ABSTRACT

BACKGROUND: Caregivers play a crucial role in assessing the oral health-related quality of life (OHRQoL) of young individuals with autism spectrum disorder (ASD). AIM: This study assessed the impact of sociodemographic and oral conditions on OHRQoL and family dynamics in young individuals with and without autism, as perceived by guardians. DESIGN: This comparative cross-sectional study included young individuals aged 6 to 14 years and their guardians. Data were collected at a specialized institution and dental schools. Guardians completed the Parental-Caregiver Perceptions Questionnaire (P-CPQ), Family Impact Scale (FIS), and sociodemographic and oral habits questionnaires. The sociopsychological need for orthodontic treatment was assessed using the Index of Orthodontic Treatment Need (IOTN). RESULTS: The sample included 144 youths and caregivers. The ASD group had higher P-CPQ and FIS scores. Factors associated with poorer perceived OHRQoL included higher youth age, lower caregiver education, higher IOTN scores, teeth clenching (RR = 1.20; 95% CI: 1.01-1.41), and lip sucking. Lower parental education (RR = 1.75; 95% CI: 1.10-2.80) and higher IOTN scores from the caregiver's perspective impacted family dynamics. CONCLUSION: Caregivers of young individuals with ASD perceived a lower OHRQoL, and families in this group were more affected by sociodemographic and oral conditions.

ABSTRACT Objective: To determine the prevalence of the need for orthodontic treatment, anxiety, and probable sleep bruxism and its association in adolescents. Material and Methods: A cross-sectional study was conducted with 294 adolescents aged between 11 and 16 years. Orthodontic treatment need was determined using the Dental Health Component of the Index of Orthodontic Treatment Needs (IOTN-DHC). Anxiety symptoms were assessed using the Brazilian version of the Multidimensional Anxiety Scale for Children (MASC). The probable sleep bruxism was identified based on the American Academy of Sleep Medicine (AASM) questionnaire. Descriptive data analyses were performed. Simple logistic regression models were applied between each independent variable and the outcome (anxiety score). Variables with p<0.20 in the individual (raw) analyses were studied in a multiple logistic regression model, with p≤0.05 remaining in the final model. Based on the regression models, the prevalence ratios were estimated with the respective 95% confidence intervals. Results: 68.7% of the adolescents had probable sleep bruxism, and 35.4% had a moderate or severe normative need for orthodontic treatment. Adolescents aged up to 12 years (OR=1.82; CI: 1.10-3.02), females (OR=2.67; CI: 1.64-4.34), and with a moderate or severe need for orthodontic treatment according to the IOTN-DHC (OR=1.76; CI: 1.06-2.90), are more likely to have a higher anxiety score. The prevalence of adolescents with a moderate or severe need for normative orthodontic treatment by the IOTN-DHC is 35.4% (95%CI: 29.9-40.8%), while the perceived need for treatment by the IOTN-AC is 14.0% (95%CI: 10.0-17.9%). Adolescents with a high degree of anxiety were more likely to have probable sleep bruxism (OR=3.64; CI: 1.06-12.50). Conclusion: Female adolescents up to 12 years of age and with a moderate or severe need for orthodontic treatment are more likely to have higher levels of anxiety; adolescents with a high degree of anxiety are more likely to have probable sleep bruxism.

ABSTRACT

El objetivo de este estudio fue determinar la prevalencia de anomalías dentomaxilares y la necesidad de tratamiento ortodóncico en escolares pertenecientes al pueblo originario de Rapa Nui. Se realizó un estudio descriptivo, observacional, transversal y no probabilístico. Se evaluaron 85 alumnos entre primero básico y primero medio, entre los 6 y 16 años de edad, de dos colegios de Rapa Nui. Se realizó un examen clínico estandarizado, realizado por dos investigadores previamente calibrados, donde se completó una ficha clínica diseñada para el estudio. Para el análisis de las anomalías dentomaxilares se realizó un escaneo digital de la cavidad oral completa para su posterior análisis. De 85 estudiantes evaluados, 75 (88,2 %) se encontraban afectados por algún tipo de anomalía dentomaxilar en el plano sagital, vertical y/o transversal, independiente de su gravedad. Un 87,1 % de los estudiantes necesitan tratamiento ortodóncico según el Índice de Necesidad de Tratamiento Ortodóncico, de los cuales un 57,7, % se clasificó dentro de los rangos de moderado, grave y muy grave. Existe una alta prevalencia de anomalías dentomaxilares en los escolares pertenecientes al pueblo originario Rapa Nui, encontrándose por sobre las cifras nacionales como internacionales, lo cual da como resultado que la mayoría de los alumnos evaluados necesiten de algún tipo tratamiento ortodóncico, ya sea preventivo, interceptivo y/o correctivo.


The aim of this study was to determine the prevalence of dentomaxillary anomalies and the need for orthodontic treatment in schoolchildren belonging to the Rapa Nui native people. A descriptive, observational, cross- sectional and non-probabilistic study was carried out. Eighty- five students between first grade and first middle school, between 6 and 16 years of age, from two schools of Rapa Nui were evaluated. A standardized clinical examination was performed by two previously calibrated investigators, where a clinical record designed for the study was completed. For the analysis of dentomaxillary anomalies, a digital scan of the full mouth was performed for subsequent analysis. Of 85 students evaluated, 75 (88.2 %) were affected by some type of dentomaxillary anomaly in the sagittal, vertical and/or transversal plane, regardless of its severity. Some 87.1 % of the students needed orthodontic treatment according to the Orthodontic Treatment Need Index, of which 57.7 % were classified within the moderate, severe and very severe ranges. There is a high prevalence of dentomaxillary anomalies in schoolchildren belonging to the Rapa Nui native people, which is above the national and international figures, resulting in the majority of the students evaluated needing some type of orthodontic treatment, whether preventive, interceptive and/or corrective.

