It is well known that the perinatal period supposes a considerable risk of relapse for women with bipolar disorder (BD) and recurrent major depressive disorder (rMDD), with the consequences that this entails. Therefore, the authors sought to provide a critical appraisal of the evidence related to specific risk factors for this population with the aim of improving the prevention of relapses during pregnancy and postpartum. The authors conducted a systematic review assessing 18 original studies that provided data on risk factors for relapse or recurrence of BD and/or rMDD in the perinatal period (pregnancy and postpartum). Recurrences of BD and rMDD are more frequent in the postpartum period than in pregnancy, with the first 4-6 weeks postpartum being especially complicated. In addition, women with BD type I are at higher risk than those with BD type II and rMDD, and the most frequent presentation of perinatal episodes of both disorders is a major depressive episode. Other risk factors consistently repeated were early age of onset of illnesses, severity criteria, primiparity, abrupt discontinuation of treatment, and personal or family history of perinatal affective episodes. This review shows that there are common and different risk factors according to the type of disorder and to perinatal timing (pregnancy or postpartum) that should be known for an adequate prevention of relapses.

RESUMEN El médico, fisiólogo y psicólogo alemán Wilhelm Wundt ha sido y es considerado el creador de la llamada psicología experimental, por su abordaje de conceptos psicológicos hasta entonces intuitivos, mediante métodos propios de la fisiología del siglo xix. Emergió de esta manera una nueva disciplina, la psicología científica, en el contexto físico de un laboratorio de ciencias en la ciudad alemana de Leipzig. Sin embargo, la labor de Wundt no se limitó únicamente al estudio de la psicología desde un punto de vista científico. Por una parte, se ha destacado su gran capacidad de convocatoria, aglutinadora y organizativa de un laboratorio original donde pudieron formarse investigadores de varias partes del mundo; y, por otra, modificó sustancialmente el modo de analizar la psicología, centrando la atención de los investigadores en el estudio pormenorizado de la consciencia mediante la utilización del método introspectivo.


ABSTRACT The physician, physiologist and psychologist Wilhelm Wundt has been and is considered the founder of experimental psychology due to his approach to psychological subjects, until then intuitive, through the XIX century methods. Thus, a new discipline, the Scientific Psychology, emerged in the venue of a laboratory in the town of Leipzig, Germany. Nevertheless, Wundt´s labor was not restricted to the study of psychology from a scientific perspective: on one hand, his great convocational, organizing and group-working capacity attracted lots of investigators from around the world to be trained. In n original laboratory, On the other, he substantially modified the study of psychology centering the researchers' attention in the detailed analysis of consciousness through the use of the method called introspection.

RESUMEN La ayahuasca pertenece al grupo de alucinógenos de origen vegetal con los que comparte su capacidad de alterar el estado de conciencia y provocar alteraciones perceptivas. Empleada tradicionalmente en forma de brebaje en ritos ceremoniales con el propósito de alcanzar experiencias de contenido espiritual o sanatorio, su uso se ha extendido a lo largo de centurias hasta nuestro actual contexto cultural dentro del cual es empleada por pequeños grupos guiados por fines de crecimiento personal. La experiencia suele durar unas horas y los efectos suelen ser autolimitados. Se describe el caso clínico de una paciente que, tras el consumo experimental de ayahuasca, desarrolla un episodio maníaco con síntomas psicóticos. Se realiza asimismo una búsqueda sistemática de casos publicados de sintomatología psicótica relacionada con el consumo de ayahuasca. A nivel experimental, la relación entre ayahuasca y experiencias psicóticas es poco conocida, si bien se ha determinado un buen perfil de seguridad de la sustancia en administraciones puntuales en sujetos sanos. En el presente estudio, los resultados sugieren que la aparición de episodios psicóticos asociados con consumo de ayahuasca es un fenómeno infrecuente que parece estar relacionado con características de vulnerabilidad tales como historia personal o familiar de trastorno mental grave o el uso concomitante de otros compuestos tóxicos o drogas, especialmente cannabis.


SUMMARY Ayahuasca belongs to the group of plant-based or vegetal hallucinogens sharing their alterating effects on the state of consciousness and causing perceptual alterations. Traditionally used as a concoction in ceremonial rites aimed to achieve experiences of spiritual or curative content, its use has expanded throughout centuries, reaching our current cultural context, where it is used by small groups guided by purposes of personal growth. The experience usually lasts a few hours and the effects are usually self-limited. We describe the case of a patient who developed a manic episode with psychotic symptoms after experimental consumption of ayahuasca. In addition, a systematic review of published cases of sustained psychotic symptoms after the consumption of ayahuasca is carried out. There are few experimental studies of the relationship between ayahuasca and psychotic experiences, although a good safety profile has been determined in single administrations to healthy subjects. In the present study, the results suggest that the appearance of psychotic episodes associated with ayahuasca consumption is an infrequent phenomenon and seem to be related to vulnerability characteristics such as personal or family history of severe mental disorder or the concomitant use of other drugs, particularly cannabis.
