ABSTRACT

Most redox processes that break/form bonds involve net 2e- changes, and many are coupled to protons. Yet most proton-coupled electron transfer (PCET) studies focus on 1e-/1H+ reactions. Reported here is a family of molecular models that undergo tunable 2e-/2H+ redox changes. Complexes [(X2bpy)RuII(en*)2](PF6)2 and [(X2bpy)RuIV(en*-H)2](PF6)2 have been synthesized with bpy=2,2'-bipyridine with 4,4'-subtitutions X=-NMe2, -OMe, -Me, -H, -CF3; and en*=2,3-dimethyl-2,3-butanediamine. They have been characterized by IR, UV-vis, and NMR spectroscopies, XRD, electrochemistry, mass spectrometry, DFT and (TD)DFT computations. The introduction of electron-withdrawing and donating groups at the 4,4'-position of the bpy ligand affects the complexes' redox potentials, pKa's, and Bond Dissociation Free Energies (BDFEs) of the N-H bonds in the en* ligands. The average BDFEs for the overall 2e-/2H+ PCET span over 5 kcal/mol. Notably, these complexes all show marked potential inversion over an extended range, ΔpKa>25 units and ΔE0>1.4 V. Potential inversion remains despite the electronic influence of bpy's substitutions which regulate N-H properties several bonds apart by trans-effect over dπ-molecular orbitals at the Ru center. The experimental and computational results presented in this work support the presence of strong coupling between electrons and protons, for modelling insights of 2e-/2H+ transfer reactivity.

ABSTRACT

Structural variation is a source of genetic variation that, in some cases, may trigger pathogenicity. Here, we describe two cases, a mother and son, with the same partial inverted duplication of the long arm of chromosome 8 [invdup(8)(q24.21q24.21)] of 17.18 Mb, showing different clinical manifestations: microcephaly, dorsal hypertrichosis, seizures and neuropsychomotor development delay in the child, and a cleft lip/palate, down-slanted palpebral fissures and learning disabilities in the mother. The deleterious outcome, in general, is reflected by the gain or loss of genetic material. However, discrepancies among the clinical manifestations raise some concerns about the genomic configuration within the chromosome and other genetic modifiers. With that in mind, we also performed a literature review of research published in the last 20 years about the duplication of the same, or close, chromosome region, seeking the elucidation of at least some relevant clinical features.

ABSTRACT

Objective: The atrio-ventricular and ventricle-arterial double discordance (DD) or corrected transposition of the great arteries is a rare heart disease, it occurs in 0.02-0.07 of every 1,000 live newborns. The objective of the study is to describe the diagnosis, treatment and evolution of a series of patients with DD. Method: A retrospective and descriptive study was carried out, reviewing the records of patients diagnosed with DD in the last 22 years. Descriptive statistics were performed. Numerical variables were obtained using means and standard deviation and categorical variables using frequencies and percentages. Results: Thirty patients were studied in 22 years with a ratio of 1.5:1 for men, with a mean age of 20 months. The situs was solitus in 24/29 patients (82.7%). Ventricular septal defect was the most frequent lesion in 25/29 (86.2%) Tricuspid insufficiency in 70%. Four patients diagnosed with pulmonary atrial hypertension. With atrio-ventricular block 20%. One with Wolff-Parkinson-White syndrome. Surgical treatment was carried out in 70% of patients. Eight with Glenn procedure (26.6%) and 4 with Fontan surgery (13.3%). Follow-up ranged from 1 month to 17 years. Five died (16.6%). Of the 25 patients in follow-up, 18 patients (72%) had normal ventricular function, 5 with Grade II Ross classification (20%) and 2 in Grade III (8%). Conclusions: The quality of life of these patients is improving and there is still controversy in the literature about the ideal time to perform the most appropriate surgical procedure.

