RESUMO

We present the case of a 43 year old patient diagnosed with rhegmatogenous retinal detachment and retinoschizis, a rare case of disease association. Surgery is recommended and we practice 23 gauge vitrectomy, laser retinopexy, criopexy in the periphery and internal heavy oil tamponade. Postoperatory evolution was favorable.

RESUMO

We present 3 cases of postsurgery endophthalmitis, with good initial operatory technique, which were admitted in our hospital within variable time, to which a second surgery was performed, with good postoperative evolution, without any inflammatory signs and preserving the eye. We analyse the pre and post-operative treatment of endophthalmitis, but also the ways to prevent the appearance of this post-operative complication.

RESUMO

PURPOSE: A review regarding the pathophysiology of AMD as shown in the literature RESULTS: Targets in AMD treatment include: 1. Protection against oxidative stress; 2. Prevention of the accumulation of lipofuscin; 3. Reduction or elimination of chronic inflammation; 4. Changes involving the participation of complement inflammatory phenomena; 5. Changes in the phenomena of chronic inflammation which do not involve the participation of complement (eg. Mitochondria and extracellular matrix). The Neovascularization process includes: 1. Production of angiogenic factor; 2. Release of angiogenic factor; 3. The binding of factors to extracellular receptors and activation of intracellular signaling; 4. Activation of endothelial cells with basement membrane degradation; 5. Endothelial cell proliferation; 6. Endothelial cell migration; 7. Remodeling of extracellular matrix; 8. Tube formation; 9. Vascular stabilization. CONCLUSIONS: Therapy inAMD, based on physiological characteristics of early and late stages, is possible nowadays. It is possible to apply a specific treatment for each stage of AMD, but effective treatment requires combinations of specific therapeutic remedies involving different pathophysiological pathways.

RESUMO

We present the case of a 65 years old pacient which was admitted for the sudden decrease of visual acuity in the left eye, accompanied by ocular pain and conjunctival hiperemia, simptoms appeared after an ocular trauma. After the clinical and paraclinical examination we determined the diagnosis of OS: Penetrating ocular trauma with retention of a foreign body; posttraumatic cataract. Surgical treatement was warrented and we performed OS : Facoemulsification + PFK implant in sulcus + 23 Ga posterior vitrectomy + peeling of the posterior hyaloid membrane + extraction of the foreign body + LASER endofotocoagulation + transscleral cryotherapy + SF6 gas injection. The post-operatory evolution was favorable.

OBJECTIVES: This study evaluates the efficiency of toric artificial intraocular lenses in correcting the astigmatism of pacients operated by facoemulsification. METHODS: A retrospective study was carried in which took part during the following period: february 2008 - february 2010. The study was carried on a number of 37 eyes. The pacients were evaluated by: refractometry, keratometry, corneal topography biometry, biomicroscopic examination of the eye (anterior and poterior poles), aplanotonometry the calculation of the IOL dioptric value and implantation axes with the help of the Alcon-acrysof calculator. RESULTS: AV without correction > 1/2 in 98 % of cases. AV without correction > 2/3 in 62 % of cases. AV with correction > 5/6 in 85 % of cases. Medium residual astigmatism between 0,25-0,75D CONCLUSIONS: The implant of toric IOL is a viable option in the treatement of preoperative astigmatism in selected cases.

RESUMO

We present the case of a 48 year old man who was admitted for the decreased visual acuity of the left eye for about 20 years, when a left sided facial trauma had occured. Clinical examination and lab exams revealed the cause of this change: ocular contusive posttraumatic status for about 20 years (anamnestic), iridodialysis, corectopie, traumatic cataract, PVR, tractional retinal detachment. Surgical treatment was warranted. We performed cataract extraction, posterior vitrectomy with laser fotocoagulation and injection of silicon oil 1000. We followed the pacient's post-operative evolution and we extracted of the silicone oil six months later. The evolution was favorable.

RESUMO

We present the case of a 57 years old woman which was admitted for the amputation of the infero-nasal visual field of the right eye, change observed by the pacient for 2 weeks. Clinical examination and lab exams revealed the cause of this change: a rhegmatogenous retinal detachment in the superior temporal quadrant. Surgical treatement was warranted. We did a posterior vitrectomy with trans-scleral crioapplications and injection of silicone oil 1000. We followed the patient's evolution post-operative and we observed the development of a complicated cataract which was treated in a second surgery together with the extraction of the silicone oil. The evolution was favorable.