Resumo A decisão pela extração de dentes permanentes para a solução de problemas ortodônticos considera, além da extração em si, outros fatores, dentre eles, as características clínicas do esmalte dos dentes a serem extraídos. O objetivo deste artigo foi apresentar como a presença de defeitos de esmalte dentário (DDE) influenciou na tomada de decisão e modificou o plano de tratamento ortodôntico proposto para o paciente. A severidade do defeito, o número de dentes afetados e a presença e complexidade de má oclusão são fatores que influenciaram na definição do plano de tratamento. Quanto mais grave os defeitos e mais complexa a má oclusão, maior a dificuldade no planejamento do tratamento e, em muitos casos, é indicado incorporar a contribuição de vários especialistas na tomada de decisões. Foi possível demonstrar que o tratamento da má oclusão associada a DDE com a exodontia dos dentes mais severamente afetados apresentou resultados estético e funcional adequados. Concluiu-se que um tratamento bem-sucedido da má oclusão com resultado estético dentofacial adequado, saudável e funcional, requer a compreensão da inter-relação entre os componentes estruturais bucais, oclusão dentária e estética facial.(AU)


Abstract The decision to extract permanent teeth to solve orthodontic problems considers, in addition to the extraction itself, other factors, and among them are the clinical characteristics of the enamel of the teeth to be extracted. The objective of this article was to present how the presence of dental enamel defects (DED) influenced decision-making and modified the proposed orthodontic treatment plan for the patient. The severity of the defect, the number of affected teeth and the presence and complexity of the malocclusion are factors that influenced the definition of the treatment plan. The more severe the defects and the more complex the malocclusion, the greater the difficulty in planning the treatment and, in many cases, it is indicated to incorporate the contribution of several specialists in the decision-making process. It was possible to demonstrate that the treatment of malocclusion associated with DED with the extraction of the most severely affected teeth, presented adequate aesthetic and functional results. It is concluded that a successful treatment of malocclusion with adequate, healthy, and functional dentofacial aesthetic results, requires understanding the interrelationship between oral structural components, dental occlusion and facial aesthetics. Abstract Objective: To assess the need for orthodontic treatment among Nepalese high school students. Material and Methods: This is a quantitative, cross-sectional descriptive study. The sample comprises 938 children (537 males and 401 females) with an age group above 14 years. The subjects were selected voluntarily from seven different schools of Kathmandu valley using a multistage sampling technique. The Index of Orthodontic Treatment Need comprises two components: Dental Health Component (DHC) and Aesthetic Component (AC). Two trained and calibrated examiners performed the oral examination. Results: On analysis of the DHC component, it was found that 21% had no need, 18.1% had mild/little need, 24.3% had moderate/borderline need, 35.8% had severe need, and 0.7% had extreme treatment need. Similarly on analysis of AC component, it was found that 33% were AC-1, 30.8% were AC-2, 7.2% were AC-3, 8.2% were AC-4, 2.1% were AC-5, 3.6% were AC-6, 1.8% were AC-7, 7.4% were AC-8, 1.8% were AC-9, and 3.9% were AC-10. Conclusion: The Index of Orthodontic Treatment Need can be used as a tool for planning dental health resources and prioritizing the treatment need of different populations (AU).

Abstract Objective: To evaluate the psychosocial impact of malocclusion and self-esteem in adolescents in the Amazon region. Material and Methods: A cross-sectional study was carried out with 212 adolescents with 12-year-old enrolled in all public schools in the Boca do Acre (Amazonas, Brazil). Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) assessed the psychosocial impact of malocclusion. The self-perception of the need for orthodontic treatment was evaluated by the Aesthetic Component (AC) of the Index of Orthodontic Treatment Need (IOTN) and Global Negative Self-evaluation (GSE), the adolescent's self-esteem. The malocclusion was clinically evaluated by the Dental Aesthetic Index (DAI). The variables with p<0.20 in the individual analyses were tested in multiple logistic regression models, and those with p<0.10 remained in the model. The adjusted odds ratio (OR) was estimated with a 95% confidence interval (CI). Results: Adolescents with low self-esteem were 2.20 (95% CI: 1.23-3.93) times more likely to have a more significant impact on dental aesthetics (p<0.05). When verified by domains, the adolescents with low self-esteem had 2.33 (95% CI: 1.31-4.17) and 1.93 (95% CI: 1.09-3.42) times more likely to impact the psychological and social domains of the PIDAQ, respectively. Conclusion: Self-esteem influenced adolescents' perception of dental aesthetics in the domains related to psychological and social impact (AU).

Introduction: Orthodontic movement can cause painful symptoms, especially in the early stages of treatment. Objective: This study aimed to compare the performance of chewing gum and ibuprofen in pain control during the initial period of orthodontic treatment. Material and method: A randomized blind clinical trial, with an allocation ratio of 1:1, was developed with patients aged ≥18 years old. The sample size was established considering a significance level of 5% and test power of 80%, resulting in a minimum of 30 volunteers per group (n=90). Participants were paired regarding sex, age, the severity of malocclusion, defined by the Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN), and crowding, determined by Little's irregularity index. The sample was randomly allocated to three groups: Group I (control) placebo; Group II chewing gum; and Group III Ibuprofen. Pain perception was evaluated by the Visual Analog Scale (VAS) in the first 24, 36, and 48 hours after activation of the orthodontic appliance. The data were analyzed by generalized linear models for repeated measures in time. Result: No statistically significant difference (p>0.05) was observed among the groups for the methods of pain therapy evaluated in 24, 36, and 48 hours post-activation. Conclusion: There was no difference among the method used for pain control during the orthodontic treatment.