ABSTRACT

INTRODUCTION: Reactance inversion (RI) has been associated with impaired peripheral airway function in persistent asthma. However, there is little to no data about the difference between asthmatic children with and without RI. This study aimed to detect clinical and lung function differences in moderate-severe asthmatic children with and without RI. METHODS: This study was conducted between 2021 and 2022 in asthmatic school-age children. Impulse oscillometry (IOS) and spirometry were performed according to ATS/ERS standards. RESULTS: A total of 62 patients, with a mean age of 8.4 years, 54.8% were males and were divided into three groups: group 1 (32.3%) with no RI, group 2 (27.4%) with RI but disappearing after bronchodilator test and group 3 (40.3%) with persistent RI after bronchodilator test. Children in groups 2 and 3 had significantly lower birth weights than in group 1. Group 2 had lower gestational age compared to group 1. FEV1 and FEF25-75 of forced vital capacity were significantly lower in groups 2 and 3. In group 3, R5, AX, R5-20, and R5-R20/R5 ratios were significantly higher. Bronchodilator responses (BDR) in X5c, AX, and R5-R20 were significantly different between groups and lower in group 3. CONCLUSION: RI is frequently found in children with moderate-severe persistent asthma, particularly in those with a history of prematurity or low birth weight. In some patients, RI disappears after the bronchodilator test; however, it, persists in those with the worst pulmonary function. RI could be a small airway dysfunction marker.

ABSTRACT

Uterine inversion is a rare and infrequent disease. The case of a patient with a non-puerperal uterine inversion that had to be treated with a total abdominal hysterectomy is presented in this study. With no important history of disease, she attended the emergency department presenting abdominal pain in the last 10 days associated with vaginal bleeding and mass sensation. The physical examination revealed a protruding necrotic-like mass through the vagina, hence the suspicion of a myoma nascens. During her hospital ward stay, she presented hemodynamic instability, urgent abdominal hysterectomy had to be done which revealed uterine inversion. Non-puerperal uterine inversion is infrequent. Precise clinical diagnosis is important supported by diagnostic imaging if available. The surgical intervention is necessary, giving a good prognosis. Vaginal hysterectomy is not easy in this type of cases, therefore total abdominal hysterectomy is recommended.

ABSTRACT

OBJECTIVES: When air irrigation is used for caloric stimulation in patients with a perforated ear, warm irrigation may elicit a nystagmus that initially beats in the opposite direction of what is expected for warm irrigations, which is referred to as "caloric inversion". This study aimed to investigate the disease group in which caloric inversion appeared in patients who underwent caloric testing and to classify the patterns of caloric inversion. METHODS: We conducted a retrospective review of bithermal caloric test results that were collected in our dizziness clinic between 2005 and 2022. Caloric inversion was defined when nystagmus induced by caloric stimulation appeared in the opposite direction to that expected. The incidence of caloric inversion among all patients who underwent bithermal caloric tests was calculated. To confirm the clinical diagnoses of the patients with caloric inversion, their clinical records were reviewed. RESULTS: Out of 9923 patients who underwent bithermal caloric tests, 29 patients (0.29%) showed a caloric inversion. The most common clinical diagnosis was chronic otitis media (21 of 29, 72%). Of the 21 patients with chronic otitis media, 20 patients showed a caloric inversion by warm air irrigation and one patient showed caloric inversion by cold air stimulation. Patients with clinical diagnoses other than chronic otitis media such as sudden sensorineural hearing loss, benign paroxysmal vertigo of childhood and recurrent vestibulopathy showed caloric inversion by warm air irrigation. Caloric inversion by warm water irrigation was observed in patients with lateral semicircular canal cupulopathy and recurrent vestibulopathy. Two patients (one with Meniere's disease and one with age-related dizziness) showed caloric inversion by cold water irrigation. CONCLUSION: Caloric inversion can be observed in various diseases other than chronic otitis media with tympanic membrane perforation. Special care should be taken in the interpretation of caloric test results. LEVEL OF EVIDENCE: Level 4.

ABSTRACT

Most studies modeling population mobility and the spread of infectious diseases, particularly those using meta-population multi-patch models, tend to focus on the theoretical properties and numerical simulation of such models. As such, there is relatively scant literature focused on numerical fit, inference, and uncertainty quantification of epidemic models with population mobility. In this research, we use three estimation techniques to solve an inverse problem and quantify its uncertainty for a human-mobility-based multi-patch epidemic model using mobile phone sensing data and confirmed COVID-19-positive cases in Hermosillo, Mexico. First, we utilize a Brownian bridge model using mobile phone GPS data to estimate the residence and mobility parameters of the epidemic model. In the second step, we estimate the optimal model epidemiological parameters by deterministically inverting the model using a Darwinian-inspired evolutionary algorithm (EA)-that is, a genetic algorithm (GA). The third part of the analysis involves performing inference and uncertainty quantification in the epidemic model using two Bayesian Monte Carlo sampling methods: t-walk and Hamiltonian Monte Carlo (HMC). The results demonstrate that the estimated model parameters and incidence adequately fit the observed daily COVID-19 incidence in Hermosillo. Moreover, the estimated parameters from the HMC method yield large credible intervals, improving their coverage for the observed and predicted daily incidences. Furthermore, we observe that the use of a multi-patch model with mobility yields improved predictions when compared to a single-patch model.

