RESUMO

Autism spectrum disorder (ASD) as a serious neurodevelopmental condition requires intensive and comprehensive interventions, particularly interventions found to be effective through rigorous research. The National Professional Development Center on Autism Spectrum Disorders (NPDC) was formed in the USAin order to conduct a comprehensive review of ASD-related evidence-based practices (EBPs) and to create amodelof implementing such practices for children, adolescents and young adults (from preschool to high school education level). The NPDC final review, being the most comprehensive to date (the initial search included 29,105 articles) identified 27 ASD-focused EBPs. In addition to the resulting matrix of the autism-related EBPs, and the e-learning moduleson the identified interventions, the NPDC model includes as well the Autism Program Environment Rating Scale (APERS) for external evaluation and self-assessment, goal attainment scaling (GAS) and coaching program manual. To date, the model has been implemented in 12 states in the USAand is being introduced in several other countries including Australia, Sweden, Saudi Arabia, and Poland. The purpose of this article is to present the NPDC model and its components, along with the relevant research.

BACKGROUND: The efficacy of early intervention programs for children with autism has been emphasized in many studies. However, access for people with autism to professional services in Poland is very limited and the burden of supporting a child's development often falls on parents, especially in families with low socioeconomic status and families living far away from big cities. Using Internet resources in planning and delivering individualized intervention may be useful. This study examined the effects of a home program of intervention based on the AutismPro system with elements of consultative therapy. METHODS: Ten families of children with a diagnosis of autism participated in the project; nine of them completed the 6-month program of intervention. Parents were taught to use the AutismPro system and implement the intervention techniques in a home setting. Modification of the intervention program to suit individual children's needs and evaluations of children's progress were performed during consultation meetings with therapists. The pre- and post-treatment measurement of child development was performed using the Psychoeducational Profile - Revised (PEP-R). RESULTS: Children involved in the study progressed in total PEP-R scores and on the PEP-R subtest of fine motor, gross motor, non-verbal and verbal cognitive skills and eye-hand integration. Parents expressed positive opinions on the program. CONCLUSIONS: The results suggest that an intervention which combines the use of the Internet support tool and professional consultations may provide benefits to the children with autism. There are, however, methodological limitations of the study to be taken into account when interpreting the results.

BACKGROUND: Fracture of the femur is the most frequent late complication in patients with soft tissue sarcomas (STS) who receive external beam radiotherapy after limb-sparing surgery. AIM: To reduce the risk of bone fracture following radiotherapy of STS of the thigh, we minimized the dose to the femur and to surrounding normal tissues by applying intensity modulated radiation therapy (IMRT). We report preliminary results of post-surgery IMRT of the thigh in patients with STS in this extremity. MATERIALS AND METHODS: 10 adult patients undergoing post-operative radiotherapy of STS of the thigh were treated using IMRT. Clinical IMRT plans with simultaneous integrated boost (SIB) and 3-phase three-dimensional conformal radiotherapy (3D-CRT) were designed to adequately treat the planning target volume and to spare the femur to the largest extent possible. Dose distributions and dose-volume histograms were compared. RESULTS: For either technique, a comparable target coverage was achieved; however, target volume was better covered and critical structures were better spared in IMRT plans. Mean and maximum doses to OAR structures were also significantly reduced in the IMRT plans. On average, the mean dose to the femur in 3D-CRT plans was about two times higher than that in IMRT plans. CONCLUSION: Compared with 3D-CRT, the application of IMRT improves the dose distribution within the concave target volumes and reduces dose to the OAR structures without compromising target coverage.
