RESUMO

PURPOSE: The global cervical cancer burden is disproportionately high in low- and middle-income countries (LMICs), and outcomes can be governed by the accessibility of appropriate screening and treatment. High-dose-rate (HDR) brachytherapy plays a central role in cervical cancer treatment, improving local control and overall survival. The American Brachytherapy Society (ABS) and Indian Brachytherapy Society (IBS) collaborated to provide this succinct consensus statement guiding the establishment of brachytherapy programs for gynecological malignancies in resource-limited settings. METHODS AND MATERIALS: ABS and IBS members with expertise in brachytherapy formulated this consensus statement based on their collective clinical experience in LMICs with varying levels of resources. RESULTS: The ABS and IBS strongly encourage the establishment of HDR brachytherapy programs for the treatment of gynecological malignancies. With the consideration of resource variability in LMICs, we present 15 minimum component requirements for the establishment of such programs. Guidance on these components, including discussion of what is considered to be essential and what is considered to be optimal, is provided. CONCLUSIONS: This ABS/IBS consensus statement can guide the successful and safe establishment of HDR brachytherapy programs for gynecological malignancies in LMICs with varying levels of resources.

RESUMO

The γ-H2AX assay was investigated as an alternative to the time-consuming dicentric chromosome assay (DCA). Radiation doses to 25 radiotherapy patients were estimated in parallel by DCA and the γ-H2AX assay. The γ-H2AX assay yielded doses in line with the calculated equivalent whole body doses in 92% of the patients, whereas the success rate of DCA was only 76%. The result shows that the γ-H2AX assay can be effectively used as a rapid and more precise alternative to DCA.

In the present study, we evaluated the radioprotective effect of carvacrol (CVC) against X-radiation-induced cellular damage in cultured human blood lymphocytes. By MTT assay, the LD50 doses of CVC and X-radiation to lymphocytes were determined to be 100 µg/ml and 4 Gy, respectively. To explore the radioprotective effect of CVC, the cultured lymphocytes were treated with 100 µg/mL of CVC 30 min prior to 4 Gy irradiation. Subsequently, the radiation-induced damage was screened by micronuclei (MN) and dicentric chromosome (DC) frequencies and comet assay. The percentage of cell death was evaluated by acridine orange/ethidium bromide (AO/EB) staining. The radiation-induced oxidative stress was estimated by assessing the changes in the levels of enzymatic antioxidants and lipid peroxidation markers. Compared with the sham control, we observed increases in MN and DC frequencies, comet attributes, % cell death, and lipid peroxidation with a concomitant decrease in the antioxidant status of the lymphocytes treated with radiation alone. Pre-treatment of lymphocytes with CVC (100 µg/mL) altered those changes mediated by radiation. These results clearly indicate that CVC may be an effective radioprotector against X-radiation. It has the ability to scavenge the free radicals produced and to protect cells from radiation-induced cell damage.

The 4 MV photon beam offers equal build-up region behavior like Co-60 beam and it plays a major role in head and neck and pediatric radiotherapy. In this study an attempt is made to study the head scatter factor (SC) for 4 MV photon beam using locally designed PMMA and Brass miniphantoms. The SC is measured in combination of PMMA miniphantom with 0.6 cc chamber and Brass miniphantom with 0.6 cc and 0.13 cc chambers. The measured SC is compared with the literature data and it agrees within ± 1.98%. The study reveals that either 0.13 cc or 0.6 cc chamber with PMMA or Brass phantom materials can be used for SC measurements in a 4 MV photon beam. The variation of SSD does not alter the head scatter factor. The collimator exchange effect is found to be within 1, and it is less than that of other linear accelerators. It is also found that the presence of internal wedge has significant contribution to head scatter factor. The Phantom scatter factor is also calculated and it agrees within ±1% with published data.