Introdução: A movimentação ortodôntica pode causar sintomatologia dolorosa, principalmente nas fases iniciais do tratamento. Objetivo: Este estudo teve como objetivo comparar o desempenho da goma de mascar e do ibuprofeno no controle da dor durante o período inicial do tratamento ortodôntico. Material e método: Foi desenvolvido um ensaio clínico randomizado cego, com razão de alocação de 1:1, com pacientes com idade ≥ 18 anos. O tamanho da amostra foi estabelecido considerando um nível de significância de 5% e poder do teste de 80%, resultando em um mínimo de 30 voluntários por grupo (n=90). Os participantes foram pareados quanto ao sexo, idade, gravidade da má oclusão, definida pelo Componente de Saúde Bucal (DHC) do Índice de Necessidade de Tratamento Ortodôntico (IOTN), e apinhamento, determinado pelo índice de irregularidade de Little. A amostra foi distribuída aleatoriamente em três grupos: Grupo I (controle) placebo; Goma de mascar Grupo II; e Grupo III Ibuprofeno. A percepção da dor foi avaliada pela Escala Visual Analógica (EVA) nas primeiras 24, 36 e 48 horas após a ativação do aparelho ortodôntico. Os dados foram analisados por modelos lineares generalizados para medidas repetidas no tempo. Resultado Não foi observada diferença estatisticamente significativa (p>0.05) entre os grupos para os métodos de terapia da dor avaliados em 24, 36 e 48 horas pós-ativação. Conclusão: Não houve diferença entre o método utilizado para controle da dor durante o tratamento ortodôntico.

Introduction: The Dental Aesthetic Index (DAI) is considered an essential tool in the association of the aesthetic and clinical elements of occlusion. It plays a key role in the timely and essential orthodontic treatment. Objective: to determine the orthodontic treatment required in students of a Peruvian public institution using the DAI. Material and Methods: A descriptive, observational, cross-sectional study was carried out during the months of June to July 2016 in a Peruvian educational institution. The sample consisted of 120 students. The use of the DAI allowed to assess the orthodontic treatment required, through the 10 occlusal conditions and regression indicators that constitute a linear formula, with the following components: no treatment required, elective treatment, desirable treatment, and priority treatment, according to the severity of the malocclusion as normal, defined, severe, and very severe, respectively. Results: 53.3% (64) of the students required priority orthodontic treatment due to presenting DAI=43.03. Between the ages of 12-14 years, 56.7% (34), 8.3% (5), and 28.3% (17) required priority, desirable, and elective orthodontic treatment, respectively. The need for priority orthodontic treatment was more prevalent in females accounting for 57.6% (38). 95% (57) of the students from rural areas required orthodontic treatment. Conclusion: The need for orthodontic treatment in a Peruvian sample using the Dental Aesthetic Index was priority orthodontic treatment, mostly in females with ages ranging between 12-14 years.


Introducción: El Índice Estético Dental (DAI) es considerada una herramienta indispensable en la aso-ciación de los elementos estéticos y clínicos de la oclusión, influyendo directamente en el tratamiento de ortodoncia oportuno y requerido. Objetivo: determinar el tratamiento de ortodoncia requerido en estudiantes de una institución pública peruana haciendo uso del DAI. Material y Métodos: Estudio de diseño descriptivo, observacional, con corte transversal, ejecutado durante los meses junio a julio de 2016, en una institución educativa peruana. La muestra fueron 120 estudiantes. La observación mediante el uso del instrumento DAI permitió valorar el tratamiento de ortodoncia requerido, mediante las 10 condiciones oclusales e indicadores de regresión que constituyen una fórmula lineal, con categorías de: No requiere tratamiento, tratamiento electivo, trata-miento deseable, tratamiento prioritario, de acuerdo a la severidad de la maloclusión presente como oclusión normal, definida, severa y muy severa, respectivamente. Resultados: Estudio de diseño descriptivo, observacional, con corte transversal, ejecutado durante los meses junio a julio de 2016, en una institución educativa peruana. La muestra fueron 120 estudiantes. Background: To correlate the need for orthodontic treatment between the self-perception of Chilean adolescents from 14 to 18 years old with the observation of a dentist using the same assessment scale, as well as to determine if covariates such as gender, age and type of school influence the self-perception of the adolescent and the examiner. Material and Methods: Cross-sectional descriptive study of adolescents aged 14 to 18 years from public, subsidized and private schools in Temuco, Chile. The probability sample is stratified by course, from first to fourth year, a total of 414 students participated, according to the eligibility criteria. The photographic score of the aesthetic component (AC) of the Index of Orthodontic Treatment Need (IOTN) was used. The statistical analysis of the data was performed with the SPSS Statistics program v.23. Results: 94.9% of the adolescents perceived themselves as having good aesthetics. The examiner considered that 77% presented this condition, p<0.00. Males perceived themselves better than females. At age 15, 1.7% of students considered themselves to have poor aesthetics, p<0.01. From the examiner's perspective, aesthetics are related to type of school, p<0.00. Conclusion: Adolescents perceive themselves better aesthetically than do the evaluators. The school type factor, according to the IOTN-AC examiner, shows a higher proportion of students with no need for orthodontic treatment in private schools, and a threshold need in municipal and subsidized institutions.