ABSTRACT

In this work, the preparation of dense blended membranes, from blends of poly(vinylidene fluoride) (PVDF), poly(ether sulfone) (PES) and polyethyleneimine (PEI) or Fumion®, with possible applications in alkaline fuel cell (AEMFC) is reported. The blended PEI/Fumion® membranes were prepared under a controlled air atmosphere by a solvent evaporation method, and were characterized regarding water uptake, swelling ratio, thermogravimetric analysis (TGA), infrared spectroscopy (FT-IR), scanning electron microscopy (SEM), ion exchange capacity (IEC), OH- conductivity and novel hydroxide ion exchange rate (HIER), which is related to the mass transport capacity of the OH- ions through the membrane. The effect of the chemical composition on its morphological and anion exchange properties was evaluated. It was expected that the usage of a commercial ionomer Fumion®, in the blended membranes would result in better features in the electrical/ionic conductivity behaviour. However, two of the membranes containing PEI exhibited a higher HIER and OH- conductivity than Fumion® membranes, and were excellent option for potential applications in AEMFC, considering their performance and the cost of Fumion®-based membranes.

ABSTRACT

Multiple drug resistance (MDR) is the tumor's way of escaping the cytotoxic effects of various unrelated chemotherapeutic drugs. It can be either innate or acquired. MDR represents the end of the therapeutic pathway, and it practically leaves no treatment alternatives. Reversing MDR is an unfulfilled goal, despite the important recent advances in cancer research. MDR, the main cause of death in cancer patients, is a multi-factorial development, and most of its known causes have been thoroughly discussed in the literature. However, there is one aspect that has not received adequate consideration - intracellular alkalosis - which is part of wider pH deregulation where the pH gradient is inverted, meaning that extracellular pH is decreased and intracellular pH increased. This situation interacts with MDR and with the proteins involved, such as P-gp, breast cancer resistance protein, and multidrug associated resistance protein 1. However, there are also situations in which these proteins play no role at all, and where pH takes the lead. This is the case in ion trapping. Reversing the pH gradient to normal can be an important contribution to managing MDR. The drugs to manipulate pH exist, and most of them are FDA approved and in clinical use for other purposes. Furthermore, they have low or no toxicity and are inexpensive compared with any chemotherapeutic treatment. Repurposing these drugs and combining them in a reasonable fashion is one of the points proposed in this paper, which discusses the relationship between cancer's peculiar pH and MDR.

ABSTRACT

In tilapia aquaculture, the cultivation of single-sex animals is extremely widespread, as it allows for the standardization of lots, in addition to improving the general performance of the animals. However, it is possible that hormonal inversion interacts with other factors, such as environmental and nutritional management, and modulates the immune response and antioxidant system of animals in a distinct manner. In order to test this hypothesis, an experiment was carried out using Nile tilapia larvae six days after hatching, divided into four experimental groups: NI (non-inverted animals), I (sexual inverted animals), NI + M (non-inverted supplemented with microencapsulated products) and I + M (sexual inverted and supplemented with microencapsulated products; half of which were subjected to transportation-related stress after 28 days of the experiment. At the end, the survival rate was evaluated; the gene expression of heat shock protein (HSP70), interleukin 1 beta (IL-1ß) and cyclooxygenase-2 (COX 2) via RT-PCR; also evaluated were the activity of catalase (CAT) and superoxide dismutase (SOD) enzymes, as well as the total antioxidant capacity by 2,2-diphenyl-1-picryl-hydrazyl (DPPH). Animals from the I and I + M groups had the highest survival rate (p < 0.001) regardless of transportation stress. The highest expressions of HSP70 were found in the NI group (p < 0.001, with and without transportation). For the IL-1ß gene, there was an increase in expression for animals belonging to groups NI and NI + M (no transportation); and NI (p < 0.0001, with transportation). Increased COX 2 expression was observed for all groups after transportation (p < 0.0001). The highest SOD activities were observed in groups I and I + M (without transportation, p = 0.0004), and I (with transportation, p < 0.0001). The transportation decreased the total antioxidant capacity of DPPH in all treatments (p < 0.001). Finally, when evaluating all of the results together, we came to a conclusion that sex inversion improves the immune response and antioxidant profile of animals under stressful conditions when associated with microencapsulated dietary supplementation.

