material doped with 0.005 mol L-1 of Bi, with a toxicity level approximately 40% lower. Photoelectrocatalysis proved to be the most efficient CIP degradation technique. The highest degradation rate was observed for materials doped with 0.005 mol L-1 of Bi, with an efficiency of 46%, which is 1.4 times more efficient than photolysis. These results demonstrate that materials doped with low amounts of Bi can be effectively used as photoanodes for drug degradation, as their performance is superior, and the final product generated exhibits low toxicity to living organisms.

Adequate housing protects from diarrhea, which is a substantial health concern in low- and middle-income countries. The purpose of this study was to quantify the relationship between severe diarrhea and housing features at the municipal level to help in public health planning. Regression analyses were performed on annual (2000-2012) datasets on Brazilian municipalities (5570) in six household feature categories (e.g., waste management) and four severe diarrhea outcomes (e.g., diarrhea deaths of under-5 children). Household data were not available elsewhere of this magnitude and granularity, highlighting the scientific value-add of this study. Municipalities were clustered prior to regression analysis because of data heterogeneity. The compositional household feature data were also subjected to principal component analysis to diminish feature variable multicollinearity. The highest explanatory power was found for diarrhea deaths of under-5 children (R2 = 10-22 %), while those in the over-5 population were the least best explained (R2 = 0.3-7 %). Household features predicted diarrhea outcomes more accurately in the "advanced" housing municipality cluster (R2 = 16-22 %) than in the "mid-level" (R2 = 7-20 %) and "basic" (R2 = 6-12 %) ones (over-5 diarrhea deaths excluded). Under-5 children's diarrhea death prevalence was three times higher in the "basic" cluster than in the "advanced" cluster. Importantly, the impact of waste management was overall the largest of all household features, even larger than those of WASH, i.e., water supply, sanitation, and household drinking water treatment. This is surprising in the context of existing literature because WASH is generally regarded as the most important household factor affecting gastrointestinal health. In conclusion, public health interventions could benefit from customizing interventions for diarrhea outcomes, municipality types, and household features. Waste management's identified stronger association with diarrhea compared to WASH may have important implications beyond the water field and Brazil.

ABSTRACT

This study aimed to evaluate the level of counting by indicator microorganisms, identify the microbial ecology, detect Listeria monocytogenes and Salmonella sp., and determine the presence of virulence genes and biofilm formation. A total of 480 samples were collected from the surfaces of the equipment and utensils using sterile swabs for the detection of L. monocytogenes and Salmonella sp. and counting mesophilic aerobes, Enterobacteriaceae, Escherichia coli, and Pseudomonas sp. The microbial ecology was evaluated by sequencing the 16S rRNA gene. Genes for virulence and biofilm formation were analyzed and adhesion capacity was evaluated for L. monocytogenes and Salmonella sp. The mesophilic aerobe count was the highest in the dairy processing facility, followed by the pork and poultry slaughterhouses. L. monocytogenes was detected in all facilities, with the highest detection in the pork slaughterhouse, followed by the poultry and dairy facilities. Salmonella sp. was only detected in the dairy. Isolates of L. monocytogenes and Salmonella sp. showed poor adhesion to polystyrene surfaces, virulence genes, and biofilm formation. The frequent contaminants in the slaughterhouses were Pseudomonas, Acinetobacter, and Aeromonas in poultry, Acinetobacter, Pseudomonas, and Brevundimonas in pork, and Pseudomonas, Kocuria, and Staphylococcus in dairy. Our results provide useful information to understand the microbiological risks associated with contamination.

INTRODUCTION: Diarrheal disease, particularly in children under 5 years old, remains a global health challenge due to its high prevalence and chronic health consequences. Public health interventions that reduce diarrheal disease risk include improving access to water, sanitation, and hygiene. Although Peru achieved the 2015 Millennium Development Goal (MDG) indicators for water access, less progress was achieved on sanitation. Furthermore, many Indigenous Peoples were overlooked in the MDG indicators, resulting in a prioritization of Indigenous Peoples in the 2030 Sustainable Development Goals (SDGs). This study aimed to estimate the prevalence of childhood diarrhea, characterize access to water and sanitation, and determine the association of childhood diarrhea with water access and sanitation indicators in 10 Shawi Indigenous communities along the Armanayacu River in the Peruvian Amazon. METHODS: A cross-sectional survey (n=82) that captured data on diarrheal disease, sociodemographic variables, and water and sanitation exposures was conducted in 10 Shawi communities. Nutritional status of children under 5 was also assessed via physical examination. Descriptive and comparative statistics were conducted. RESULTS: A small proportion (n=7; 8.54%) of participating children reported an episode of diarrhea in the previous month. Almost half (46.30%) of participating children had stunting, wasting, or both. Although not statistically significant, children living in households that used latrines were 4.29 times (95% confidence interval (CI) 1.01-18.19) more likely to report an episode of diarrhea than children living in households that practiced open defecation. Although not statistically significant, children living in households that used water treatment methods were 4.25 times (95%CI 0.54-33.71) more likely to report an episode of diarrhea than children living in households that did not. CONCLUSION: The prevalence of childhood diarrhea was lower for Shawi than for other Amazon areas. The higher prevalence of childhood diarrhea in households that used latrines and water treatments warrants further investigation into local risk and protective factors. These Shawi communities scored low for the WHO/UNICEF Joint Monitoring Programme indicators for water and sanitation, indicating that they should be prioritized in future water, sanitation, and hygiene initiatives. Research will be required to understand and incorporate local Indigenous values and cultural practices into water, sanitation, and hygiene initiatives to maximize intervention uptake and effectiveness.