Correlacionar la necesidad de tratamiento ortodóncico, entre la autopercepción de adolescentes con el diagnóstico de un evaluador odontólogo, utilizando la misma escala de valoración, así también determinar si las variables como el género, la edad y la dependencia educacional influyen en la autopercepción del adolescente y la observación del examinador. Material y Métodos: Estudio descriptivo de corte transversal, en adolescentes de 14 a 18 años de escuelas públicas, subvencionadas y privadas de Temuco-Chile. Muestreo probabilístico estratificado por cursos, de primero a cuarto medio con una muestra de 414 estudiantes, según los criterios de elegibilidad. Se utilizó el score fotográfico del componente estético (AC) del Índice de Necesidad de Tratamiento de Ortodoncia (INTO). El análisis estadístico de los datos fue realizado con el programa SPSS Statistics v.23. Resultados: El 94,9% de los adolescentes se autoperciben con una buena estética, el examinador considera que un 77% presenta esta condición, p<0,00. Los varones se perciben mejor que las damas. Los adolescentes de 15 años un 1,7% considera tener mala estética, p<0,01. Desde la perspectiva del examinador la estética se relaciona con la dependencia educacional, <0,01. Conclusión: Los adolescentes se autoperciben mejor estéticamente que lo diagnosticado por evaluadores odontólogos. El factor dependencia educacional según INTO-AC examinador, muestra mayor proporción de estudiantes sin necesidad de tratamiento ortodóncico en los establecimientos privados, y necesidad límite en los públicos y subvencionados.

ABSTRACT

A Epidemiologia tornou-se um ramo da ciência muito utilizado para a verificação das necessidades da população, o planejamento de ações e a organização dos serviços em saúde. Para isso, primeiramente, é fundamental realizar o diagnóstico situacional, a partir de dados específicos, em busca da compreensão e atuação sobre os problemas de saúde encontrados em coletividade. As oclusopatias têm sido uma das condições bucais mais prevalentes e, em consequência, houve o crescimento da demanda por tratamento ortodôntico. Nesta perspectiva, índices oclusais foram elaborados para medir a ocorrência das oclusopatias tendo em vista a priorização da necessidade de tratamento. O conhecimento da temática em escala global permitiu uma visão ampliada do problema e das estratégias desenvolvidas em saúde bucal. Neste contexto, a presente pesquisa teve os seguintes objetivos: 1) realizar uma revisão sistemática das evidências científicas existentes sobre a prevalência da necessidade de tratamento ortodôntico em adolescentes na faixa etária de 12 à 15 anos de idade; 2) investigar a prevalência das oclusopatias e a necessidade normativa de tratamento ortodôntico em adolescentes de 12 anos de idade. Na revisão sistemática foram incluídos trabalhos nacionais e internacionais publicados nas bases de dados Cochrane, Embase, Portal Regional da Biblioteca Virtual em Saúde (BVS), PubMed, Scientific Electronic Library Online (Scielo), Scopus e Web of Science (WOS). A busca inicial não se limitou a nenhum filtro disponível nas bases de dados e utilizou os seguintes descritores: prevalence, epidemiology, malocclusion, index orthodontic treatment need, child e adolescent. Termos utilizados na literatura científica também foram empregados: orthodontic treatment need, index for need of orthodontic treatment, index of orthodontic treatment needs, IOTN index, dental aesthetic index e DAI index. Foram incluídas publicações que avaliaram a prevalência da necessidade de tratamento ortodôntico em adolescentes entre 12 à 15 anos de idade utilizando o Índice de Estética Dentária (DAI) e/ou Índice de Necessidade de Tratamento Ortodôntico (IOTN) em estudos epidemiológicos transversais, sem restrições de ano do periódico e idioma. Do total de artigos encontrados (n=2255), 525 estudos permaneceram após a remoção por duplicidade. Os títulos e resumos destas publicações foram avaliados e aplicando os critérios de exclusão, selecionou-se 38 artigos. Para elegibilidade, os critérios de inclusão foram empregados e 11 estudos foram eleitos. Os textos completos destas publicações foram obtidos para leitura e análise da qualidade metodológica. Com relação aos achados da necessidade de tratamento ortodôntico global, houve variabilidade nos resultados, mas observou-se que a prevalência foi considerada alta. Quanto às ações desenvolvidas, foram sugeridas a educação em saúde bucal para os adolescentes e pais com foco na prevenção e no tratamento interceptativo precoce das oclusopatias, bem como, inserção da ortodontia nos programas de saúde com a implantação de mais centros especializados. A segunda etapa desta pesquisa consistiu em um estudo observacional e transversal, realizado com 461 adolescentes de 12 anos de idade, matriculados em escolas públicas de um município de médio porte do Estado de São Paulo. A oclusão e a necessidade de tratamento ortodôntico foram avaliadas por meio da Classificação de Angle e do DAI. Os dados foram analisados utilizando estatística descritiva e as distribuições absolutas e percentuais das variáveis categóricas foram tabuladas. A verificação da associação entre os resultados dos exames clínicos para diagnóstico de oclusopatias utilizando a Classificação de Angle e o DAI foi realizada por meio do teste G, adotando-se um nível de significância de 5%. O processamento e a análise dos dados foram realizados com auxílio do Programa Epi-InfoTM versão 7.2. (Center for Disease Control and Prevention). O diagnóstico da oclusão, segundo a Classificação de Angle, mostrou que 8,89% apresentavam oclusão normal, 56,83% oclusopatia em Classe I, 24,08% em Classe II e 10,20% em Classe III. A prevalência da necessidade de tratamento ortodôntico, segundo o DAI, foi de 52,69% dos adolescentes considerando tratamento eletivo à obrigatório. Houve associação entre os resultados dos exames clínicos para diagnóstico de oclusopatias utilizando a Classificação de Angle com o DAI (p <0,0001). A prevalência das oclusopatias e da necessidade normativa de tratamento ortodôntico foi elevada, sendo 18,44% dos adolescentes com necessidade de tratamento obrigatório. Os índices foram eficazes para a identificação das oclusopatias, entretanto por avaliarem aspectos clínicos distintos, poderiam ser utilizados concomitantemente para aprimorar o diagnóstico das oclusopatias(AU)


