[Density of lymphocyte beta-2 adrenoreceptors in the course of hypertension]. / Plotnost' limfotsitarnykh beta2-adrenoretseptorov v techenie gipertonicheskoi bolezni.

Baseline density of intact lymphocyte beta2 adrenoreceptors (B max., 3H-DHA) was compared to dynamics of left ventricular hypertrophy (LVH) and frequency of essential hypertension (EH) complications within 7-9-year follow-up in 27 patients with EH stage II. No significant correlation was noted between B max and LVH changes. In a subgroup of 9 patients with initially very high B max overall number of complications reached 77.8% against 27.5%) (p<0.05) in control. These patients also developed acute myocardial infarctions more frequently (33.3% versus 0%, p<0.05). Such complications as strokes, coronary failure occurred with the same frequency. More extended trials are needed to infer on prognostic value of lymphocytic beta2-adrenoreceptor density.

[A comparative evaluation of the importance of transesophageal electrical stimulation of the left atrium and of bicycle ergometry in the diagnosis of ischemic heart disease in patients with arterial hypertension]. / Sravnitel'naia otsenka znacheniia chrespishchevodnoi élektricheskoi stimuliatsii levogo predserdiia i veloérgometrii v diagnostike ishemicheskoi bolezni serdtsa u bol'nykh arterial'noi gipertoniei.

To make a comparative assessment of transesophageal left atrial pacing (TLAP) and bicycle ergometry (BE) in the diagnosis of coronary heart disease (CHD) in patients with arterial hypertension (AH) of different origin, the authors examined 56 patients. The patients underwent TLAP and selective coronary angiography, of them 39 patients had BE testing. No significant differences were found in their specificity (73 and 78%) and sensitivity (92 and 81%) between TLAP and BE, respectively. The maximum heart rate in CHD patients with AH was significantly higher (130 +/- 6 per min) during TLAP than that in BE (112 +/- 5 per min, p > 0.05), ischemic changes occurring at the same value of double product despite the mode of myocardial ischemia induction in these functional tests (240 +/- 10 and 236 +/- 12 arbitrary units, respectively; p < 0.05). The mean systolic blood pressure was higher in TLAP and in BE (210 +/- 10 and 185 +/- 8 mm Hg). This follows that TLAP and BE finding are comparable and no profound changes occur in TLAP, hence it can be recommended for wide application in the diagnosis of CHD in AH patients.

New electrocardiographic criteria of left-ventricular hypertrophy (LVH) have been identified at automated analysis of ECG-35 cartograms (71 normal and 88 LVH recordings). Simultaneous application of 2 sums of S and R amplitudes (S32 + R14 and S30 + R19) obtained from the leads other than Wilsonian chest leads enabled a 1.5-fold increase in the sensitivity of electrocardiographic LVH diagnosis, as compared to the Sokolov-Lyon criteria, without loss of specificity. The use of summary R and S areas at the same points on the cartogram further improves the method's sensitivity.
