ABSTRACT: Recently, the coronavirus disease 2019 (COVID-19) epidemic has greatly threatened global public health. The responsibility of healthcare-associated infection control professionals (ICPs) is to prevent and control the nosocomial infections. The mental health status of ICPs deserves more attention, however, the correlational research is still lacking. This study aims to investigate the incidence and risk factors of mental health status among ICPs in China during the outbreak of COVID-19.A national cross-sectional survey was performed. The online questionnaire was completed by 9228 ICPs from 3776 hospitals throughout China. Data collection tools were used, including demographics data questionnaire, the Chinese version of the 12-item general health questionnaire (GHQ-12) and the Chinese version of the psychological capital questionnaire (PCQ) for medical staff. Univariate and multivariable analyses were conducted.The total score of mental health of Chinese ICPs was 3.45 ±â€Š2.57. 5608 (60.77%) ICPs might have mental health problems. The psychological capital was in the upper-middle level with an average score of 3.72 ±â€Š0.38. An increased mental health problem risk was associated with the greater self-efficacy and working in the public hospital; a significantly lower risk was obtained by working in the second-class hospital rather than in the third-class hospitals. Besides, mental health problem risk of ICPs working in hospitals of the western economic region or northeast economic region was more significant than that in hospitals of the central economic region. However, a lower risk was caused by the unmarried than married, and working years in department ≤1 year contributed to the lower risk than that >20 years. Moreover, fewer working hours per week, higher values of hope, and optimism each were contributed to a lower risk.Chinese healthcare-associated ICPs were under different levels of mental health problems in fighting against COVID-19. More importantly, we should actively deal with the mental health problem of ICPs and help them get rid of psychological disorders.

RESUMO

Iatrogenic femoral artery pseudoaneurysm (IFAP) caused by invasive procedures is one of the common complications for endovascular interventions. We present a case of a young male patient with a complex IFAP as a result of iatrogenic femoral artery puncture. The defective femoral artery was repaired with combined bovine pericardial tube and autologous great saphenous vein grafts. Computed tomography angiography showed the grafts were still patent one year after the surgery.

OBJECTIVES: To analyze the CT manifestations,clinical symptoms, laboratory test and the interrelationship of them with secondary invasive pulmonary aspergillosis (IPA) in hematologic patients so as to improve the diagnosis and prognosis of this disease. METHODS: A total of 37 hematologic patients suspected of IPA in a tertiary referral center were selectedfrom Jun. 2004 to Sep. 2016 and the CT characteristics, clinical manifestations, laboratory test of these patients were retrospectively analyzed. RESULTS: There were 8 patients confirmed as IPA by sputum culture or bronchoalveolar lavage fluid culture among the 37 hematologic patients.The sensitivity of CT in the diagnosis of IPA was 75.0%, the specificity was 79.3%, the positive predictive value was 42.9%, and the negative predictive value was 91.3%. Multiple patchy shadows in both lungs were the most common CT signs of suspected and confirmed patients, followed by nodules, air bronchogram, cavitary lesions, consolidation, mass shadows, halo, and crescent sign. Cough, expectoration, and fever were the most common symptoms among the suspected and confirmed patients. ß-D-glucan (G)/galactomannan antigen (GM) tests for the most patients were negative, but the number of people with elevated procalcitonin (PCT) in the confirmed patients (57.1%) was higher than that in the suspected patients (31.4%). Results of sputum culture, PCT, G/GM test, and clinical manifestations were compared between the CT lesion progression group and the absorption group, and no significant difference was observed. CONCLUSIONS: Hematologic patients are susceptible to IPA. CT features showed no obvious specificity, and the G/GM test was normal in most patients. So when hematologic patients have clinical symptoms such as cough, expectoration and fever, the possibility of combination with IPA should be considered as soon as possible, and relevant examinations such as sputum culture, bronchoalveolar lavage fluid culture and pathological examination should be completed for early diagnosis and treatment to improve the prognosis of patients.