Epidemiology has become a branch of science widely used to define the needs of the population, plan actions and organize health services. For this, first, it is essential to perform the situational diagnosis, based on specific data, in search of understanding and acting on the health problems found in the community. Malocclusion has been one of the most prevalent oral conditions and, as a result, there has been an increase in demand for orthodontic treatment. In this perspective, occlusal indexes were developed to measure the occurrence of malocclusions in order to prioritize the need for treatment. The knowledge of the theme on a global scale allowed an expanded view of the problem and the strategies developed in oral health. In this context, the present research had the following objectives: 1) to carry out a systematic review of the existing scientific evidence on the prevalence of the need for orthodontic treatment in adolescents aged 12 to 15 years old; 2) to investigate the prevalence of malocclusions and the normative need for orthodontic treatment in 12-year-old adolescents.The systematic review included national and international works published in the Cochrane, Embase, Regional Portal of the Virtual Health Library (VHL Regional Portal), PubMed, Scientific Electronic Library Online (Scielo), Scopus and Web of Science (WOS) databases. The initial search was not limited to any filter available in the databases and used the following descriptors: prevalence, epidemiology, malocclusion, index orthodontic treatment need, child and adolescent. In addition, terms used in the scientific literature were also used: orthodontic treatment need, index for need of orthodontic treatment, index of orthodontic treatment needs, IOTN index, dental aesthetic index and DAI index. Were included publications that evaluated the prevalence of the need for orthodontic treatment in adolescents between 12 and 15 years of age using the Dental Aesthetic Index (DAI) and / or Orthodontic Treatment Needs Index (IOTN) in cross-sectional epidemiological studies, without restrictions on year of the journal and language. Of the total articles found (n = 2255), 525 studies remained after removal due to duplication. The titles and abstracts of these publications were evaluated and applying the exclusion criteria, 38 articles were selected. For eligibility, the inclusion criteria were used and 11 studies were elected. The complete texts of these publications were obtained for reading and analysis of methodological quality. As for the actions developed, oral health education recommendations were suggested for adolescents and parents with a focus on prevention and early interceptive treatment of malocclusions, as well as insertion of orthodontics in health programs with the implementation and development of specialized centers. The second stage of this research consisted of an observational, crosssectional, survey-type study conducted with 461 12-year-old adolescents enrolled in public schools in the medium-sized municipality of the State of São Paulo. Occlusion and the need for orthodontic treatment were assessed using the Angle Classification and the DAI. The data were analyzed using descriptive statistics and the absolute and percentage distributions of categorical variables were tabulated. The verification of the association between the results of clinical exams for the diagnosis of malocclusions using the Angle Classification and the DAI was performed using the G test, adopting a significance level of 5%. Data processing and analysis were performed using the Epi-InfoTM version 7.2 program. (Center for Disease Control and Prevention). The diagnosis of occlusion, according to the Angle Classification, showed that 8.89% had normal occlusion, 56.83% occlusion Class I, 24.08% Class II and 10.20% Class III. The prevalence of the need for orthodontic treatment, according to the DAI, was 52.69% of adolescents considering elective treatment to mandatory treatment. There was an association between the results of clinical examinations for the diagnosis of malocclusions using the Angle Classification with the DAI (p <0.0001). The prevalence of malocclusions and the normative need for orthodontic treatment was high, with approximately 18,44% of adolescents in need of mandatory treatment. The indices were effective for the identification of malocclusions, however, by evaluating different clinical aspects, they could be used concomitantly to improve the diagnosis of malocclusions(AU)

Abstract Introduction Orofacial pain is a critical TMD symptom that can influence physical and social capacity. Objective To evaluate the association of temporomandibular disorders (TMD) symptoms with affective relationships and demographic variables in young adults. Material and method A cross-sectional study involving 395 young adults was developed. Diagnostic Criteria for TMD, anxiety, and depression were collected from questionnaires. The Dental Health Component of the Index of Orthodontic Treatment Need measures the orthodontic treatment need. Questionnaires also contained questions related to the previous orthodontic treatment. Logistic regression models were adjusted, estimating crude odds ratio with the 95% confidence intervals. The variables with p<0.20 in the analyses were assessed in a multiple logistic regression model, remaining with p≤0.10. Result There was no significant association of TMD symptoms with sex, age, medication use for pain, previous orthodontic treatment, orthodontic treatment need, anxiety, and depression (p>0.05). Individuals without an affective relationship are 1.78 (95%CI: 0.99-3.17) times more likely to report TMD symptoms. Conclusion Affective relationships showed an association with TMD symptoms in young adults.


Resumo Introdução A dor orofacial é um sintoma crítico da DTM que pode influenciar a capacidade física e social. Objetivo Avaliar a associação dos sintomas das desordens têmporomandibulares (DTM) com as relações afetivas e variáveis demográficas em adultos jovens. Material e método Estudo transversal envolvendo 395 adultos jovens foi realizado. Os critérios diagnósticos para DTM, ansiedade e depressão foram coletados a partir de questionários. O Componente de Saúde Bucal do Índice de Necessidade de Tratamento Ortodôntico mensurou a necessidade de tratamento ortodôntico. Os questionários também continham questões relacionadas ao tratamento ortodôntico anterior. Modelos de regressão logística foram ajustados, estimando odds ratio bruto com os intervalos de confiança de 95%. As variáveis com p<0.20 nas análises foram avaliadas em modelo de regressão logística múltipla, permanecendo as variáveis com p≤0.10. Resultado Não houve associação significativa dos sintomas de DTM com sexo, idade, uso de medicamentos para dor, tratamento ortodôntico prévio, necessidade de tratamento ortodôntico, ansiedade e depressão (p>0.05). Indivíduos sem relacionamento afetivo têm 1.78 (IC95%:0.99-3.17) vezes mais chance de relatar sintomas de DTM. Conclusão Os relacionamentos afetivos mostraram associação com os sintomas de DTM em adultos jovens.

