RESUMO
Based on literature data and the results of own authors' research, the review article considers the efficacy and safety of sildenafil, including its long-term use. Application of this drug leads to an improvement of erectile function in patients of all ages, regardless of etiology, severity and duration of erectile dysfunction (ED). Drug has long-term efficacy. Sildenafil affects both arterial and venous blood flow to the penis, which makes it indicated in vasculogenic erectile dysfunction first. Treatment with sildenafil is accompanied by improvement of the cavernous electrical activity, which justifies its use in neurogenic form of the disease. According to results of IIEF questionnaire, sildenafil provides quick and lasting therapeutic effect. Efficacy and safety of sildenafil is rated as good. As for short-term and long-term use, sildenafil does not cause dependence and addiction.

The authors present a comparative estimation of therapeutic efficiency of conventional antibacterial therapy, coherent and incoherent impulse quantum irradiation at a constant magnetic field in combined therapy of 130 patients with acute pyelonephritis. Incoherent impulsive quantum irradiation at a constant magnetic field showed high clinical efficacy manifesting in early normalization of microcirculation of the affected kidney.

The time course of thyroid function and hypophyseal thyrotropic activity were investigated in 182 patients with diffuse toxic goiter of severe and moderately severe forms of thyrotoxicosis before operation and in short- and long-term periods after operation using radioimmunoassay in a hospital and outpatient clinic. The determination of blood T3, T4 and TSH at the height of clinical manifestations of thyrotoxicosis and before operation was shown to reflect the gravity of disease and could serve as a diagnostic test of the efficacy of preoperative preparation. A T4 concentration index varied at different time after operation. A considerable decrease in blood TSH on the first days after the operation and its rise in the following 6 mos. resulted from the patients' adaptation to new conditions and were of transient nature in the overwhelming majority of the patients. Final assessment of the thyroid and pituitary function was possible only 12 mos. after operation.

The time course of the basal level of the thyrotropic hormone (b TSH) and its concentration after thyroliberin (TRH) stimulation was studied in 28 patients with diffuse toxic goiter suffering from severe thyrotoxicosis before the onset of drug therapy, directly before operation, early and long after the operation. The TRH test was shown as a diagnostic test of efficacy of drug therapy. A higher level of b TTH with a lowered reaction to TRH in the first 6 mos. after operation was of compensatory nature requiring no substitution therapy. The restoration of the endocrine axis hypothalamus-pituitary gland-thyroid was noted 1 year after the operation.