Lipid droplets (LDs) are dynamic cytoplasmic organelles present in most eukaryotic cells. The appearance of LDs in neurons is not usually observed under physiological conditions, but is associated with neural diseases. It remains unclear how LD dynamics is regulated in neurons and how the appearance of LDs affects neuronal functions. We discovered that mutations of two key lipolysis genes atgl-1 and lid-1 lead to LD appearance in neurons of Caenorhabditis elegans. This neuronal lipid accumulation protects neurons from hyperactivation-triggered neurodegeneration, with a mild decrease in touch sensation. We also discovered that reduced biosynthesis of polyunsaturated fatty acids (PUFAs) causes similar effects and synergizes with decreased lipolysis. Furthermore, we demonstrated that these changes in lipolysis and PUFA biosynthesis increase PUFA partitioning toward triacylglycerol, and reduced incorporation of PUFAs into phospholipids increases neuronal protection. Together, these results suggest the crucial role of neuronal lipolysis in cell-autonomous regulation of neural functions and neurodegeneration.

RESUMO

A simple, eco-friendly, and efficient methodology for performing radical cyclizations of enynes/dienes with alcohols in water has been established. This methodology showed ease of scale up, and it was designed to use mild reaction conditions and no catalyst. It was also designed to employ K2S2O8 as a green oxidant and water as the solvent, conditions making this process clean and easy to operate, hence achieving the criteria of green chemistry.

BACKGROUND: Currently, both minimally invasive plate osteosynthesis (MIPO) and intramedullary nailing are the two most commonly used methods of treatment in distal tibial fractures, but controversy still exists regarding the clinical effects of 2 techniques. Our purposes were to compare MIPO and intramedullary nailing for distal tibia shaft fractures by assessing functional outcomes and complications. METHODS: Data were collected retrospectively from the charts of patients treated for distal tibial extra-articular fractures between May 2012 and July 2018. All cases were performed by a single surgeon. Institutional review board approval in the Second Affiliated Hospital of Army Medical University was obtained prior to conducting chart review and analysis. The criteria for inclusion in the study were being aged at least 18 years at the time of diagnosis and having a closed or type I open fracture of the distal third of the tibial diaphysis. The primary outcome compared between the 2 groups was the American Orthopedic Foot and Ankle surgery score. The secondary outcome measures in this trial included Olerud and Molander Ankle Score, radiographic outcomes, and complications. Statistical analysis was performed using SPSS version. P values < .05 were considered statistically significant. RESULTS: We hypothesized that MIPO would be associated with better functional outcomes and fewer complications. TRIAL REGISTRATION: This study protocol was registered in Research Registry (researchregistry5808).

In this paper, a concept about the aeroengine fan health monitoring approach is proposed based on the compressive-sensing-based acoustic mode detection method. Utilizing only a few acoustic sensors, possible accidents inducing the change of the fan noise mode spectrum can be inferred. To enable such a concept, the array design strategy and optimization method are first studied by maximizing the incoherence of the so-called sensing matrix. The performance of the designed array is examined in both simulation and experimental studies. Then, the idea of fan noise monitoring is conceptually demonstrated in wind tunnel tests by taking into account possible accidental scenarios with foreign body intrusions. The simulation and experimental results suggest that under such circumstances remarkable changes appear in the azimuthal mode spectrum from fan noise. Finally, it is demonstrated that the fan noise variation can be successfully detected by the compressive sensing method with just six sensors. In this way, the foreign body intrusion can be further diagnosed through the combination of compressive sensing and mode detection. Overall, the results confirm the potential capability of the proposed concept for future aeroengine health monitoring applications.

To propose the architectural layout for the big general hospital in the face of public health emergencies, we analyzed the conditions, methods, problems and countermeasures for the reconstruction of the isolation ward from the existing medical building layout of a general hospital. The affected areas met the requirements of isolation ward in the reconstruction, and realized the corresponding partition and separation of people. But the cost of occupying the medical room should be concerned. General hospital should be alerted to potential risks of public health emergencies. The characteristics of different construction types, defects, and the function of the hospital should be considered in the construction, rebuilding, and expansion of the hospital, which shouldnot only meet the needs of the development of the hospital daily usage but also consider dealing with emergent public health events. We can adopt the reasonable layout, including setting up a firewall-like device between the channel and the floor, an ordinary ward at ordinary times, and an independent space for emergency by pulling down the gate. This strategy can not only avoid the problem of low utilization rate of the space occupied by the corresponding area in the ward for diseases spread by air and droplets, maximizing the efficiency of the medical site, but also avoid the problem of emergency response to the temporary reconstruction.

