ABSTRACT

Acute and late radiation-induced injury on skin and subcutaneous tissues are associated with substantial morbidity in radiation therapy, interventional procedures and also are of concern in the context of nuclear or radiological accidents. Pathogenesis is initiated by depletion of acutely responding epithelial tissues and damage to vascular endothelial microvessels. Efforts for medical management of severe radiation-induced lesions have been made. Nevertheless, the development of strategies to promote wound healing, including stem cell therapy, is required. From 1997 to 2014, over 248 patients were referred to the Radiopathology Committee of Hospital de Quemados del Gobierno de la Ciudad de Buenos Aires (Burns Hospital) for the diagnosis and therapy of radiation-induced localized lesions. As part of the strategies for the management of severe cases, there is an ongoing research and development protocol on 'Translational Clinical Trial phases I/II to evaluate the safety and efficacy of adult mesenchymal stem cells from bone marrow for the treatment of large burns and radiological lesions'. The object of this work was to describe the actions carried out by the Radiopathology Committee of the Burns Hospital in a chronic case with more than 30 years of evolution without positive response to conventional treatments. The approach involved the evaluation of the tissular compromise of the lesion, the prognosis and the personalized treatment, including regenerative therapy.

ABSTRACT

Breast radiotherapy during childhood may cause unpredictable outcomes in soft tissue growth and may be responsible for most iatrogenic-related breast hypoplasias. Poor local skin quality and subcutaneous atrophy poses a difficult problem for the use of alloplastic tissues. Clinical use of a deep inferior epigastric perforator flap (DIEP) in oncological breast surgery is a common practice; however few studies have described its application in benign situations and no previous report addressed this subject. The authors indicate the use of the DIEP flap to correct severe hypomastia after previous childhood radiotherapy for hemangioma treatment, followed by unsuccessful alloplastic tissue reconstruction. For selected patients the DIEP flap may constitute a new alternative for complications of radiotherapy treatment due to the possibility of large tissue transfer with minimal donor area morbidity. The quantity of flap tissue necessary to restore the breast mound, the perforator vessel size, and the quality of suitable recipient vessels should be carefully evaluated. However, the need for microsurgical training as well as the extended surgical time are the main limiting factors for the use of this flap.

ABSTRACT

Se presentan dos casos clínicos, el primero es un niño de un año y cinco meses de edad portador de un hemangioma labial superficial, y el segundo es una niña de cinco años de edad con un hemangioma nasal de tipo mixto. Ambos fueron tratados con láser de colorantes pulsado, obteniéndose una remisión de casi un 100 por ciento en el primer caso y superior al 70 por ciento en el segundo. Se revisa la literatura del tratamiento de los hemangiomas de la infancia con láser de colorantes pulsado.

Os autores consultaram vários artigos com a finalidade de avaliar a conduta diagnóstica e terapêutica neste tumor. Observou-se que os meios diagnósticos podem variar desde a Ultra-sonografia (USG) até a Ressonância Nuclear Magnética (RNM), e que para o tratamento pode-se adotar desde a conduta conservadora até o transplante hepático ou radioterapia local.

We describe a 15-year-old boy with hemangiomatosis of bone and hypophosphatemic rickets. The rickets was ameliorated by irradiation of the skeletal lesions.

Os autores examinaram 48 mulheres irradiadas na regiäo mamária no período infantil a fim de estabelecer os efeitos dos tipos de radiaçöes e suas doses sobre o desenvolvimento da mama e da aréola, e as mudanças cutâneas devidas à irradiaçäo. Foram encontradas leves e graves disparidades de crescimento em mais de 50//dos casos, assim como mudanças cutâneas. Esses resultados parecem ser principalmente relacionados à posiçäo do hemangioma, ao tipo de irradiaçäo e dose total recebida. No entanto, a idade na época do tratamento e o tipo de hemangioma näo parecem influenciar o resultado sucessivo