ABSTRACT Objective: To evaluate the prevalence and severity of malocclusion in children suffering from β-thalassemia and to assess orthodontic treatment need using Grainger's Treatment Priority Index (TPI) and index of orthodontic treatment need (IOTN)-dental health component (DHC). Methods: A cross-sectional study was conducted on 200 transfusion-dependent children diagnosed with homozygous β-thalassemia and 200 healthy school children aged 11-17 years. The TPI and IOTN-DHC data was recorded for both groups. Total TPI score for each subject was calculated and graded according to malocclusion severity estimate (MSE). Independent sample t-test was used to compare mean TPI scores, overjet and overbite between thalassemic and healthy children. Chi-square test was used to compare the frequency of IOTN-DHC grades, Angle's classification, and MSE grades between thalassemic and healthy children. Results: The most prevalent malocclusion was Class I in normal children (67.5%) and Class II in thalassemic children (59%). The mean overjet and overbite were significantly (p<0.001) greater in thalassemic children than in healthy children. Severe tooth displacements were 3.5 times greater in thalassemic children, compared to controls. A greater proportion of thalassemic children were in IOTN grades 3 and 4, compared to the controls (p<0.001). MSE grades 4 and 5 were significantly (p<0.001) more prevalent in thalassemic children, compared to the controls. Conclusion: There is a high prevalence of Angle's Class II malocclusion in thalassemic children. Majority of these children are categorized in higher grades of IOTN-DHC and TPI-MSE, showing a great severity of malocclusion and high orthodontic treatment needs.


RESUMO Objetivo: Avaliar a prevalência e gravidade da má oclusão em crianças que sofrem de beta-talassemia e mensurar a necessidade de tratamento ortodôntico usando o Índice de Prioridade de Tratamento (IPT) de Grainger e o Índice de Necessidade de Tratamento Ortodôntico (Index of Orthodontic Treatment Need - IOTN) - Componente de Saúde Dental (Dental Health Component - DHC). Métodos: Um estudo transversal foi conduzido com 200 crianças com diagnóstico de beta-talassemia homozigótica dependentes de transfusão e 200 crianças saudáveis em idade escolar, entre 11 e 17 anos. Os dados do IPT e do IOTN-DHC foram documentados para ambos os grupos. A pontuação total do IPT para cada sujeito foi calculada e classificada de acordo com a Estimativa de Severidade da Má oclusão (ESM). O teste t para amostras independentes foi usado para comparar os escores médios de IPT, sobressaliência e sobremordida, entre as crianças saudáveis e as com talassemia. O teste qui-quadrado foi usado para comparar a frequência dos escores do IOTN-DHC, a classificação de Angle e os escores do ESM entre crianças com beta-talassemia e crianças saudáveis. Resultados: A má oclusão mais prevalente foi a Classe I em crianças normais (67,5%) e a Classe II em crianças com beta-talassemia (59%). Os valores médios de sobressaliência e a sobremordida foram significativamente (p< 0,001) maiores em crianças com beta-talassemia do que em crianças saudáveis. Os deslocamentos dentários graves foram 3,5 vezes maiores em crianças com beta-talassemia em comparação com os controles. Uma proporção maior de crianças com beta-talassemia estava nos escores 3 e 4 do IOTN em comparação com os controles (p <0,001). Os escores 4 e 5 de ESM foram significativamente (p< 0,001) mais prevalentes em crianças com beta-talassemia em comparação com os controles. Conclusão: Há uma alta prevalência de má oclusão de Classe II de Angle em crianças com beta-talassemia. A maioria dessas crianças é categorizada em escores superiores de IOTN-DHC e IPT-ESM, mostrando uma grande gravidade de má oclusão e alta necessidade de tratamento ortodôntico.

RESUMEN: Objetivo: Determinar la necesidad de tratamiento ortodóncico en adolescentes de 12 años de Diego de Almagro, Chile, según el Índice de Estética Dental (IED). Materiales y Métodos: estudio descriptivo, observacional, transversal y no probabilístico. Se aplicó el IED en 97 estudiantes de 12 años de la localidad Diego de Almagro con un examen clínico estandarizado por una investigadora previamente calibrada, respetando las recomendaciones de la Organización Mundial de la Salud. La muestra fue de tipo censal. Se analizaron los criterios IED descriptivamente. Resultados: De 118 adolescentes, 97 reunían los criterios de inclusión, 17.52% obtuvo puntajes del IED ≤ 25, indicativo de oclusión normal o maloclusión mínima, lo que determina tratamiento innecesario o poco necesario; 16.49% obtuvo una puntuación entre 26 y 30, maloclusión manifiesta y una necesidad de tratamiento optativa; 16.49% obtuvo una puntuación entre 30 y 35, maloclusión severa y una necesidad de tratamiento deseable por el paciente; 49.48% presentó un puntaje de IED ≥ 36, indicativo de maloclusión muy severa o discapacitante con una necesidad de tratamiento obligatoria. Conclusiones: Existe una alta necesidad de tratamiento ortodóncico obligatorio en la población adolescente de 12 años, según el IED en que el 65.97% presentaba una maloclusión severa o muy severa.