ABSTRACT

Objetivo:o artigo defende a constitucionalidade do marco legal do saneamento, com foco nas controvérsias relacionadas à repartição de competências entre União, estados e municípios. Metodologia:tratou-se de um estudo jurisprudencial em que foi analisado o inteiro teor das ações diretas de inconstitucionalidade submetidas ao Supremo Tribunal Federal,as quais discutem a constitucionalidade da Lei nº14.026/2020, bem como outras ações de controle de constitucionalidade relacionadas ao tema. O texto normativo foi obtido em sítio na internet, assim como o inteiro teor dos acórdãos das ações. Resultados:foram encontrados julgados anteriores que convergem para o que determinou o legislador na lei que instituiu o marco legal do saneamento. Conclusão:a despeito das críticas dirigidas ao novo marco legal do saneamento básico, sua formulação obedeceu aos critérios impostos pela Constituição Federal de 1988e aos posicionamentos do Supremo Tribunal Federal no exercício do seu poder de interpretação do texto constitucional.


Objective: the article defends the constitutionality of the Legal Sanitation Framework, focusing on controversies related to the division of powers between the Union, states and municipalities. Methods: it was a jurisprudential study in which the entire content of the direct actions of unconstitutionality submitted to the Federal Supreme Court,which discuss the constitutionality of Law n. 14.026/2020, as well as other judicial review cases related to the subject. The normative text was obtained on a website, as well as the full content of the judgmentsof the lawsuits. Results: previous judgments were found that converge with what the legislator determined in the Legal Sanitation Framework. Conclusion: despite the criticism directed at the new Legal Sanitation Framework, its formulation followed the criteria imposed by the Federal Constitution of 1988andthe positions of the Federal Supreme Court in the exercise of its power to interpret the constitutional text.


Objectivo: el artículo defiende la constitucionalidad del Marco Jurídico Sanitario, centrándose en controversias relacionadas con la división de poderes entre la Unión, los estados y los municipios. Metodología: se trató de un estudio jurisprudencial en el que se analizó todo el contenido de la ADI presentada al Tribunal Supremo Federal en la que se discute la constitucionalidad de la Ley n. 14.026/2020, así como otros casos de control de constitucionalidad judicial relacionados con el tema. El texto normativo fue obtenido del sitio web, así como todo el contenido de los dictámenes de la Corte. Resultados: la investigación logró encontrar precedentes legales que convergen con lo determinado por el legislador en el Marco Jurídico Sanitario. Conclusión: a pesar de las críticas dirigidas al nuevo Marco Jurídico Sanitario, su formulación siguió los criterios impuestos por la Constitución Federal de 1988, así como las posiciones del Supremo Tribunal Federal en el ejercicio de su facultad de interpretación del texto constitucional.

ABSTRACT

Resumo Este trabalho aborda os desafios contemporâneos do setor de saneamento básico brasileiro. As características de rentabilidade e dinamismo setorial puseram os serviços estatais de A & E no foco da agenda das reformas estruturais sob o argumento de que os governos não dispõem de recursos necessários para universalizar a cobertura. A agenda da reforma dos serviços de A & E advoga a concessão a empresas privadas da comercialização do abastecimento de água e do tratamento do esgotamento sanitário. Em convergência com esta agenda de reforma estrutural, a aprovação em 15 de julho de 2020 da Lei 14.026 alterou em profundidade o marco legal do saneamento, adotando integralmente a pauta da desestatização do setor. O novo marco regulatório veta o contrato de programa, torna obrigatória a licitação para contratação do serviço de saneamento, estabelece diretrizes nacionais para que estados e municípios acessem recursos federais e institui o regime de regulação nacional por meio da Agência Nacional de Águas e Saneamento Básico. O modelo cooperativo praticado no Sistema Único de Saúde (SUS) pode servir como referência para a construção de um pacto social no setor.