ABSTRACT

ABSTRACT The research presented below corresponds to the analysis of public finances, specifically the Municipal Territorial Tax Site (SFTM), of the municipality of Santiago de Cali, Colombia, in the communes 21 [low-low socioeconomic status], 10 [middle socioeconomic status] and 22 [high-high socioeconomic status], during the period 2012-2015, in which an investment was made by the municipality in order to satisfy the needs of the population and improve people's quality of life. To carry out the analysis, the Development Plans of Communes and Townships, the Annual Operational Investment Plans (POAI) and the executed budget of the SFTM were used as tolos, comparing the per capita investment of the intervened problems between the studied communes. As a result of this analysis, failures in the allocation and execution of resources were identified, since sensitive problems were not prioritized for communes 21 and 10, such as security, job training, care for the elderly and the political participation of social organizations, which are considered important for the economic and social development of the less favored communes.

ABSTRACT

Abstract: Objective: Evaluating the behavior of intrapulmonary pressures with the inversion of the inspiration expiration index, during elective videolaparoscopic cholecystectomy. Material and methods: A phase three controlled clinical trial was conducted on intrapulmonary pressures during laparoscopic cholecystectomies at the «Manuel Ascunce Domenech¼ University Hospital, from September 2016 to February 2020, the universe consisted of patients who met the inclusion criteria, from which a sample of 106 patients was selected, by a simple randomizing method. The primary source of data collection was the medical records and the result of a survey. These data were collected in a model designed in correspondence with the bibliography reviewed. Results: Measured by variation in peak inspiratory pressure and plateau pressure after pneumoperitoneum, with significant decrease of both after inversion of I:E index. More than half of the patients presented arterial hypertension, arrhythmias and arterial hypotension, without them being related to the application of the inverted I:E index. Conclusions: The inversion of inspiratory expiration contributed to improve the effects of pneumoperitoneum on intrapulmonary pressures, with adequate oxygenation and no cardiovascular effects.

ABSTRACT

Chromosomal inversions are known to play roles in adaptation and differentiation in many species. They involve clusters of correlated genes (i.e., loci in linkage disequilibrium, LD) possibly associated with environmental variables. The grasshopper "species complex" Trimerotropis pallidipennis comprises several genetic lineages distributed from North to South America in arid and semi-arid high-altitude environments. The southernmost lineage, Trimerotropis sp., segregates for four to seven putative inversions that display clinal variation, possibly through adaptation to temperate environments. We analysed chromosomal, mitochondrial and genome-wide single nucleotide polymorphism data in 19 Trimerotropis sp. populations mainly distributed along two altitudinal gradients (MS and Ju). Populations across Argentina comprise two main chromosomally and genetically differentiated lineages: one distributed across the southernmost border of the "Andes Centrales," adding evidence for a differentiation hotspot in this area; and the other widely distributed in Argentina. Within the latter, network analytical approaches to LD found three clusters of correlated loci (LD-clusters), with inversion karyotypes explaining >79% of the genetic variation. Outlier loci associated with environmental variables mapped to two of these LD-clusters. Furthermore, despite the complex geographical history indicated by population genetic analyses, the clines in inversion karyotypes have remained stable for more than 20 generations, implicating their role in adaptation and differentiation within this lineage. We hypothesize that these clines could be the consequence of a coupling between extrinsic postzygotic barriers and spatially varying selection along environmental gradients resulting in a hybrid zone. These results provide a framework for future investigations about candidate genes implicated in rapid adaptation to new environments.