BACKGROUND: Presbyastasis is a term used to describe the balance deficiences that appear in older individuals. One of the main features of presbyastasis is the bilateral vestibular hipofunction. The bilateral vestibular hipofunction affects the vestibulo-ocular reflex (VOR) leading to oscillopsia. MATERIAL AND METHOD: A comparative study between two groups ofpatients: a group of 16 patients diagnosed with presbyastasis and a second control group includind 11 healthy subjects. The aim of the study was to asses the disturbances of the visual acuity in pacients with presbyastasis. The static visual acuity and the dynamic visual acuity were measured using a Snellen chart. RESULTS: The dynamic visual acuity was severely impaired in patients with presbyastasis in wich the difference between the static and the dynamic visual acuity represented more then four lines. CONCLUSION: The dynamic vizual acuity can be used in the diagnostic protocol of the balance impairments that appear in bilateral vestibulopaties, such as presbyastasis.

PURPOSE: To evaluate the results of AcrySof Toric intraocular lens (Alcon) implantation to correct pre-existing astigmatism and rotational stability in patients having cataract surgery or not. METHODS: Retrospective study three surgical teams, between February 2009-August 2010, included 37 eyes with more than 1 diopter (D) of preexisting corneal astigmatism having or not cataract surgery. RESULTS: Rotation at 6 M post-op: 32 patients (87%)--> rotation between aprox. 0-5 degrees, 4 patients (11%)--> rotation between A 5-10 degrees, 1 patient (2%) rotation of A 20 degrees. The mean refractive cylinder decreased significantly after surgery: 34 (91.90%) of cases between 0.25-0.75 D--2 (5.40%) of cases 1.5 D--1 (2.70%) of cases > 1.5 D. CONCLUSIONS: In conclusion the results indicate that TheAcrySof Toric IOL is a viable option of preoperative corneal astigmatism in carefully selected cases. The Toric IOL showed good rotational stability The most frequent cause of turnover is the: viscoelastic left in the capsular bag (early rotation); fibrosis of the bag (late rotation).

RESUMO

The lesions of the pancreas represent a very interesting chapter of its pathology. Although they seem very much alike, the different anatomo- pathological forms have well defined individual features, which impose the therapeutical attitude. The authors have analyzed a group of 62 patients, admitted in our clinic between 1974 and 2003. Most of them had pancreatic pseudocysts (54-86%); 6 (10%) had cystadenomas; 1 (2%)--cystadenocarcinoma and 1 (2%)--solid and papillary cystic tumour. The diagnostic methods are largely presented and compared to the literature; it is emphasized the importance of imaging procedures and of the histo-pathological examination. The different therapeutical procedures, their advantages and limits are also analyzed--the necessity of a conservative attitude, with the total or partial preservation of the pancreas, is almost unanimously recognized.

RESUMO

Age related macular degeneration (ARMD) is a disease in which the retinal pigment epithelium (RPE) is damaged in the central retinal area. In the exudative form, the vision loss is due to choroidal neovascularization, while in the nonexudative or atrophic form, there is a vision loss because of the retinal pigment epithelium atrophy. Treatment which proved to be efficient in lowering the risk of severe vision loss in the exudative form includes laser photocoagulation, photodynamic therapy, transpupilary thermotherapy and as surgical treatment, the controversial subretinal membrane extraction. In all these situations the RPE is damaged by the disease itself and by the therapeutic procedure too. Retinal translocation is a surgical procedure that intends to remove the neurosensory retina from an area with damaged RPE to an healthy RPE area, through a 360 degrees retinotomy or through a limited one. This paper present some ARMD cases treated by subretinal membrane extraction and one by limited retinal translocation. The question which is still remaining is which are the risks and benefits for the following treatment procedures: laser photocoagulation, photodynamic therapy, transpupilary thermotherapy and surgical approach?

Vitreous hemorrhage has been the most common indication of posterior vitrectomy in diabetic retinopathy. Now, the leader is tractional retinal detachement and many others: rhegmatogenous retinal detachement, fibrovascular proliferations membranes, huge premacular hemorrhage. During vitrectomy procedure it may be necessary internal tamponade using air, expanding gases, silicon oil and/or perfluorocarbon liquids.

RESUMO

The authors analyze 13 cases of small bowel tumors. There are emphasized the difficulties of diagnosis, which is rarely established before the laparotomy. In the same time, clinical features are few and operations are often performed too late, in emergency, due to tumors complications (perforation, bleeding, occlusion). There are also presented new etiopathogenic hypothesis, recently mentioned in literature. Surgical treatment is followed by many post-operative complications. Five-years survival is low, under 40% of patients.