Abstract This paper describes the challenges currently facing Brazil's basic sanitation sector. The sector's characteristic profitability and dynamism have brought State-run Water & Sewage (W&S) services into the spotlight of the structural reform agenda on the argument that governments do not have the resources necessary to universalize coverage. There is a pattern of W&S services' operating with a surplus, which helps explain the intense dispute in recent years in favour of legal changes to the market position of CESBs and local providers to favour an expanding role for private agents. Converging with the structural reform agenda, Law 14,026, sanctioned on 15 July 2020, made far-reaching changes to the legal framework for sanitation and provided for the State's complete withdrawal from the sector. The new regulatory framework prohibited programme contracts, required tender processes for sanitation service contracts, encouraged regionalization without CESBs, set out national guidelines for States and municipalities to access federal funding and introduced a national regulation regime under the National Waters and Basic Sanitation Agency. The model of cooperation that operates in the SUS could serve as a reference for constructing a social pact in the sector.

ABSTRACT

Objective: To investigate the relationship between the prevalence of schistosomiasis and soil-transmitted helminthiasis with variables related to access to water, sanitation and solid waste in Latin American and Caribbean (LAC) countries. Method: A systematic review was performed in the LILACS, PubMed, Web of Science, and SciELO databases. Studies published between 1950 and August 2021, with an ecological design and a focus on population groups (states, municipalities and/or districts), having the prevalence of infection by Schistosoma mansoni, Ancylostoma sp., Necator americanus, Ascaris lumbricoides or Trichuris trichiura as primary variable and access to water, sewage and/or solid waste as explanatory variables were included. Open access articles with full text available in English, Spanish, or Portuguese were considered. The risk of bias and the quality of the studies were assessed according to the Joanna Briggs Institute manual. Results: Of 2 714 articles, nine were eligible, published between 1994 and 2021 and covering 22 LAC countries and 14 350 municipalities. All articles had moderate methodological quality. Environmental variables indicated an association between water supply and solid waste collection with schistosomiasis; water supply with ascariasis, trichuriasis and hookworm; and sewage with ascariasis and hookworm. Except for one article, which had regional coverage for LAC, all the others were developed in Brazil. Conclusion: There is a clear need to expand research on the association between household and collective health conditions and parasitic diseases for all endemic countries in LAC to support environmental strategies to control these diseases.


Objetivo: Investigar la relación entre la prevalencia de esquistosomiasis y geohelmintiasis y las variables de acceso al agua, el saneamiento y el manejo de residuos sólidos en los países de América Latina y el Caribe. Métodos: Se realizó una revisión sistemática en las bases de datos LILACS, PubMed, Web of Science y SciELO. Todos los artículos fueron de calidad metodológica moderada. Se incluyeron estudios publicados entre 1950 y agosto del 2021, con diseño ecológico y atención en agregados demográficos (estados, municipios o distritos), que tuvieran como resultado principal la prevalencia de infección por Schistosoma mansoni, Ancylostoma spp., Necator americanus, Ascaris lumbricoides o Trichuris trichiura y como variable explicativa el acceso al agua, el saneamiento y el manejo de residuos sólidos. Se analizaron artículos de texto completo y acceso libre en español, inglés o portugués. El riesgo de sesgo y la calidad de los estudios se evaluaron según las normas del manual del Instituto Joanna Briggs. Resultados: De los 2 714 artículos, hubo 9 que cumplieron con los requisitos establecidos; estos se publicaron entre 1994 y el 2021 y abarcaron 22 países y 14 350 municipios de América Latina y el Caribe. Las variables ambientales indicaron una relación del abastecimiento de agua y la recolección de residuos sólidos con la esquistosomiasis; del abastecimiento de agua con la ascariasis, la tricuriasis y la anquilostomiasis; y del saneamiento con la ascariasis y la anquilostomiasis. Con excepción de un artículo que abarcó la Región de América Latina y el Caribe, todos los demás se realizaron en Brasil. Conclusiones: Es evidente la necesidad de ampliar las investigaciones sobre la relación entre las condiciones sanitarias domésticas y colectivas y las enfermedades parasitarias en todos los países de América Latina y el Caribe donde son endémicas, con el fin de formular estrategias centradas en el medio ambiente para controlar esas enfermedades.