This information is stored in the sediment record at the lake bottom, and to make it available for paleoclimate research, potential target sites with undisturbed and continuous sediment sequences need to be identified. Different geophysical methods are suitable to identify, explore, and characterize sediment layers prior to sediment core recovery. Due to the high resolution, reflection seismic methods have become standard for this purpose. However, seismic measurements cannot always provide a comprehensive image of lake-bottom sediments, e.g., due to lacking seismic contrasts between geological units or high attenuation of seismic waves. Here, we developed and tested a complementary method based on water-borne electrical-resistivity tomography (ERT) measurements. Our setup consisted of 13 floating electrodes (at 5 m spacing) used to collect ERT data with a dipole-dipole configuration. We used a 1D inversion to adjust a layered-earth model, which facilitates the implementation of constraints on water depth, water resistivity, and sediment resistivity as a priori information. The first two parameters were readily obtained from the echo-sounder and conductivity-probe measurements. The resistivity of sediment samples can also be determined in the laboratory. We applied this approach to process ERT data collected on a lake in southern Mexico. The direct comparison of ERT data with reflection seismic data collected with a sub-bottom profiler (SBP) showed that we can significantly improve the sediment-thickness estimates compared to unconstrained 2D inversions. Down to water depths of 20 m, our sediment thickness estimates were close to the sediment thickness derived from collocated SBP seismograms. Our approach represents an implementation of ERT measurements on lakes and complements the standard lake-bottom exploration by reflection seismic methods.

ABSTRACT

The seismic data inversion from observations contaminated by spurious measures (outliers) remains a significant challenge for the industrial and scientific communities. This difficulty is due to slow processing work to mitigate the influence of the outliers. In this work, we introduce a robust formulation to mitigate the influence of spurious measurements in the seismic inversion process. In this regard, we put forth an outlier-resistant seismic inversion methodology for model estimation based on the deformed Jackson Gaussian distribution. To demonstrate the effectiveness of our proposal, we investigated a classic geophysical data-inverse problem in three different scenarios: (i) in the first one, we analyzed the sensitivity of the seismic inversion to incorrect seismic sources; (ii) in the second one, we considered a dataset polluted by Gaussian errors with different noise intensities; and (iii) in the last one we considered a dataset contaminated by many outliers. The results reveal that the deformed Jackson Gaussian outperforms the classical approach, which is based on the standard Gaussian distribution.

ABSTRACT

Purpose: Gender dysphoria (GD) is an incompatibility between biological sex and personal gender identity; individuals harbor an unalterable conviction that they were born in the wrong body, which causes personal suffering. In this context, surgery is imperative to achieve a successful gender transition and plays a key role in alleviating the associated psychological discomfort. In the current study, a retrospective cohort, we report the 20-years outcomes of the gender-affirming surgery performed at a single Brazilian university center, examining demographic data, intra and postoperative complications. During this period, 214 patients underwent penile inversion vaginoplasty. Results: Results demonstrate that the average age at the time of surgery was 32.2 years (range, 18-61 years); the average of operative time was 3.3 h (range 2-5 h); the average duration of hormone therapy before surgery was 12 years (range 1-39). The most commons minor postoperative complications were granulation tissue (20.5 percent) and introital stricture of the neovagina (15.4 percent) and the major complications included urethral meatus stenosis (20.5 percent) and hematoma/excessive bleeding (8.9 percent). A total of 36 patients (16.8 percent) underwent some form of reoperation. One hundred eighty-one (85 percent) patients in our series were able to have regular sexual intercourse, and no individual regretted having undergone GAS. Conclusions: Findings confirm that it is a safety procedure, with a low incidence of serious complications. Otherwise, in our series, there were a high level of functionality of the neovagina, as well as subjective personal satisfaction.

ABSTRACT

I employ the idea of de-existing, in this and other papers, as a result from an urge to name and delimit usually transient situations limited to the psychic sphere. It differs from the idea of killing, active verb that implies the action of a subject, in physical or metaphorical terms. As well as it also differs from the idea of dying, connected to something more definitive in natural or figurative terms. De-existing refers to a momentary, temporary situation, sometimes long-lasting, in which, due to disinvestment, the subject goes into a process of failure as psychic subject. It may be de-existed in different levels of effacement in a spectrum that extends between the active situation as subject and the passive condition of object of the death drive (AU)