ABSTRACT Objective: To determine the orthodontic treatment need in 12-year-old adolescents from the city of Diego de Almagro, Chile, according to the Dental Aesthetics Index (DAI) Materials and Methods: descriptive, observational, cross-sectional and non-probabilistic study. The DAI was applied to 97 12-year-old students belonging to the city of Diego de Almagro, Atacama Region. The sample was of census type. The information was obtained through a standardized clinical examination performed by a previously calibrated researcher, fulfilling the recommendations described by the World Health Organization for this type of studies. The DAI criteria were analyzed descriptively. Results: 97 out of 118 students met the inclusion criteria, 45 (46.3%) were males and 52 (53.7%) females; 17.52% of the children examined obtained DAI scores ≤ 25, indicating a normal occlusion or minimal malocclusion, which determines that the treatment is unnecessary or not necessary; 16.49% obtained a score between 26 and 30, indicating a manifest malocclusion and an optional treatment need; 16.49% obtained a score between 30 and 35 with severe malocclusion and a treatment need desirable by the patient; 49.48% had a DAI score ≥ 36, indicating a very severe or disabling malocclusion with a compulsory treatment need. Conclusions: There is a high mandatory orthodontic treatment need in the 12-year-old adolescent population, based on the fact that 65.97% of the studied sample through DAI presented a severe or very severe malocclusion.

ABSTRACT

This cross-sectional study aimed to establish a relationship between the Normative Need for Orthodontic Treatment (NNOT) and oral health among Mexican adolescents aged 13-15 years old. A convenience sample of 424 subjects in Mexico City participated in the study. The dependent variable used was NNOT, which was determined via the dental health component (grades 4 and 5) of the Index of Orthodontic Treatment Need (IOTN). The variables for oral health were as follows: caries experience, oral hygiene, self-reported temporomandibular joint pain, and self-reported bruxism. Logistic regression models were fitted to determine the association between NNOT and oral health. The prevalence of NNOT was 66.0% (280/424), and the crowding was the most prevalent occlusal anomaly with 36.1% (n = 135). Multivariate models showed that subjects with NNOT were more than twice as likely to present poor hygiene (OR = 2.56; p = 0.001) as subjects presenting crowding (>4 mm) (OR = 1.99; p = 0.004) and increased overjet (>6 mm) (OR = 1.74; p = 0.046). Those schoolchildren who presented anterior guidance were 72% less likely to present NNOT (OR = 0.28; p < 0.001). In conclusion, the risk of presenting NNOT in Mexican adolescents is high, with a prevalence of over 50% of which the most prevalent occlusal anomaly was crowding. On the other hand, poor oral hygiene was associated with crowding and increased overjet.

INTRODUCTION: The purpose of this research was to evaluate the esthetic impact of orthodontic treatment need in adolescents. METHODS: This cross-sectional study was performed with 1172 adolescents aged 12 (n = 638) years and 15 (n = 534) years from Salvador, Bahia, Brazil. The esthetic impact was the following outcome variables: Aesthetic Component of the Index of Orthodontic Treatment Need and the Oral Aesthetic Subjective Impact Scale. The normative orthodontic treatment need was assessed by the Dental Health Component of the Index of Orthodontic Treatment Need (IOTN-DHC) and by the Dental Aesthetic Index (DAI). Logistic regression models were adjusted, estimating crude odds ratio with the respective 95% confidence intervals. The variables with P ≤0.20 in the individual analyses were tested in multiple logistic regression models, remaining the variables with P ≤0.05. RESULTS: The results showed a statistically significant association for the esthetic impact (Aesthetic Component of the Index of Orthodontic Treatment Need and the Oral Aesthetic Subjective Impact Scale) when assessing the normative orthodontic treatment need by the DAI (P <0.05). However, there was no significant association between subjective esthetic impact and normative needs when using the IOTN-DHC (P >0.05). CONCLUSIONS: DAI was more associated with the adolescents' esthetic concern than IOTN-DHC and should be the Index of Orthodontic Treatment Need to be selected to complement the patient's self-perception.

Describe the prevalence of malocclusions, dental caries and the need for orthodontic treatment according to the Index of Orthodontic Treatment Need (IOTN) among Aymara schoolchildren, Indian group has lived in the highlands of the Andes Mountains. 76 Aymara schoolchildren between 5 and 15 years old in Colchane, Chile, where a cross-sectional study was conducted. DMFT/dmft index, Dental component (DHC) of IOTN and demographics were determined. Frequencies and mean (SD) were calculated. Bivariate analyses were conducted using Fisher's exact to test the association between sex and stage of dentition (p<0.05). Stata version 14 was used for all analyses. Among the 76 participants, 47.4 % (N=36) were male. Participants were on average 9.96 years old (SD=2.5). Over half presented dental caries (61.8 %, n=47) and 36.8 % (N=28) had missing teeth due to caries. The DMFT index was 2.1 (SD=1.9). The most frequent biotype was brachyfacial 88.2 % (N=67). Malocclusion was observed in 81.6 % (N=62) of participants. Class I, was the most frequent (67 %, N=51) malocclusion in the different stages of dentition. The most prevalent treatment need was grade 4 and 5 "need treatment" of the IOTN, observed in 71 % (N=54) of the sample with significant differences according to the stage of dentition (p<0.05). In this study, a high prevalence of malocclusion and a high percentage of dental caries was observed in Aymara children.