ER stress occurs in many physiological and pathological conditions. However, how chronic ER stress is alleviated in specific cells in an intact organism is an outstanding question. Here, overexpressing the gap junction protein UNC-9 (Uncoordinated) in C. elegans neurons triggers the Ire1-Xbp1-mediated stress response in an age-dependent and cell-autonomous manner. The p38 MAPK PMK-3 regulates the chronic stress through IRE-1 phosphorylation. Overexpressing gap junction protein also activates autophagy. The insulin pathway functions through autophagy, but not the transcription of genes encoding ER chaperones, to counteract the p38-Ire1-Xbp1-mediated stress response. Together, these results reveal an intricate cellular regulatory network in response to chronic stress in a subset of cells in multicellular organism.

The novel human coronavirus (SARS-CoV-2) causes the coronavirus disease 2019 (COVID-19) pandemic worldwide. Control of COVID-19 pandemic is vital for public health and is the prerequisite to maintain social stability. However, the origin and transmission route of SARS-CoV-2 is unclear, bringing huge difficult to virus control. Monitoring viral variation and screening functional mutation sites are crucial for prevention and control of infectious diseases. In this study, we developed a user-friendly software, named BioAider, for quick sequence annotation and mutation analysis on large-scale genome-sequencing data. Herein, we detected 14 substitution hotspots within 3,240 SARS-CoV-2 genome sequences, including 3 groups of potentially linked substitution. NSP13-Y541C was crucial substitution which might affect the unwinding activity of the viral helicase. In particular, we discovered a SR-rich region of SARS-CoV-2 distinct from SARS-CoV, indicating more complex replication mechanism and unique N-M interaction of SARS-CoV-2. Interestingly, the quantity of SSRX repeat fragments in SARS-CoV-2 provided further evidence of its animal origin. Overall, we developed an efficient tool for rapid identification of viral genome mutations which could facilitate viral genomic studies. Using this tool, we have found critical clues for the transmission route of SARS-CoV-2 which would provide theoretical support for the epidemic control of pathogenic coronaviruses.

The ongoing coronavirus disease 2019 (COVID-19) pandemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has imposed a significant impact on social and economic activities. As a high infectious pathogen, the existence of SARS-CoV-2 in public space is very important for its transmission. During the COVID-19 pandemic, hospitals are the main places to deal with the diseases. In this work, we evaluated the exposure risk of SARS-CoV-2 in hospital environment in order to protect healthcare workers (HCWs). Briefly, air and surface samples from 6 different sites of 3 hospitals with different protection levels were collected and tested for the SARS-CoV-2 nucleic acid by reverse transcription real-time fluorescence PCR method during the COVID-19 epidemic. We found that the positive rate of SARS-CoV-2 nucleic acid was 7.7 % in a COVID-19 respiratory investigation wards and 82.6 % in a ICUs with confirmed COVID-19 patients. These results indicated that in some wards of the hospital, such as ICUs occupied by COVID-19 patients, the nucleic acid of SARS-CoV-2 existed in the air and surface, which indicates the potential occupational exposure risk of HCWs. This study has clarified retention of SARS-CoV-2 in different sites of hospital, suggesting that it is necessary to monitor and disinfect the SARS-CoV-2 in hospital environment during COVID-19 pandemic, and will help to prevent the iatrogenic infection and nosocomial transmission of SARS-CoV-2 and to better protect the HCWs.