ABSTRACT

Escherichia coli is a well-characterized micro-organism in scientific literature. Similarly, quaternary ammonium compounds (QACs) are historical sanitizers in food processing. However, the use of QACs has been questioned due to bacterial resistance in some studies. Therefore, this study aimed to compare effects of single and mixed cultures of E. coli strains of different serogroups with either high (six strains) or low (five strains) resistance to QACs. Twenty-five combinations of strains with either high (H)- or low (L)-QAC resistance were analyzed (H + H vs. L + L). After exposure to QAC, combinations with statistical differences (p < 0.05) compared with individuals were selected and an inactivation model determined using GInaFit®. Only one combination of two strains (C23 and C20) with low-QAC resistance (mixture T18) had greater resistance (p < 0.05) than the individual isolates. The combination T18 and individual strain C23 presented a Weibull model, whereas the other isolated strain (C20) presented a biphasic inactivation model with a shoulder. Whole genome sequencing determined that unlike C20, C23 carried yehW, which may have led to Weibull inactivation. Possibly, very rapid interaction of C20 with the QAC favored increased survival of C23 and overall persistence of the T18 mixture. Consequently, our results indicate that individual E. coli with low-QAC resistance can synergistically interfere with QAC inactivation.

ABSTRACT

Background and objective: the self-styled population of Marajoara is vulnerable due to the neglect of basic services governed by the constitution, such as basic sanitation, health and education. Therefore, the search and mapping of epidemiological data are necessary in order to have a real dimension of how certain diseases behave in the region. Therefore, this study aimed to analyze the space-time distribution of neglected tropical diseases and their relationship with socio-environmental indicators in the Marajó Archipelago, Pará, Amazon. Methods: this is a descriptive and ecological study, with a time series, with a quantitative approach, which assessed the incidence rate of neglected tropical diseases in the 16 municipalities that make up Marajó from 2007 to 2016. Analyzes of socioeconomic and population data were carried out, extracted from the Federation of the Industries of the State of Rio de Janeiro (FIRJAN System) and the Brazilian Institute of Geography and Statistics. Results: according to the survey, American cutaneous leishmaniasis, dengue and tuberculosis were the most reported diseases in total. All municipalities had at least one disease in the period. The spatial distribution of the numbers of cases of tropical diseases in all the municipalities that make up the Archipelago between 2007 and 2016 was carried out. Conclusion: the anthropization of the environment has favored the proliferation of vector agents and, consequently, the incidence of diseases related to inadequate basic sanitation, whose configuration is the result of a disorganized urbanization process and historically exclusive development, with a lack of investment, making this damage even more dramatic for the population.(AU)


Justificativa e objetivo: a população autodenominada de marajoara encontra-se em vulnerabilidade devido às negligências de serviços básicos regidos na constituição, como saneamento básico, saúde e educação. Logo, a busca e o mapeamento de dados epidemiológicos são necessários para que se tenha uma dimensão real de como se comportam determinados agravos na região. Diante disso, o estudo objetivou analisar a distribuição espaço-temporal das doenças tropicais negligenciadas e sua relação com indicadores socioambientais no Arquipélago do Marajó, Pará, Amazônia. Métodos: este estudo é do tipo descritivo e ecológico, de série temporal, com abordagem quantitativa, que avaliou a taxa de incidência das doenças tropicais negligenciadas nos 16 municípios que compõem o Marajó no período de 2007 a 2016. Foram realizadas análises de dados socioeconômicos e populacionais extraídos da Federação das Indústrias do Estado do Rio de Janeiro (Sistema FIRJAN) e do Instituto Brasileiro de Geografia e Estatística. Resultados: de acordo com o levantamento, a leishmaniose tegumentar americana, a dengue e a tuberculose foram os agravos de maior notificação no total. Todos os municípios apresentaram, ao menos, uma doença no período. Realizou-se a distribuição espacial dos números de casos de doenças tropicais em todos os municípios que compõem o Arquipélago entre 2007 e 2016. Conclusão: a antropização do ambiente vem favorecendo a proliferação de agentes vetoriais e, consequentemente, a incidência de doenças relacionadas ao saneamento básico inadequado, cuja configuração é fruto de um processo de urbanização desordenado e desenvolvimento historicamente excludente, com ausência de investimentos, tornando esse prejuízo ainda mais dramático para a população.(AU)

Por lo tanto, la búsqueda y el mapeo de datos epidemiológicos son necesarios para tener una dimensión real de cómo se comportan ciertas enfermedades en la región. Por lo tanto, el estudio tuvo como objetivo analizar la distribución espacio-temporal de las enfermedades tropicales desatendidas y su relación con los indicadores socioambientales en el Archipiélago de Marajó, en el estado de Pará, Amazonas. Métodos: se trata de un estudio descriptivo y ecológico, con serie temporal, con enfoque cuantitativo, que evaluó la tasa de incidencia de las enfermedades tropicales desatendidas en los 16 municipios que componen el Marajó de 2007 a 2016. Se realizaron análisis de datos socioeconómicos y poblacionales extraídos de la Federación de Industrias del Estado de Rio de Janeiro (Sistema FIRJAN) y del Instituto Brasileño de Geografía y Estadística.