Describir la prevalencia de maloclusiones, caries dental y la necesidad de tratamiento de ortodoncia según el Índice de Necesidad de Tratamiento de Ortodoncia (IOTN), en escolares Aymaras, grupo indígena que vive en el altiplano de la Cordillera de los Andes. Estudio transversal de 76 escolares Aymaras de 5 a 15 años de Colchane, Chile. Se determinó el índice COPD/ceod, el componente dental (DHC) del IOTN y la demografía. Se calcularon las frecuencias y la media (DE). Se realizaron análisis bivariados utilizando el exacto de Fisher para probar la asociación entre el sexo y la etapa de la dentición (p<0,05). Se utilizó la versión 14 de Stata para todos los análisis. Entre los 76 participantes, 47,4 % (N = 36) eran hombres. Los participantes tenían un promedio de 9,96 años (DE = 2,5). Más de la mitad presentó caries dental (61,8 %, n = 47) y 36,8 % (N = 28) tenían dientes perdidos por caries. El índice DMFT fue 2,1 (DE = 1,9). El biotipo más frecuente fue el braquifacial 88,2 % (N = 67). Se observó maloclusión en el 81,6 % (N = 62) de los participantes. La Clase I fue la maloclusión más frecuente (67 %, N = 51) en las diferentes etapas de la dentición. La necesidad de tratamiento más prevalente fue la definida con los grados 4 y 5 de "necesidad de tratamiento" del IOTN, observada en el 71 % (N = 54) de la muestra con diferencias significativas según la etapa de la dentición (p <0,05). En este estudio, se observó una alta prevalencia de maloclusión y un alto porcentaje de caries dental en niños aymaras.

ABSTRACT

Aim: To evaluate the influence of social capital on self-perception related to orthodontic treatment need. Methods: A cross-sectional study was conducted with a sample of 578 11-16 years-old adolescents from a city in southern Brazil. Social capital was evaluated using the Social Capital Questionnaire for Adolescent Students (SCQ-AS). Index of Orthodontic Treatment Need (IOTN) assessed malocclusion and self-perception related to orthodontic treatment need. Sociodemographic aspects of adolescents were also evaluated. Individual analyses were performed, relating the study variables to the outcome, estimating the odds ratio with the respective confidence intervals of 95%. The variables with p<0.20 in the individual analyses were tested in the multiple logistic regression models, and those with p<0.10 remained in the model. Results: Social capital did not influence the self-perception related to orthodontic treatment need. Adolescents with high orthodontic needs were 5.35 (CI 95%: 2.68 to 10.65) times more likely to perceived orthodontic treatment need (p <0.05). Crowding and dental absence were associated with self-perception related to orthodontic treatment need (p <0.05). Conclusions: Social capital did not influence the self-perception related to orthodontic treatment need

Resumen: Objetivo: Caracterizar la necesidad de tratamiento ortodóntico en su componente de salud dental y estético en escolares de 11 a 12 años de la Institución Educativa Mariscal Gamarra del Cusco - Perú, 2019. Materiales y métodos: La muestra estuvo constituida por 142 escolares de una institución educativa estatal, se usó el Índice de Necesidad de Tratamiento Ortodóntico (INTO) con sus dos componentes. El Componente de Salud Dental (CDS) es realizado por el Odontólogo y el Componente Estético (CE) es percibido por el paciente. Resultados: Se obtuvo 15,49% sin necesidad de tratamiento y 33.10% de leve necesidad de tratamiento y 28,87% moderada necesidad de tratamiento en su Componente de Salud Dental y el 74,65% se presentó sin necesidad de tratamiento en su Componente Estético. Conclusión: Al examen por parte del profesional casi la mitad de los estudiantes no necesitan tratamiento y por otro lado, tres de cuatro estudiantes se perciben sin necesidad de tratamiento de ortodoncia.

O componente de saúde bucal (CDS) é realizado pelo dentista e o componente estético (CE) é percebido pelo paciente. Resultados: foram obtidos 15,49% sem necessidade de tratamento e 33,10% para uma leve necessidade de tratamento e 28,87% para uma necessidade moderada de tratamento em seu Componente de Saúde Bucal e 74,65% apresentaram sem a necessidade de tratamento em seu Componente Estético. Conclusão: No exame realizado pelo profissional, quase metade dos estudantes não necessita de tratamento e, por outro lado, três em cada quatro percebem a si mesmos sem a necessidade de tratamento ortodôntico.


Abstract: Objective: To describe the need for orthodontic treatment in its dental and aesthetic health components in schoolchildren aged 11 to 12 at the Mariscal Gamarra Educational Institution of Cusco - Peru, 2019. Materials and methods: The sample consisted of 142 schoolchildren from a public school. The Index of Orthodontic Treatment Need (IOTN) was used with the two components; the Dental Health Component, determined by the dentist, and the Aesthetic Component, as perceived by the patient. Results: According to the results, 15.49% did not need any treatment, 33.10% had a slight need for treatment, and 28.87% a moderate need for treatment in the Dental Health component; 74.65% presented no need for treatment in the Aesthetic Component. Conclusion: Almost half the students did not need any treatment as reported after the health professional exam. On the other hand, three out of four students showed no need for orthodontic treatment.

Abstract Objective: To determine whether psychosocial status is associated with orthodontic treatment needs in high school students using the Psychosocial Impact of the Dental Aesthetic Questionnaire (PIDAQ) and the Index of Orthodontic Need (IOTN). Material and Methods: This cross-sectional study comprised 95 adolescents, both genders, aged 15-17 years old. PIDAQ was used to assess psychosocial status. The IOTN Aesthetic Component (IOTN-AC) and the IOTN Dental Health Component (DHC) was used to assess treatment needs. Examination of the oral cavity was carried out with the help of the IOTN ruler by observing the MOCDO components: missing teeth, overjet, crossbite, displacement, and overbite. The Chi-square test was used to determine the association of psychosocial status and orthodontic treatment need. The reliability test was carried out by using Cronbach's Alpha test (0.978). Results: There was an association between psychosocial status and orthodontic treatment need based on the IOTN-AC (p=0.001) and between psychosocial status and orthodontic treatment need based on the IOTN-DHC (p=0.140). Conclusion: There is an association between psychosocial status based on the PIDAQ and subjective orthodontic treatment need based on the IOTN-AC, but there is no association between psychosocial status based on the PIDAQ and objective orthodontic treatment need based on the IOTN-DHC in students of SMAN 27 Jakarta.