Cadmium (Cd) contamination is the most common and extensive heavy metal pollution in the farmland of China. Phytoremediation is considered as a promising measure for Cd-contaminated soil remediation, but the remediation efficiency still needs to be enhanced. Biochar as an effective amendment medium is widely manufactured and studied for the soil remediation of heavy metals. In this study, a greenhouse pot trial was conducted to investigate the effects of cornstalk biochar on Cd accumulation of Beta vulgaris var. cicla L. (Beta vulgaris) in Cd contaminated soil. The Cd availability, speciation and nutrients in soil, biomass and Cd chemical forms in the Beta vulgaris root were studied to explore the mechanism that how the cornstalk biochar promoted Cd accumulation in Beta vulgaris. Biochar amendment reduced the DTPA-extractable Cd concentration and stimulated the growth of root. Compared to the Beta vulgaris without biochar treatment, the results of 5% biochar amendment showed that the root dry weight of Beta vulgaris increased to 267%, Cd accumulation in Beta vulgaris increased to 206% and the Cd concentration in leaves and roots increased by 36% and 52%, respectively. Additionally, after 5% biochar was applied to soil, the total content of organic matter-bound Cd and residual Cd increased by 38%, while the content of Fe-Mn oxides-bound Cd decreased by 40%. Meanwhile, Cd may mainly bind to the root cell wall and the ratio of NaCl-extracted Cd to HAc-extracted Cd increased to 166% with 5% biochar amendment. According to our study, Cd in soil can be removed by Beta vulgaris and phytoremediation efficiency can be improved with biochar amendment. The combination of phytoremediation and biochar amendment is a promising strategy for the Cd-contaminated soil remediation.

A wavelet-based beamforming method is performed in this investigation to analyze moving acoustic sources in the time-frequency domain, which is of scientific significance and practical importance. The particular problem considered here is the interaction noise from an airfoil and the downstream rotor in the presence of a moving flow, which could find realistic applications in next-generation underwater and aviation systems. A realistic experimental setup is prepared with a rotating blade and the airfoil in an anechoic wind tunnel. The results show that the wavelet-based beamforming method is very suitable for unsteady sound source imaging, which would be able to strengthen the time-frequency analysis capability of acoustic imaging tests and, consequently, possibly leads to deepened physical insights of various transient and moving systems in underwater and aerospace systems.

Fluorescent carbon dots (CDs) have numerous important applications, but enhancing the fluorescence emission and overcoming fluorescence quenching are still big challenges. Here, fluorescence-enhanced carbon dots (named hr-CDs) were prepared from sustainable hydrogenated rosin, using a simple hydrothermal method in a water solvent. The hr-CDs were mainly composed of graphitized carbon cores with surface functional groups. With the increase in the concentration to hr-CDs aqueous solutions, the distance between the carbon cores decreased, which resulted in the formation of J aggregates and the enhanced blue fluorescence emission. Even in the solid state, the hr-CDs show fluorescence emission because the surface functional groups could prevent π-π stacking interactions between the carbon cores. The hr-CDs show excellent resistance to photobleaching under intense ultraviolet light (200 mW/cm2). Vibrations and rotations of graphitized carbon core are restricted by low temperature and high viscosity, leading to increased radiative transition and thus increase in fluorescence intensity. The pH value in the range of 3.99-9.87 and anions have little effect on the fluorescence emission of hr-CDs. The fluorescence emission of the hr-CDs was selectively quenched by Fe3+ and can thus be used to detect Fe3+. The hr-CDs also have good biocompatibility and show the same ability in cell nuclear staining as 4',6-diamidino-2-phenylindole (DAPI).

The time to positivity (TTP) of blood cultures has been considered a predictor of clinical outcomes for bacteremia. This retrospective study aimed to determine the clinical value of TTP for the prognostic assessment of patients with Escherichia coli bacteremia. A total of 167 adult patients with E.coli bacteremia identified over a 22-month period in a 3500-bed university teaching hospital in China were studied. The standard cut-off TTP was 11 h in the patient cohort. The septic shock occurred in 27.9% of patients with early TTP (⩽11 h) and in 7.1% of those with a prolonged TTP (>11 h) (P = 0.003). The mortality rate was significantly higher for patients in the early than in the late group (17.7% vs. 4.0%, P < 0.001). Multivariate analysis showed that an early TTP (OR 4.50, 95% CI 1.70-11.93), intensive care unit admission (OR 8.39, 95% CI 2.01-35.14) and neutropenia (OR 4.20, 95% CI 1.55-11.40) were independently associated with septic shock. Likewise, the independent risk factors for mortality of patients were an early TTP (OR 3.80, 95% CI 1.04-12.90), intensive care unit admission (OR 6.45; 95% CI 1.14-36.53), a Pittsburgh bacteremia score ⩾2 (OR 4.34, 95% CI 1.22-15.47) and a Charlson Comorbidity Index ⩾3 (OR 11.29, 95% CI 2.81-45.39). Overall, a TTP for blood cultures within 11 h appears to be associated with worse outcomes for patients with E.coli bacteremia.
